Artificial Insemination in Infertile Couples with Abnormal Findings of Sperm (이상정자소견을 갖은 불임환자에서 인공수정)

  • Artificial insemination with donor sperm(AID) or husband sperm(AIH) has been a major form of treatment for the infertile couples with severe male factors. The conception rate in oligoasthenoteratozoospermia is very low. Therefore the aim of this study was to examine those factors associated with improved success rate. The results are obtained as the follows; 1. The husband semenalysis(n=639) revealed normal findings in 32.2%. The abnormal findings included the oligospermia(18.5%), oligoasthenospermia(20.7%), oligoasthenotetatozoospermia(8.5%) and azoospermia(20.1 %). 2. The causes of abnormal semenalysis are idiopathic(most common), varicocele, congenital anomalies, vaso-vasostomy, etc.. 3. The semen washing to improve sperm motility and concentration was effective in case of over 20 ${\times}lO^6$/ml sperm count, but there was no significant improvement in case of be low 10-5${\times}lO^6$/ml sperm count. 4. The improvement of motility after cryopreservation depended on the initial sperm concentration. 5. The pregnancy rates following AIH are higher in normospermia than oligospermia.

Induction of Apoptosis in the Testes of SD Rats After Exposure to 2-Bromopropane

  • Exposure to 2-Bromopropane has been known to cause degeneration of male germ cells. However the mechanism underlying this process is poorly understood. The objective of this study was to determine whether or not the exposure of male Sprague-Dawley rats to 2-BP induces apoptosis in male germ cells. Male rats(N=3 or 4 in each group) were orally administered either with the corn oil vehicle (10 ml/kg body weight) or with 2-BP (3,500 mg/kg) once a day for 3 days. The presence of apoptosis was determined by TUNEL detection in situ and by an increase in DNA fragmentation. A low spontaneous incidence of apoptosis was observed in vehicle control animals, especially in pre-meiotic germ cells of stages I-VI and stages XII-XIV the seminiferous tubules. In 2-BP exposure rats, the incidence of apoptosis markedly increased at 4 h, reached a peak at 8 h (about 7-fold over control), and then decreased rapidly to control level by 48 h after the last administration. Although apoptosis induced by 2-BP occurred in all stages of germ cells, it was most pronounced in spermatogonia and early spermatocytes in stages I-VI and stages XII-XIV. Taken together, our results suggest that apoptosis is involved in the toxicity of testicular germ cells resulting in oligospermia or azoospermia after exposure to 2-BP.

Prevalence of Y chromosome microdeletions among infertile Mongolian men

  • Objective: Y chromosome microdeletions are the second most common genetic cause of male infertility after Klinefelter syndrome. The aim of this study was to determine the patterns of Y chromosome microdeletions among infertile Mongolian men. Methods: A descriptive study was performed on 75 infertile men from February 2017 to December 2018. Y chromosome microdeletions were identified by polymerase chain reaction. Semen parameters, hormonal levels, and testis biopsy samples were examined. Results: Among 75 infertile men, two cases of Y chromosome microdeletions were identified. The first case had an AZFa complete deletion and the other had an AZFc partial deletion. This study found that the proportion of Y chromosome microdeletions among infertile Mongolian men was 2.66%. Conclusion: The findings can be applied to in vitro fertilization and assisted reproductive technology, and our results will help clinicians improve treatment management for infertile Mongolian couples.

Effects of Y Chromosome Microdeletion on the Outcome of in vitro Fertilization (남성 불임 환자에서 Y 염색체 미세 결손이 체외 수정 결과에 미치는 영향)

  • Objective: To determine whether the presence of Y-chromosome microdeletion affects the outcome of in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) program. Methods: Fourteen couples with microdeletion in azoospermic factor (AZF)c region who attempted IVF/ICSI or cryopreserved and thawed embryo transfer cycles were enrolled. All of the men showed severe oligoasthenoteratoazoospermia (OATS) or azoospermia. As a control, 12 couples with OATS or azoospermia and having normal Y-chromosome were included. Both groups were divided into two subgroups by sperm source used in ICSI such as those who underwent testicular sperm extraction (TESE) and those used ejaculate sperm. We retrospectively analyzed our database in respect to the IVF outcomes. The outcome measures were mean number of good quality embryos, fertilization rates, implantation rates, $\beta$-hCG positive rates, early pregnancy loss and live birth rates. Results: Mean number of good quality embryos, implantation rates, $\beta$-hCG positive rates, early pregnancy loss rates and live birth rates were not significantly different between Y-chromosome microdeletion and control groups. But, fertilization rates in the Y-chromosome microdeletion group (61.1%) was significantly lower than that of control group (79.8%, p=0.003). Also, the subgroup underwent TESE and having AZFc microdeletion showed significantly lower fertilization rates (52.9%) than the subgroup underwent TESE and having normal Y-chromosome (79.5%, p=0.008). Otherwise, in the subgroups used ejaculate sperm, fertilization rates were showed tendency toward lower in couples having Y-chromosome microdeletion than couples with normal Y-chromosome. (65.5% versus 79.9%, p=0.082). But, there was no significance statistically. Conclusions: In IVF/ICSI cycles using TESE sperm, presence of V-chromosome microdeletion may adversely affect to fertilization ability of injected sperm. But, in cases of ejaculate sperm available for ICSI, IVF outcome was not affected by presence of Y-chromosome AZFc microdeletion. However, more larger scaled prospective study was needed to support our results.

