Using DITI to examine the pattern of subcutaneous heat of the climacteric patients (DITI를 이용한 갱년기 환자의 체열분포 양상)

  • 1. Objectives Among many symptoms that the climacteric patients complain of, the major subjective symptom is both heat in the upper part and cold in the lower part of the body(e.g foot, knee, and leg). We use DITI as a method to prove the symptoms of climacteric patients such as both heat in the upper part and cold in the lower part of the body, diagnose these symptoms, and follow up the progress of the clinical treatments. 2. Subjects and Methods We compare the subcutaneous heat of the climacteric patients with the symptom of heat in the upper part and cold in the lower part of the body with young women without any specific disease. The age distribution of the experimental group is 28 patients in 40's, and 22 in 50's. The mean age of the experimental group is $48.5{\pm}4.25$. The age distribution of the control group is 17 patients in teen, 26 in 20's, 7 in 30's. The mean age of the control group is $23.31{\pm}6.45$. We measure the subcutaneous heat on the cheeks, upper arms, palms, thighs, knees, dorsum pedis, lower back, hip, upper abdomen, lower abdomen of both groups and analyse the results. 3. Statistical methods All the results are statiscally analysed using student T-test of Microsoft Exel program. Statistically significant value by the analysis of variance procedure is P<0.05. 4. Results 1. The subcutaneous heat of the whole body of the experimental group shows hypothermia compared with control group, and the hypothermic pattern gets severe on the periphery. ${\Delta}T$ between the right and left side of the body in experimental group is larger than that of control group. 2. ${\Delta}T$ between the cheeks and the hypogastric in the experimental group is significantly larger than that of the control group, which shows cold face. 3. ${\Delta}T$ between the upper arms and the palms in the experimental group is significantly larger than that of the control group, which shows cold hand. 4. ${\Delta}T$ between the thighs and the knees in the experimental group is significantly larger than that of the control group, which shows cold knee. 5. ${\Delta}T$ between the thighs and the feet in the experimental group is significantly larger than that of the control group, which shows cold foot. 6. ${\Delta}T$ between the hip and the lower back in the experimental group is significantly larger than that of the control group. That shows the Lower back is warmer than the hip. 7. ${\Delta}T$ between the upper and the lower abdomen in the experimental group is significantly larger than that of the control group, which shows cold hypogastric.

5 Cases of Climacteric Syndrome Patients with Hwa-Byung (火病을 동반한 갱년기 환자 치험 5례)

  • Objectives: The purpose of this study is to evaluate the effects of Korean traditional treatments for Climacteric Syndrome Patients with Hwa-Byung. Methods: From February 2015 to May 2015, We treated 5 Cases Patients with Korean traditional treatments for a month. We measured treatment effects by Menopause Rating Scale (MRS), Beck's Depression Inventory (BDI), Visual Analogue Scale (VAS) and Digital Infrared Thermal Imaging (DITI). Results: After treatment, First, the scores of MRS, BDI were decreased. Second, the symptoms such as hot flush, pantalgia, palpitation, insomnia, fatigue were significantly improved. Also, the difference of 膻中 (CV17, Danjung) and 關元 (CV4, Guanyuan) temperatures were significantly decreased after treatment. Conclusions: This study suggests that Korean traditional treatments such as Gamicheonglijagam-hwan, acupuncture, moxibustion are effective on Climacteric Syndrome Patients with Hwa-Byung.

A Clinical Case Study to Evaluate the Hot flush and Abdominal Cold Hypersensitivity Relief Efficacy of Cheonglijagamtang in Climacteric Women by DITI (청리자감탕(淸離滋坎湯)으로 호전된 갱년기 여성의 상열감(上熱感)과 복부냉증(腹部冷症)에 대한 DITI를 이용한 평가)

  • Purpose : To quantitative analyse the hot flush and abdominal cold hypersensitivity relief efficacy of Cheonglijagamtang in climacteric women by DITI. Methods : The patients was a 59-year-old climacteric woman who was suffered by hot flush and abdominal cold hypersensitivity. The patient was treated by herb medicine, acupuncture treatment, moxa treatment and physical treatment. The progress of symptoms was evaluated by checking the change of VAS, and inspecting the DITI. Results : Visual analog scale of hot flush and abdominal cold hypersensitivity in this climacteric women have decreased after treatment. The temperature of two abdominal site(RN12.RN4) after treatment get higher than those before treatment. Difference of acupoint-Indang and other abdominal site(RN12.RN4)'s temperature have decreased after treatment. Conclusion : The result suggest that DITI can be used for the diagnosis of hot flush and coldness relief efficacy in climacteric women.

