The purpose of this study was to understand the meaning and nature of long term boarding care of children provided by grandparents in surrogate foster homes. Data were collected from 10 children 15 to 18 years of age by in depth interviews and observation from May 2007 to July 2007. Analysis of data was by the phenomenological analytic method of Giorgi(1970). Seven essential themes were extracted: (1) ambivalence about being labeled "family headed by a child," (2) recognition of the burden to grandparents with appreciation for their care, (3) resentment and yearning for parents, (4) economic hardship, (5) discord with grandparents, (6) maladjustment to school life, and (7) overcoming such obstacles as poor economic conditions and loneliness.
In this paper, we developed the care system for older people living alone using the RFID technologies and the living informations. The care system store living informations, extracted through a unconstrained detection method by the RFID tags and readers, into a monitering server. The unconstrained detection method improved a weakness of existing systems that detected a living informations through an infrared sensor, ultrasonic sensor, camera, consumed quantity of the tap water or gas. The result of this study will playa very important role, as a part of a composite older-welfare services. Also, in the future, accumulated living informations will be allowed for a health data of older peoples.
Background : This study is to identify the inappropriate hospital services for elderly inpatients over 65 years in general hospital with acute care functioning. Consequently elderly inpatient care and the management of long-term care facilities are key issues for current government health policy. Method : The survey was conducted for two months for all inpatients over 65 in 7 general hospitals, 6 work sampling days randomly selected. In each survey day, the subjective judgement by medical staff on the degree of acute care needs and by nursing unit manager on hospital services of each inpatients was also conducted. Result : The total number of cases collected are 2,541 elderly inpatients, according to subjective judgements by medical staff on inpatient condition. However 46.8% of cases are turned out to be non-acute care group. The frequency of medical services provided to non-acute group are 2~3 vital sign checks per day 78.2%, IV injection 40.1%, antibiotics medication 20.2%. Conclusion : Lots of elderly patients' who are staving in acute hospitals, at present need to be transferred to long-term care facilities. However, there was been shortage of long-term care facilities. It is expected to identify the need of elderly inpatients and therefore, to provide cost-effective, appropriate and good quality health services to elderly inpatients depending on their needs.
Purpose: The purpose of this research was to determine the effects of hospice and palliative care education on knowledge of hospice and palliative care and attitude toward death in nursing students. Method: Subjects of this study included 49 nursing students. Using a one-group pretest-posttest quasi-experimental research design, we conducted experiments to determine the effects of a hospice and palliative care education during a period of seven weeks from August through November 2009. The program was composed of seven sessions (total: 28hrs). The SPSS/Win 18.0 program was used for analysis of collected data. Results: A significant change in knowledge of hospice and palliative care and attitude toward death was observed after the intervention. Conclusion: The hospice and palliative care education was effective for nursing students in improving their attitude toward death and in increasing knowledge of hospice and palliative care. Therefore, I recommend generous application of this program to nursing students in order to improve knowledge of hospice and palliative care and attitude toward death.