Effect of Human Follicular Fluid (hFF) and Temperatures on the Motility of Testicular Sperm Cultured In Vitro (고환 내 정자의 체외배양 중 운동성에 미치는 인간 난포액과 온도의 영향)

  • The aim of this study was to evaluate the improvement of testicular sperm motility following different culture conditions such as human follicular fluid (hFF) and temperature. Testicular tissues obtained from azoospermia (n=21) were minced into small pieces by blade and recovered sperm suspension were cultured in Ham's F10 with or without 40% hFF at different temperatures (Group I: 37$^{\circ}C$/with hFF, Group II: 32$^{\circ}C$/withGroup III: 37$^{\circ}C$/without, Group IV:32$^{\circ}C$ /without The motility and viability of sperm were monitored during culture for 48 hours. Initial motility of testicular sperm was 10.9$\pm$1.9%. After 24 hours culture, sperm motility was 23.5$\pm$2.1% (Group I), 8.1$\pm$1.1% (Group II), 10.4$\pm$ 1.4% (Group III) and 4.0$\pm$0.8% (Group IV), respectively. After 48 hours, the motility had been changed as 32$\pm$2.3% (Group I), 14.3$\pm$1.7% (Group II), 5.3 $\pm$1.4% (Group III) and 4.3$\pm$0.9% (Group IV). In hFF group (I and II), sperm motility of group I cultured at 37$^{\circ}C$ was higher than those of group II at 32$^{\circ}C$. But, sperm viability of group I cultured at 37$^{\circ}C$ was lower than those of group II at 32$^{\circ}C$ (54.4$\pm$4.1% vs. 59.4$\pm$3.7%) after cultured for 48 hours. We acquired the best motility of testicular sperm when performed in vitro culture for 48 hours in hFF supplemented medium at 37$^{\circ}C$. Increase of sperm motility by in vitro culture could be useful tool fur human TESE-ICSI program.

Analysis of Semen Parameters in α1,3-Galactosyltransferase-/- Boars

  • It is very difficult to get the information about semen quality analysis in transgenic pigs because of limited numbers and research facilities. Therefore, in the present study, we analyzed the semen quality of transgenic boars generated for xenotransplantation research. Briefly, the semen samples were collected from 5 homozygous ${\alpha}1,3$-Galactosyltransferase knock-out ($GalT^{-/-}$) transgenic boars and immediately transported to the laboratory. These semen samples were decupled with DPBS and conducted to analyze semen parameters by a computer-assisted semen analysis (CASA) system. The boar semen were examined all 12 parameters such as total motility (TM), curvilinear velocity (VCL), straight line velocity (VSL), average path velocity (VAP), and hyperactivated (HYP), etc. In results, among the 5 $GalT^{-/-}$ boars, three boars (#134, 144, and 170) showed normal range of semen parameters, but #199 and 171 boars showed abnormal ranges of semen parameters according to standard ranges of semen parameters. Unfortunately, #171 boar showed azoospermia symptom with rare sperm counts in the original semen. Conclusively, assessment of semen parameters by CASA system is useful to pre-screening of reproductively healthy boar prior to natural mating and artificial insemination for multiplication and breeding.

Studies on Liquefaction of Semen (정액(精液)의 액화(液化)에 관한 연구(硏究))

  • The human semen ejaculated in a form of liquid state, coagulates immediately after ejaculation, and then liquefies again. However, the mechanisms of neither coagulation and liquefaction of semen have not been explained clearly so far, and very limited numbers of report are available, although the spermatology and andrology made rapid progress. This clinical study has been undertaken to investigate the liquefaction phenomena and practicability of the results might be applied to fertility and infertility problems. As a preliminary study, in this report the liquefaction time of various semen groups is measured and analysed. The following results are obtained: 1. An average liquefaction time of semen of a total of 60 subjects: 25 minutes. 2. An average liquefaction time of semen according to sperm count: 1) Normospermia group (20 cases): 34 minutes. 2) Oligospermia group (20 cases): 21 minutes. 3) Azoospermia group (20 cases): 20 minutes. 3. An average liquefaction time of semen according to abstinence period: 1) Less than 3 days group (30 cases): 22 minutes. 2) More then 5 days group (30 cases): 28 minutes. In conclusion: 1. The liquefaction time of semen of the normospermia group is longer than oligospermia group or azoosermia group. 2. The liquefaction time of semen may not be greatly influenced by the various factors such as abstinence period, semen volume, semen pH, age of the subjects and so on. 3. In routine semen analyses, it is recommended to begin the analysis at least 25 minutes after the ejaculation. 4. Further studies are required in conjunction with practical application of liquefaction mechanism in infertility and fertility control.

