Usefulness of Tuberculin Skin Test by Tuberculin PPD RT23 2 TU (Tuberculin PPD RT23 2 TU를 이용한 투베르쿨린 피부반응 검사의 의의)

  • Background : Tuberculin skin test is a method to examine M. tuberculosis infection and has been used all over the world. But various factors make it difficult to understand testing results. In 2000, the American Thoracic Society recommended that skin test results should be decided by considering risk factors of the tested. In Korea, high tuberculosis infection rate and BCG vaccination rate make it difficult to differentiate current infection, past infection, and no infection by the skin test. This study was attempted to examine a negative predictive value of the skin test to understand how the skin test acts on deciding administration of anti-tuberculosis drug. Methods : From Mar. 1 to Jul. 31 in 2001, the test was performed for patients hospitalized in Department of Internal Medicine, Hallym University College of Medicine, Chunchon, Korea by administering Tuberculin PPD RT23 2 TU (0.1 ml)to them that has been currently used in Korea based on Mantoux method. They were decided to be infected with tuberculosis bacilli by following diagnostic standard: 1) tuberculosis bacilli was cultured in sputum by microbiological diagnostic standard or Acid-fast bacilli was proven on a microscopic examination or 2) tuberculosis bacilli was not proven in the aforesaid microbiological test by clinical diagnostic standard, while there was opinion or symptom suitable for tuberculosis by radiographic or histological standard so the doctor decided to apply the tuberculosis treatment. Results : In this study, total 210 patients except 20 patients (8.7%) among 230 hospitalized patients were evaluated. Their average age was 60±16.8 years, and male-female rate was 1.28 : 1 (male: 118, female: 92). Number of patient, who was diagnosed and decided as tuberculosis, was 53(25.2%). Pulmonary tuberculosis was found in 45 patients (84.9%); 22 patients were decided to be positive in the Acid-fast bacilli smear test by microbiological examination (culture positive: 13, culture negative: 9), and 23 patients were decided to be tuberculosis patients by clinical diagnosis standard. Tuberculosis pleuritis was found in 8 patients (15.1%); 4 patients were diagnosed and decided by histological standard, and 4 patients were decided and treated by clinical standard. In differentiating patients into 'Negative' and 'Positive' by the skin test standard of the American Thoracic Society, negative predictive value 92.3%, positive predictive value 47.3%, sensitivity and specificity were 83%, 68.8%, respectively. Conclusion : In hospitalized respiratory patients, there was high negative predictive vlaue 92.3% by tuberculin skin test, therefore skin test would be a important factor for deciding administration of anti-tuberculosis drug on negative skin test patient.

Effects of Temperature and Concentration on the Rheological Properties of the Biopolymer Produced by Bacillus sp. K-1 Strain and Mutants (Bacillus sp. K-1과 변이주들에 의해 생산된 Biopolymer의 물성에 미치는 온도 및 농도의 영향)

  • The rheological Voperties of biopolymers produced by Bacilli sp. K-1 and its mutant strains(KM-21, KM-83) were studied at the temperature ranges with 20∼80$^{\circ}C$, at the concenration of 2∼6%, at the pH ranges from 3.0 to 9.0 and at the shear rate of 9.3-930sec-1 The apparent viscosity of biopolymers was decreased with increasing shear rate, and thereby biopolymers showed pseudoplastic characteristics. It was found that the apparent viscosity models respected 19 temperature, concentration and both temperature and concentration were expressed by Arrhenius Model, Exponential Model and combined of the above two Models. Therefore, the apparent viscosity could be predictable by Arrhenius and Exponential Models with high R2.

Detection and Identification of Bacteriocins Produced by Propionibacteria Isolated from Commercial Swiss Cheese Products

  • Wild propionibacteria isolated from different commercial swiss cheese samples were tested for antimicrobial activities. In initial screening, six of these Propionibacterium isolates showed antagonistic activity against 10 selected indicator organisms by the deferred method. In next, only two Propionibacterium strains JW6 and JW14 showed antibacterial activity in the cell-free supernatants by the modified well diffusion method. Propionibacterium strains JW6 and JW14 were finally identified as bacteriocin producers which exhibited a bactericidal effect against closely related species. The antimicrobial substances were proteins, since their activities were completely destroyed following several degradative enzyme treatments. The bacteriocins showed a narrow inhibitory spectrum of activity against two propionibacteria and two bacilli of strains tested in this study.

Isolation of Bacillus amyloliquefaciens Strains with Antifungal Activities from Meju

  • Bacilli with fibrinolytic activities were isolated from traditionally-prepared Meju and some of these strains showed strong antifungal activities. One isolate, MJ1-4, showed the strongest antifungal activity. MJ1-4 and other isolates were identified as B. amyloliquefaciens strains by recA gene sequencing and RAPD-PCR results. B. amyloliqufaciens MJ1-4 efficiently inhibited an Aspergillus spp.-producing aflatoxin B1 ($AFB_1$) and a Penicillium spp.-producing ochratoxin (OTA) in addition to other fungi. Antifungal activity of B. amyloliquefaciens MJ1-4 culture reached its maximum (40 AU/mg protein) in LB or TSB medium around 48 hr at $37^{\circ}C$. Antifungal activity of the concentrated culture supernatant was not decreased significantly by protease treatments, implying that the antifungal substance might not be a simple peptide or protein. Considering its antifungal and fibrinolytic activities together, B. amyloliquefaciens MJ1-4 can serve as a starter for fermented soyfoods such as Cheonggukjang and Doenjang.

