임신 중 곰팡이 노출로 아토피피부염 발병에 미치는 영향 (Effect of mold exposure during pregnancy on the development of offspring's atopic dermatitis)

  • Background: Atopic dermatitis one of the most common chronic skin diseases, is caused by various environmental and genetic factors. Methods: A total of 2609 healthy newborns who were enrolled in the COCOA study (COCOA) from 2008 to 2015 were surveyed for indoor environmental exposure to fungi during gestation and then diagnosed postnatally for atopic dermatitis. The fungi collected during the gestation of 20 normal subjects and 20 infants that developed atopic dermatitis were identified using Illumina's MiSeq platform and analyzed for their diversity and species. Results: A total of 2,609 respondents were surveyed (52.8% male and 47.2% female) Children, 1, 2, and 3 years old diagnosed with atopic dermatitis comprised 15.2%, 15.7%, and 14.1% of the respondents, respectively. The prevalence of exposure to mold during gestation was 1.46 (95% CI, 1.05-2.04) and 1.52 (95% CI, 0.95-2.43), in the first and third years after birth, respectively. One-year-old children with atopic dermatitis and no fungal markers detected in the bathroom environment during gestation accounted for less than 5% (aOR, 1.51; 95%CI, 0.96-2.38) and in the group less than 5 ~ 30% (aOR, 2.21; 95%CI, 1.00-4.89), 3-year-old children had an increased prevalence of atopic dermatitis of more than 30% (aOR, 9.48, 95%CI 1.42-63.13). Conclusions: Exposure to indoor fungi during gestation and infancy is associated with the development of atopic dermatitis in children. The phyla and genera of the fungi in the indoor house dust differed during gestation. This suggests that exposure to indoor fungi during gestation may be associated with the development of atopic dermatitis in children. Future research will be necessary to establish the underlying mechanisms.

  • Many patients assume that allergic reactions against foods are responsible for triggering or worsening their allergic symptoms. Therefore, it is important to identify patients who would benefit from an elimination diet, while avoiding unnecessary dietary restrictions. The diagnosis of food allergy depends on the thorough review of the patients's medical history, results of supplemented trials of dietary elimination, and in vivo and in vitro tests for measuring specific IgE levels. However, in some cases the reliability of such procedures is suboptimal. Oral food challenges are procedures employed for making an accurate diagnosis of immediate and occasionally delayed adverse reactions to foods. The timing and type of the challenge, preparation of patients, foods to be tested, and dosing schedule should be determined on the basis of the patient's history, age, and experience. Although double-blind, placebo-controlled food challenges(DBPCFC) are used to establish definitively if a food is the cause of adverse reactions, they are time-consuming, expensive and troublesome for physician and patients. In practice, An open challenge controlled by trained personnel is sufficient especially in infants and young children. The interpretation of the results and follow-up after a challenge are also important. Since theses challenges are relatively safe and informative, controlled oral food challenges could become the measure of choice in children.

  • Atopic dermatitis, caused by immune hyper-responsiveness, is wide spread in humans as well as in the dogs, especially in industrialized condition. Pet dogs are generally exposed to the same environment as their owners, and a significant portion of these animals are also known to suffer from this allergic disease. However, diagnosis and treatment methods of atopic dermatitis in animals have not been well established. We explored the possibility of using recently developed PG102 for the treatment of atopic dermatitis in the canine population. PG102 is a water soluble extract prepared from Actinidia arguta, and has been shown to produce significant therapeutic effect in variable allergy animal models. After oral administration of PG102 at a dose of 12.5 mg/kg daily for 4 weeks, severity of disease was greatly improved. IgE is one of representative members used to diagnose allergic diseases in humans. However, it is not well established whether there is any correlation between the serum level of IgE and atopic dermatitis. Our data indicated that dogs diagnosed to have atopic dermatitis contained higher level of serum IgE than the normal dogs and that treatment of dogs with PG102 significantly lowered the serum level of IgE. Taken together, this study demonstrated that PG102 treatment yielded significant amelioration of canine atopic dermatitis and down-regulation of serum IgE and that the serum level of IgE can be used as a convenient member for diagnosis of atopic dermatitis in dogs.

