단안에 국소적으로 발현된 색소정맥옆망막맥락막 위축 1예 (A Case of Unilateral Focal Pigmented Paravenous Retinochoroidal Atrophy)

  • 목적: 단안에 국소적으로 발생한 색소정맥옆망막맥락막 위축(pigmented paravenous retinochoroidal atrophy) 1예를 경험하여 이를 보고하고자 한다. 증례요약: 46세 여자 환자가 건강검진상에서 발견된 우안 망막 이상 소견으로 내원하였다. 나안시력은 양안 모두 1.0이었다. 세극등현미경으로 시행한 전안부 검사상 특이 이상 소견은 관찰되지 않았다. 안저검사상 우안의 상측 망막정맥을 따라 국소적으로 분포된 뼈 조각 모양의 색소침착이 동반된 망막맥락막위축 소견이 관찰되었다. 안저자가형광상 우안의 망막맥락막위축의 경계에서 다소 과형광을 보이면서 병변의 대부분은 저형광 소견을 보였다. 빛간섭단층촬영상 해당 병변은 망막색소상피층의 위축 및 뭉침이 혼재되어 있었고, 맥락막층에서 맥락막모세혈관층이 얇아진 소견을 보였다. 형광안저촬영상 병변 내 창문비침 및 형광차단이 혼재되어 관찰되었으며 이는 망막색소상피층의 위축 및 뭉침 부위와 일치하였다. 결론: 단안에 국소적으로 발생한 색소정맥옆망막맥락막 위축 1예를 경험하여 이를 국내 최초로 보고하는 바이다.

Hippocampal Sclerosis: Correlation of MR Imaging Findings with Surgical Outcome

  • Objective: Atrophy and a high T2 signal of the hippocampus are known to be the principal MR imaging findings of hippocampal sclerosis. The purpose of this study was to determine whether or not individual MRI findings correlate with surgical outcome in patients with this condition. Materials and Methods: Preoperative MR imaging findings in 57 consecutive patients with pathologically-proven hippocampal sclerosis who underwent anterior temporal lobectomy and were followed-up for 24 months or more were retrospectively reviewed, and the results were compared with the postsurgical outcome (Engel classification). The MR images included routine sagittal T1-weighted and axial T2-weighted spin-echo images, and oblique coronal T1-weighted 3D gradient-echo and T2-weighted 2D fast spin-echo images obtained on either a 1.5 T or 1.0 T unit. The images were visually evaluated by two neuroradiologists blinded to the outcome; their focus was the presence or absence of atrophy and a high T2 hippocampal signal. Results: Hippocampal atrophy was seen in 96% of cases (55/57) [100% (53/53) of the good outcome group (Engel class I and II), and 50% (2/4) of the poor outcome group (class III and IV)]. A high T2 hippocampal signal was seen in 61% of cases (35/57) [62% (33/53) of the good outcome group and 50% (2/4) of the poor outcome group]. All 35 patients with a high T2 signal had hippocampal atrophy. 'Normal' hippocampus, as revealed by MR imaging, occurred in 4% of patients (2/57), both of whom showed a poor outcome (Engel class III). The presence or absence of hippocampal atrophy correlated well with surgical outcome (p<0.01). High T2 signal intensity did not, however, significantly correlate with surgical outcome (p>0.05). Conclusion: Compared with a high T2 hippocampal signal, hippocampal atrophy is more common and correlates better with surgical outcome. For the prediction of this, it thus appears to be the more useful indicator.

하지에 생긴 양성 일지성 근위축 2예 (Two Cases of Benign Monomelic Amyotrophy of the Lower Extremities)

  • Benign monomelic amyotrophy(BMA) is an uncommon cause of progressive mildly disabling atrophy and weakness of a limb. It predominantly affects the distal upper limb of young men. I present two men with BMA of the lower extremities. Although the disorder seemed clinically confined to a arm or leg, I confirmed by electromyography evidence of denervation in the other extremities. I reviewed the literatures and discuss the differential diagnosis. BMA is a diagnosis of exclusion that requires consideration in men with unilateral arm or leg atrophy and weakness.

Dermatomyositis sine dermatitis, a rare phenotype of idiopathic inflammatory myopathy

  • Dermatomyositis (DM) is characterized by progressive proximal limb weakness and typical skin manifestations. The histological findings that show perifascicular atrophy and deposition of membrane attack complex are pathognomic features of DM. Dermatomyositis is categorized into classical DM and non-classical DM, which includes amyopathic DM and DM sine dermatitis. DM sine dermatitis is seldom described because of its rarity, making the diagnosis more challenging. We report a case of DM sine dermatitis, a rare phenotype of DM.

  • Amyotrophic lateral sclerosis Is an unusual pattern of distal muscular atrophy with permanent bulbar sign. Vocal fold paralysis and velopharyngeal insufficiency(VPI) due to soft palate paralysis Is occasionally associated with distal muscular atrophy. Recently we experienced a case of amyotrophic lateral sclerosis whose symptom was initially expressed with hoarseness and VPI. So we report a case with review of literature.

