Possibility of the Use of Public Microarray Database for Identifying Significant Genes Associated with Oral Squamous Cell Carcinoma

  • There are lots of studies attempting to identify the expression changes in oral squamous cell carcinoma. Most studies include insufficient samples to apply statistical methods for detecting significant gene sets. This study combined two small microarray datasets from a public database and identified significant genes associated with the progress of oral squamous cell carcinoma. There were different expression scales between the two datasets, even though these datasets were generated under the same platforms - Affymetrix U133A gene chips. We discretized gene expressions of the two datasets by adjusting the differences between the datasets for detecting the more reliable information. From the combination of the two datasets, we detected 51 significant genes that were upregulated in oral squamous cell carcinoma. Most of them were published in previous studies as cancer-related genes. From these selected genes, significant genetic pathways associated with expression changes were identified. By combining several datasets from the public database, sufficient samples can be obtained for detecting reliable information. Most of the selected genes were known as cancer-related genes, including oral squamous cell carcinoma. Several unknown genes can be biologically evaluated in further studies.

Malignant Transformation Rate and P53, and P16 Expression in Teratomatous Skin of Ovarian Mature Cystic Teratoma

  • Objective: To investigate the incidence of malignant transformation and P53 and P16 expression in teratomatous skin of ovarian mature cystic teratoma. Materials and Methods: Data on ovarian teratoma specimens in nearly 10 years were reviewed. P53 and P16 expression were detected by immunohistochemistry in 25 cases of teratomatous skin of ovarian mature cystic teratoma, 20 cases of squamous cell carcinoma and 2 cases of squamous cell carcinoma originated from teratomatous skin. Results: Of 1913 cases of ovarian mature cystic teratoma in nearly 10 years, only two cases of squamous cell carcinoma were found in teratomatous skin, with malignant transformation rate of 0.1045%. P53 expression was detected in 2 cases squamous cell carcinoma originated from teratomatous skin and P16 overexpression in one. There were no expressions of P53 and P16 in 25 cases of teratomatous skin of ovarian mature cystic teratoma. Of 20 cases of squamous cell carcinoma P53 overexpression (positive rate of 55%) was detected in 11 cases, P16 overexpression (positive rate of 35%) in 7 cases. The positive rates of P53 and P16 expression in squamous cell carcinomas were significantly higher than that in the teratomatous skins (p< 0.001, p= 0.002). Conclusions: There was low risk of malignant transformation in teratomatous skin of ovarian mature cystic teratoma which can be explained by lower P53 and P16 expressionin teratomas than that in squamous cell carcinoma.

기저세포암, 편평세포암 및 피부내암의 최신지견 (Recent advances of basal cell carcinoma, squamous cell carcinoma, and carcinoma in situ)

  • Incidence of basal and squamous cell carcinoma is increasing among Korean. With increasing this incidence we need to attend the management and prevention of these cutaneous malignancies. Basal cell carcinoma (BCC) is the most common skin malignancy. Sun exposure is the most important environmental cause of BCC. Daily sun protection especially avoiding sun exposure between 10 a.m. and 4 p.m. provides most effective prevention against chronic ultraviolet-induced skin damage. Hedgehog pathway inhibitors are used locally advanced and metastatic BCC lesions in Korea. Until recently, there are no reports whether it developed resistance of Hedgehog pathway inhibitors with advanced BCC in Korean patients. Among malignant skin tumors, 19.1% were squamous cell carcinoma (SCC). The incidence of SCC was rapidly increased over the past years. However, the incidence rate of SCC in Japan has not been changed from the period 1976-1980 to 1986-1990. Sentinel lymph node biopsy for SCC did not provide diagnostic value. Bowen's disease and Erythroplasia of Queyrat are considered as carcinoma in situ of the skin. Erythroplasia of Queyrat have worse prognosis with a higher rate of malignant degeneration. Dermoscopy may helpful tool in assisting the noninvasive diagnosis of carcinoma in situ of the skin.

