LONGITUDINAL CLINICAL STUDY ON THE EFFICACY OF OSSEOINTEGRATED DENTAL IMPLANTS IN PARTIALLY EDENTULOUS KOREAN PATIENTS: AN 8-YEAR PROSPECTIVE STUDY

  • This study was to evaluate the clinical evidence for the success and the predictability of the osseointegrated dental implants in the partially edentulous Korean patients. 201 patients have received total of 502 Branemark implants, which were restored with either single or multiunit fixed superstructures at the Implants Clinic, Yonsei University Dental Hospital. The clinical and radiographic evaluations carried out on the patients for maximum 8 years were assessed annually for peri-implant inflammation, implant mobility (PTV), Plaque Index (PI), Gingival Index (GI), Bleeding on Probing (BOP), Keratinized Mucosa width and any changes in the surrounding bone level. The radiographs were taken at completion of the restoration, and annually thereafter. On the last recall appointments the patients filled a questionnaire consist of 29 questions in four categories. The cumulative non-failure rate of success was 93.9%. The first year mean bone loss was 0.3mm and less than 0.2mm annually thereafter. The periodontal parameters, keratinized tissue width and periotest values stabilized after initial changes in the first few years. The questionnaire has shown general satisfactory responses in all four aspect of dental implants treatment, including chewing efficacy, comfort, aesthetics and speech. The results support the predictability and success of the long-term rehabilitation of implant supported prostheses in partially edentulous Korean patients.

A Fatal Case of Naegleria fowleri Meningoencephalitis in Taiwan

  • After bathing at a hot spring resort, a 75-year-old man presented to the emergency department because of seizure-like attack with loss of conscious. This is the first case of primary amebic meningoencephalitis (PAM) caused by Naegleria fowleri in Taiwan. PAM was diagnosed based on detection of actively motile trophozoites in cerebrospinal fluid using a wet-mount smear and the Liu's stain. The amoebae were further confirmed by PCR and gene sequencing. In spite of administering amphotericin B treatment, the patient died 25 days later.

Gastric Xanthoma in the Pediatric Population: A Possible Herald for Malignancy?

  • Gastric xanthoma is frequently an incidental finding on upper endoscopy in adults. Gastric xanthomas (GX) can be mistaken for malignancies and warrant prompt histologic diagnosis. The underlying etiology is not fully understood; however, it has been linked to Helicobacter pylori gastritis and gastric cancer. GX in the pediatric population is largely unreported in the literature. Because of the relative rarity, documentation with case reports are essential to provide as much data as possible to see if there is a correlation between GX and malignant potential in the pediatric population. Our group is reporting two cases, a 10-year-old male and a 7-year-old male, both who presented with chronic dysphagia, upper abdominal pain, nausea, vomiting, and loss of appetite. Upper endoscopies for both patients revealed small polypoid lesions located in the antrum with foamy histiocytes on histology, leading to the diagnosis of gastric xanthoma.

Whole Liver Palliative Radiotherapy for Patients with Massive Liver Metastases

  • Background: The purpose of this retrospectively study was to examine the effectiveness and tolerability of a radiotherapy technique for the palliation of symptomatic liver metastases. Materials and Methods: Twentyseven patients with liver metastases were enrolled and received targeted whole liver irradiation consisting of mean 1, 8 Gy in five to twelve fractions to a total mean dose 17Gy. Symptoms at baseline were hepatic pain (26 patients), lost of weight (6), lack of appetite (2), and night sweats (1). Seventeen patients (63%) had failed previous treatment with chemotherapy and/or high-dose steroids. Results: Individual symptom response rates were 100% at 4 weeks. Partial or complete global symptomatic responses were noted in 11 patients (40%) after 2 months. After 3 months, 8 patients (28%) reported loss of pain. The treatment was well tolerated with one patient (3%) experiencing grade 3 toxicity (one vomiting and one diarrhoea). Overall the median survival time was 4.9 months (range 1 - 14 months). One year survival was 39%. Conclusions: This is simple and well-tolerated treatment but to achieve good palliation effects we should carefully selected patients whose conventional treatment does not bring good analgesic control.

  • An optical switch composed of identical, equally-spaced $Ti:LiNbO_3$ three-waveguides was designed and fabricated. Patterned Ti was diffused into z-cut $LiNbO_3$ substrates. $SiO_2$ buffer layer was evaporated to reduce the propagation loss of TM mode, and Al electrodes of CPW structure were built on the layer for switching of the guided beam. The optical switching phenomenon was confirmed when a beam of ${\lambda}=1.3{\mu}m$ was launched into the center waveguide and an electric field was applied to detune the three waveguides symmetrically.

