Arterial stiffness index, physical activity and food and nutrient intake: cross-sectional study in adults aged 40 years and older

  • Objectives: This study aimed to investigate arterial stiffness index, physical activity, and food and nutrient intake in middle-aged adults over 40 years when the incidence of cardiovascular disease begins to increase. Methods: This study included 106 subjects (48 males and 58 females) aged between 40 and 64 years. The arterial stiffness index (brachial-ankle pulse wave velocity [baPWV], and ankle-brachial index [ABI]) were measured using a blood pressure pulse wave testing device. Physical activity was assessed using the Korean version of the Global Physical Activity Questionnaire, and food and nutrient intake was calculated using the Food Frequency Questionnaire. Results: The mean age of the subjects was 54.4 years. Although the ABI of the subjects was within the normal range, they were divided into tertiles to compare physical activity and food and nutrient intake. In males, the time spent on moderate to vigorous physical activity (MVPA) was significantly higher in T3 (600.6 min/week) than in T1 (304.4 min/week). In females, the time spent in sedentary behavior was significantly lower in T3 (294.5 min/week) than in T1 (472.1 min/week). In addition, the frequency of fish consumption was significantly higher in T3 (1.27 frequency/day) than in T1 (0.64 frequency/day) in females. Polyunsaturated fatty acid (PUFA) and ω-3 fatty acid intake, adjusted for energy intake, were significantly positively correlated with ABI (r = 0.200 and r = 0.218, respectively). Conclusions: High MVPA (in males), low sedentary behavior (in females), and PUFA and ω-3 fatty acid intake through fish consumption may be associated with low peripheral artery stiffness. Therefore, arteriosclerosis can be prevented through physical activity and proper dietary therapy.

Systolic blood pressure measurement algorithm with mmWave radar sensor

  • Blood pressure is one of the key physiological parameters for determining human health, and can prove whether human cardiovascular function is healthy or not. In general, what we call blood pressure refers to arterial blood pressure. Blood pressure fluctuates greatly and, due to the influence of various factors, even varies with each heartbeat. Therefore, achievement of continuous blood pressure measurement is particularly important for more accurate diagnosis. It is difficult to achieve long-term continuous blood pressure monitoring with traditional measurement methods due to the continuous wear of measuring instruments. On the other hand, radar technology is not easily affected by environmental factors and is capable of strong penetration. In this study, by using machine learning, tried to develop a linear blood pressure prediction model using data from a public database. The radar sensor evaluates the measured object, obtains the pulse waveform data, calculates the pulse transmission time, and obtains the blood pressure data through linear model regression analysis. Confirm its availability to facilitate follow-up research, such as integrating other sensors, collecting temperature, heartbeat, respiratory pulse and other data, and seeking medical treatment in time in case of abnormalities.

맥파 전달 속도(PWV) 측정을 위한 특징점 검출 알고리즘 개발 (Development of Feature Points Detection Algorithm for Measuring of Pulse Wave Velocity)

  • The compliance and stiffness of artery are closely related with disease of arteries. Pulse wave velocity(PWV) in the blood vessel is a basic and common parameter in the hemodynamics of blood pressure and blood flow wave traveling in arteries because the PWV is affected directly by the conditions of blood vessels. However, there is no standardized method to measure the PWV and it is difficult to measure. The conventional PWV measurement has being done by manual calculation of the pulse wave transmission time between coronary arterial proximal and distal points on a strip chart on which the pulse wave and ECG signal are recorded. In this study, a pressure sensor consisting of strain gauges is used to measure the blood pressure of arteries in invasive method and regular ECG electrodes are used to record the ECG signal. The R-peak point of ECG is extracted by using a reference level and time windowing technique and the ascending starting point of blood pressure is determined by using differentiation of the blood pressure signal and time windowing technique. The algorithm proposed in this study, which can measure PWV automatically, shows robust and good results in the extraction of feature points and calculation of PWV.

  • The conventional studies of the aging effect on cardio-vascular system are mostly obtained from human experiences and can only provide local information. In this study, the effect of fatigue fracture of arterial wall on the blood pressure was investigated by hemodynamic analysis. The aging process was simulated with three stages. The pulse wave velocity increased with age. The systolic blood pressure increases consistently but diastolic blood pressure decreases with age. The simulated results show in good accordance with the clinical ones.

