  • This study was performed to compare the dental arch dimensions of urban and rural high school females according to the region of residence and facial type. A model and cephalometric analysis was made from 48 urban and 48 rural high school females. The results of this study were obtained as follows : 1. Arch width in the urban group were similar to the rural group. 2. Lingual arch length at upper 1st premolar level in the urban group was larger than the rural group and lingual arch length at lower canine level in rural group was larger than the urban group. 3. Palatal height at canine level in the rural group was higher than the urban group. 4. Palatal heights at 2nd premolar and 1st molar level had correlation with the VERT index. In comparison of lingual arch dimensions according to Rickett's facial group, palatal heights at 2nd premolar and 1st molar level in dolichofacial group were lower than other groups.

Gonial Angle에 따른 두부 및 안면부 골격의 변화에 관한 연구 (THE VARIATION OF THE CRANIO FACIO-SKELETAL COMPLEX ACCORDING TO CHANGE OF THE GONIAL ANGLE)

  • In order to know the variation of the associated craniofacial skeletal angle and linear distance according to the change of gonial angle, the roentgenographic cephalo metric study was undertaken in Korean normal cephalometric analysis and eletric computer. The fallowing results were optained. 1) The size of gonial angle is mainly depend on the lower gonial angle. The mean of upper gonial gngle is almost same in normal occlusion group$^*$ and malocclusion group. 2) It was resulted on normal group and malocclusion group by F test that the number of parameters that were significant at 5% level of confidence were 14 parameters in normal occlusion group and 22 parameters in malocclusion group. 3) Ramus height and mandibular body length increased and facial ratio is decreased as gonial angle decreased. 4) MP-T, overbite depth and Y-axis angle is not related to gonial angle in normal occlusion group, but in moloclusion group, as gonial angle decreased, MP-T and overbite depth is increased and Y-axis angle is decreased. 5) SN-MP, OP-MP, PP-MP is increased as gonial anglel increased, it was mainly depend on the lower gonial angle.

  • The study was performed to establish the cephalometric standards of Hellman dental age III B, IV A, IV C groups of the age of puberty and to aid for the case analysis and diagnosis of malocclusion. A roentgenocephalometric study was made from 365 subjects, that consist of 162 males, 203 females with normal occlusion, acceptable profile and no history of orthodontic and prosthodontic treatment. The results of this study were obtained as follows: 1. The tables of standards from the measurements by age, sex group were made. 2. All linear measurements of skeletal pattern in male were greater than in females. 3. The Bjork's sum was reduced gradually by aging in group I $396^{\circ}$, group II $395^{\circ}$, and group III $393^{\circ}$. 4. Posterior facial height to anterior facial height was 63% in group I, 64% group II, and 67% in group III. 5. The angulation of SNA and SNB were $81^{\circ}$ & $78^{\circ}$ in group I, $81^{\circ}$ & $78^{\circ}$ in group II, and $82^{\circ}$ & $79^{\circ}$ in group III.

  • This study was performed to establish the cephalometric standards and to observe the nature of anatomic fit in the internal structural relationship of the craniofaciodental complex of the normal Korean by means of Moyers' method. Lateral cephalograms of 143 males and 144 females with normal occlusion and acceptable profile from 6 to 25 years of age, which were consisted of 5 groups that were 6 year-, 9 year-, 12 year-, 15 year- and adult-group were obtained. Data were gathered by traced digitizing the cephalograms and were statistically analyzed. The findings can be summerized as follows. 1. Norms of Korean males, females and both sexes in each group were established. 2. There was little significant sexual dimorphism in the form of craniofacial skeleton of all age groups. 3. The height and length of craniofacial skeleton was alike in each sexes in the 6 year-, 9 year- and 12 year-group, whereas it was larger in male than in female in the 15 year- and adult-group. 4. There were no significant sexual differences in the internal structural relationship of the craniofacial skeleton in all age groups.

