Congenital Bronchoesophageal Fistula in Adult -Report of Three Cases- (성인의 선천성 식도기관지루 3례 보고)

  • We have experienced three cases of congenital bronchoesophageal fistula which is rare and usually has an insidious clinical course. The patients included a thirty year old man and thirty six, thirty eight year old women respectively. Bronchiectatsis was found in all three cases, and bronchoesophageal fistula was found in one case preoperatively by esophagography and esophagoscopy, and other two cases operative field. The fistula was found between right lower esophagus and right lower lobe in all cases and esophageal diverticulum in one case. So they belonged to type I[1 case , II[2 cases of Braimbridge and Keith`s classification of congenital bronchoesophageal fistula . The fistulectomy was performed in all cases and concomitant lobectomy [2 cases and bilobectomy [1 case were done. There were toxic hepatitis in two cases and prolonged air leakage in one case postoperatively. They were discharged on recovered state and have continued to do well.

  • Fourteen cases of diaphragmatic hernias surgically treated at the Dept. of Thoracic & Cardiovascular Surgery in Kosin Medical Center from Jan. 1979 to Feb. 1991, were reviewed in this study. This report includ 9 cases of traumatic diaphragmatic hernia, 5 cases of congenital diaphragmatic hernia. Among the traumatic hernias, 5 cases were blunt trauma and 4 cases were penetrating trauma. Five cases of congenital diaphragmatic hernias were presented, including 2 cases of esophageal hiatal hernia, 2 cases of Bochdalek's hernia and a case of Morgagni's hernia. Operation was performed in all patients. Results of all treated cases were excellent.

  • We have experienced 5 cases of Pectus excavatum corrected by Ravitch method without use of K-wire or metal bar from Jan. 1985 to Apr. 1986. All cases were male. The symmetric depression was in 4 cases, asymmetric one was in one case. The chief complaints were dyspnea on exertion, frequent U.R.I. and inferiority. The flail chest was developed in all cases, but it was insignificant in all cases except one who needed assist ventilation. Mediastinitis was developed in one case one week postoperatively but well managed with drainage. A distance from the posterior surface of sternum to the anterior surface of vertebral body was estimated preoperatively and postoperatively. Postoperative result was evaluated as the change of distance postoperatively. The change was 1.5cm minimally to 3.5cm maximally. Late results were "Excellent" in 3 cases and "Good" in 2 cases.uot; in 2 cases.

  • Congenital diaphragmatic hernia is an important cause of respiratory distress in the newborn. Eventration, with which these conditions are easily confused, may also Produce severe respiratory distress in infancy or be an asymptomatic radiographic finding. Harrington`s classification of diaphragmatic hernias into two categories, traumatic and nontraumatic, is most widely accepted. Nontraumatic hernias are [1] the congenital types, composed of the posterolateral [Bochdalek], those through the esophageal hiatus, the parasternal [/Morgagni], and those through a defect left by partial absense posteriorly, and [2] the acquired types, composed of those through the esophagea/hiatus [sliding and paraesophageal] and those the sites mentioned above under the congenital hernias. During the period from 1970 up to October 1978, 21 cases of diaphragmatic hernia were treated in department of cardiovascular and thoracic surgery. 11 cases of Bochdalek hernias, 1 case of Morgagni hernia, 5 cases of diaphragmatic eventration and 3 cases of hiatal hernia [2 cases of paraesophageal and 1 cases of sliding type], were experienced. 3 cases of 20 died of respiratory insufficiency, 2 cases of mortality were combined with left lung hypoplasia with Bochdalek hernia.

  • Eventration of the diaphragm is a rare anomaly, the cause of which still is not understand completely. Recently we were experienced two cases of diaphragmatic eventration which were successfully treated with surgical diaphragmatic plication at the department of thoracic and cardiovascular surgery, college of medicine, chosun university. Specific complication were not noticed after surgical repair of diaphragmatic eventration with good results.

  • There are more patients with tracheal stenosis lately due to the increased use of tracheostomy and assisted ventilation. Recently we have experienced two cases of post tracheostomy tracheal stenosis and successfully performed circumferential resection and end-to-end anastomosis of the trachea. Postoperative tracheostomy was not performed. The postoperative courses were uneventful and they are now conducting an usual life.

  • Cardiac myxoma is most significant benign tumors constituting nearly 50% of all primary cardiac tumors. Its diagnosis is important because prognosis is excellent by surgical therapy and is usually fatal if unrecognized and untreated. Recently the development of diagnostic procedure and cardiac surgery increased the accuracy of diagnosis and the opportunity of successful treatment. Two cases of left atrial myxoma removed successfully were presented.

  • Bronchogenic cysts,though known as a relatively common malformation of the mediastinum, were rarely discussed in Korea literatures. Since the surgical removal of asymptomatic lesions was adopted as the reasonable therapeutic principle,the incidence of bronchogenic cysts has been found higher than previously supposed. Four cases of bronchogenic cysts operated on in the department of chest surgery, SNUH, were reported and the related literatures were reviewed.
