Effects of Innovativeness on Customer Satisfaction in Long-Term Care Hospitals: The Effect of Internal Capacity and Location Strategy in Hospital (요양병원의 혁신성이 고객만족도에 미치는 영향 : 내부역량과 입지전략의 매개효과)

  • In recent years, over-supply of hospitals has made hospital management more difficult and hospitals have introduced innovative hospital management to enhance customer satisfaction. The purpose of this study is to investigate the effect of innovativeness of hospitals on customer satisfaction by using mediating effects of hospital capacity and location strategy. The subjects of this study were selected from 120 patients and caregivers who were hospitalized in Seoul and Metropolitan area hospitals and conducted questionnaire and statistical analysis. The results of this study are as follows: Firstly, this paper shows hospital Innovativeness does not significantly affect customer satisfaction. Unlike private companies, it is urgent to develop innovativeness measuring tools that are unique to hospitals differentiated from those of general companies. Secondly, although the impact of Innovativeness on hospital internal competency and location strategy was similar, location strategy(${\beta}=0.357$) had a greater impact on customer satisfaction than internal competency(${\beta}=0.283$). This suggests that the medical institution should take precedence over the detailed preparations based on its location marketability, traffic infrastructure, building sales and medical concentration from the time of its opening. Thirdly, this paper confirms through empirical analysis that the relationship between hospital Innovativeness and customer satisfaction is completely mediated by internal capacity and location strategy. The hospital's Innovativeness is affecting customer satisfaction by enhancing the hospital's internal competencies and inducing an active attitude toward establishing a location strategy.

Development of prediction models of chlorine bulk decay coefficient by rechlorination in water distribution network (상수도 공급과정 중 재염소 투입에 따른 잔류염소농도 수체감소계수 예측모델 개발)

  • This study developed prediction models of chlorine bulk decay coefficient by each condition of water quality, measuring chlorine bulk decay coefficients of the water and water quality by water purification processes. The second-reaction order of chlorine were selected as the optimal reaction order of research area because the decay of chlorine was best represented. Chlorine bulk decay coefficients of the water in conventional processes, advanced processes before rechlorination was respectively $5.9072(mg/L)^{-1}d^{-1}$ and $3.3974(mg/L)^{-1}d^{-1}$, and $1.2522(mg/L)^{-1}d^{-1}$ and $1.1998(mg/L)^{-1}d^{-1}$ after rechlorination. As a result, the reduction of organic material concentration during the retention time has greatly changed the chlorine bulk decay coefficient. All the coefficients of determination were higher than 0.8 in the developed models of the chlorine bulk decay coefficient, considering the drawn chlorine bulk decay coefficient and several parameters of water quality and statistically significant. Thus, it was judged that models that could express the actual values, properly were developed. In the meantime, the chlorine bulk decay coefficient was in proportion to the initial residual chlorine concentration and the concentration of rechlorination; however, it may greatly vary depending on rechlorination. Thus, it is judged that it is necessary to set a plan for the management of residual chlorine concentration after experimentally assessing this change, utilizing the methodology proposed in this study in the actual fields. The prediction models in this study would simulate the reduction of residual chlorine concentration according to the conditions of the operation of water purification plants and the introduction of rechlorination facilities, more reasonably considering water purification process and the time of chlorination. In addition, utilizing the prediction models, the reduction of residual chlorine concentration in the supply areas can be predicted, and it is judged that this can be utilized in setting plans for the management of residual chlorine concentration.

Analysis of the Relationships according to the Frame (f/s) Change of Cine Imaging in Coronary Angiographic System: With Focus on FOV Enlargement and Live Zoom (심장 혈관 조영장치에서의 프레임 레이트(f/s) 변화에 따른 상관 관계 분석 : FOV 확대와 Live Zoom을 중점으로)

  • This study aimed to investigate the difference of X-ray exposure by comparing and analyzing absorbed dose according to changes in the number of frames in coronary angiography, also depending whether the zoom mode is FOV enlargement or Zoom Live. Moreover, for appropriate frame selection measures for examination, including the effect of frame change on the image quality, were sought by measuring the noise strength expressed by the standard deviation (SD), the signal to noise ratio (SNR) and contrast to noise ratio (CNR). The study was conducted with an anthropomorphic phantom on an angio-system. The linear relationship between the frame rate and the radiation dose was evident. On the contrary, the indices of image quality (SD, SNR, and CNR) were almost constant irrespective of the number of frames. The difference depending on the zoom mode was not statistically significant for DAP, air kerma, and SD (p > 0.05). However, SNR and CNR were statistically different between FOV enlargement and Zoom Live. In conclusion, since the image quality was not degraded significantly with the decreasing frame rate from 30, 15, to 7.5 f/s and the radiation dose evidently decreases in almost exactly linear proportion to the decreasing frame rate, the number of frames per second needs to be maintained as low as reasonably achievable. As for the dependence on the zooming mode, the Live Zoom mode showed statistically significant improvement in the image quality indices of SNR and CNR and it justifies active use of the Live Zoom mode which enables real-time image enlargment without additional radiation dose.