하악 전방운동시 과두의 운동궤적에 관한 컴퓨터 분석 (A COMPUTER ANALYSIS ON THE CONDYLAR PATH IN MANDIBULAR PROTRUSIVE MOVEMENT)

  • The purpose of this study were to compare the condylar path and the anterior angle of glenoid fossae, and analyze the condylar path, and classify the patterns of condylar path. Thirty-three male and female dental students with normal occlusion and masticatory system ranging in age from 24 to 27, without present symptoms and any history of TM joint disturbance, were selected for this study. Then, sixty-six TM joint were examed. Transcranial radiographs of TM joint and Cephalometric radiographs under mandibular protrusive movement were obtained. They were taken at six positions on protrusive movement. The results obtained were as follows : 1. The sequence of the frequeny of condylar movement patterns were concave curve, "S"shape curve, convex curve, and reverse "S"shape. 2. The horizontal distance of the greatest changed position of condylar angle averaged 2.6mm. 3. The condylar path angle averaged 36.91 degree. 4. The angle of the anterior slope of glenoid fossae averaged 37.28 degree. 5. The distance fo condylar movement averaged 5.98mm and the distance of condylar horizontal movement averaged 4.71mm. 6. The slope of articular eminence was correlated to the condyalr path and three were no significant differences between right and left side of the anterior slope of articular eminence, and the condyalr path angle.

Comparison of the effects on the pharyngeal airway space of maxillary protraction appliances according to the methods of anchorage

  • Background: The purpose of the study is to compare the effects on the pharyngeal airway space of skeletal anchored face mask with those of tooth-borne facemask. Methods: We used two types of facemask for maxillary protraction, the tooth-borne facemask (TBFM) and the skeletal anchored facemask (SAFM), and evaluated the effects of each facemask on the pharyngeal airway. Twenty-eight patients (mean age 10.3 years) were treated with the TBFM and 24 patients (mean age 11.2 years) were treated with the SAFM. Lateral cephalometric radiographs were taken before treatment (T1) and after treatment (T2) to assess changes in the dimensions of the upper airway. Statistical analysis was performed with independent t tests, matched t tests, Mann-Whitney U tests, and Kruskal-Wallis tests. Results: There were marked increases in upper airway dimensions in both groups following treatment, but the SAFM group had a significantly greater increase in airway dimensions than the TBFM group. Also, the SAFM subgroups showed more improved airway measurements than the TBFM subgroups in both the superior and inferior pharyngeal airways. Conclusions: SAFM is more effective than TBFM in increasing upper airway dimensions.

횡문근육종의 항암제-방사선치료 후 치아발육장애 (FAILURE OF ODONTOGENESIS AFTER CHEMO-RADIATION THERAPY FOR RHABDOMYOSARCOMA)

  • This report details a case of 8-year-old girl showing failure of odontogenesis after chemo-radiation therapy for the rhabdomyosarcoma at the age of 4. The observed results were as follows : 1. Past history revealed that she had received for a total radiation dose of 4430cGy, 29 fractions in 6 weeks and chemotherapy with vincristine, actinomycin D and cytoxan, followed as maintenance phase for 2 years. 2. The patient was symptom -free and appointed for the treatment of multiple dental caries. 3. Oral examination showed hypoplastic enamel on whole erupted permanent teeth and showed retarded eruption. 4. Conventional radiograms showed failure of root development including abrupt cessation of root formation and root agenesis, and microdontia, missing teeth, irregular enamel, dislocation of the impacted teeth. Additional finding showed good healing bone pattern on the left mandibular ramus and angle area. 5. Cephalometric analysis revealed failure of bite raising due to incomplete eruption of all the first molars and made it possible to suspect entrapped mandibular growth and then Class II tendency growth. 6. There was correlation between the time of chemo-radiation therapy and the damage of the teeth.