  • This paper deals with the noise evaluation and noise reduction of a reciprocating air-compressor. The reciprocating air-compressor is widely used in the small and medium sized industrial firms, many employees exposed and irritated by their noise in the workplace. Thus, appropriate noise control actions should be taken to prevent hearing loss due to the its noise exposure. Lead-wrapping techniques are employed to identify the contribution of principal noise sources which are generally known to be motor, belts, suction valves, discharge valves, moving parts, and flow-induced noise caused by edges or discontinuities along the flow path including expansions, contractions, junctions and bends. As a result, the main noise sources of the air-compressor are categorized by the suction and discharge noise, valve noise, and compressed air tank noise. Based on the investigations, mufflers are designed to reduce both the suction and discharge noise and the compressed-air tank noise. Instead of the conventional valve plate, polyethylene resin is used the reduction of valve impact noise. In addition, attempts are made to reduce the valve noise propagation to the cylinder head and the compressor tank by using the insulation casings. As a result of the countermeasure plans, a noise reduction up to 10dB(A) could be achieved for the air-compressor.

Oct4 resetting by Aurkb–PP1 cell cycle axis determines the identity of mouse embryonic stem cells

  • In embryonic stem cells (ESCs), cell cycle regulation is deeply connected to pluripotency. Especially, core transcription factors (CTFs) which are essential to maintaining the pluripotency transcription programs should be reset during M/G1 transition. However, it remains unknown about how CTFs are governed during cell cycle progression. Here, we describe that the regulation of Oct4 by Aurora kinase b (Aurkb)/protein phosphatase 1 (PP1) axis during the cell cycle is important for resetting Oct4 to pluripotency and cell cycle related target genes in determining the identity of ESCs. Aurkb starts to phosphorylate Oct4(S229) at the onset of G2/M phase, inducing the dissociation of Oct4 from chromatin, whereas PP1 binds Oct4 and dephosphorylates Oct4(S229) during M/G1 transition, which resets Oct4-driven transcription for pluripotency and the cell cycle. Furthermore, Aurkb phosphormimetic and PP1 binding-deficient mutations in Oct4 disrupt the pluripotent cell cycle, lead to the loss of pluripotency in ESCs, and decrease the efficiency of somatic cell reprogramming. Based on our findings, we suggest that the cell cycle is directly linked to pluripotency programs in ESCs.

Role of cysteine at positions 67, 161 and 241 of a Bacillus sphaericus binary toxin BinB

  • Binary toxin consisting of BinA and BinB from Bacillus sphaericus is toxic to mosquito larvae. BinB is responsible for specific binding to the larval gut cell membrane while BinA is crucial for toxicity. To investigate functional role of cysteine in BinB, three cysteine residues at positions 67, 161, and 241 were replaced by alanine or serine. Mutations at these positions did not affect protein production and overall structure of BinB. These cysteine residues are not involved in disulfide bond formation between BinB molecules. Mosquito-larvicidal assays revealed that C67 and C161 are essential for toxicity, whereas C241 is not. Mutations at C67 and C161 resulted in weaker BinA-BinB interaction. The loss of toxicity may be due to the reduction of interactions between BinA and BinB or BinB and its receptor. C67 and C161 could also play a part during conformational changes or internalization of the binary toxin into the target cell.

Experimental study on non-linear throughflow characteristics of rockfill gabion weir (돌망태 보 통과류의 비선형적 흐름 특성에 관한 실험적연구)

  • As the flow velocity and Reynolds number increase in rockfill porous media, the flow deviates from Darcy conditions. In this study, the permeability tests of rock column specimen and laboratory gabion weir model were carried out to investigate a head loss behaviour of flow through rockfill deposition in small river artificial recharge. Through column test, the nonlinear relationships between flow velocity and hydraulic gradient and coefficients were determined and the correlation formula of hydraulic mean radius and coefficients was proposed. The flow velocities and discharges in voids estimated by proposed equations were well matched with the measured values of laboratory gabion weir model.

Study on a design of Band Pass Filter C-band using silicon substrate (실리콘 기판을 이용한 Ku-band용 Band Pass Filter 설계에 관한 연구)

  • In this paper, we designed a Ku-band BPF(Band Pass Filter) by microstrip line that most usually used a microwave device design and fabrication. Here a substrate of designed BPF were silicon substrate(${\varepsilon}_r=11.8$), and metal line was copper and silver/copper structure. And a configration of BPF was used hairpin pattern. A center frequency of designed BPF was 10GHz and their FBW(Fractional Band Width) was 20%(2GHz). It presented simulated results obtained for a 10GHz filter which yields an insertion loss of 0.1dB that ripple value related chebyshev reponse. Finallt we tried to make that a 30dB attenuation frequency was 20% of center frequency.