This study was designed to obtain basic data for development of evaluation tool which would be needed to measure the outcome of general quality nursing care of individual patient. The purpose of this study was to analyze and classify the outcome indicators of quality nursing care. The 29 articles of quality nursing care and outcome measures were selected coveniently, and analyzed to classify the outcome indicators of quality nursing care using open coding method. The results of this study were as follows: 1. Quality nursing care was defined as level of excellence of nursing care to achieve good patient outcome. 2. The 6 domains of which were health status, satisfaction, self care, patient progress and prognosis, and compliance were identified in outcome indicators of quality nursing care 3. Seven indicators of health status domain which were perceived health status, quality of life, well-being, daily activities, physical-physiological status, psychoemotional status, and social role functioning were identified. 4. Two indicators of satifaction domain which were patient satisfaction and family satisfaction were identified. 5. Three indicators of self care domain which were skill, knowledge, and home management were identified. 6. Seven indicators of patient progress and prognosis domain which were change of clinical status, resolution of nursing diagnosis and problem, days of stay, dicahrge state, recovery state, survival were identified. 7. compliance with therapeutic direction compliance was identified as an indicator of compliance domain. 8. Purpose: The purpose of this study was to compare the costs and benefits of home nursing care services between public health centers (PHC) and private hospitals. Method: Participants were 105 patients who had received home nursing care services from a private hospital or public health center. From a societal perspective, the researcher identified the costs and benefits of the services using performance data and calculated the net benefit and benefit/cost ratio. Result: The net benefit of the home nursing care service based in the PHC was 165.9 million won and benefit/cost ratio was 2.0, while the net benefit of the home nursing care services by the private hospital was 141.1 million won and benefit/cost ratio was 1.7. Both types of programs were economically validated. Conclusion: Home nursing care services were basically efficient as the results showed a positive net benefit. A cost-benefit analysis indicated that the PHC-based home nursing care services were more efficient than that of the private hospital. With limited human resources and management standards in public health centers, results suggest the need for a more systematic management of the home nursing care service to improve the health of this vulnerable community population.
The purpose of this study was 1) analyze the current state of Sanhojoriwon; and 2) to suggest the new model for the community based mother infants health care delivery system: strategies of TMIC are related to Public Health policy, cost-effectiveness, mother infant care provision of medical professionalism, and so on. Method: Forty-seven workers from seventeen Sanhojoriwon participated to analyze several aspects of Sanhojoriwon. Using a questionnaire developed at Korean Sanhojori Research Forum (KSARF), such as the traditional and medical concept of the Sanhojori, postpartum care, Korean traditional postpartum care, job description on women and infant care at Sanhojoriwon, professional management, health care policy and the educational need. Results: Based on the descriptive study results, the TMIC, the community based transitional mother infants care center was suggested as a new model for the cyclic public health care system related on the reproductive health, using an already existing related center, Sanhojoriwon. Also, several strategies were presented on the TMIC.

Purpose: To explore nurses' compliance with standards for critical care nursing practice in intensive care units (ICUs) and to provide basic data for high quality of critical care nursing. Method: A total of 616 participants from 61 ICUs which are graded from 1 to 7 throughout the nation were surveyed. Data were collected from February 9 to February 27, 2009 using a questionnaire consisting of 58 questions including 50 nursing activities as indicators. Results: The rate of ICU nurses' compliance with standards for critical care nursing practice was high. As for individual standards, compliance with the standard of assessment was the highest, followed by implementation, diagnosis, and planning in that order. There were differences in compliance according to nurses' ICU experience, work place (unit), and ICU grade. A shortage of manpower was considered as a main cause for noncompliance. Conclusion: The present compliance rate with standards for critical care nursing practice by ICU nurses was identified. Therefore, compliance rate can be used to promote quality of critical care nursing and development of educational programs for ICU nurses.

Purpose : This study aimed to explore and describe good nursing experiences of patients with cancer. Methods : Data were collected using individual in-depth interviews with 15 males and 8 females who were hospitalized in a Korean cancer hospital. The transcribed script was analyzed using Colaizzi's phenomenological method. Results : As a result, three theme clusters and nine themes were identified. The three theme clusters were as follows: "being kind and expert in every step along the cancer care trajectory," "taking an omnipotent mediator role supporting patients' well-being," and "providing professional care touching patients' mind and body." Three themes were assigned to each cluster to illustrate the given phenomena. Conclusion : Cancer patients wanted considerate understanding and sincere care from nurses while they experienced physical and emotional suffering owing to the cancer, treatment trajectory, and hospital system. To improve the quality of nursing care for cancer patients, patient-centered care combined with nurses' expertise in oncology care must be provided based on the insights from our study's findings.

Purpose: The purpose of this study was to examine nurses' cognition of the diagnosis related group (DRG) in long-term care hospitals and changes in nursing care after application of the DRG system. Method: This study was a descriptive survey involving 161 nurses working in 12 long-term care hospitals located in Gwangju city and Chonnam area. Data were analyzed using the SPSS 18.0 version program. Data analyses utilized descriptive test, chi-square test, Fisher's exact probability test, t-test, and one-way ANOVA. Result: There was no change in cognition of DRG in 55.3% of the nurses, and 26.1% takes to 'change positively'. More than half of the respondents (57.8%) agreed to the application of DRG. After application of DRG, the nurses responded 'there were changes in nursing care' in 23 of the 25 care items. Two items had an increase in nursing care. Conclusions: No distinct changes in nursing care were evident after DRG application. Therefore, there is need to provide education programs related to DRG for nurse in long-term care hospitals.