Accuracy of virtual 3-dimensional cephalometric images constructed with 2-dimensional cephalograms using the biplanar radiography principle

  • Purpose: The purpose of this study was to evaluate the accuracy of virtual 3-dimensional (3D) cephalograms constructed using the principle of biplanar radiography by comparing them with cone-beam computed tomography (CBCT) images. Materials and Methods: Thirty orthodontic patients were enrolled in this study. Frontal and lateral cephalograms were obtained with the use of a head posture aligner and reconstructed into 3D cephalograms using biplanar radiography software. Thirty-four measurements representing the height, width, depth, and oblique distance were computed in 3 dimensions, and compared with the measurements from the 3D images obtained by CBCT, using the paired t-test and Bland-Altman analysis. Results: Comparison of height, width, depth, and oblique measurements showed no statistically significant differences between the measurements obtained from 3D cephalograms and those from CBCT images (P>0.05). Bland-Altman plots also showed high agreement between the 3D cephalograms and CBCT images. Conclusion: Accurate 3D cephalograms can be constructed using the principle of biplanar radiography if frontal and lateral cephalograms can be obtained with a head posture aligner. Three-dimensional cephalograms generated using biplanar radiography can replace CBCT images taken for diagnostic purposes.

Changes in longitudinal craniofacial growth in subjects with normal occlusions using the Ricketts analysis

  • Objectives: This study was designed to define the Korean norm values for the Ricketts analysis. Methods: In this longitudinal study, lateral cephalograms of 31 subjects with normal occlusion were taken biennially from ages 9-19 years. Cephalometric measurements were performed. Parameters for which the 10-year change did not exceed one standard deviation were defined as unchanged. The means and standard deviations for the measured parameters were determined for each age group. Results: No significant changes in growth were observed in the molar relationship, incisor overjet, incisor overbite, mandibular incisor extrusion, interincisor angle, lower incisor tip (B1) to A point-Pogonion (A-PO) plane, upper incisor tip (A1) to A-PO plane, B1 inclination to A-PO, A1 inclination to A-PO, B1 inclination to Frankfurt plane (FH), convexity, lower facial height, facial axis, maxillary depth, maxillary height, palatal plane to FH, cranial deflection, ramus Xi position, or porion location. Continual changes over the 10 years of growth were observed in the maxillary first molar distal position to pterygoid true vertical plane, facial depth, mandibular plane to FH, anterior cranial length, mandibular arc, and corpus length. Conclusions: Clinicians can apply the Korean norms at age 9 as determined in this study when using the Ricketts analysis. The patient's age at the beginning of treatment and their sex should be taken into consideration when drawing visual treatment objectives.

Multiscale Clustering and Profile Visualization of Malocclusion in Korean Orthodontic Patients : Cluster Analysis of Malocclusion

  • Understanding the classification of malocclusion is a crucial issue in Orthodontics. It can also help us to diagnose, treat, and understand malocclusion to establish a standard for definite class of patients. Principal component analysis (PCA) and k-means algorithms have been emerging as data analytic methods for cephalometric measurements, due to their intuitive concepts and application potentials. This study analyzed the macro- and meso-scale classification structure and feature basis vectors of 1020 (415 male, 605 female; mean age, 25 years) orthodontic patients using statistical preprocessing, PCA, random matrix theory (RMT) and k-means algorithms. RMT results show that 7 principal components (PCs) are significant standard in the extraction of features. Using k-means algorithms, 3 and 6 clusters were identified and the axes of PC1~3 were determined to be significant for patient classification. Macro-scale classification denotes skeletal Class I, II, III and PC1 means anteroposterior discrepancy of the maxilla and mandible and mandibular position. PC2 and PC3 means vertical pattern and maxillary position respectively; they played significant roles in the meso-scale classification. In conclusion, the typical patient profile (TPP) of each class showed that the data-based classification corresponds with the clinical classification of orthodontic patients. This data-based study can provide insight into the development of new diagnostic classifications.
