The Remodeling of the Posterior Edentulous Mandible as Illustrated by Computed Tomography (전산화 단층사진술에 의해 예증된 구치부 무치악 하악골의 골개조)

  • Purpose: The aim of this study was to analyze radiologically the location and course of the mandibular canal and to observe the alveolar and basal bone changes during the remodeling procedures of atrophic mandible. Materials and Methods: CT scanning was performed on dry 30 edentulous or partially dentulous mandibles. In 48 edentulous lower halves, measuring areas were determined by three points in the length of the mandibular canal. The distance from the mandibular canal towards cranial and caudal edges, buccal and lingual external borders of the body of the mandible were measured. A statistical comparison between the mean values of different classes of mandibular body was carried out in the selected areas. Results: The distance between the mandibular canal and caudal borders of the body of the mandible and lingual borders dose not change in the atrophic process of mandible. The mandibular canal within the mandible courses downwards from mandibular foramen towards mesial and subsequently it gets to the mental foramen. The distance between the mandibular canal and buccal external border of basal bone changes similar to the change of cranial borders of alveolar bone in the atrophic process of mandible. Conclusion: CT scanning was very effective and practicable to analyze the location and course of the mandibular canal and to observe the alveolar and basal bone changes of atrophic mandible. Also more detailed investigation of basal bone changes observed during the remodeling procedures of atrophic mandibles seems reasonable to rely on the massive anthropologic collections of atrophic mandibles combined with CT scanning.

Histopathological Studies on the Effect of Persimmon Leaves on Cadmium Poisoning in Mice (감잎이 마우스의 카드뮴 중독에 미치는 병리조직학적 관찰)

  • This study was undertaken to find out the effect of persimmon leaves on histopathological changes of cadmium toxicity in mice. Seventy two BALB/c mice of male were divided into a control group(A) and five experimental groups (B, C, D, E, F) : group A received tap water and basal diet, group B received tap water and diet supplemented with 3% persimmon leaves alone, group C received basal diet and 300 ppm cadmium, group D, E and F received basal diet supplemented with 1, 3% and 7% persimmon leaves and 300 ppm cadmium respectively. Cadmium dissolved in tap water was used, and the persimmon leaves were mixed with feed. All mice were dissected on the 84th day. Pathological changes in liver, kidney, cortical osseous tissue of femoral shaft, bone trabecular of femur, and epiphyseal cartilage plate of femur were observed. Group B showed no significant changes as the control group. But group C showed the unclearness of specific cells in liver, the loss of architecture and necrosis of hepatocyte, degeneration and necrosis of renal convoluted tubules, desquamation and vacuolization of the greater part of the renal tubular epithelium, marked thinning of the cortical osseous tissue in femoral shaft, reduction of cancellous bone volume and decreaswe of trabecular number, and marked thinning of epiphyseal cartilage plate in femur. On the other hand, persimmon leaves-treated group showed a little convalescent changes an maintained their normal architectures in liver, kidney, cortical osseous tissue of femoral shaft, bone trabecular of femur, and epiphyseal cartilage plate of femur.

Histopathological Studies on the Effect of Korean Safflower (Carthamus tinctorious L.) Seed Powder on Cadmium Poisoning in Mice (홍화씨분말이 카드뮴 중독에 미치는 병리조직학적 관찰)

  • This study was undertaken to find out the effect of Korean safflower seed powder on histopathological changes of cadmium toxicity in mice. Fifty BALB/c mice were divided into a control group(A) and four experimental groups(B, C, D, E) : group A received tap water and basal diet, group B received tap water and diet supplemented with 3% Korean safflower seed powder alone, group C received basal diet and 300 $\mu\textrm{g}$/g of cadmium, group D and E received basal diet supplemented with 3% and 10% Korean safflower seed powder and 300$\mu\textrm{g}$/g of cadmium respectively. Cadmium dissolved in tap water was used, and the Korean safflower seed powder were mixed with feed. All mice were dissected on the 56th day. Histopathological changes in liver, kidney, lung, cortical osseous tissue of femoral shaft, bone trabecular of femur, and epiphyseal cartilage plate of femur were observed. Group B showed no significant changes compared with the control group. But group C showed the unclearness of specific cells in liver, the loss of architecture and focal necrosis of hepatocyte, the glomerular swelling, degeneration and necrosis of convoluted tubules, desquamation and vacuolization of the greater part of the renal tubular epithelium, the marked congestion and thickness of the wall of alveolus in lung, slightly thinning of the cortical osseous tissue in femoral shaft, reduction of cancellous bone volume and marked narrowness of bone trabecular, marked thinning of epiphyseal cartilage plate and irregular arrangement of columnar structure of cartilage cells. On the other hand, Korean safflower seed powder-treated group showed a little convalescent changes and maintained their normal architectures in liner, kidney, lung, cortical osseous tissue of femoral shaft, bone trabecular of femur and epiphyseal cartilage plate of femur.

