Facilitators and Barriers in the Use of a Checklist by Insurance Physicians during Work Ability Assessments in Depressive Disorder

  • Objectives: Depressive disorder (DD) is a complex disease, and the assessment of work ability in patients with DD is also complicated. The checklist depression (CDp) has recently been developed to support such work ability assessments and has been recommended for implementation in insurance medicine, starting with an analysis of the organisational and social contexts. The aim of this study was to identify the potential facilitators and barriers in the use of the CDp by insurance physicians (IPs) during work ability assessments of employees on sick leave due to DD. Methods: A qualitative research was conducted based on semi-structured interviews. The participants were IPs with at least one year of work experience in performing work ability assessments. The interviews were audiotaped, transcribed and analysed qualitatively. Results: Ten IPs (7 males, 3 females; mean 53 years) were interviewed. Important facilitators, which emerged for use of the CDp, were an oral introduction for colleagues and staff, support from management, valuing the increased transparency in work ability assessments with using the CDp, having adequate time for assessments as well as modification of the appearance (colour, plasticised form) and content (clarifying aspects of the examples) of the assessment tool. The fear of the loss of autonomy, lack of added value of the CDp, high workload, inadequate instructions and lack of time were mentioned as barriers. Conclusion: Adequate introduction to the use of CDp and the fear of the loss of autonomy of IPs need special attention in planning its implementation.

Loss of Primary Cilia Results in the Development of Cancer in the Murine Thyroid Gland

  • Communications at the interface between the apical membrane of follicular cells and the follicular lumen are critical for the homeostasis of thyroid gland. Primary cilia at the apical membrane of thyroid follicular cells may sense follicular luminal environment and regulate follicular homeostasis, although their role in vivo remains to be determined. Here, mice devoid of primary cilia were generated by thyroid follicular epithelial cell-specific deletion of the gene encoding intraflagellar transport protein 88 (Ift88). Thyroid follicular cellspecific Ift88-deficient mice showed normal folliculogenesis and hormonogenesis; however, those older than 7 weeks showed irregularly dilated and destroyed follicles in the thyroid gland. With increasing age, follicular cells with malignant properties showing the characteristic nuclear features of human thyroid carcinomas formed papillary and solid proliferative nodules from degenerated thyroid follicles. Furthermore, malignant tumor cells manifested as tumor emboli in thyroid vessels. These findings suggest that loss-of-function of Ift88/primary cilia results in malignant transformation from degenerated thyroid follicles.

Effects of Aging and Aging Method on Physicochemical and Sensory Traits of Different Beef Cuts

  • Wet and dry aging methods were applied to improve the quality of three different beef cuts (butt, rump, and sirloin) from Hanwoo cows (quality grade 2, approximately 50-mon-old). After 28 d of wet aging (vacuum packaged; temperature, $2{\pm}1^{\circ}C$) and dry aging (air velocity, 2-7 m/s; temperature, $1{\pm}1^{\circ}C$; humidity, $85{\pm}10%$), proximate composition, cooking loss, water holding capacity, shear force, color, nucleotides content, and sensory properties were compared with a non-aged control (2 d postmortem). Both wet and dry aging significantly increased the water holding capacity of the butt cuts. Dry aging in all beef cuts induced lower cooking loss than that in wet-aged cuts. Shear force of all beef cuts was decreased after both wet and dry aging and CIE $L^*$, $a^*$, and $b^*$ color values in butt and sirloin cuts were higher in both wet and dry aging (p<0.05) groups than those in the non-aged control. Regardless of the aging method used, inosine-5'-monophosphate content among beef cuts was the same. The sensory panel scored significantly higher values in tenderness, flavor, and overall acceptability for dry-aged beef regardless of the beef cuts tested compared to non- and wet-aged cuts. In addition, dry-aged beef resulted in similar overall acceptability among the different beef cuts, whereas that in wet-aged meat was significantly different by different beef cuts. In conclusion, both wet and dry aging improved the quality of different beef cuts; however, dry aging was more suitable for improving the quality of less preferred beef cuts.

A Methodology for Selection of Habitat Management Areas for Amphibians and Reptiles Considering Soil Loss (토양유실을 고려한 양서파충류의 서식지 관리지역 선정방법)