반전 용적맥파 신호를 이용한 심박 모니터링 시스템 (The Heart Rate Monitoring System using Inverted Photoplethysmography)

  • 광용적맥파는 심박과 동맥 혈액의 산소포화도 등을 측정하기 위하여 널리 사용된다. 본 논문에서는 요골동맥에서 780nm와 940nm 적외선 LED를 사용하여 PPG 신호를 측정할 수 있는 모니터링 시스템을 구현하였다. 구현된 시스템은 여러 상황에서 특성이 서로 다른 8종의 LED와 2종의 광전 센서를 조합하여 요골동맥에서 광전용적맥파를 측정하였다. 요골동맥에서 측정된 파형들은 반전되어서 기록된다. 이렇게 반전된 파형은 수축기의 신호는 감소시키고, 확장기의 신호는 증가시킨다. 시스템을 통하여 투과형 방식과 반사형 방식에서 각각의 신호 검출이 적합한 환경과 최적의 센서 조합을 파악하여 모니터링 시스템을 구현하고, 각 조합별 그 결과 피검자의 요골동맥에서 안정적으로 사용할 수 있는 심박신호 측정 시스템을 개발하였다.

펜타입 압저항 센서를 활용한 연령별 맥파 특성 및 맥파의 대사증후군에의 적용 가능성 평가 (Characteristics of Pulse Waves in Various Age Categories and Applicability of Pulse Wave to Metabolic Syndrome Using Pen-type Piezoresistive Sensor)

  • Objectives : The purpose of this study was to confirm that the pulse analyzer is useful for analyzing characteristics of variables of pulse waves in age categories, evaluating pulse waves of the metabolic syndrome group, compared with those of the non-metabolic syndrome group in Korean adults. Methods : The pulse wave variables were measured in Guan of all 1,056 subjects by the pulse analyzer, using a pen-type piezoresistive sensor. The physical measurement, blood test and survey were also performed by each subject. Results : In the age categories, height of pre-incisura (h2), height of tidal wave (h3), area of percussion wave (Aw), and width of percussion wave (w) increased in accordance with increase in age. While ratio of systolic period area (As) went up according to the increase of age, ratio of diastolic period area (Ad) went down. Radial augmentation index (R-AI), h2/h1, h3/h1, w/t and angle of percussion wave went up by aging, generally. Aw rate (Aw/At) also increased. Among the metabolic syndrome group, in the ages of 19 and 44, ratio of systolic period area (As) was higher and ratio of diastolic period area (Ad) was lower than in the non-metabolic group. w/t, Aw/At, and angle of percussion wave were higher than in the non-metabolic syndrome group. Among the metabolic syndrome group over the age of 60, height of pre-incisura (h2), height of tidal wave (h3), total area (At), area of percussion wave (Aw), radial augmentation index (R-AI), h2/h1 and h3/h1 were higher than in the non-metabolic syndrome group. Conclusions : The pulse analyzer is useful to analyze arterial stiffness in the age categories and in the metabolic syndrome group by some measures.

산양삼(山養蔘) 약침(藥鍼)이 유방암절제술 여성의 심박변이도, 맥파전달속도에 미치는 영향 (Effect of Heart Rate Variability, Pulse Wave Velocity in Women of Breast Cancer Patients Care by Mountain Cultivated Ginseng Pharmacopuncture)

  • Objectives: The aim of this experiment is to know the effect of Mountain Cultivated Ginseng Pharmacopuncture on Heart Rate Variability (HRV), Pulse wave velocity (PWV) in Middle Aged Women. Methods: We investigated on 40 women of breast cancer patients. First, we measured their Heart Rate Variability(HRV), Pulse wave velocity (PWV) and then Mountain Cultivated Ginseng Pharmacopuncture $20m{\ell}$ were injected on them. After 30 minutes, we measured Heart Rate Variability(HRV), Pulse wave velocity(PWV) again. As a result, method of non-equivalent control group non-synchronized design were used for evaluation. Results: In HRV, Mean HRV is significantly decreased from 69.15 to 63.34 after injection. Mean RR is significantly increased from 877.20 to 962.10 after injection. SDNN is significantly increased from 32.56 to 41.34 after injection. PNN50 is significantly decreased after injection. RNSSD, SDSD, TP, VLF is significantly increased after injection. Stress Resistance ability is significantly increased from 37.55 to 44.60 after injection. And In PWV, E-R, E-L, H-R, H-L is significantly decreased after injection. Conclusions: Effect of Mountain Cultivated Ginseng Pharmacopuncture on Heart Rate Variability (HRV) increased adaptability of autonomic nervous system and on Pulse wave velocity (PWV) decreased arterial stiffness.