구강편평상피암종에서 DCC 유전자의 역할 (ROLE OF DCC(DELETED IN COLORECTAL CANCER) GENE IN ORAL SQUAMOUS CELL CARCINOMA)

  • Chromosome 18q alteration plays a key role in colorectal tumorigenesis, and loss of heterozygosity at 18q is associated with a poor prognosis in colon cancer. DCC(Deleted in Colorectal Cancer) is a putative tumor- suppressor gene at 18q21 that encodes a transmembrane protein with structural similarity to neural cell adhesion molecule that is involved in both epithelial and neuronal cell differentiation. DCC is implicated in regulation of cell growth, survival and proliferation. Thus, tumor progression in squamous cell carcinoma, stomach cancer, colorectal cancer correlates with downregulation of DCC expression. The mechanism for DCC suppression is associated with hypermethylation of the DCC gene promoter region. Hence, the goal of this study is to identify the promoter methylation responsible for the down-regulation of DCC expression in oral squamous cell carcinoma. 12 of tissue specimens for the study are excised and gathered from 12 patients who are diagnosed as SCC in department of OMS, dental hospital, dankook university. To find expression of DCC in each tissue samples, immunohistochemical staining, RT-PCR gene analysis and methylation specific PCR are processed. The results are as follows. 1. In the DCC gene RT-PCR analysis, 5(41.6%) of 12 specimens of oral squamous cell carcinoma did not expressed DCC gene. 2. In the promoter methylation specific PCR analysis, 5(41.6%) of 12 specimens showed promoter methylation of DCC gene. 3. In the immunohistochemical staining of poor differentiated and invasive oral squamous cell carcinoma, loss of DCC expression was observed. These findings suggest that methylation of the DCC gene may play a role in loss of gene expression in invasive oral squamous cell carcinoma.

새열낭종 기원의 편평세포암종 1예 (A Case of Branchiogenic Squamous Cell Carcinoma)

  • Branchiogenic carcinoma is extremely rare and is defined as a malignant degeneration within the confines of epithelial remnants derived from the embryonal branchial apparatus. Two major diagnostic criteria are histologic proof of transitional area from normal cyst epithelium to invasive squamous cell carcinoma and absence of an identifiable primary carcinoma elsewhere. A 62-year old woman visited our department complaining of a non-tender, movable mass in left upper lateral neck. After a complete mass excision, histopathologic diagnosis of the surgical specimen was branchiogenic squamous cell carcinoma. I report a case of branchiogenic carcinoma with literature review.

새열낭종에서 기원한 암종으로 오인된 전이성 편평세포암종 1예 (A Case of Metastatic Squamous Cell Carcinoma Misrecognized as Branchiogenic Squamous Cell Carcinoma)

  • The existence of primary branchiogenic carcinoma is controversial. In 1950, Martin et al. established four criteria for the diagnosis of primary branchiogenic carcinoma. In 1989, Khafif et al. proposed new modified criteria, which are currently most recognized in the literature. A 54-year-old woman presented the well-defined, fluctuant, painless mass on her left neck and underwent a complete excision under the clinical diagnosis of the branchial cleft cyst. The initial pathological impression was a branchiogenic squamous cell carcinoma. However, it did not coincide with a true primary branchiogenic carcinoma clinically. After the guided biopsy of suspicious areas found a squamous cell carcinoma of the tongue base, the patient was treated by combination chemotherapy with radiotherapy. Thus, we report this case with a review of the literature.

상악동에 발생한 기저양 편평상피세포암 (BASALOID SQUAMOUS CELL CARCINOMA IN THE MAXILLARY SINUS)

  • Basaloid squamous cell carcinoma(BSCC) is uncommon and distinct variant of squamous cell carcinoma that arises mostly in the upper aerodigestive tract and aggressive, high grade tumor with an increased tendency to be deeply invasive, multifocal, and metastatic even at the initial presentation. The typical microscopic features of carcinoma with a basaloid pattern in intimate association with a squamous component helps in diagnosis of this tumour. Since Wain's report in 1986, BSCC of oral cavity, the palate, floor of the mouth, nasopharynx, oropharynx and mastoid region have been reported. However, BSCC in the nasal cavity or in the paranasal sinuses is rare and there are few reports in the Korean literature. We had experienced a case of basaloid squamous cell carcinoma that occurred in the left maxillary sinus of 72-year-old woman and reported with review of the clinical and pathologic features from the literature.