  • Purpose: The purpose of this study is to analyze the characteristics of the pulse energy in women who complain of climacteric and menopausal symptoms. Methods: We analyzed the values of pulse energy on 45 climacteric and menopausal women. The patients' symptoms were assessed by Kupperman's index and Menopause Rating Scale (MRS). Statistical analysis was performed using SPSS 18.0 for window program, one way ANOVA, pearson correlation coefficient. Results: The results were as follows. 1. The pulse energy of the Left Chon was significantly largest and Left Cheok was comparatively smallest. 2. There was no statistically significant correlation in the pulse energy of Left Chon minus Left Cheok, Kupperman's index, MRS total sum score. Conclusions: The results suggest that the pulse energy of the Left Chon tends to be increased and Left Cheok decreased in climacteric and menopausal women.

  • Purpose: The purpose of this study is to investigate the characters of Yangdorak in climacteric women. Methods: We analyzed the measurement value of Yangdorak on 37 climacteric women, aged 45-60 years. The patient's symptoms were assessed by modified Kupperman's index. Statistical analysis was performed using SPSS 13.0 for windows program. one-sample t-test. pearson correlation coefficient. Results: The results were as follows. 1. The average value of Yangdorak in 37 patients with climacteric women was 29.02$\pm$16.85. 2. According to the comparison of mean value with the each meridian value of Yangdorak. H1, H5 is high, F2, F3, F5 is low significantly. 3. The number of Pyesaek is more than Gyeokcha. 4. The Yangdorak value(F2) had significant correlation with modified Kupperman's index. Conclusion: The result suggests that Yangdorak values in H1, H5, F2, F3, F5 are related to climacteric. Further study on the yangdorak and climacteric syndrome measured by Kupperman's index is recommended.

A Systemic Review of Recent Randomized Controlled Trials of Insomnia in Climacteric Syndrome Treated with Acupuncture (갱년기 증후군으로 인한 불면의 침 치료 무작위 대조군 최신 연구의 체계적 문헌 고찰과 메타 분석)

  • Objectives: This study was aimed to review the effect of acupuncture for insomnia in climacteric syndrome. Methods: Researchers searched data through 5 online databases up to December 2021. The data was limited to randomized controlled trial studies on patients diagnosed with insomnia in climacteric syndrome. Results: 8 Randomized controlled trials were included in this study. 6 of studies were published in Chinese, in China. 2 were published in English. Intervention of treatment group were composed of simple acupuncture. Intervention of control group were made up with Estazolam, Alprazolam, Lorazepam and sham-acupuncture. Outcome measurements were Total effective rate, Pittsburgh Sleep Quality Index (PSQI), Kupperman index, Menopause Quality of Life scales (Men-QoL), the level of hormones (E2, FSH, LH) etc. In all outcome measurements, treatment group were more effective than control group and it was statistically significant. Conclusions: Acupunture therapy is effective for treating insomnia in climacteric syndrome. Pittsburgh Sleep Quality Index (PSQI) is most frequently used for sleep assessment indicator. 神門 (HT7) is the most frequently used acupoint for treatment group. However because insomnia in climacteric syndrome is easy to recur, additional long-term research is needed.

Practical use of HRV as barometer of climacteric symptom (갱년기 증상의 척도로서의 심박 변이도의 활용에 관한 연구)

  • Purpose : To investigate relation between Heart rate variability(HRV) and Kupperman's index. Methods : From 2004, 3, 15 to 2004, 4, 10, a total 18 female patients(age 40-61) was recruited. but 2 patients was excluded. one was under hormone raplacement therapy, the other was under drug treatment. We measured patient's HRV by SA-2000E(Medicore Co., Ltd., Korea), and they fulfilled kupperman's index and qustionnaire. Results : The mean age of patient was 50.13${\pm}$5.78 years. 6 of 16 patients was menopause, 12 of 16 patients had menstruation. half of 16 patients had a understanding about climacteric syndrome. Investigating relation between HRV and kupperman's index, kupperman's index had positive correlation with heart rate significantly. Kupperman's index had negative correlation with SDNN significantly. Kupperman's index had negative correlation with RMS-SD significantly. Conclusion : In this study, we presented relation between HRV and kupperman's index. Furthermore, large scale study is required.

