  • To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed and produced a composite that is consisted of calcium phosphate and collagen. Human umbilical cord origin pepsin treated type I atelocollagen was used as the structural matrix, by which sintered or non-sintered carbonate apatite was encapsulated to form an inorganic-organic composite. With cross linking atelocollagen by UV ray irradiation, the resistance to both compressive and tensile strength was increased. Collagen degradation by the collagenase induced collagenolysis was also decreased.

Development of Physical Fitness Standard Indicators According to the Bone Age in Youth (유소년의 골연령에 따른 체력 표준지표 개발)

  • This study aims to evaluate physical fitness according to the bone age of youth, and ultimately provide basic data for balanced development of youth through physical fitness standard indicators according to the bone age. A total of 730 youth aged 11 to 13 years in bone age and 11 to 13 years in chronological age were selected as subjects; and after taking X-ray films to calculate the bone age, they were evaluated by using the TW3 method. A total of 2 components in physique, which were stature and weight, were measured using a stadiometer(Hanebio, Korea, 2021) and Inbody 270(Biospace, Korea, 2019). A total of 7 components in physical fitness were measured as well, which included muscular strength (Hand Grip Strength), balance (Bass Stick Test), agility (Plate Tapping), power (Standing Long Jump), flexibility (Sit&Reach), muscular endurance (Sit-Up), and cardiovascular endurance (Shuttle Run). Descriptive statistics and independent t-test were conducted for data processing using the SPSS PC/Program(Version 26.0), and it was considered significant at the level of p< .05. The results of this study may be summarized as follow. First, the result of comparing the bone age and the chronological age of 11 to 13 years old in physical fitness, males showed significant difference in muscular strength, power, muscular endurance, and cardiovasular endurance. In females, muscular strength, balance, agility, power, flexibility, muscular endurance, and cardiovascular endurance showed significant difference. Second, physical fitness standard indicators were presented for each gender and age (11-13 years old) of youth according to the bone age; and based on this, physical fitness standard indicators, which are basic data for physical fitness evaluation according to the bone age of youth, were presented.

Removal Torque and Histomorphometric Investigation of Surface Modified Commercial Implants: An Experimental Study in the Rabbit Tibia (상용화된 치과용 임플란트의 뒤틀림 제거력 및 조직학적 분석 연구: 가토 경골에서의 연구)

  • The methods of surface modification of commercial implants were various according to the manufacturer. Surface modification of implant may produce diverse physical and chemical surface characteristics resulted from the treatment method and treatment condition. As a result, the bone response might be different. Even though surface modified implants have been used clinically, most researches are focusing on the bone response of surface modified implants comparing to machined implants rather than surface modified commercial implants. This study compare and analyze bone responses of 4 surface modified commercial implants with different shapes and surfaces. Eighty surface modified commercial implants with 4 different surface characteristics were installed in the tibia of white Newzealand rabbits. Biomechanical stability tests and histomorphometric evaluation were done. The results were as follows: 1. Surface modified commercial implants showed stable osseointegration at 6 weeks after installation. 2. Histomorphometric evaluation showed that there was no significant differences in bone to implant contact among 4 different commercial titanium implants. In comparing the implants with different shape the measurement of bone growth in subcortical area would be more reliable than entire bone to implant contact length. 3. Resonance Frequency Analysis showed that there was no significant differences among 4 types of implants, even though they were significantly different in installation. 4. There was significant differences in interfacial shear strength among 4 type of implants. 5. It is difficult to observe accurate bone to implant interface using Micro-CT. However, it is possible to measure the entire contact length of the implant to the bone.

Identification of novel susceptibility genes associated with bone density and osteoporosis in Korean women

  • Purpose: Osteoporosis is a common calcium and metabolic skeletal disease which is characterized by decreased bone mass, microarchitectural deterioration of bone tissue and impaired bone strength, thereby leading to enhanced risk of bone fragility. In this study, we aimed to identify novel genes for susceptibility to osteoporosis and/or bone density. Materials and Methods: To identify differentially expressed genes (DEGs) between control and osteoporosis-induced cells, annealing control primer-based differential display reverse-transcription polymerase chain reaction (RT-PCR) was carried out in pre-osteoblast MC3T3-E1 cells. Expression levels of the identified DEGs were evaluated by quantitative RT-PCR. Association studies for the quantitative bone density analysis and osteoporosis case-control analysis of single nucleotide polymorphism (SNPs) were performed in Korean women (3,570 subjects) from the Korean Association REsource (KARE) study cohort. Results: Comparison analysis of expression levels of the identified DEGs by quantitative RT-PCR found seven genes, Anxa6, Col5a1, Col6a2, Eno1, Myof, Nfib, and Scara5, that showed significantly different expression between the dexamethason-treated and untreated MC3T3-E1 cells and between the ovariectomized osteoporosis-induced mice and sham mice. Association studies revealed that there was a significant association between the SNPs in the five genes, ANXA6, COL5A1, ENO1, MYOF, and SCARA5, and bone density and/or osteoporosis. Conclusion: Using a whole-genome comparative expression analysis, gene expression evaluation analysis, and association analysis, we found five genes that were significantly associated with bone density and/or osteoporosis. Notably, the association P-values of the SNPs in the ANXA6 and COL5A1 genes were below the Bonferroni-corrected significance level.