  • 폐의 편평상피암종에서 기관지표면상피의 화생에 대해서는 많은 이들에 의해서 밝혀진 바 있지만 그 외의 다른 종류의 폐암과 연관된 기관지 표면상피의 변화에 대해서는 그 예가 많지 않고 소세포폐암종에 대해서는 더욱 그러하다. 한편 소세포암종의 조직학적 분류와그아형에 대해서 지난수십년 간많은 관심이 모아져 왔다. 이 종량의 아형을분류하는기본적인 근거는 그 형태학적 판정 기준에 있다. 1967년 WHO폐암종 분류법이 제긍된 이후 3여러 기관에서 SCLC의 임상경과나 치료에 대한 반응, 그 아형을 연구하였다. 1988년 IASLC에서는 소세포암종을 그 형태학적 구분으로 세가지 아형 : Small cell carcinoma (2) Mixed small cel1/1arge ceil carcinoma (3) Combined small cell carcinoma로 분류하고 예후와 치료에 대한 반응의 차이에 대해서 언급하였다. 최근 저자들은 68세 남자환자에서 소세포암종에서 국소적으로 편평양분화를 보이는 세포가 함께 혼합되어 있는 주종괴와 분리되 어서 존재하는 기관지 상피세포에 편평상피암종이 동반된 예를 전폐 절제 술을 하고 조직 검사상 경험하였기에 문헌고찰과 함께 보고하는 바이다.

구강편평상피암종에서 stromal cell-derived factor-1의 발현 (Stromal cell-derived factor-1 (SDF-1) expression in the oral squamous cell carcinoma)

  • Purpose: Chemokines are structurally related, small polypeptide signaling molecules that bind to and activate a family of transmembrane G protein-coupled receptors, the chemokine receptors. Recently, interaction between the chemokine receptor CXCR4 and its ligand, stromal cell-derived factor 1 (SDF-1 or CXCL12), has been found to play an important role in tumorigenicity, proliferation, metastasis and angiogenesis in many cancers such as lung cancer, breast cancer, melanoma, glioblastoma, pancreatic cancer and cholangiocarcinoma. Hence, the goal of this study is to identify the correlation of clinicopathological factors and the up-regulation of SDF-1 expression in oral squamous cell carcinoma. Material and methods: We studied the immunohistochemical staining of SDF-1, quantitative RT-PCR (qRT-PCR) of SDF-1 gene in 20 specimens of 20 patients with oral squamous cell carcinoma. Results: 1. In the immunohistochemical study of poor differentiated and invasive oral squamous cell carcinoma, the high level staining of SDF-1 was observed. And the correlation between immunohistochemical SDF-1 expression and tumor nodes metastases (TNM) classification of specimens was significant.($x^2$ test, P < 0.05) 2. In the SDF-1 gene qRT-PCR analysis, SDF-1 expression was more in tumor tissue than in carcinoma in situ tissue. Paired-samples analysis determined the difference of SDF-1 mRNA expression level between the cancer tissue and the carcinoma in situ tissue.(Student's t-test, P < 0.05) Conclusion: These findings suggest that up-regulation of the SDF-1 may play a role in progression and invasion of oral squamous cell carcinoma.

Extraordinarily aggressive cutaneous sarcomatoid squamous cell carcinoma of the face: a case report

  • Sarcomatoid squamous cell carcinoma (SSCC), a biphasic malignant tumor consisting of atypical squamous epithelial and mesenchymal elements mixed with epithelioid and spindle cells, is a variant of squamous cell carcinoma. Cutaneous SSCC is very rare and aggressive and has a poor prognosis. Here, we report a case of cutaneous SSCC with satellites and in-transit metastases. A 79-year-old woman presented with a protruding mass on the left temporal area sized 1.2×1.0 cm. The punch biopsy report indicated keratoacanthoma or well-differentiated squamous cell carcinoma. The size of the tumor increased to 2.7×2.0 cm after 8 days. An excisional biopsy was performed with a 2 mm safety margin. The tumor was identified as SSCC with a clear resection margin. Reoperation was performed thrice with an increased safety margin of 10 mm; however, the cancer recurred along with satellites and in-transit metastases. Chemoradiotherapy was administered; however, the size of the tumor increased along with satellites and in-transit metastases. The patient expired 162 days after the initial excision. Complete excision and immediate multidisciplinary approach should be combined during the early stages due to the aggressiveness and poor prognosis of cutaneous SSCC with satellites and in-transit metastasis.
