소아청소년의 게임장애와 중독 (Gaming Disorder and Addiction in Children and Adolescents)

  • Objectives: Gaming disorder has been viewed as a disease in the DSM-5 and ICD-11. Its essential symptoms are loss of control over gaming, gaming becoming a markedly prioritized activity over other activities of daily living, and continued and excessive use of gaming despite negative problems occurring. Methods: Children and adolescents are especially vulnerable to gaming disorder because the striatal pathways related to reward develop earlier than the control regions of the prefrontal cortex. It is also associated with decreased dopamine D2 receptors. Addiction is related to 'want' and is explained by incentive-sensitization. In addition, allostasis, in which homeostasis is continuously achieved at a new target value, is also related to gaming disorder. In addition, personality causes, unchangeable factors, and external factors can influence on the onset of gaming disorder. Results: Prevention is the best solution for gaming disorder, and the role of parents is important. For gaming disorder, bupropion is used, cognitive-behavioral therapy and family-based therapy are also beneficial. Herbal medicine treatment such as Antler velvet and ginseng can be effective. Electroacupuncture and acupuncture using PC6, SP6, and LR3 has a correlation with relieving Internet craving. Ear-acupuncture was also effective in treating addiction. Conclusion: Psychologically, 'want' is an intense longing for reward and motivation, and is related to addiction. This 'want' may rather be related to avoidance, and game addiction in children and adolescents may be due to wanting to escape from academic stress or avoidance of comparison. Therefore, the importance of 'like', which gives pleasure in itself, increases. It can also be explained with Sasang Constitutional Medicine.

초등학생 스마트폰 중독 개입교육 프로그램의 개발 및 적용 (A Study on Development and Application of An Educational Program to Intervene with Elementary School Students' Smartphone Addiction)

  • 본 연구는 초등학생을 대상으로 개발한 스마트폰중독 개입교육 프로그램이 최근 빠른 추세로 증가하고 있는 초등학생의 스마트폰 과다사용 감소에 효과적인가를 규명하는데 그 연구 목적을 두었다. 이러한 목적을 달성하기 위해 중독현상을 나타내는 사람들의 심리치료에 효과성이 검증된 인지행동치료를 기반으로 기존에 개발된 인터넷중독 개입프로그램을 분석하고, 이를 토대로 스마트폰중독 개입을 위한 교육프로그램을 개발 적용하였다. 본 연구의 결론을 제시하면 다음과 같다. 첫째, 스마트폰중독 개입교육 프로그램은 비합리적인 사고와 습관적 행동으로 스마트폰을 과다하게 사용하는 학생들의 스마트폰 중독 경향을 감소시키는데 효과가 있다. 둘째, 스마트폰중독 개입교육 프로그램은 일상생활장애, 내성과 같은 스마트폰 중독의 부정적 요인들에 대한 행동의 변화를 촉진하고, 초등학생의 스마트폰 사용에 대한 긍정적인 인지를 재구성하는데 도움이 될 것이다.

Brain Neuroadaptative Changes in Adolescents with Internet Addiction : An FDG-PET Study with Statistical Parametric Mapping Analysis

  • Objectives : Internet addiction or pathologic internet use is one of the major mental health problems in children and adolescents in Korea. Internet addiction is defined as uncontrollable, markedly time-consuming internet use, which lasts for a period of at least six months. Internet addiction results in poor academic performance and negative parent-child relationships. By using $^{18}F$-fluorodeoxyglucose-positron emission tomography (FDG-PET), we investigated the effects of internet addiction on functional changes occurring in the adolescent brain. Methods : Adolescent patients with an internet addiction (4 boys and 2 girls; $15.6{\pm}1.2$ years) participated in this study. Eight healthy young adults (5 males and 3 females; 18-30 years old) with no previous history of psychiatric illness also participated as normal controls. Brain FDG-PET data was obtained with the participants in the resting condition and with no addictive stimuli. Results : Statistic parametric mapping analysis of the brain FDG-PET data revealed hypometabolic changes in the visual information processing circuits and hypermetabolic changes in the prefrontal areas in the adolescents with internet addiction, as compared with normal controls (p<.001). Conclusion : These results suggest a neuronal adaptation to excessive visual stimulation and synaptic plasticity due to internet addiction.