Topographical Analysis of the Potholes in Jakgwaecheon Stream in Ulsan (울산 작괘천의 포트홀에 관한 지형분석)

  • This report is based on the investigation of potholes which are formed by fluvial erosion. A pothole is called so because it is a hole that looks like a coffeepot. The results of previous studies are applied to the 'Jakgwaecheon Porthole' of this study. The study is focused on the dimension and morphology of the Pothole and investigates the effects of stream sediments, river flow, geological structural lines, etc. on the formation of potholes. As a result of measuring 61 potholes in this area, we recognized that the elliptical dish-shaped cross sections are dominant and inferred that their longitudinal direction on the plain is affected by the direction of the stream flow. Also, 'Jakgwaecheon Pothole' is very characterized in terms of scale and morphology. Furthermore, it is harmonious with the beautiful landscape, humanity, and historical values and it can be suggested that it is qualified to be registered as a geoheritage structure.

Field Application and Maintenance of sidewalk concrete block for PV Power Generation (태양광 발전을 위한 보도형 콘크리트 블록의 현장 적용과 유지관리)

  • In order to fulfill the obligation to voluntarily reduce greenhouse gas emissions under the Paris Climate Agreement, the proportion of coal and nuclear power generation is reduced worldwide and national efforts are being made to spread renewable energy including solar power generation. Korea also intends to increase the proportion of renewable energy generation to 30~35% by 2040 by introducing laws and regulations. In addition, while the country is trying to apply solar power generation to sidewalks and roads, there is no research related to it in Korea. Therefore, as a precedent study to develop solar power generation roads, solar power generation concrete blocks applicable to sidewalks and plazas were developed and the applicability was evaluated by constructing them on the site. As a result of indoor experiment, compressive strength was measured by 25.5~35.7MPa and flexural strength was measured by 5.1~10.5MPa, which showed that all domestic standards were satisfied. However, the higher the unit cement amount, the lower the strength was measured according to the mixing of the broken fine aggregate. The absorption rate was 5.7%, which satisfied the domestic standard of 7% or less. As a result of the freeze-thawing test, the reduction rate of the compressive strength after 100 cycles was up to 6.3%. As a result of measuring the settlement amount after construction, the maximum of 2.498mm was measured and irregular settlement occurred in the overall area, which is because the resolution of the sand layer was poor during construction. Maintenance techniques of sidewalk concrete block and solar panel need to be established more efficiently through long-term operation in the further.

Geometrical Analysis on Parts of Load Limit Valve for Static Structural Test of Aerospace Flight Vehicles (항공우주 비행체 정적구조시험용 하중제한밸브 부품 형상 분석)

  • Free body diagram analysis is done for key parts of pilot stage of LLV (Load Limit Valve) which is used to protect overload for static structural test of aerospace flight vehicle. It is shown through the analysis that diameter ratio($D_2)^{ten}/D_2)^{comp}$) of two poppets in a pilot stage must be equal to piston area ratio($A_{comp}/A_{ten}$) of a hydraulic actuator for making a poppet open consistently at constant force applied by an actuator. The result of the analysis is verified by measuring geometries of the poppets in the four different LLVs which are corresponding to four actuators with different capacity and have been used after being imported in this laboratory. Results of "Adjuster resolution tests" with two different pilot stages show the max. deviation of Fi(actuator force in instant of opening poppet) from average Fi obtained for each turn of adjuster is 0.3KN and max. deviation of the Fi normalized by average Fi of each turn of adjuster is 3.7%. From the results, it is verified that the two pilot stages with same poppet diameter ratio make a poppet consistently open at Fis within ${\pm}3.7%$ deviation from the average Fi. The deviation is shown to be caused from frictional force of O-ring in the poppet. Additionally, design factors for poppet spring and adjuster, which are also key parts of the pilot stage, are distinguished and procedure for deciding the factors are also shown in this study.

Assessment of NATM tunnel lining thickness and its behind state utilizing GPR survey (GPR탐사를 통한 NATM터널(무근)라이닝의 두께 분포 및 배면상태 평가)

  • In this study, lining thickness distribution and its behind state (particularly, its void state) were analyzed using the GPR survey data performed on three currently operating NATM tunnels. Results of GPR analysis showed that void areas were mostly detected between concrete lining and primary support, particularly, near the crown of the tunnels. The lining thickness in the left-hand side of the tunnel was different from that of the right-hand side by 8.6~253.5 mm when measured in transverse direction. It was also found that longitudinal cracks were prevailed in the area lining thickness was sharply changed. Longitudinal thickness distribution at the crown was also studied and tested by performing 3 goodness-of-fit tests in order to find the most suitable thickness distribution. Normal distribution (or similar distribution) fit most suitably to the measured data if the measured average thickness was larger than designed one; Gamma and/or Inverse Gauss distribution fit to the measured data reasonably well if the measured average thickness was less than the designed value of thickness. Since actual lining thickness can be a potential index when assessing the state and safety of the unreinforced NATM tunnel lining, measuring of the lining thickness with GPR survey might be needed rather than assuming the thickness is always constant and same with the designed value.

