  • In order to evaluate the role of cytopathologic diagnosis of sputum, bronchial washing and bronchial blushing in the diagnosis of lung cancer, we performed this study. The patients included in this study had undergone sputum, bronchial washing and brushing cytology over the 20-month period of 1985 through 1987. The total number of specimens was 5,495 of 2,242 patients, including 4,830 sputa and 665 bronchial washing and brushings. The average number of sputa and bronchial washings and brushings per case was 2.4 and 1.2 respectively. Among them, about 10% were unsatisfactory specimen, and three-fourths were negative specimens. In sputum cytology, the diagnosis of "atypical cells" was given to 3%, "suspicious for malignancy" was given to 1 %, and "malignancy" was given to 13%. In bronchial washing and brushing cytology, the diagnosis of "atypical cells", "suspicious for malignancy" and malignancy" was given to 6%, 3%, and 20% respectively The cases diagnosed as "atypical cells" in cytology were actually malignancy in 95% and 84.8% of sputum and bronchial washing and brushings respectively, and the "suspicious for malignancy" were actually malignancy in 100% in both methods. The detection rates of malignancy were 504% and 55.2% in sputum and bronchial washing and blushing respectively, and the specificity was 100% in both methods. The accuracy of cell typing was 92% in sputum and 89.7% in bronchial washing and blushing.

  • Background: The rising prevalence of asthma may be associated with the rising prevalence of obesity in developed nations. There are several studies showing that obesity increases the risk of asthma in adults. We investigated the association of each body composition scale and bronchial hyper-responsiveness. Methods: This study involved a retrospective review of the existing records for 279 subjects with respiratory symptoms, who underwent a pulmonary function test, a methacholine challenge test and a body composition test between May 2007 and June 2009. Results: Of the 279 subjects, 179 (64%) were female. There was a statistically significant difference in fat free mass and in fat free mass index between the normal bronchial responsiveness group and bronchial hyper-responsiveness group (p=0.036; p=0.000). There was no significant differences in body mass index, in fat mass and fat free mass index in the normal bronchial responsiveness group and bronchial hyper-responsiveness group in males. However in females, body mass index and fat free mass index were increased in the bronchial hyper-responsiveness group (p=0.044; p=0.000). Total body water (kg), fat free mass (kg) and soft lean mass (kg) were significantly different between the normal bronchial responsiveness group and bronchial hyper-responsiveness group (p=0.002; p=0.000; p=0.000). Conclusion: This study showed significant differences in fat free mass and in fat free mass index between the normal bronchial responsiveness group and the bronchial hyper-responsiveness group. In females, BMI, soft lean mass, and total body water showed significant differences between the normal bronchial responsiveness group and the bronchial hyper-responsiveness group. We concluded that bronchial hyper-responsiveness was associated with not only body mass index but also fat free mass index in female bronchial asthma.

  • Lung cancer is a type of cancer with high mortality; its 5-year survival rate is at a low 14%. Related cytological tests include sputum, bronchial brushing, bronchial washing and fine needle aspiration cytology test etc. From the test specimens in which sputum, bronchial brushing, bronchial washing, and fine needle aspiration cytology were performed, the sensitivity, specificity and accuracy between cytology test and histology test. In the sputum test, sensitivity was 27.71% and specificity was 98.02%, and the bronchial brushing test showed sensitivity of 93.33% and a specificity of 91.3%. The bronchial washing test was a sensitivity of 53.7% and its specificity was 98.9%, and the fine needle aspiration cytology test showed sensitivity and specificity were 88.46% and 72.97%, respectively. In the specimens diagnosed as normal at the sputum test, malignant diagnosis was found in 21 specimens of bronchial brushing, 30 cases of bronchial washings and 37 cases of fine needle aspiration cytology specimens. In the specimens diagnosed as normal at the bronchial washing test, malignant diagnosis was found in 5 specimens of sputum, 7 specimens of bronchial brushin and 1 cases of fine needle aspiration cytology. One specimens found to be normal in fine needle aspiration cytology turned out to maligant in sputum test. The result of this research shows that, in diagnosis lung cancer, a test method of high sensitivity and specificity should be pursued. However, depending on the location and malignancy of the illness, diagnosis may not be obtained in some cases. Therefore, we conclude that the cytological tests performed for lung cancer testing such as sputum, bronchial brushing, bronchial washing, and fine needle aspiration cytology should be carried out in a mutually complementary manner.

  • To evaluate the effect of damaged bronchial epithelium on epithelial permeability and physiologic changes of the airway in patients with bronchial asthma, we measured the concentration of protein and albumin in bronchoalveolar lavage fluid (BALF), the width of intercellular junction and bronchial hyperreactivity in 22 patients with bronchial asthma and 21 healthy subjects. The results were as follows: 1) The concentration of protein in BALF from patients with bronchial asthma was higher when compared with that of normal subjects ($237{\pm}182$ vs $113{\pm}78\;{\mu}g/mL$, p<0.05), and the concentration of albumin was also higher than that of normal subjects ($116{\pm}126$ vs $32{\pm}88\;{\mu}g/mL$, p<0.05). The ratio of the concentration of protein in BALF to that in serum was increased in patients with bronchial asthma when compared with that of normal subjects ($0.35{\pm}0.30$ vs $0.16{\pm}0.11%$, p<0.05). 2) The intercellular junction of bronchial epithelium was widened in 14 of 20 patients with bronchial asthma, in contrast, 5 of 14 normal subjects (p<0.05). The mean width of intercellular junction was greater when compared with that of normal subjects ($1.71{\pm}1.81$ vs $0.56{\pm}0.85\;{\mu}m$, p<0.05). The width was well correlated with the ratio of protein in BALF to that of serum (r=0.3226, p=0.047) when observed in 18 patients with bronchial asthma and 10 patients with normal subjects. 3) The bronchial hyperreactivity, expressed as $PC_{20}$, was inversely correlated with the concentration of protein in BALF (r=-0.3030, p=0.038) in when observed in 18 patients with bronchial asthma and 19 normal subjects. 4) The width of intercellular junction was well inversely correlated with $PC_{20}$ (r=-0.5006, p=0.002) when observed in 19 patients with bronchial asthma and 11 patients with normal subjects. In conclusion, increased permeability and the damage of bronchial epithelium may lead to change of bronchial hyperreactivity.