청소년 대상의 건강상태, 생활습관과 아토피 피부염의 관계 (Relationship between Health Status and Life Styles and Atopic Dermatitis in Adolescents)

  • Purpose: The aim was to explore the relationship between health status/life style and prevalence of atopic dermatitis in adolescents. Methods: Data from 1,743 adolescents (aged 13 to 18 years) derived from the fourth Korea National Health and Nutrition Examination Survey (KNHANES) were analyzed using binary logistic regression and the classification and regression tree (CART) model. Results: The prevalence rate of atopic dermatitis was 10.3%. Third quartile income class (OR=2.19, 95% CI=1.20-4.00), fair and unhealthy self-rated health (OR=1.49, 95% CI=1.06-2.11; OR= 2.73, 95% CI=1.64-4.55), past or current smoker (OR=0.41, 95% CI=0.20-0.85; OR=0.44, 95% CI=0.21-0.92), and more than average stress (OR=1.43, 95% CI=1.03-1.99) were related with increased prevalence of atopic dermatitis. The high prevalence risk groups for atopic dermatitis were adolescents who perceived themselves to be healthy on self-rated health and had more than average stress and those who perceived themselves to be unhealthy and who never smoked. Conclusion: The results of the study indicate that health status, smoking, and stress are associated with a prevalence risk for adolescent atopic dermatitis in different degrees. Therefore, for control of atopic dermatitis, various interventions for smoking and stress need to be reinforced for the high prevalence atopic dermatitis groups.

이온화된 맥반석이 아토피 피부염 유발 Nc/Nga mice의 피부손상 및 염증 완화에 미치는 영향 (The Effects of Ionized Maifanite on Skin Damages of Nc/Nga Mice after Atopic Dermatitis Elicitation)

  • Objectives : Atopic dermatitis accompanies with severe pruritus and collapse of skin barrier, inflammation. Maifanite could be used as an ointment for skin disease. However, there have been few studies about maifanite uses for atopic dermatitis. We report the anti-inflammatory and promoting skin recovery effects of ionized maifanite on damaged skin barrier with experimentally elicited atopic dermatitis. Methods : Nc/Nga mice were divided into 3 groups: control group(CON), atopic dermatitis elicited group(AE group), ionized maifanite treated group after atopic dermatitis elicitation(MT group). After 5% SDS was applied D. pteronyssinus crude extract also applied for 3 weeks to elicit atopic dermatitis-like skin disease. MT group was treated for 3 weeks with ionized maifanite. Ionized maifanite was applied once a day and voluntarily administrated. AE group and control group were treated with normal saline in the same way. Results : In MT group, skin lesions like eczema were more improved than AE group. p-ERK1/2 positive reaction was reduced in MT group. MMP-9 and substance P positive reaction at dermal papillae was also reduced in MT group. With skin angiogram, capillary vessel decreased in MT group. Also, IL-4 positive reaction cell and STAT-6 positive reaction cell reduced more in MT group than in AE group. $NF-{\kappa}B$ p65 positive reaction cell and iNOS positive reaction cell also declined more in MT group than in AE group. Conclusions : It is supposed that ionized maifanite has anti-inflammatory effects on NC/Nga mice's atopic dermatitis with suppressing IL-4 production and Th2 cell differentiation, and controlling $NF-{\kappa}B$ activation.

국내 개에서 발생한 아토피성 피부염에서 Dermatophagoides farinae의 항원 확인 (Identification of Allergens of Dermatophagoides farinae on Canine Atopic Dermatitis in Korea)