흰쥐의 탈신경근 위축에 미치는 운동의 효과 (Exercise Effects on the Atrophy of Denervated Muscles in Rat)

  • The purpose of this study was to determine the effects of mild-intensity exercise training on the denervated muscle atrophy in the sciatic nerve injured rat. Thirty-six male Sprague-Dawley rats (250~300 g) were randomly assigned into three groups; sham-denervated group (n=8), denervated group (n=8), and denervated-exercised group (n=20). Exercise consisted of treadmill running at 20 m/min speed with 0% grade for 30 min/day. The animals were decapitated at the second and sixth weeks postcrush. Soleus and medial gastrocnemius were immediately excised to be weighed. Type I and II fibers of the muscles were differentiated by m-ATPase (pH 9.4) stain, and fiber diameters were evaluated. The results were as follows: 1) The weight of the soleus and medial gastrocnemius muscles showed a tendency to increase in both the denervation-exercised groups compared to the denervated group. 2) In the 2-week denervation-exercised group, type II fiber diameter of soleus and type I fiber diameter of medial gastrocnemius were increased significantly compared to the denervated control group. 3) In the 6-week denervated-exercised group, type I fiber diameter of soleus and type II fiber diameter of medial gastrocnemius were hypertrophied significantly compared to sham-denervated group. The results of this study suggested that treadmill exercise partially prevented denervation atrophy in the soleus and medial gastrocnemius of the rat.

보행장애를 호소하는 다계통위축증 환자 1례 (A Case of Korean Medical Treatment on Multiple System Atrophy with Gait Disturbance)

  • This case report describes the effects of Korean Medicine for the gait disturbance of Multiple System Atrophy (MSA). Korean Medicine that included chuna manual therapy, pharmaco-acupuncture, herbal medicine, acupuncture, electro-acupuncture, and moxibustion was administered for 23 days. During the hospitalization period, evaluations were performed using the Unified Multiple System Atrophy Rating Scale (UMSARS), Berg Balance Scale (BBS), and 20 m Round Timed Walking Test on day 1, day 15, and day 23. The UMSARS Part I score decreased from 11 to 5, Part II score from 11 to 9, and total score from 22 to 14, and the BBS score increased from 36 to 48. The 20 m Round Timed Walking Test score also improved. The width of the gait decreased from 65 to 40 cm, and the number of staggering incidents decreased from 6 times to 2 times. The findings of this case study suggest that Korean Medicine, including Chuna manual therapy, has potential benefits in MSA patients with gait disturbance.

Dexamethasone에 의하여 유발된 근육 위축 생쥐의 비복근 근섬유에서 apoptosis와 염증 반응에 미치는 오미자 추출물의 영향 (Effects of Schisandrae Fructus Supplementation on Apoptosis and Inflammatory Response in Gastrocnemius Muscle of Dexamethasone-Induced Muscle Atrophy Mice)

  • Dried fruits of Schizandra chinensis Baillon, Fructus Schisandrae, have been widely used for many years to prevent and treat various diseases in Asian countries including Korea and Russia. It has recently been reported that extracts of Fructus Schisandrae are effective for controlling muscle and skeletal diseases. In this study, we investigated the efficacy of ethanol extract of Fructus Schisandrae (EEFS) on apoptosis and inflammatory response in gastrocnemius muscle of dexamethasone-induced catabolic muscle atrophy mice as part of natural substance discovery and functional analysis for improving muscle function. According to the results of this study, EEFS supplementation attenuated body weight gains and suppressed calf thickness loss in dexamethasone-induced muscle atrophic mice. Gastrocnemius muscle immunohistochemistry showed that expression of caspase-3 and poly(ADP-ribose) polymerase, which are representative apoptotic markers, was markedly increased in dexamethasone control mice; however, their expression was effectively reduced in the EEFS-fed mice. EEFS supplementation also prevented dexamethasone-induced increases in immunoreactivity of muscle fibers for myostatin, an important negative regulator of skeletal muscle mass. In addition, EEFS significantly normalized the increased numbers of nitrotyrosine, 4-hydroxynonenal and inducible nitric oxide synthase-positive muscle fibers compared to that found in dexamethasone control mice. These results suggest that EEFS protects dexamethasone-induced muscular atrophy by decreasing apoptosis and inflammatory responses, and EEFS is more likely to be developed as a muscle strengthening agent.

대영전(大營煎)이 불용성 근위축에서의 apoptosis 관련 단백질들의 발현변화에 미치는 영향 (The Effects of Daeyeoung-jeon on the Prevention of Disuse Muscle Atrophy in Rats)

  • Objectives : Skeletal muscle atrophy occurs in response to a variety of conditions. The unloading to muscle occurs clinically in limb immobilization, bed rest, spinal cord injury and peripheral nerve damage, resulting in significant loss of muscle mass and force production. Muscle disuse is accompanied by an increase in apoptotic signaling, which mediates some of the responses to unloading in the muscle. In this study we tested the hypothesis that Daeyeoung-jeon extract would improve muscle recovery after reloading following disuse. Method : Twenty young male Sprague-Dawley rats were used for the studies. The hindlimb immobilization was performed with casting tape to keep the left ankle joint in a fully extended position. No intervention was performed on the right leg and used as intact region. The Rats in Daeyeoung-jeon treated group (DYJ) were orally administrated Daeyeoung-jeon water extract, and rats of Control group were given with saline only. After 2 weeks of immobilization, all animals were sacrificed, and the whole gastrocnemius muscles were dissected from both legs. The morphology of right and left gastrocnemius muscles in both DYJ and Control groups were assessed by hematoxylin and eosin staining. Moreover, to investigate the immobilization-induced muscular apoptosis, the immunohistochemical analysis of Bax and Bcl-2 was carried out. Results : Daeyeoung-jeon represented the significant protective effects against the reductions of the left gastrocnemius muscles weight and average cross section area to compared with Control group. The treatment with Daeyeoung-jeon extract significantly reduced the immunoreactivity of BAX and increased the immunoreactivity of Bcl-2 in gastrocnemius muscle compared with Control group. Conclusion : Daeyeoung-jeon has protective effects against immobilization-induced muscle atrophy by regulating the activities of apoptosis-associated BAX/Bcl-2 proteins in gastrocnemius muscle.
