Nutritional Counseling for Obese Children with Obesity-Related Metabolic Abnormalities in Korea

  • Child obesity has become a significant health issue in Korea. Prevalence of obesity in school-age children in Korea has been alarmingly rising since 2008. Prevalence of obesity among infants and preschool-age children in Korea has doubled since 2008. Obese children may develop serious health complications. Before nutritional counseling is pursued, several points should be initially considered. The points are modifiable risk factors, assessment for child obesity, and principles of treatment. Motivational interviewing and a multidisciplinary team approach are key principles to consider in managing child obesity effectively in the short-term as well as long-term. Nutritional counseling begins with maintaining a daily log of food and drink intake, which could possibly be causing obesity in a child. Several effective tools for nutritional counseling in practice are the Traffic Light Diet plan, MyPlate, Food Balance Wheel, and 'Food Exchange Table'. Detailed nutritional counseling supported by a qualified dietitian is an art of medicine enabling insulin therapy and hypoglycemic agents to effectively manage diabetes mellitus in obese children.

Effects of Direct Practice of Newborn Health Assessment on Students' Nursing Clinical Competence and Self-Efficacy (신생아 건강사정 직접실습이 간호학생의 간호수행능력과 자기효능감에 미치는 효과)

  • Purpose: This study was done to examine the effect of direct practice of newborn health assessment on nursing student's clinical competence and self-efficacy and to propose effective strategies for clinical education on newborn care. Methods: Design was a nonequivalent control group quasi-experimental study. The direct practice program was composed of a lecture, demonstration, drill and feedback using a manikin, and repeated direct practice regarding newborn health assessment. Participants were 65 student nurses taking the pediatric nursing practicum in the nursery room at M hospital. The experimental group (n=33) participated in the direct practice program for newborn health assessment and the control group (n=32) received the traditional practice method. Nursing clinical competence was assessed by two nurse investigators and structured questionnaires were used to measure self-efficacy. Results: The experimental group's clinical competence was significantly higher than that of the control group (t=-4.82, p=.000). However no significant difference was found between the two groups for self-efficacy (t=1.264, p=.211). Conclusion: These findings indicate that the direct practice program is effective in improving nursing student's clinical competence, but it was not effective in increasing self-efficacy. Direct practice in various clinical education settings is recommended and longitudinal effects be evaluated.

Meta-analysis of the Diagnostic Test Accuracy of Pediatric Inpatient Fall Risk Assessment Scales

  • Purpose: This study was conducted to obtain data for the development of an effective fall risk assessment tool for pediatric inpatients through a systematic review and meta-analysis of the diagnostic test accuracy of existing scales. Methods: A literature search using Medline, Science Direct, CINAHL, EMBASE, and the Cochrane Library was performed between March 1 and 31, 2018. Of 890 identified papers, 10 were selected for review. Nine were used in the meta-analysis. Stata version 14.0 was used to create forest plots of sensitivity and specificity. A summary receiver operating characteristic curve was used to compare all diagnostic test accuracies. Results: Four studies used the Humpty Dumpty Falls Scale. The most common items included the patient's diagnoses, use of sedative medications, and mobility. The pooled sensitivity and specificity of the nine studies were .79 and .36, respectively. Conclusion: Considering the low specificity of the pediatric fall risk assessment scales currently available, there is a need to subdivide scoring categories and to minimize items that are evaluated using nurses' subjective judgment alone. Fall risk assessment scales should be incorporated into the electronic medical record system and an automated scoring system should be developed.