  • 개의 아토피성 피부염에서 Dermatophagoides farinae의 항원을 immunoblot을 통해 확인하였다. 실험은 D. farinae에 의해 아토피성 피부염이 유발된 16마리와 임상적으로 건강한 10마리 개의 혈청으로 수행하였으며, SDS-PAGE를 실시하여 D. farinae 추출물의 항원들이 15에서 220 kDa 범위 내에 존재하는 것을 관찰하였다. Immunoblot 결과, 임상적으로 건강한 10마리 개의 혈청에서는 단백띠가 보이지 않았으며, D. farinae에 의해 아토피성 피부염이 유발된 16마리 개의 혈청을 이용한 실험에서는 14마리에서 109 kDa, 13마리에서 44 kDa, 11마리에서 177 kDa, 7마리에서 27 kDa, 5마리에서 75 kDa, 3마리에서 15 kDa의 단백띠가 관찰되었다. 즉, D. farinae에 의해 아토피성 피부염이 유발된 개체 중 50% 이상에서 관찰되는 주요 항원은 177 kDa, 109 kDa, 44 kDa 항원이었다. 본 실험과 같이 주요 항원들을 확인하는 이러한 연구는 아토피성 피부염에 대한 새로운 면역 치료의 전략과 제제의 개발에 기여할 수 있을 것으로 생각된다.

가미청심련자탕(加味淸心蓮子湯)의 아토피 피부염 치료에 대한 임상고찰 (A Clinical Study on the Treatment of Atopic Dermatitis by Using Kami-chungsimyeunjatang(加味淸心蓮子湯))

  • Objectives The purpose of this study is to investigate the clinical effect of Kami-chungsimyeunjatang on atopic dermatitis and to survey the general characteristics in children with atopic dermatitis. Methods 30 patients suffering from atopic dermatitis were treated with water extract of Kami-chungsimyeunjatang and the clinical evaluation were made by SCORAD index system before and after treatment. We also investigated some characteristics of improvements by using questionnaire. Results 1. Among 30 patients diagnosed as atopic dermatitis, 18 of the people were male(60%) and 12 of the people were female(40%). 2. Among 30 patients diagnosed as atopic dermatitis, 24 of them have family history of allergic disease(80%) and 18 of them have past history of allergic disease(60%). 3. The distribution of nursing method, 13 of them (44%) had powdered milk, 10(33%) of them had both breast milk and powdered milk, 7 of them (23%) had only breast milk. 4. 20 patients(67%) who have atopic dermatitis were less than 1 year old and 6 of them (20%) were between 1 or 2 years old. The initial lesion of 22(74%) of the patients were face and neck. 5. 9 patients(30%) of them answered that the symptoms are getting worse especially during the winter while 8 patients(25%)of them said summer. 6. The mean period of treatment value was $4.6{\pm}2.20$ months. 7. After treatment, there was statistically significant decrease(p<0.05) of extent, intensity, subjective symptoms and total SCORAD index score(p=0.001). 8. After treatment, 28(93%) of the patients were improved. Conclusions Considering the above results, we speculate that Kami-chungsimyeunjatang is effective in the treatment of atopic dermatitis, and further studies are needed with more clinical cases of hematological evaluation.

Cinnamomum camphora Leaves Alleviate Allergic Skin Inflammatory Responses In Vitro and In Vivo

  • In this study, we investigated the therapeutic potential of Cinnamomum camphora leaves on allergic skin inflammation such as atopic dermatitis. We evaluated the effects of C. camphora leaves on human adult low-calcium high-temperature keratinocytes and atopic dermatitis mice. C. camphora leaves inhibited Macrophage-derived chemokine (an inflammatory chemokine) production in $interferon-{\gamma}$ (10 ng/mL) stimulated Human adult low-calcium high-temperature keratinocytes in a dose dependent manner. C. camphora leaves suppressed the phosphorylation of janus kinase signal transducer and activator of transcription 1. C. camphora leaves also suppressed the phosphorylation of extracellular signal-regulated kinase 1/2, a central signaling molecule in the inflammation process. These results suggest that C. camphora leaves exhibits anti-inflammatory effect via the phosphorylation of signal transducer and activator of transcription 1 and extracellular signal-regulated kinase 1/2. To study the advanced effects of C. camphora leaves on atopic dermatitis, we induced experimental atopic dermatitis in mice by applying 2,4-dinitrochlorobenzene. The group treated with C. camphora leaves (100 mg/kg) showed remarkable improvement of atopic dermatitis symptoms: reduced serum immunoglobulin E levels, smaller lymph nodes with reduced thickness and length, decreased ear edema, and reduced levels of inflammatory cell infiltration in the ears. Interestingly, the effects of C. camphora leaves on atopic dermatitis symptoms were stronger than those of hydrocort cream, a positive control. Taken together, C. camphora leaves showed alleviating effects on the inflammatory chemokine production in vitro and atopic dermatitis symptoms in vivo. These results suggest that C. camphora leaves help in the treatment of allergic inflammation such as atopic dermatitis.