  • Congenital esophago-bronchial fistula without atresia is very rare and usually demonstrate more insidious clinical effects, and patients may reach adult life before the diagnosis. We had been experienced a typical case of esophago-bronchial fistula without atresia and document the case. A 42-year-old women was admitted to our hospital due to frequent choking symptom and attacks of pneumonia, and patient status was in chronic pulmonary infection status such as most probably bronchiectatic state of right middle & lower lobe including significant destructive changes. We had confirmed esophago-bronchial fistula without atresia with esophagoscopy, esophagogram, and chest CT, and performed esophago-bronchial fistulectomy and right middle & lower lobectomy. Its belongs to type II of Braimbridge`s classification for congenital esophago-bronchial fistula. Postoperation course was smooth and uneventful.

  • In humans, an immediate allergic response to an allergen snay appear as either bronchial asthma unaccompanied by anaphylaxis or a severe bronchial constriction and spasm accompanied by anaphylaxis. Much experimental evidence with isolated asthmatic tissues has shown that histamine is released during the allergic reaction. Paradoxically, antihistamine treatment is ineffective in reversing bronchial asthma or anapllylaxis in a man. The present study objective is to search for protective agents(In-Sam-Yun-Pai-San-Ga-Gam-Bang) against bronchial constriction in treating a anaphylaxis. The result from this study is as follows. A single dose of In-Sam-Yun-Pai-San-Ga-Gam-Bang extract(3ml/kg body weight) 2 hrs prior to histamine-induced bronchial constriction test demonstrated significant protection. Based on the above result, It is considered that In-Sam-Yun-Pai-San-Ga-Gam-Bang can be used in bronchial asthma.

  • Bronchial carcinoid was classified as one of the bronchial adenoma group and it was regarded as benign tumor because of its uniform histologic feature, predictable clinical course and good survivability after surgical resection. But recently, many authors described that bronchial carcinoid has low grade malignancy for its lymphatic spread and distant metastasis, especially atypical carcinoid. We had a case of typical bronchial carcinoid in 20 year old female patient, and successful operation was performed by bilobectomy of middle and lower lob of right lung. Postoperative course was uneventful and discharged without any complication.

  • The injuries to the bronchi have been reported with increasing frequencies. The most common cause of such injuries is compression or crushing chest trauma. Early diagnosis and emergent repair should be done for the good prognosis. We report 3 cases who had bronchial injuries after traffic accident. Our operative procedures were a primary bronchial repair for 17 months old boy, a pneumonectomy for delayed recognition and a bronchoplasty procedure 63 days after trauma. All these bronchial ruptures were successfully treated and discharged.

  • Bronchoplasty has gained popularity in the selected cAses of bronchogenic carcinoma with poor pulmonary reserve, and also has been a choice of treatment for obstructive bronchial diseases since it can cure patient completely with preservation of pulmonary function. From Apr. 1990 to hpr. 19'96 two methods of bronchoplastic procedures, d patch dilating bronchoplasty and a segmental bronchial resection with end-to-end anastomosis, were performed with or without concominant pulmonary resection in 13 patients with benign bronchial stenosis and obstruction. The patients were 8 men and 5 women with average age of 43years(range 19 to 64 years). Patch dilating bronchoplasty using autogenous perichondrium and pericardium was applied in 5 cases of bronchial stenosis. Antecedan diseases of bronchial stenosls were 3 inflammatory bronchiectas is, and 2 endobronchial tuberculose is mixed with bronchi, ectas is. Segmental bronchial resection with end-to-end anastomosis was applied in 8 cases of bronchial obstruction, which were caused by endobronchial tuberculosis in 6 and cicatrization after trauma and foreign body in one case each. Bronchial obstructive symptoms and signs including recurrent pulmonary infection, dyspnea and wheezing were disappeared postoperatively with satisfactory recovery of physical activity. There was no operative mortality. Morbidity occured in 2 patients which were one case of unstability of applied bronchial patch resulting atelectasis and one case of bronchial restenosis at the anastomotic site. Based upon our experiences, we conclude that bronchoplastic procedure can be done with great success in patients with lung atelectasis caused by bronchial obstruction or stenosis and it restores physiologic function of collapsed lung with acc ptable complication.

  • We studied the association of nonspecific bronchial hyperresponsiveness with general characteristics, exposure concentration, respiratory symptoms, chest x-ray findings, past histories and pulmonary function. We determined bronchial hyperresponsiveness by methacholine challenge test. And we conducted a respiratory symptom questionnaire and performed spirometry on 111 workers occupationally exposed to isocyanates in various industries. About 21.6% of subjects had bronchial hyperresponsiveness. No significant differences were observed between the hyperresponsive and non-responsive group with respect to age, sex, employment period, height, and smoking histories. Cough and breathlessness were significantly associated with the bronchial hyperresponsiveness. The hyperresponsive group had more experience of bronchitis and asthma in the past than the non-responsive group. The lower $FEV_1\;and\;FEV_1%$ were closely related with bronchial hyperresponsiveness. Bronchial hyperresponsiveness seems to be associated with some of respiratory symptoms, past histories and pulmonary function parameters in workers exposed to isocyanates.