  • As disaster risk and climate change volatility increase, there are more efforts to adapt to disasters such as forest fires, floods, and landslides. Most of the research, however, is about influence of human activities on disaster and there is few research on disaster adaptation for species. Previous studies focusing on biodiversity in selecting conservation areas have not addressed threats of disaster in the habitats for species. The natural disasters sometimes play role of drivers of ecological successions in the long run, but they might cause serious problems for the conservation of vulnerable species which are endangered. The purpose of this study is to determine whether soil loss (SL) is effective in selecting habitat management areas for amphibians and reptiles. RUSLE model was used to calculate soil loss (SL) and the distribution of each species (SD) was computed with MaxEnt model to find out the biodiversity index. In order to select the habitat management area, we estimated the different results depending if value of soil loss was applied or not by using MARXAN, a conservation priority selection tool. With using MARXAN, conservation goals can be achieved according to the scenario objectives, and the study has been made to meet the minimum habitat area. Finally, the results are expressed in two; 1) the result of soil loss and biodiversity with MATRIX method and 2) the result of regional difference calculated with MARXAN conservation prioritization considering soil loss. The first result indicates that the area with high soil loss and low species diversity have lower conservation values and thus can be managed as natural disturbances. In the area where soil loss is high and species diversity is also high, it becomes where a disaster mitigation action should be taken for the species. According to the conservation priorities of the second result, higher effectiveness of conservation was obtained with fewer area when it considered SL in addition to SD, compared to when considered only biodiversity. When the SL was not taken into consideration, forest area with high distribution of species were important, but when SL considered, the agricultural area or downstream of the river were represented to be a major part of habitats. If more species data or disaster parameters other than soil loss are added as variables later, it could contribute as a reference material for decision-making to achieve various purposes.

A Case Report of an Ataxia Patient with Sensory Loss Diagnosed with Thalamic Infarction (감각장애를 동반한 Thalamic Ataxia 환자 치험 1례)

  • This report is about a case of an ataxia patient with sensory loss diagnosed with thalamic Infarction. The patient had symtoms of gait disturbances and dysmetria. We used Korean medicine treatment modalities including acupunture, electroacupuncture, moxibustion and herb medicines. The improvement of ataxia was evaluated by International Cooperative Ataxia Rating Scale and Balance master system. After treatment, decreased scores of International Cooperative Ataxia Rating Scale and change of the trace of Balance master system showed that symtoms of ataxia were improved. Sensory deficits and other conditions were also getting better. This report suggests that Korean medicine could have a therapeutic effect for Thalamic ataxia.

  • Automotive wheel bearing grease helps to reduce stresses and prevent wear of wheel bearings. But it is easily contaminated by water and other contaminants. In this study, we investigated the property change of automotive wheel bearing grease under water contamination. The result showed that some properties such as dropping point, work penetration and oxidation stability were not influenced by water content. However, most of properties such as work stability, water washout characteristics, leakage tendency, oil separation, evaporation loss and rust protection became worse after water was added. This is thought that added water makes the interaction weak between thickener and base oil of grease.

  • Highly chemo- and regioselective reaction of hydroxybenzenes with ${\alpha},{\beta}$-unsaturated compounds in acidic ionic liquid l-butyl-3-methylimidazolium hydrogen sulphate ([BMIM]HSO4) was reported for the first time. A series of oxa-Michael adducts and Friedel-Crafts alkylated products were synthesized with good yields. The acidic ionic liquid could be easily recycled for at least 5 times with only minor loss in activity.

  • We experienced a case of primary hepatic actinomycosis which was initially diagnosed by means of fine needle aspiration. The patient was a 31-year-old emaciated man with a 2-month history of 10 kg weight loss, right upper quadrant pain and flank pain. The liver was palpable and tender on physical examination. Computerized tomography scan of the liver showed two ill-defined hypodense masses in gallbladder fossa and inferior pole of right lobe. Hepatocellular carcinoma was clinically suspected. A CT-guided fine needle aspiration was peformed. Microscopically, smears showed numerous radiating clusters of filamentous bacteria with many neutrophils and monocytes in necrotic background. The symptoms were improved by incision and drainage and massive administration of penicillin.

  • Effect of reentrant type bowl geometry on combustion characteristics was investigated in a D.I. diesel engine. The main factor was the cupola height of bowl center and the reentrant angle of combustion chamber, and the cylinder pressure, engine performance and emissions of the engine using the total 11 kinds of the combustion chamber were measured by test. The results are as follows. The NOx decreases by increasing the cupola height of bowl center because it makes the decreasing of maximum combustion pressure by the heat loss and smooth combustion from good airflow. The smoke increases by increasing the reentrant angle at high speed range of the engine, but decrease at low and medium speed range until the reentrant angle becomes $15^{\circ}$.

Idiopathic Concurrent Gastric Cardiac and Pyloric Achalasia in a Dog (개에서 특발성 위 분문부 및 유문부 괄약근이완무력증 1례)

  • A 4-month-old, intact male, Tosa with a history of a regurgitation, vomiting, and weight loss for three weeks was presented to Animal Medical Center, Chonbuk National University. In Serial plain radiographs, a severely distended stomach was seen and ultrasonogram revealed a nonfunctional pylorus with normal layer comparable with an obstruction of pyloric region by pyloric achalasia. An esophagram and endoscopy revealed normal peristalsis with failure of the lower esophageal sphincter to open, supporting the diagnosis of esophageal achalasia. Megaesophagus was observed on reradiograph and esophagram 11 days later. The clinical signs and esophageal dilation were resolved without resorting to any treatment.