Effects of microbial fertilizers on the yield and major characteristics of altari radish (알타리무 재비시 토양미생물제 처리가 수량 및 주요형질에 미치는 영향)

  • This study was conducted to investigate the effects of microbial fertilizers on the yield of altari radish, chemical components of plant and soil, and the microbial floras. Six microbial fertilizers, MPK+Husk+Palma, Husk+Palma, MPK+Compost, Compost, Bio livestock cattle system (BLCS) cattle dropping, and Tomi, were used in this experiment. The yield of altari radish was increased in treatments of all microbial fertilizers. The concentration of all chemical components in plant were not significantly different. Whereas the amount of $P_2$$O_{5}$ in soil was increased in Compost treatment, the other components were not significantly different compared with control. The total bacteria and bacilli in soil were increased in Tomi, Husk+Palma, and MPK+Husk+Palma treatments, and actinomycete and fungi were increased in Tomi treatment.

  • studied on the physicochemistrical property of the clean active water depositting on the ceramic stone by the mineral-Oriental medicine. The results are as follows. The cluster of the clean active water is observed 53-66.5㎐ for contral water. The clean active water is observed 7.2 ph and 1.3330 refractive index for contral water. In the chemical experimental test by ICP, the clean active water is observed 102℃ in boiling point and 4.8㎎/100g V.E for 0㎎g/100g of contral water. In the antibiotic experimentation, an anion, the dimintion index of bacilli, radiation degree and radiate energy of the clean active water increased for contral water. And in the deodorization experimentation, the clean active water is observed 95% deodorization index.

Mediastinitis Caused by Prevotella Intermedia/Nigrescens Occurred after Acupuncture -A case report- (침술후 발생한 Prevotella intermedia/nigrescens에 의한 종격동염 -치험1례-)

  • A 54-year-old male patient was admitted due to dyspnea with chest pain and reddish swelling in the right lteral neck and right upper which developed 2 or 3 days age. He was treated with acupuncture in the neck about one week ago prior to admission. CT scans of the chest showed density of air and abnormal soft tissue which suggested abscess of the anterior mediastinum and subcutaneous tissue. He underwent cervical and mediastinal drainage with closed thoracostomy and antibiotic therapy. Black-pigmente anaerobic nonspore-forming gram-negative bacilli were isolated from the mediastinal pus and were identified as Prevotella intermedia/nigrescens upon performing biochemical tests and API rapid ID 32A (bioMeriux, France) kit. He underwent decortication of the right side because of loculated empyema on 41st postoperative day. He was fully recovered and discharged on the 82nd hospital day. This was thought to be another case of descending necrotizing mediastinitis not caused by peritonsilar abscess but by cellulitis developed after acupuncture. Early deterction of mediastinits and aggressive drainage of mediastinal abscess are important.

Development of Biofilter System for Ammonia Removal in Livestock Facility (축산 시설의 암모니아 가스 제거용 바이오 필터 시스템 개발)

  • The purpose of this study was to develop a pilot scale bio-filter system removing ammonia gas with microorganisms. The system consisted of chaff(filter medium), a blower, a temperature sensor, a moisture sensor, a solenoid valve, and a heating system. Temperature and moisture contents were controlled via a PC to provide the microorganisms with proper environment. The microorganisms used in this study were Bacillius. coagulans NLRI T-6 and Pseudononas. putida NLRI S-21 of bacilli. Performance tests were performed to evaluate gas removal rate during 20 days. The result was shown that the removal rate was high in early days and gradually dropped below 90% without injecting the microbes. However, it was shown that when injecting the microbes, the removal rate was almost 100% and pH value was maintained at between 7 and 9 during the whole twenty-day period.

Antibiotic Resistance Survey of Gram Negative Bacilli in Daejeon Area (대전지역에서 그람음성간균의 항생제 내성률 조사)

  • During 2006, the antibiotic resistance rate were investigated in gram negative bacteria. Resistance to piperacillin were detected at 60% in Escherichia coli, and 37% in Klebsiella pneumoniae. Ceftriaxone were detected 58% in Enterobacter cloacae, 52% in Acinetobacter baumannii, 43% in Enterobacter aerogenes and 76% were detected in Serratia marcescens. Between 1998 and 2007 antibiotic resistance rate were decreased in seven types antibiotic drugs. but, ceftazidime were increased from 12 to 20% during this times. In addition, E. coli, E. cloacae, A. baumannii and E. aerogenes were more isolated from May to June and K. pneumoniae and S. marcescens were more isolated from July to September. We should monitor and control antibiotic use and regularly survey antibiotic resistance patterns among pathogens in the hospital.

Four Cases of Cutaneous Tuberculosis (최근 경험한 피부결핵 4례)

  • The incidence of the cutaneous tuberculosis has shown a steady decline over the past decades. This parallels the decreasing incidence of pulmonary tuberculosis. We experienced 5 cases of cutaneous tuberculosis from January 1990 to February 1991. We present herin 4 cases of cutaneous tuberculosis. They were 3 cases of vulgaris and 1 case of tuberculosis verrucosa cutis. Mantoux tests were done except one case and were reactive in all cases. Culture for Mycobactelium tuberculosis were done but Mycobacterium tuberculosis were not cultivated in the all cases. Histopathological findings showed tuberculoid granulomas in the dermis except one case and no acid fast bacilli were demonstrated on AFB stains.