청소년을 위한 스마트폰 과다사용 예방교육 프로그램 개발 및 예비효과연구 (Development and Preliminary Outcome Study of Smartphone Overuse Prevention Education Program for Adolescents of Middle School)

  • This study developed the prevention education program of smartphone overuse for adolescents in middle school and testified its outcome. Finally the 4-session prevention education program for smartphone overuse was developed. 1st session was composed of psychoeducation about smartphone overuse and its side effect by quiz question. 2nd session smartphone addiction and brain change, 3rd session motivational enhancement program for smartphone overuse, 4th session cognitive behavioral program for it. The outcome was testified by internet addiction scale, smartphone addiction scale, and school adaptation scale for 2 classes of middle school students(experimental and control class). 2 Main results were as follows; First, the smartphone addiction total score and cyberworld orientation of the students participating in it were statistically significantly reduced. The difficulty of daily living and abstinence symptom due to smartphone overuse of them tended to reduce. Second, internet addiction total and all subscores of them were not changed. Third, class adaptation of them was statistically significantly increased and school adaptation total score and teacher adaptation of them tended to increase. Its meaning, implication and limitation were discussed.

Wild Ginseng Attenuates Repeated Morphine-Induced Behavioral Sensitization in Rats

  • Many studies have suggested that the behavioral and reinforcing effects of morphine are induced by hyperactivation of the mesolimbic dopaminergic system, which results in increases in locomotor activity, c-Fos expression in the nucleus accumbens (NAc), and tyrosine hydroxylase (TH) in the ventral tegmental area (VTA). In order to investigate the effect of wild ginseng (WG) on treating morphine addiction, we examined the behavioral sensitization of locomotor activity and c-Fos and TH expression in the rat brain using immunohistochemistry. Intraperitioneal injection of WG (100 and 200 mg/kg), 30 min before administration of a daily injection of morphine (40 mg/kg, s.c.), significantly inhibited morphine-induced increases in c-Fos expression in NAc and TH expression in VTA as well as in locomotor activity, as compared with Panax ginseng. It was demonstrated that the inhibitory effect of WG on the behavioral sensitization after repeated exposure to morphine was closely associated with the reduction of dopamine biosynthesis and postsynaptic neuronal activity. It suggests that WG extract may be effective for inhibiting the behavioral effects of morphine by possibly modulating the central dopaminergic system and that WG might be a useful resource to develop an agent for preventing and treating morphine addiction.

황련해독탕(黃連解毒湯)과 그 주성분인 황련(黃連) 및 황련(黃連)의 주성분인 berberine이 니코틴 중독 흰쥐에 미치는 영향 (The Effect of Hwangryunhaedok-tang and its Major Component Coptidis Rhizoma, Berberine on Nicotine Addiction in Rats)

  • Objectives: The purpose of this study was to investigate the effect of Hwangryunhaedok-tang, its major component Coptidis Rhizoma, and the major component of Coptidis Rhizoma, berberine, on the expression of behavioral sensitization induced by the repeated administration of nicotine. Methods: Experimental ratswere divided into 3 groups and each group treated with saline, nicotine, and Hwangryunhaedok-tang. Experiments were performed on 4 further groups, each group treated with saline, nicotine, Coptidis Rhizoma, and berberine. To obtain rats addicted todrugs, nicotine was injected repeatedly. After 3 days of withdrawal phase, on the 11th day, rats were re-exposed to nicotine and thus able to induce the expression of the activity sensitization phenomenon successfully. To examine the change of c-Fos in the brain of experimental rats with the nicotine-induced activity sensitization, their neurological activation rate was assessed by c-Fos staining with Fos-like immunihistochemistry of the dopamine releasing area, the striatum and the nucleus accumbens. Results: The result demonstrate that Hwangryunhaedok-tang, its major component Coptidis Rhizoma and berberine,the major component of Coptidis Rhizoma, significantly reduced the walking activity rate in the drug addict models induced by nicotine administration, and Coptidis Rhizoma and its major component berberine reduce the marker of the neurological activation c-Fos, and thus revealed an effect of suppressing drug addiction. Conclusions: These results suggest the possibility of Hwangryunhaedok-tang, its major component Coptidis Rhizoma and the major component of Coptidis Rhizoma, berberine,as therapeutic for nicotine addiction.