  • Osteoporosis, Greek for "porous bone," is a bone disease characterized by a decrease in bone strength, microarchitectural changes in the bone tissues, and an increased risk of fracture. An imbalance of bone resorption and bone formation may lead to chronic metabolic diseases such as osteoporosis. Wolfiporia extensa, known as "Bokryung" in Korea, is a fungus belonging to the family Polyporaceae and has been used as a therapeutic food against various diseases. Medicinal mushrooms, mycelium and fungi, possess approximately 130 medicinal functions, including antitumor, immunomodulating, antibacterial, hepatoprotective, and antidiabetic effects, and are therefore used to improve human health. In this study, we used osteoclast and osteoblast cell cultures treated with Wolfiporia extensa mycelium water extract (WEMWE) and investigated the effect of the fungus on bone homeostasis. Subsequently, we assessed its capacity to modulate both osteoblast and osteoclast differentiation by performing osteogenic and anti-osteoclastogenic activity assays. We observed that WEMWE increased BMP-2-stimulated osteogenesis by inducing Smad-Runx2 signal pathway axis. In addition, we found that WEMWE decreased RANKL-induced osteoclastogenesis by blocking c-Fos/NFATc1 via the inhibition of ERK and JNK phosphorylation. Our results show that WEMWE can prevent and treat bone metabolic diseases, including osteoporosis, by a biphasic activity that sustains bone homeostasis. Therefore, we suggest that WEMWE can be used as a preventive and therapeutic drug.

Effect of surface anodization on stability of orthodontic microimplant

  • Objective: To determine the effect of surface anodization on the interfacial strength between an orthodontic microimplant (MI) and the rabbit tibial bone, particularly in the initial phase aft er placement. Methods: A total of 36 MIs were driven into the tibias of 3 mature rabbits by using the self-drilling method and then removed aft er 6 weeks. Half the MIs were as-machined (n = 18; machined group), while the remaining had anodized surfaces (n = 18; anodized group). The peak insertion torque (PIT) and the peak removal torque (PRT) values were measured for the 2 groups of MIs. These values were then used to calculate the interfacial shear strength between the MI and cortical bone. Results: There were no statistical differences in terms of PIT between the 2 groups. However, mean PRT was significantly greater for the anodized implants ($3.79{\pm}1.39$ Ncm) than for the machined ones ($2.05{\pm}1.07$ Ncm) (p < 0.01). The interfacial strengths, converted from PRT, were calculated at 10.6 MPa and 5.74 MPa for the anodized and machined group implants, respectively. Conclusions: Anodization of orthodontic MIs may enhance their early-phase retention capability, thereby ensuring a more reliable source of absolute anchorage.

Optimal Condition of Hydroxyapatite Powder Plasma Spray on Ti6Al4V Alloy for Implant Applications

  • Optimal conditions for HA plasma spray-coating on Ti6Al4V alloy were investigated in order to obtain enhanced bone-bonding ability with Ti6Al4V alloy. The properties of plasma spray coated film were analyzed by SEM, XRD, surface roughness measurement, and adhesion strength test because the film's transformed phase and crystallinity were known to be influential to bone-bonding ability withTi6Al4V alloy. The films were formed by a plasma spray coating technique with various combinations of plasma power, spray distance, and auxiliary He gas pressure. The film properties were analyzed in order to determine the optimal spray coating parameters with which we will able to achieve enhanced bone-bonding ability with Ti6Al4V alloy. The most influential coating parameter was found to be the plasma spray distance to the specimen from the spray gun nozzle. Additionally, it was observed that a relatively higher film crystallinity can be obtained with lower auxiliary gas pressure. Moderate adhesion strength can be achievable at minimal plasma power. That is, adhesion strength is minimally dependent on the plasma power. The combination of shorter spray distance, lower auxiliary gas pressure, and moderate spray power can be recommended as the optimal spray conditions. In this study, optimal plasma spray coated films were formed with spray distance of 70 mm, plasma current of 800 A, and auxiliary gas pressure of 60 psi.