Development of Capacitive Type Humidity Sensor using Polyimide as Sensing Layer (폴리이미드를 감지층으로 이용한 정전용량형 습도센서 개발)

  • In this paper, we fabricated a capacitive humidity sensor with an IDT(Interdigitated) electrode using commercial polyimide containing fluorine, and its properties were measured and analyzed. First, in order to analyze the composition of commercial polyimide, EDS analysis was performed after patterning process on a silicon wafer. The area of the humidity sensor was $1.56{\times}1.66mm^2$, and the width of the electrode and the gap between the electrodes were $3{\mu}m$ each. The number of electrodes was 166 and the length of the electrode was 1.294mm for the sensitivity of the sensor. The fabricated sensor showed that the sensitivity was 24 fF/%RH, linearity <${\pm}2.5%RH$ and hysteresis <${\pm}4%RH$. As a result of measuring the capacitance value according to the frequency change, the capacitance vlaue decreased with increasing frequency. Capacitance deviations with 10kHz and 100kHz were measured as 0.3pF on average.

Machine Learning Model to Predict Osteoporotic Spine with Hounsfield Units on Lumbar Computed Tomography

  • Objective : Bone mineral density (BMD) is an important consideration during fusion surgery. Although dual X-ray absorptiometry is considered as the gold standard for assessing BMD, quantitative computed tomography (QCT) provides more accurate data in spine osteoporosis. However, QCT has the disadvantage of additional radiation hazard and cost. The present study was to demonstrate the utility of artificial intelligence and machine learning algorithm for assessing osteoporosis using Hounsfield units (HU) of preoperative lumbar CT coupling with data of QCT. Methods : We reviewed 70 patients undergoing both QCT and conventional lumbar CT for spine surgery. The T-scores of 198 lumbar vertebra was assessed in QCT and the HU of vertebral body at the same level were measured in conventional CT by the picture archiving and communication system (PACS) system. A multiple regression algorithm was applied to predict the T-score using three independent variables (age, sex, and HU of vertebral body on conventional CT) coupling with T-score of QCT. Next, a logistic regression algorithm was applied to predict osteoporotic or non-osteoporotic vertebra. The Tensor flow and Python were used as the machine learning tools. The Tensor flow user interface developed in our institute was used for easy code generation. Results : The predictive model with multiple regression algorithm estimated similar T-scores with data of QCT. HU demonstrates the similar results as QCT without the discordance in only one non-osteoporotic vertebra that indicated osteoporosis. From the training set, the predictive model classified the lumbar vertebra into two groups (osteoporotic vs. non-osteoporotic spine) with 88.0% accuracy. In a test set of 40 vertebrae, classification accuracy was 92.5% when the learning rate was 0.0001 (precision, 0.939; recall, 0.969; F1 score, 0.954; area under the curve, 0.900). Conclusion : This study is a simple machine learning model applicable in the spine research field. The machine learning model can predict the T-score and osteoporotic vertebrae solely by measuring the HU of conventional CT, and this would help spine surgeons not to under-estimate the osteoporotic spine preoperatively. If applied to a bigger data set, we believe the predictive accuracy of our model will further increase. We propose that machine learning is an important modality of the medical research field.

A Study of the CT MAR using Single-Source and Dual-Source Devices: Practical Comparison using Animal Phantom Fabrication (단일 선원 장치와 이중 선원 장치 비교를 이용한 전산화단층촬영 금속인공물 감소에 대한 연구: 동물팬텀 제작을 이용한 실측적인 비교)

  • This study aims to compare and evaluate the image differences between single and dual sources in applying a technique to reduce metal artifacts using dual energy CT. Discovery CT 256 (GE, USA) as a single source device and Somatom Definition Flash (Siemens Health Care, Forchheim, Germany) as a dual source device. The self-made phantom (pigs with medical titanium screws inserted) was quantitative and qualitatively evaluated under the same conditions by varying the dose under the same conditions using a dual energy CT. The evaluation method was compared by measuring SNR for metal artifacts (scattering, stripe) generated by metal inserts, divided around bones and around tissues. There was a difference in images in the method of reducing metal artifacts between single-source and dual-source devices. In a single source device, the linearized prosthesis by metal implantation showed a greater decrease than the image obtained from a double source device, and the surrounding tissue was well observed without interference from the artifact. In dual-source devices, scattering and stripe artifacts caused by metal inserts decreased more than on a single source device, and signals from adjacent tissues surrounding the metal implant were well observed without diminishing. If the examination is conducted separately between single source and dual source devices depending on whether the area to which the patient is intended to be viewed during the examination is adjacent to the metal insert or the total tissue surrounding the metal insert, it is believed that diagnostic helpful images can be obtained.
