Thirty two cases of patient with Thoracolumbar Compression Fracture were treated and observed clinically from November, 1997, to August, 1998, in the Dept. of Acupuncture and Moxibustion, Chon-an Oriental Medical Hospital, Taejon University. The results were obtained as follows; 1. The distribution of sex was male 6 cases(18.7%), female 26 cases(81.3%) and the distribution of age was seventies 17 cases(53.2%) the most. 2. The distribution of the causes was Slip-down 17 cases(53.2%) the most. 3. The distribution of the period before admission was within a week 19 cases(59.4%) the most. 4. The distribution of treatment duration in descending order: Under a week and two weeks under four weeks was the most number in 11 cases(59.4%) each. 5. The distribution of the injured level was L1 body 15 cases(18.3%) the most. 6. In the distribution of grading clinical severity on admission, the most cases were Gr.IV(18cases, 56.3%). 7. In the distribution of clinical symptoms on admission, 30 patients(21.9%) complained lower back pain and disturbance in turning over. 8. In the distribution of treatment result, the most cases(18 cases, 56.3%) was good. 9. The distribution of treatment duration according to the clinical grade on admission was as follows: Two weeks under four weeks was the most number in 8 cases(25%) in the group of Gr.IV, under two weeks was the most number in 6 cases(18.7%) in the group of Gr.III, and under four weeks was the most number in 2 cases(6.2%) in the group of Gr.II. 10. The distribution of treatment result according to the clinical grade on admission was as follows: Good was the most number in 10 cases(31.2%) in the group of Gr.IV, 6 cases(10.7%) in the group of Gr.III and 2 cases(6.2%) in the group of Gr.II. 11. The distribution of treatment result according to the treatment duration was as follows: Good was the most number in 4 cases(12.5%) under two weeks, 6cases(18.7%) two weeks under four weeks, and 6 cases(18.7%) four weeks under six weeks.

Objective: The majority of embryo transfers (ETs) to date have been performed on day 3 to reduce the potential risk of developmental arrest of in vitro cultured embryos before ET. Development of sequential media has significantly improved culture conditions and allowed blastocyst transfer on day 5. While day 5 ET provides higher clinical pregnancy outcomes with reduced risks of multiple pregnancies, it still has potential risks of developmental arrest of IVF embryos. The aim of this study was to evaluate the clinical outcomes of day 4 ETs and compare the efficacy of day 4 ET with day 5 ET. Methods: From 2006 to 2009, a total of 747 fresh IVF-ET cycles were retrospectively analyzed (day 4, n=440 or and day 5, n=307). The cycles with any genetic factors were excluded. The rates of matured oocytes, fertilization, good embryos, and clinical pregnancy of the two groups were compared. The chi-square test and t-test were used for statistical analysis. Results: There were no significant differences between the two groups with respect to the mean age of the females and rates of matured oocytes. The pregnancy outcomes of day 4 ET (40.7%) were similar to those of day 5 ET (44.6%). The implantation rate of day 5 ET (24.2%) was significantly higher than that of day 4 ET (18.4%) (p=0.003). Conclusion: Day 4 ET can be chosen to avoid ET cancellation in day 5 ET resulting from suboptimal circumstances in the IVF laboratory, but the decremented quality of embryos for transfer and the decreased pregnancy rate must be taken into consideration.
Objective: Ths research was to investigate the efficacy of bee venom theraphy on peripheral facial paralysis. Methods: Thirty patients were deyided into two groups with fifteen patients each. Group A and B were both treated with basic oriental medicine treatment, and also group A was treated with bee venom aqua-acupunture additionaly. The groups were evaluated by Yanagihara's unweighted grading system before treatment, and at every one week till four weeks after treatment. Results: The Yanagihara's scores of group B were higher than those of group A before treatment, after one week and two weeks, but not statistically significant. After three and four weeks, the Yanagihara's scores of group A were higher than those of group B, but not statistically significant. The improvement indexes of group A were higher than those of group B from the first week and statistically significant after three and four weeks. Conclusion: Combinding bee venom aqua-acupunture theraphy on peripheral facial paralysis was more efficacious than the only use of basic oriental medicine treatment.
Purpose : To evaluate the neurologic abnormalities in the donor limb after contralateral C7 transfer in brachial plexus injury. Materials and Methods : From August 1996 to December 1999, five patients with brachial plexus injury were treated with contralateral C7 nerve root transfer. The average follow up was 16 months(range, 5 to 36 months). The clinical findings were assessed using the British Medical Research Council Grading System, and also measured grip power, pinch power of hand and two point discrimination of the fingers. Results : We had no difference in shoulder abduction and elbow flexion after contralateral C7 transfer. The grip and pinch strength were recovered within 4weeks. Sensory loss occurred in all patients and was noted to be more severe on index and middle finger. Four patients recovered within 2 weeks, one continued till one year. Subjective numbness and pain on percussion minimally persisted until last follow-up. Conclusion : The division of the C7 nerve root resulted in minimal and temporary functional deficit in the donor upper limb.
Background: Sonoelastography (SE) is a new technique that can assess differences in tissue stiffness, the purpose of this study was to evaluate the ability of SE to assess the long head of biceps tendon alteration. Methods: Forty shoulders of 36 consecutively registered patients with clinical symptoms and conventional ultrasonography findings of biceps tendinitis or tendinosis, and 40 asymptomatic shoulders of 20 healthy volunteers were assessed with SE. Transverse and longitudinal images of long head of biceps tendon were obtained using SE. SE images were performed by one orthopedic surgeon and evaluated by two orthopedic surgeons using an experimentally proven color grading system. Results: The transverse images of SE showed a sensitivity of 87.5%, a specificity of 95.0% and a accuracy of 91.3%, the longitudinal images of SE showed a sensitivity of 92.5%, a specificity of 90.0% and a accuracy of 91.3%. Inter-observer reliability of SE was in 'almost perfect agreement' with a weighted kappa coefficient of 0.83. Conclusions: SE is valuable in the detection of the intratendinous and peritendinous alterations of biceps tendon, and has excellent accuracy and excellent correlation with conventional ultrasound findings.