측정 자세 및 상완 혈압에 의한 맥파 특성 변화 (Change of arterial pulse wave characteristic by measurement posture and brachial blood pressure)

  • 본 연구에서는 비침습적으로 압력 맥파를 검출하는 토노메트리 방식의 맥파 측정 장비인 SphygmoCor(AtCor, Australia)를 사용하여 측정자세에 따라 맥파를 측정하였다. 측정 결과를 통해 선 자세, 앉은 자세, 누운 자세에 따른 맥파의 분석 지표들의 변화를 분석하였다. 또한 측정된 데이터를 저혈압군, 정상혈압군, 고혈압군으로 구분하여 혈압군에 따른 맥파의 비교를 수행하였다. 그 결과 자세에 따른 요골동맥의 파형에서 유의차를 보인 분석지표는 맥파의 진폭에 해당되는 압력 보다 주요 피크가 발생되는 시간에서 나타났다. 맥압의 경우 맥파증대계수(AI)나 중심동맥압과 요골동맥압의 비율과 같이 맥압 간의 비율이 자세에 따른 변별력이 있었다. 사후검정에 따라 각 자세별로 상호 간에 모두 유의한 차이를 보인 분석지표는 주파(P_$T_1$)와 반사파(P_$T_2$)의 시간, 심박출지속시간(ED), 심박수(HR)로 나타났다. 혈압군에 대한 비교에서 대부분 맥파의 시간에 해당되는 지표보다 주요 피크의 진폭에 해당되는 맥파의 압력을 나타내는 지표가 혈압군에 따른 유의차를 보였다.

Cardiovascular risk factors of early atherosclerosis in school-aged children after Kawasaki disease

  • Purpose: The aim of this study was to determine whether school-aged children with Kawasaki disease (KD) have an increased risk for early atherosclerosis. Methods: The study included 98 children. The children were divided into the following groups: group A (n=19), KD with coronary arterial lesions that persisted or regressed; group B (n=49), KD without coronary arterial lesions; and group C (n=30), healthy children. Anthropometric variables and the levels of biochemical markers, including total cholesterol, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), apolipoprotein A, apolipoprotein B, homocysteine, highsensitivity C-reactive protein (hs-CRP), and brachial artery stiffness using pulse wave velocity were compared among the three groups. Results: There were no significant differences in blood pressure and body index among the three groups. Additionally, there was no sex-specific difference. Moreover, the levels of triglyceride, HDL-C, apolipoprotein A, and hs-CRP did not differ among the three groups. However, the levels of total cholesterol (P =0.018), LDL-C (P =0.0003), and apolipoprotein B (P =0.029) were significantly higher in group A than in group C. Further, the level of homocysteine and the aortic pulse wave velocity were significantly higher in groups A and B than in group C (P=0.0001). Conclusion: School-aged children after KD have high lipid profiles and arterial stiffness indicating an increased risk for early atherosclerosis.

Elevation of heart-femoral pulse wave velocity by short-term low sodium diet followed by high sodium diet in hypertensive patients with sodium sensitivity

  • BACKGROUND/OBJECTIVES: We compared changes in heart-femoral pulse wave velocity (hfPWV) in response to low sodium and high sodium diet between individuals with sodium sensitivity (SS) and resistance (SR) to evaluate the influence of sodium intake on arterial stiffness. SUBJECTS/METHODS: Thirty-one hypertensive and 70 normotensive individuals were given 7 days of low sodium dietary approach to stop hypertension (DASH) diet (LSD, 100 mmol NaCl/day) followed by 7 days of high sodium DASH diet (HSD, 300 mmol NaCl/day) during 2 weeks of hospitalization. The hfPWV was measured and compared after the LSD and HSD. RESULTS: The hfPWV was significantly elevated from LSD to HSD in individuals with SS (P = 0.001) independently of changes in mean arterial pressure (P = 0.037). Conversely, there was no significant elevation of hfPWV from LSD to HSD in individuals with SR. The percent change in hfPWV from the LSD to the HSD in individuals with SS was higher than that in individuals with SR. Subgroup analysis revealed that individuals with both SS and hypertension showed significant elevation of hfPWV from LSD to HSD upon adjusted analysis using changes of the means arterial pressure (P = 0.040). However, there was no significant elevation of hfPWV in individuals with SS and normotension. CONCLUSION: High sodium intake elevated hfPWV in hypertensive individuals with SS, suggesting that high sodium intake increases aortic stiffness, and may contribute to enhanced cardiovascular risk in hypertensive individuals with SS.