Bone changes after bilateral sagittal split osteotomy for mandibular prognathism (하악 전돌증 환자의 양측성 하악지 시상분할 골절단술 후 골 변화)

  • Purpose: The purpose of this research was to study bone changes after bilateral sagittal split osteotomy through fractal analysis and measurement of mandibular cortical thickness. Materials and Methods: This study included twenty-two prognathic patients who underwent bilateral sagittal split osteotomy. Panoramic radiographs of these patients were taken immediately before operation and at 1 month, 6 months, and 12 months postoperatively. The fractal dimension was measured by the box-counting method in the region of interest centered on both the basal and interdental bones between the first and second mandibular molars. Measurements of mandibular cortical thickness were taken both in the area between the first and second mandibular molars and at the osteotomy site. Changes of fractal dimension and cortical thickness over four stages were statistically analyzed. Results: The fractal dimension of the mandibular basal bone before surgery and after 1 month, 6 months and 12 months were $1.4099{\pm}0.0657,\;1.382{\pm}0.0595,\;1.2995{\pm}0.0949,\;and\;1.4166{\pm}0.0676$, respectively (Repeated-measures ANOVA, P<0.001). However, no statistically significant differences were noted in interdental fractal dimensions among the four stages. Mandibular cortical thickness between the first and second mandibular molars before operation and after 1 month, 6 months and 12 months was $3.74{\pm}0.48mm,\;3.63{\pm}0.47mm,\;3.41{\pm}0.61mm\;and\;3.55{\pm}0.66mm$ (P<0.01), respectively. Mandibular cortical thickness at the osteotomy site at each of the four stages was $3.22{\pm}0.44mm,\;2.87{\pm}0.59mm,\;2.37{\pm}0.61mm\;and\;2.64{\pm}0.62mm$, respectively (P<0.001). Conclusion: This study suggests that the mandibular tissue continued decreasing for 6 months postoperatively and then increased over the subsequent 6 months.

  • Vocal folds injury from vocal abuse is important topics of phonosurgery. Recent advances in diagnostic equipment, phonosurgery and speech analysis equipment have provided a lot of Information about fine movement of the vocal folds. However, predicting the reaction of the vocal folds to phonatory trauma remains difficult. The vocal folds need to withstand great vibratory and shearing stress and anchoring fibers of basement membrane Bone play a role in maintaining structural integrity of histologically different epidermis and superficial layer of lamina propria(cover of vocal folds). The purpose of this study is to demonstrate the changes of anchoring fibers in vocal polyp using transmission electron microscope. Various defects were observed : a irregular thickening of basement membrane, a near absence of normal anchoring fiber, a lot of electron dense material in superficial layer of lamina propria, a destruction of hemidesmosome and many vesicles carrying electron dense material In basal keratinocyte. These observations were suggestive of a hyperactivity of basal keratinocyte of vocal folds epithelium in response to vibratory stress.

Stability of dental, alveolar, and skeletal changes after miniscrew-assisted rapid palatal expansion

  • Objective: Miniscrew-assisted rapid palatal expansion (MARPE) is a means for expanding the basal bone without surgical intervention in young adults. Here, we assessed the differences in dental, alveolar, and skeletal measurements taken before (T0), immediately after (T1), and 1 year after (T2) MARPE. Methods: Twenty-four patients (mean age, 21.6 years) who had undergone MARPE and cone-beam computed tomography at T0, T1, and T2 were included. Changes in the following parameters were compared using paired t-tests: intercusp, interapex, alveolar, nasal floor, and nasal cavity widths; inclination of the first molar (M1) and its alveolus; and thickness and height of the alveolar bone. A linear mixed-effects model was used to determine variables that affected periodontal changes in the M1. Results: MARPE produced significant increases in most measurements during T0-T2, despite relapse of some measurements during T1-T2. The alveolar thickness decreased on the buccal side, but increased on the palatal side. The alveolar crest level at the first premolar moved apically. Changes in the thickness and height of the alveolar bone were affected by the corresponding initial values. Conclusions: MARPE can be used as an effective tool for correcting maxillomandibular transverse discrepancy, showing stable outcomes 1 year after expansion.