Purpose: For hypertrophic scars and keloids no universally effective treatment modality exists. Surgical revision, intralesional steroid injection, silicone gel sheeting, pressure, laser, and others have been used with variable success, but many treatments are associated with high recurrence rates. Although optimal treatment remains undefined, successful treatment can be obtained through a combined therapeutic approach. Methods: We used three therapeutic modalities in combination, which are intralesional injection of triamcinolone acetonide, silicone gel sheeting, and 585 nm flashlamp-pumped pulsed dye laser. Fifty-eight cases of hypertrophic or keloid scar were treated by combined therapeutic regimen for mean period of 18 months. The changes of thickness, color, and pliability of scars were evaluated with clinical photographs by grading scale. Results: As summing the grades and categorizing the result into three group, we obtained 28% good, 67% fair, and 5% poor results. There was a desirable improvement of scars with insignificant adverse effects. Conclusion: Combination of intralesional steroid injection, silicone gel sheeting, and pulsed dye laser can lead to successful treatment of hypertrophic scar and keloid.
Objectives : This study is to investigate the effect of the sasang constitutional prescription with peeling and MTS on Acne patients. Methods : We treated 81 acne patients with sasang constitutional prescription with peeling and MTS. After treatments we compared the before and after photos, and estimated by Korean Acne Grading System. Results & Conclusions : 1. Patients were comprised of 59 females and 22 males. And there were 30 adolescent and 51 postadolescent acne patients. Out of 81 acne patients, there were many 41 postadolescent female acne patients. 2. Sasang constitutional distribution was comprised of 74 Soyangin, 5 Soeumin and 2 Taeeumin 3. Yangkyuksanhwa-tang(48.1%) and Yangdokbackho-tang(40.7%) were used much in 81 acne patients. 4. Before the treatment, 92.3% of Yangkyuksanhwa-tang group and 63.6% of Yangdokbackho-tang group had mild acne symptom and acne grade mean of Yangdokbackho-tang group was higher than another groups. 5. Peeling times mean of 81 acne patients was 2.54 times and MTS times mean was 4.57 times. Acne grade was improved when peeling times were higher. 6. Before the treatment, 81 acne patients were comprised of 81.5% mild symptom and 18.5% severe symptom patients and acne grade mean was 2.75. After the treatment. there were only mild symptom patients and acne grade mean was 1.04.
A 25-years-old woman with mandibular prognathism underwent a mandibular setback by way of mandibular sagittal split ramus osteotomy (MSSRO). After 2 days of operation, she developed difficulty of closing her right eye. The blink reflex test and motor nerve conduction study of the right orbicularis oris muscle were revealed right facial neuropathy of unknown origin and House-Brackmann facial nerve grading system (HBFNGS) grade V. For treatment, we initially prescribed oral prednisolone and nimodipine including physical therapy. The samples consisted of 11 facial nerve palsy patients caused by MSSRO and were analysed about onset of facial nerve palsy, postoperative HBFNGS, final HBFNGS, treatment method and recovery time. At 10 weeks of treatment of nimodipine, she had completely regained normal function (HBFNGS grade I) of the right facial nerve. The clinical results lead to assume a fast recovery of facial nerve function by the nimodipine medication, whereas average time of recovery is 16.32 weeks in references. Despite of the limited one patient treated, the result was very promising with respect to a faster recovery of the facial nerve function. Considering the use of nimodipine treatment for peripheral facial nerve palsy following a surgical approach with an anatomically preserved nerve can be recommended.
Objective : To investigate how general characteristics and life habits of patients affect severity of acne which was determined by Korean Acne Grading system(KAGS). To adress correlation between the severity of acne and subjective stresses from which patients suffer. Methods : 86 patients answered to questionaire sheet on their general characteristics and life habits and rated their level of stresses as a number in the range between 0 and 10. Then an oriental medical doctor evaluated subject's acne grades, based of KAGS, through observations on papules, nodules, and scars before (medical) treatments. All data (collected) were analyzed using SPSS 22 (software for windows). To clearly demonstrate association of subjects general characteristics, life habits with KAGS grades, Chi-square tests were performed. The correlation between the KAGS grades and the level of stresses was determined using one-way ANOVA followed by Scheffe's post-hoc analysis. Results & Conclusion : Majority of subjects were women, students between 15 and 25 years old, and patients with normal range of Body Mass Index(BMI). Dysmenorrhea presence and premenstrual exacerbation of acne had statistically significant relationship, suggesting their potential influences on KAGS grades as major factors.
Objectives : The purpose of this study was to analyze the general features of patients with idiopathic faical paralysis. Methods : We investigated the various features and HBGS (House-Brackmann Grading System) of 505 patients who were diagnosed with idiopathic facial paralysis. Results and Conclusions : 1. The sex distribution was 264 male cases, and 241 female cases, and the majority of them were in their 40s. 2. Overwork was the most frequent contributing factor to their paralysis. 3. The treatment of most patients was completed within 2 months. 4. Their recovery was faster if their first hospital visit was sooner and their paralysis was partial. On average, the patients recovered within 19.5 to 27.8 days and it took much less time than natural recovery. 5. When looking at the BMI of the selected patients for this study, 62.00% were obese while 14.37% were under-weight. Among the obese patients, there were more with Xu mai (虛脈) than Shi mai (實脈). For the under-weight patients, most had Shuo mai (數脈) while none had Chi mai (遲脈).