상황버섯, 감초 복합추출물의 항산화 및 인체적용 아토피 개선 효능 연구 (Anti-oxidant Activities and Anti-atopic Dermatitis Effect of Combined Extract of Phellinus linteus and Glycyrrhiza uralensis)

  • Objectives : This study investigated the anti-oxidant activities and improving effect of Phellinus linteus and Glycyrrhiza uralensis Extract (PGE) on Atopic Dermatitis. Methods : 2,2′-diphenyl-1-picrylhydrazyl (DPPH) radical, 2,2′-azino-bis 3-ethylbenzothiazoline-6-sulphonic acid (ABTS) radical, Hydrogen peroxides scavenging activities and Superoxide dismutase (SOD)-like activities were used for the measurement of anti-oxidant ability. Cytotoxicity of PGE in Raw 264.7 cell was evaluated by MTT assay. To evaluate the anti-atopic dermatitis effect of PGE, a total of 33 patients with atopic dermatitis were observed trans epidermal water loss, skin moisture content, modified SCORAD index of atopic dermatitis and pruritic degree after applying the PGE for 4 weeks. Results : PGE scavenged DPPH ($IC_{50}=25ppm$) effectively, ABTS and Hydrogenperoxides scavenged similar to BHA. As for the SOD-like activity, it had lower effect than ascorbic acid, but it comparable activities in 500ppm. There was no cytotoxicity at PGE at concentrations of 10,000ppm. In clinical research about PGE on patients with atopic dermatitis, skin condition was improved. After 4 weeks, the application of PGE increased skin moisture content from 19.43 to 31.22. Moreover, it reduced the skin temperature (from 32.5 to 31.9), skin pH (from 5.39 to 5.22), trans epidermal water loss (from 39.03 to 24.46) and pruritus score (from 6.07 to 3.87). In addition, the Modified SCORAD index decreased from 31.28 to 20.3. Conclusions : In conclusion, PGE possesses anti-oxidant and anti-atopic dermatitis activities, thus it could be potentially valuable as anti-atopic dermatitis material.

아토피 피부염의 주증상에 대한 문헌적 고찰 (A Literal Study of the Main Symptoms of Atopic Dermatitis)

  • Objective : To find the main symptoms associated with atopic dermatitis and to get the utmost out of information about cause, treatment are contribute in the treatment of atopic dermatitis rather than looking for ways through how to connect modern atopic dermatitis and a listed symptom of classic literature. Method : In this research, we recorded about the main symptoms of atopic dermatitis such as itch, erythema, edema, pus and furuncle. Also we interpreted original text by extracting a sentence containing the terms like red[赤], pus[膿], flare[發赤], dermatosis[溞], swelling[洋], light red[紅], itch[瀁], worm running[蟲行], juice[汁], rash[癮疹], variegation[疹斑] and summarized cause of symptoms, disease sign, prescription. Results : There are the cause of many symptoms accompanied by atopic dermatitis in the body that recorded unpreparedness[虛], Wind[風], Fever[熱], unpreparedness & interruption of vitality hole[氣穴不通], unpreparedness & Wind, Wind Fever[風熱], Wind Coldness[風寒], Wind Humidity[風濕], Humidity Fever[濕熱], Wind Humidity Fever, Wind of Lungs[肺風], Wind of Lungs and Spleen[脾肺風], the Kidney Wind poison[腎臟風毒], Wind of Lungs Fever, Worm[蟲], miscellaneous vitality[雜氣], unpreparedness and Fever of Heart[心虛熱], old sore of kidney poison[久瘡腎臟風毒].