  • This study was Performed to understand Pediatric Pain management status and nurses' knowledge and attitudes toward it. In addition. it aimed to provide basic data in order to establish effective nursing intervention strategies by confirming the barriers of effective pain management in practice. The subjects were 195 nurses working in pediatric units(general pediatric unit. oncology unit, neonatal unit. neonatal ICU, Pediatric ICU) of 8 university hospitals and one general hospital. Data was collected by the questionnaire from the 3rd of August to the 20th of September in 1999. The instrument developed by Sanna(1999) to measure nurses' knowledge and their attitudes and the other tool by Cleeland(1984) to evaluate barriers in effective Pain management was used. Results of this study are summarized as follows 1. Most nurses learn about the pain management knowledges from through regular curriculum of nursing school(62.0%) And almost nurses(90.8%) don't used to utilize pain assessment tool, but the 'Faces Rating Scale' is the most frequently used by nurses. 2. The use of pain medication(65.6%) is most frequently taken by nurses as pain management and is followed by massage (55.9%) , distraction(27.7%). 3. Nurses' knowledge level is moderate (Mn=3.07). Nurses don't seem to understand pediatric Physical development (Mn = 2.86), psychological development(Mn=2.94) well, meanwhile they seem relatively Quite knowledgeable about the way pain emerges. 4. Nurses' attitudes toward pain is based on behavioral and physiological responses to Pain. They believe 'changes in behavior are a way of assessing pain in child' and 'acute pain increases the number of respiration' Nurses are ready to accept pediatric pain, but are not positive in adopting pain intervention in practice. 5. The barriers of effective pain management are inadequate assessment of pain and pain relief(81.5%), ineffective incorporation among health professionals(80%), and lack of equipment or skills(80.0%).

  • Purpose: The purpose of this study was to determine the needs for children's health promotion education programs as perceived by child care center teachers and mothers. Methods: This study conducted a survey of 88 child care center teachers and 70 mothers of preschool children in Seoul from January 4 to February 5, 2016. This study aimed to characterize the current conditions of health promotion education for preschool children and the needs for health promotion education as perceived by child care center teachers and mothers of preschool children. Results: Areas of high need for health promotion education included lifestyle improvements for preventing diseases in children and awareness of the importance of health in educational objectives, standardized educational manuals, health educators as educators, child care centers as educational places, local health centers as educational support organizations, regular class hours as educational time, role play for training, and actual models in the educational medium. The educational subjects for which a high need was reported included safety and accident prevention, the role of smartphones and TV watching in mental health, and personal hygiene and disease prevention. Conclusion: These findings suggest that it is necessary to develop a health promotion education program for preschool children.

The Study on the Assistance Program for Evaluative Accreditation in terms of Child Care Teacher's and Principal's Position, Certificate Level, Expertise Level (보육시설종사자의 직위와 전문성 발달수준 및 보육교사의 자격에 따른 보육시설 조력활동에 대한 평가의 차이)

  • The purpose of this study is to investigate the principals' and teachers' assessment about the assistance program for the evaluative accreditation of child care centers. Subjects were 138 teachers and principal who work in private(family) child care center in D and G city. An instrument to survey of this study was 'Assistance Program Evaluation Scale for the Evaluative Accreditation of Child Care Centers'(Nam, 2010) and 'Self-Assessment of Developmental Levels for Kindergarten Teacher'(Baek & Jo, 2004). Data were analyzed by paired t-test and ANOVA With SPSS 18.0 Version. The results of this study were as follows: First, there were significant differences in the both 'assistance system category' and 'assistance content category' between child care teacher group and principal group. Second, there were significant differences in the 'assistance content category' between the high group and the low group in expertise level. Third, significant differences were found to exist between a second-grade certificate level of child care teacher and a first-grade certificate level of child care teacher in terms of 'assistance domain'.

The Relationship Between Trajectories of Child Maltreatment and Adolescent Suicidal Ideation (아동의 종단적 학대 경험과 청소년기 자살생각 간 관계)

  • Objective: The purpose of this study was to identify the longitudinal relationship between child maltreatment and adolescent suicidal ideation. Several studies suggest that child maltreatment is associated with adolescent suicidal ideation. However, these studies have not focused on the duration of child maltreatment. Methods: Participants were 420 adolescents (52.6% male; 47.4% female) involved in the Korea Welfare Panel Study. Data were collected via youth self-report in 2006, 2009, and 2012. A self-report assessment of children and youths' experiences of physical, emotional, or neglect had been surveyed longitudinally. Questionnaires on adolescent suicidal ideation were answered in 2012. A growth mixture model and multiple regression were conducted using Mplus 5.21. Results: The study results showed that 93.5% of the children had experienced no maltreatment; 6.5% had experienced increasing maltreatment longitudinally. The trajectory of increasing maltreatment was positively associated with adolescent suicidal ideation. Conclusion: These results show that children suffering from chronic maltreatment are subject to a high risk of suicide and therefore require continuous supervision and management.