Complications and Difficulties in Alternative Methods of Conception (인공수태(人共受胎)의 합병증(合倂症)과 난점(難點)에 관(關)한 연구(硏究))

  • Alternative methods were used in the field of infertility as the latest means for the treatment of misconception. Artificial insemination, which has been commonly used already, IVF-ET and GIFT are also used as artificicial method for conception presently. Appling such methods to the patient, following three categories should be considered; first, there should be an understanding and an agreement from the patient, second, if possible, reduce the cost of hospital expense and relieve physical and emotional problems of the patient, and third, increcrsing the pregnancy rate. Under these considerations, complication and pitfalls of artifical inseminations are a big burden to the physian and the patient. Though the conicicental complications are relatively very rare, detailed laboratoy tests, carefull examinations, and follow-up studies are necessary. In the distribution of the age groups, 40.3% was in the 25 to 29 years group, 30 to 34 years was 45%. 35 to 39 years was 8% and only 3.2% was above the 40 age groups. The range of infertile periods were from 1 to 17 years, and half of the patients were over 5 years, the other group 20% in 3 years and 2 to 4 years were 10%. Among 159 cycles of artificial insemination, there were complications such as infection(1.9%) and discomfort(5%) and abnormal bleeding (0.6%). During pregnancy, clinical abortion (1.9%) and toxemia (2.5%) were shown. Sex of new born infants were, male (68%) and female (31%). Fortunately, there were no cases of psychological complication and genetic abnormality. Indications of artificial insemination for male factors were aspermia (2.5%), azoospermia (28.8%), oligospermia (26.4%) and asthenozoospermia (1.8%), for female factors were irregular cycle (11.7%) and dysmucorrhea.

The Effect of Microsurgical Epididymal Sperm Aspiration (MESA) and Intracytoplasmic Sperm Injection (ICSI) as a Treatment for Obstructive Azoosperimia (폐색성 무정자증환자의 치료로써 미세적 부고환 정자흡입술(MESA) 및 정자직접주입법(ICSI)의 효과)

  • To present and assess the efficacy of combination of microsurgical epididymal sperm aspiration(MESA) and intracytoplasmic sperm injection(ICSI) for the treatment of infertility due to unreconstructable obstructive azoospermia or congenital bilateral agenesis of vas deferens (CBAVD), MESA was performed in the 45 husbands ( 16 CBAVD, 29 unreconstructable genital tract obstruction), followed by ICSI of oocytes recovered from the wives hyperstimulated by GnRH agonist in combination with hMG and FSH. Cleaving embryos were transfered to the uterine cavity or follopian tube(ZIFT) 18 or 24 hours after ICSI procedure. In 45 cycles of MESA, 492 oocyte complexes were recovered. ICSI was carried out on 355 metaphase II oocytes and 226 oocytes (63.7%) showed normal two pronucleus fertilization. After 198 embryos were transferred in 43 cycles, an average of 5 per cycle, 20 patients presented a positive HCG and intrauterine pregnancy was confirmed by US. So, the clinical ongoing pregnancy rate per transfer was 46.5%. Until now, 8 patients have given birth to 9 babies, 5 male and 4 female, including 1 twin. The babies were all healthy except 1 twin female baby. There was 1 miscarriage at 7 weeks and chromosomal study of abortus revealed as 45X, monosomy. These results suggested that it was possible to achieve high normal fertilization and pregnancy rate by ICSI using epididymal sperm.

Effect of 19-norandrostenedione on Steroidogenesis in Rat Testis (19-norandrostenedione이 흰쥐 정소내 스테로이드 대사에 미치는 영향)

  • 19-norandrostenedione(19-NORA) is known as an intermediate in the metabolic pathway from androstenedione to estrone. Administration of esterified 19-nortestosterone, anabolic steroid, reduces serum gonadotropin and testosterone concentration, and results in reversible azoospermia in men. 19-NORA have been isolated from testis, but its function in testis is not clear yet. Therefore, this study was designed to determine the effect of 19-NORA on steroidogenesis and on spermatogenesis. 19-NORA was administrated by single intratesticular injection to adult male rats weighing 350-400 g in dose of 1 mg/50${\mu}l$. The serum and testis were collected on 1, 3, 7, 12, 48 hr after injection. The histological differences in testis were observed by routine paraffin method. The concentrations of testosterone and estradiol in serum and in left testis were determined by the conventional radioimmunoassays. One hour after 19-NORA treatment, serum concentrations of testosterone and estradiol increased significantly, compared to those of pre-treated(0 hr) group, and reduced gradually to the control level on 7 hour after injection. The concentration of testosterone in left testis increased slightly 1 hour after injection, and estradiol level increased significantly(p<0.05). Also, testosterone and estradiol level of control group revealed no difference with pre-treated (0 hr) group. Gonad index, structure of seminiferous tubules, and the number of step 7 th spermatid were simillar to control group. The present study suggests that the elevation of testosterone level results from increment of estradiol followed by the rapid metabolism of 19-NORA at 7 hour after injection, and then testosterone concentration may be recovered to control level by feedback mechanism of hypothalamus-hypothysis-testis axis.