Benzodiazepine의 재평가 및 적절한 사용 (The Reappraisal and Appropriate Use of Benzodiazepine)

  • Benzodiazepine (BDZ) has the possibilities of development of tolerance, withdrawal symptoms, and abuse/addiction, as well as chronically adverse effects. Although many guidelines have proposed the restricted prescription of them, their uses in many psychiatric areas as well as primary practice are still wide spread. So we tried to reappraise the clinical characteristics of BDZ and then to consider the appropriate use. Firstly, meta-analyses on long-term use of BDZ indicated the cognitive impairment, which could be improved after discontinuation of BDZ. Next, there have been some evidences that the long-term use of BDZ does not develop tolerance, contrary to our concern, and maintains good anxiolytic effects. Also, physiological dependence should be discriminated from abuse/addiction, assuming the reality that the risk of BDZ abuse/addiction is surely overestimated. These issues are discussed in detail.

코로나19 스트레스가 온라인 쇼핑중독에 미치는 영향: 성별에 의해 조절된 우울의 매개효과 (Effect of COVID-19 Stress on Online Shopping Addiction: Mediating Effects of Depression on Moderated by Gender)

  • 본 연구는 성인을 대상으로 코로나19 스트레스, 우울 및 온라인 쇼핑중독 간의 연관성을 파악하고, 성별에 의해 조절된 우울의 매개효과를 확인하기 위해 수행하였다. 이를 위해 전국에 패널을 보유한 설문조사 전문기관에 의뢰하여 온라인 설문조사를 수행하였다. 수집된 성인 336명의 자료는 회귀분석에 기반한 Process Macro를 통해 분석하였다. 연구결과, 코로나19 스트레스가 증가할수록 온라인 쇼핑중독 경향이 증가하였고, 코로나19 스트레스가 온라인 쇼핑중독에 미치는 영향에서 우울의 매개효과가 있었다. 또한, 코로나19 스트레스가 온라인 쇼핑중독에 영향을 미치는 과정에서 우울의 매개효과는 여자에 비해 남자가 더욱 강하게 나타났다. 그러므로 코로나19 감염병이 장기화될수록 온라인 쇼핑과 같은 행위중독 예방 및 정신건강 관리를 위한 개입에 있어서 성별을 고려할 필요가 있다.

옥시토신의 약물중독에서 역할과 침(鍼) 관련성 (A Potential Role of Oxytocin and Acupuncture in Drug Addiction)

  • Oxytocin(OT), classically known to stimulate labour and milk ejection, contributes to play an important role in a wide range of behavioral effects including drug addiction. An increasing body of evidence suggests that OT ameliorates acute and long-term effects of commonly used drugs by means of interacting with the mesolimbic dopamine system. Mesolimbic dopamine system is thought to play a major role in the reinforcing properties of drug abuse. Oxytocin receptors in the nucleus accumbens(NAc) and ventral tegmental area(VTA) have been implicated in the regulation of reinforcing effects in abused drugs. In the same way acupuncture may attenuate the reinforcing effects of abused drugs in the NAc and VTA. We have an interest in similar liaison between the substrates of acupuncture and drug addiction that may involve OT. Here, we described the possibility that acupuncture modulates the reinforcing and sensitizing properties of abused drugs in the dopaminergic system via the regulation of activities in the oxytocinergic system. The elements in this paper are summarized as follows : neuroanatomical studies of oxytocinergic innervation and distribution of oxytocin receptors; experiments related to the methamphetamine, cocaine, morphine and ethanol; experiments related to the oxytocin and acupuncture.

흥분제(Psychostimulants)에 대한 신경생물학 측면의 고찰 -Methamphetamine 남용을 중심으로 (Neurobiological Effects of Methamphetamine Abuse on Neurotransmitters: A Review)

  • Methamphetamine (MA) is a major drug of abuse in Korea. Currently preliminary evidence suggests that MA dependence may cause long-term neural damage in human. Repeated exposure to psychostimulants such as methamphetamine results in behavioral sensitization, a paradigm thought to be relevant to drug craving and addiction in human. Sensitization alters neural circuitry involved in normal processes of incentrive, motivation, and reward. However the precise mechanism of this behavioral sensitization has not yet been fully elucidated. Repeated use of high dose MA causes neurotoxicity which is characterized by a long-lasting depletion of striatal dopamine (DA) and tyrosin hydroxylase activity of DA, DA-transporter binding sites in the striatum. The loss of DA transporters correlates with memory problems and lack of motor coordination. DA fuels motivation and pleasure, but it' s also crucial for learning and movement. This selective review provides a summary of studies that assess the neurobiological mechanisms of MA.