A Newly Designed Miniplate Staple for High Tibial Osteotomy (근위골절술을 위한 Staple 설계)

  • A biomechanical study was made to demonstrate the superior mechanical performance of the newly designed Miniplate staple to the conventional Coventry staple in high tibial osteotomy(HTO). Using twenty fresh porcine tibiae, the fixational strengh of the two different types of staple in HTO was compared. To minimize the error due to the specimen-to-specimen individuality, the bone mineral density of the tibiae was measured with a bone densitometry (Dual photon absorptionometer, Luner, USA) and those with $0.8\;{\sim}\;1.2\;gm/cm^2$ at the proximal tibia was used in the biomechanical test. Testing was performed on a material testing system (Autogram ET-5, Shimatzu, Japan) with aid of a commercial data processor (IBM 80386/ ASYST). Using two differant loading modes, 'pull-out' and 'push-out', the maximum resistant force required to release the staple from the substrate bone was recorded. In the pull-out test, ten non-osteotomized specimens were used and the staple was pullout by subjecting an axial tension on the head of the staple inserted. While in the pull-out test where ten tibiae osteotomized in the usual way of HTO were used, the staple was not directly loaded. In this testing, as a mimic condition of the natural knee, the distal part of the specimen tibia was pushed horizontally in order for the staple to be pulled out while the proximal tibia was fixed. The pull-out strength of Coventry staple and miniplate staple were found to be $27.88\;{\pm}\;5.12\;kgf$ and $182.47\;{\pm}\;32.75\;kgf$, respectively. The push-out strength of Coventry staple and miniplate staple were $18.40\;{\pm}\;4.47\;kgf$ and $119.95\;{\pm}\;19.06\;kgf$, respectively. The result revealed that miniplate staple had the pull-out/ push-out strength at least fivetimes higher than Coventry staple. Based on the measured data, it was believed that the newly designed miniplate staple could provide much better postoperative fixation in HTO. The postoerative application of long leg casting may not be needed after HTO surgery.

Comparison of Physical Characteristics and Lower Extremity Biomechanics of Elderly and Young Adult Runners (노인 러너의 신체특성과 하지관절의 생체역학적 비교)

  • Objective: The purpose of this study was to compare the physical characteristics (bone mineral density, joint muscle strength) and running biomechanics between older adults and young adult runners to understand the changes of running strategy by aging. Method: Bone mineral density (Dual Energy X-ray Absorptiometry, USA) of lower lower extremity and muscle strength (Cybex Humac Norm [DEXA], CSMI, USA) were measured to identify the physical characteristics of 10 elderly (age: $67.70{\pm}3.30yrs$, height: $1.68{\pm}0.04m$, mass: $67.70{\pm}3.80kg$) and 10 young adults (age: $21.20{\pm}0.42yrs$, height: $1.73{\pm}0.06m$, mass: $72.11{\pm}4.15kg$). Running data was collected by using an instrumented treadmill (Bertec, USA) and 7 infrared cameras (Oqus 300, Qualisys, Sweden). Two-way repeated ANOVA analysis was used to analyze results at a significant level of .05 with Bonferroni post hoc analysis. Results: Compared to the young adult group, the elderly group showed statistically significant difference in physical characteristics and in running characteristics. Elderly runners showed lower BMD and muscle strength compared with young runners (p<.05). In the running parameters, elderly runners tend to show shorter contact time and stride length compared with young runners (p<.05). In the joint angles, elderly runners showed smaller range of ankle motion compared with young runners (p<.05). Finally, elderly runners showed lower level of joint moment, joint power, and GRF compared with young runners in each running speed (p<.05). Conclusion: The running behavior of the elderly performed periodic running was similar to many variables of young adults. However, there were noticeable differences found in the ankle joints and most kinetic variables compared with young adult runners. This discrepancy may propose that elderly runners should consider appropriate running distance and intensity in the program.

Calcium release and physical properties of modified carbonate apatite cement as pulp capping agent in dental application

  • Background: Carbonate apatite ($CO_3Ap$) and silica-calcium phosphate composite (SCPC) are bone substitutes with good prospect for dental application. SCPC creates a hydroxyapatite surface layer and stimulate bone cell function while, $CO_3Ap$ induce apatite crystal formation with good adaptation providing good seal between cement and the bone. Together, these materials will add favorable properties as a pulp capping material to stimulate mineral barrier and maintain pulp vitality. The aim of this study is to investigate modification of $CO_3Ap$ cement combined with SCPC, later term as $CO_3Ap-SCPC$ cement (CAS) in means of its chemical (Calcium release) and physical properties (setting time, DTS and pH value). Methods: The study consist of three groups; group 1 (100% calcium hydroxide, group 2 $CO_3Ap$ (60% DCPA: 40% vaterite, and group 3 CAS (60% DCPA: 20% vaterite: 20% SCPC. Distilled water was employed as a solution for group 1, and $0.2mol/L\;Na_3PO_4$ used for group 2 and group 3. Samples were evaluated with respect to important properties for pulp capping application such as pH, setting time, mechanical strength and calcium release evaluation. Results: The fastest setting time was in $CO_3Ap$ cement group without SCPC, while the addition of 20% SCPC slightly increase the pH value but did not improved the cement mechanical strength, however, the mechanical strength of both $CO_3Ap$ groups were significantly higher than calcium hydroxide. All three groups released calcium ions and had alkaline pH. Highest pH level, as well as calcium released level, was in the control group. Conclusion: The CAS cement had good mechanical and acceptable chemical properties for pulp capping application compared to calcium hydroxide as a gold standard. However, improvements and in vivo studies are to be carried out with the further development of this material.