A study on the morphological changes of lower incisor and symphysis during surgical-orthodontic treatment in skeletal class III malocclusion (악교정 수술을 받은 골격성 III급 부정교합자의 치료전후 하악전치부 치조골 형태변화에 대한 연구)

  • The purpose of this study was to evaluate the morphological changes of olveolar bone and mandibular symphysis of lower incisor by presurgical orthodontic treatment and orthognathic surgery in skeletal class III malocclusion. The sample consisted of 30 adult class III malocclusion patients who have received bilateral sagittal split mandibular osteotomy. Lateral cephalograms were taken before treatment, after presurgical orthodontic treatment and 3 months after orthognathic surgery. Skeletal and symphyseal measurements were compared and the relationships between them were analysed. The results were as follow : 1. The labial and lingual alveolar bone height in presurgical and postsurgical group were decreased than that of pretreatment group. 2. The vertical measurements of the craniofacial skeleton showed reverse correlationship with anteroposterior width of basal alveolar bone, but IMPA showed correlatiionship (p<0.01) 3. The craniofacial skeleton and the change of symphyseal measurements(symphyseal length, symphyseal width) showed no correlationship. 4. The labial alveolar bone height showed correlationship with lingual alveolar bone height(p<0.001), and negative correlationship with lingual alveolar crestal width(p<0.01). Labial and lingual alveolar crestal width has negative correlationship (p<0.05). Mandibular symphyseal length and width has positive correlationship(p<0.01). 5. IMPA, LISA showed negative correlationship with labiolingual alveolar bone height and lingual alveolar width and positive correlationship with labial alveolar base bone width.

Skeletal and dentoalveolar changes after miniscrew-assisted rapid palatal expansion in young adults: A cone-beam computed tomography study

  • Objective: The aim of this study was to evaluate the skeletal and dentoalveolar changes after miniscrew-assisted rapid palatal expansion (MARPE) in young adults by cone-beam computed tomography (CBCT). Methods: This retrospective study included 14 patients (mean age, 20.1 years; range, 16-26 years) with maxillary transverse deficiency treated with MARPE. Skeletal and dentoalveolar changes were evaluated using CBCT images acquired before and after expansion. Statistical analyses were performed using paired t-test or Wilcoxon signed-rank test according to normality of the data. Results: The midpalatal suture was separated, and the maxilla exhibited statistically significant lateral movement (p < 0.05) after MARPE. Some of the landmarks had shifted forwards or upwards by a clinically irrelevant distance of less than 1 mm. The amount of expansion decreased in the superior direction, with values of 5.5, 3.2, 2.0, and 0.8 mm at the crown, cementoenamel junction, maxillary basal bone, and zygomatic arch levels, respectively (p < 0.05). The buccal bone thickness and height of the alveolar crest had decreased by 0.6-1.1 mm and 1.7-2.2 mm, respectively, with the premolars and molars exhibiting buccal tipping of $1.1^{\circ}-2.9^{\circ}$. Conclusions: Our results indicate that MARPE is an effective method for the correction of maxillary transverse deficiency without surgery in young adults.

  • Rickets is not the deposite of minerals in the skeletal tissue and the retardation of skeletal growth in growing in growing animals. This study was undertaken to investigate the histologic effects of experimental rickets on the dental structure of the albino rats, and to show the relationship between the histological effects and the pulpal disease which induced premature loss of the primary teeth. This study was based on material obtained from 40 white rats that were placed on a rachitogenic diet for a period 1 to 56 days after weaning (at 24 days). In addition, a study was made of 25 litter mates, 24 to 80 days, that were fed a normal diet. The following results were obtained: 1. Enamel formation and calcification showed no significant changes and no hypoplasia. 2. Dentin formation and calcification was retarded and disturbed. In the experimental group, predentin/calcified dentin was remarkablly increased. 3. Newly formed dentin showed interglobular texture (less homogenous calcification) and the predentin was significantly wider and thicker, and there was an irregular wave in the basal portion of the rat's incisors. 4. In cementum, Matrix formed at almost a normal rate but calcification was defective. So cementoid tissue was increasesd. 5. The formation of the alveolar bone was at almost a normal rate but calcification was retarded. The trabecular bone was filled with osteoid tissue and thicker than in normal groups.

Outcome Evaluation with Signal Activation of Functional MRI in Spinal Cord Injury

  • Objective : The authors investigated the changes of cortical sensorimotor activity in functional MRI (fMRI) and functional recovery in spinal cord injury (SCI) patients who had been treated by bone marrow cell transplantation. Methods : Nineteen patients with SCI were included in this study; ten patients with clinical improvement and nine without. The cortical sensorimotor activations were studied using the proprioceptive stimulation during the fMRI. Results : Diagnostic accuracy of fMRI with neurological improvement was 70.0% and 44.4% for sensitivity and specificity, respectively. Signal activation in the ipsilateral motor cortex in fMRI was commonly observed in the clinically neurological improved group (p-value=0.002). Signal activation in the contralateral temporal lobe and basal ganglia was more commonly found in the neurological unimproved group (p-value<0.001). Signal activation in other locations was not statistically different. Conclusion : In patients with SCI, activation patterns of fMRI between patients with neurologic recovery and those without varied. Such plasticity should be considered in evaluating SCI interventions based on behavioral and neurological measurements.