A Study about Correlation between Hot Flush and Pulse Wave Velocity(PWV)/Ankle-Brachial Index(ABI) in the Climacteric Women (갱년기 여성의 안면홍조와 맥파 전달 속도(PWV) 및 상완 발목 지수(ABI)의 상관성에 관한 연구)

  • Purpose: This study was performed to observe the correlation between Hot flush and pulse wave velocity(PWV), ankle brachial index(ABI) in the Climacteric women. Methods: We studied 63 climacteric women visiting OO hospital from 8th January 2007 to 22nd April 2009. The subjects were categorized in two groups, the hot flush group(37) and normal group(26). For the study, the subjects were selected by excluding the patients who receive treatment affect hot flush or have disease affect arterial states. We assessed PWV and ABI of two groups by Oscillometric method using VS-1000(Form PWV/ABI; Fukuda Denshi, Co., Ltd., Japan). From these results, we studied the correlation between hot flush and PWV/ABI by Wilcoxon Mann Whitney Test. Results: 1. It is shown that hot flush group was no significant correlation with normal group. There is no significant factor among R-PWV, L-PWV, R-ABI, L-ABI. However, L-ABI have shown relatively higher correlation with comparison to the other factors.(p-value<0.05) 2. R-PWV, L-PWV significantly increased in menopause group than in premenopause group though there is no relation to the hot flush. (p-value<0.01) Conclusion: Though the results showed no correlations between Hot flush and PWV/ABI in the Climacteric women, L-ABI showed relatively higher correlation with hot flush than others. R-PWV, L-PWV showed significant correlation between premenopause and menopause.

Usage of Korean Herbal Medicine for Climacteric Symptoms - Analysis of Korean Clinical Studies (갱년기 증상에 대한 한약 사용 - 국내 한의 임상연구 분석)

  • Objective : To investigate the usage of Korean herbal medicine (KHM) for climacteric symptoms, this study reviewed clinical studies conducted in Korea. Method : Literature search was performed on three Korean database; OASIS, RISS and NDSL. Studies published before July 2023 were categorized and analyzed according to the study type and herbal prescriptions. Results : Fifty-eight studies including 7 controlled trial, 27 case series and 24 single case reports were included in our review and the total number of case patients was 420. 52 prescriptions based on syndrome identifications and 11 prescriptions of Sasang constitutions were used and the most frequently used prescriptions were Soyosan(逍遙散) and Guibitang(歸脾湯). Thirty-two prescriptions were also mentioned in previous delphi research or clinical practice guideline, and the rest were not. Thirty-five studies used acupuncture combined with KHM and 10 studies used pharmacopuncture and 5 studies used herbal hipbath. Conclusion : To increase the therapeutic effect of KHM and establish KHM as an alternative to hormone treatment, various prescriptions should be used according to syndrome identifications in the clinical field. High-quality RCTs on frequently used prescriptions and the development of various type of KHM are also necessary.

Clinical Usefulness of Korean Red Ginseng in Postmenopausal Women with Severe Climacteric Disturbance

  • The aim of this study is to evaluate clinical usefulness of Korean red ginseng (RG) on various postmenopausal syndromes. Total plasminogen inhibitor-l (tPAI-l) in peripheral blood from 9 postmenopausal women with climacteric syndromes (CS) was measured before and 3 months after treatment with daily oral administration of 6 g RG and that from 8 postmenopausal women without any CS was also measured as healthy controls. Blood samples were collected in the early morning on the bed-rest. Psychological conditions of postmenopausal women with CS were measured before and 3 months after treatment with RG using simplified menopausal index (SMI). In addition, OKETSU (blood stagnation) syndrome scores and KI deficiency (generalized energy stagnation) scores proposed by Terasawa et al., were recorded before and 3 months after treatment with RG in postmenopausal women with CS and in healthy postmenopausal women. OKETSU syndrome scores and tPAI-l levels in postmenopausal patients with CS were significantly (P<0.001 and P<0.01) higher than those in healthy postmenopausal women without CS. Similarly, SMI scores and KI deficiency scores in postmenopausal patients with CS were about three-fold higher than those without any CS. When RG was administered for 3 months, KI deficiency scores and OKETSU scores as well as SMI scores declined around the levels of healthy postmenopausal women. Although tPAI-1 levels significantly (P<0.05) decreased after treatment with RG, those did not reach the levels of healthy postmenopausal women. Clinical usefulness of administration of RG to postmenopausal women with CS was confirmed from evaluation not only by modem medicine but also by traditional KAMPO medicine.