EQUIVALENT DOSE, EFFECTIVE DOSE AND RISK ASSESSMENT FROM PANORAMIC RADIOGRAPHY TO THE CRITICAL ORGANS OF HEAD AND NECK REGION (파노라마 촬영시의 두경부 주요기관의 등가선량, 유효선량 및 위험도)

  • The purpose of this study was to evaluate the equivalent and effective dose, and estimate radiation risk to the critical organs of head and neck region from the use of adult and child mode in panoramic radiography. The results were as follows. 1. The salivary glands showed the highest equivalent and effective dose in adult and child mode. the equivalent and effective dose in adult mode were 837μSv and 20.93μSv, those in child mode were 462μSv and 11.54μSv, respectively. 2. Total effective doses to the critical head and neck organs were estimated 34.21μSv in adult mode, 20.14μSv in child mode. From these datas, the probabilities of stochastic effect from adult and child mode were 2.50x10/sup -6/ and 1.47x10/sup -6/. 3. The other remainder showed the greatest risk of fatal cancer. The risk estimate were 4.5 and 2.7 fatal malignancies in adult and child mode from million panoramic examinations. The bone marrow and thyroid gland showed about 0.1 fatal cancer in adult and child mode from these examinations.

Need Assessment for Central Food Production in Child Care Center Foodservices (보육시설 급식소의 공동조리 요구도 조사)

  • Nutrition during childhood is essential for growth and maintenance of health. Good food habits developed during the childhood will contribute both to the healthy growth and the prevention of the degenerative disease of later life. Both parents and the providers in child care centers play an important role for children's good eating behavior. Therefore all child care programs should achieve recommended standards for meeting children's nutritional and educational needs in a safe, sanitary, and supportive environment to promote the healthy growth and development of children. The purposes of this study were to evaluate the foodservice management practices and assess the needs for a Central Production Unit by contacting the child care center' providers. This approach was achieved using a variety of qualitative and quantitative information including the general foodservice management practices and the needs for a Central Production Unit. An indepth face-to-face interview with structured-questionnaires was undertaken at 32 representative child-care centers in Seoul. Statistical data analysis was done using the SAS program for descriptive analysis and ANOVA. The number of national/public and private sectors were 11 respectively, followed by 10 licensed home day-care centers. Total average number of children in child-care centers was 54.3 $\pm$48.5. The foodservice productivity index in child-care centers was 4.8 minutes per meal for public child care centers, 6.0 for private child-care centers, and 9.8 for home child care centers. Home child care centers were found to have the lowest productivity index which indicated inefficient foodservice practice. The important factors in group purchasing were menus(39.6%) or close distance(39.6%) > type of foodservice operation(32.8%) > total number of meals(19.9%) > food costs(16.2%) in order. Average score of the efficiency for central food production in child-care centers was 3.80 $\pm$0.84 out of 5.

Children's Self-Concept Typology and its Effect on Internet Item Purchase Behavior and Self-Evaluation (초등학생의 자아개념 유형별 인터넷 아이템 구매행동 및 자기평가)

  • The purpose of the study was to examine the internet purchase behavior of items and self-assessment according to self-concept of elementary school students. The data were collected from 716 elementary school students by a self-administered questionnaire. Frequencies and means, Cronbach's Alpha, factor analysis, t-test, Pearson's correlation analysis, cross-tabulation analysis, cluster Analysis were conducted by SPSSWIN 12.0. The results from this study were as follows; First, from self-concept measurements, 4 factors(affective, social, schooling, Family self-concept) were extracted through factor analysis. Second, the subjects were classified into 3 clusters as self-concept types(high self-concept, middle self-concept, low self-concept) through cluster analysis. Third, the significant variables affecting internet purchase behaviors of items included grade, allowance, rank in class, the number of hours on the internet. As the self-concept gets higher, the frequence of the impulsive purchase and imitation purchases gets lower. In the contrary, as the self-concept gets higher, the self-assessment on the impulse purchases and imitation purchases also gets higher. In combination, these results suggest that irrational purchase behaviors were protected by positive self-concept, therefore it is important that children have positive a self-concept.
