The relationship between leptin adiponectin ratio and insulin resistance in healthy children (건강한 소아에서 렙틴 아디포넥틴 비와 인슐린 저항성의 관계)

  • Purpose : Leptin and adiponectin are two representative adipocytokines. Leptin increases, but adiponectin decreases, with obesity and insulin resistance. We aimed to study the relationship between the leptin/adiponectin ratio and insulin resistance in healthy children. Methods : Seventy-seven healthy children (36 boys and 41 girls) were enrolled in this study. Anthropometric measurements were performed, and the percentage of weight for height (%WFH) was calculated in each subject. Fasting plasma levels of glucose, insulin, leptin, adiponectin, testosterone, estradiol, and sex-hormone binding globulin (SHBG) were measured. The free androgen index (FAI) was used as a representative of testosterone bioactivity. The homeostasis model assessment was used to estimate the degree of insulin resistance (HOMA-IR). Results : In the boys, HOMA-IR was significantly correlated with age, pubertal stage, free androgen index (FAI), leptin, and the leptin/adiponectin ratio. HOMA-IR was also significantly related to age, percentage of weight for height (%WFH), pubertal stage, estradiol, leptin, and the leptin/adiponectin ratio in girls. The leptin/adiponectin ratio was independently related to HOMA-IR after adjusting for age, %WFH, and FAI in the boys (P<0.05). The leptin/adiponectin ratio was not independently related to HOMA-IR after adjusting for age, %WFH, and estradiol in girls. Conclusion : In non-obese healthy children, the leptin/adiponectin ratio was significantly correlated with insulin resistance. The leptin/adiponectin ratio was independently related to insulin resistance even after adjusting for age, degree of obesity, and androgen levels in healthy boys.

The Assessment of Dietary Quality in Toddler of 12-36 Months (12~36개월 유아의 식사 질 평가)

  • This research was conducted to evaluate the general dietary quality(the balance and diversity of food intake, nutrient intake and adequacy) of 12 to 36-month-old toddler, through examining their actual food intake. It was conducted for 664(male-357, female-307) healthy toddler who were outpatients from Mar. to Aug., 1999 at university hospitals and private hospitals in Seoul and kyunggi province. The anthropometric parameters were measured. The 24-hr recall method were utilized to analyze the nutrient intake by months, based on the classification of The Korea Child Gorwth Standard. As for the subjects of this research, we found that the younger the age, the more frequent the pattern of intake of only 3 kinds of food groups. The results are that 1) the group with 1 point of the DDS(Dietary Diversity Score) mostly took only foods from the milk group, 2) the group with 2 points only took foods from the grain and milk group, 3) the group with 3 points did not take foods from the fruit and vegetable group, and 4) the group with 4 points took just about all kinds of food groups except for the vegetable group. This means that most of them were not taking various kinds of food even after the age of 12 months, and that they took more than twice the recommended amount of foods from the milk group until the age of 20 months. Thus, one could find a tendency of dependance on the milk group for food intake. Food intake for the vegetable group was severely under the recommended allowance, regardless of the age. The intake of iron was less than two thirds of the recommended allowance, except for the group of 12 to 14-month-olds who had been fed with infant formula. The Nutrient Adequacy Ratio(NAR) of major nutrients(protein, calcium, phosphorous, iron, vitamin A, vitamin B1, vitamin B2, niacin, vitamin C) and the Mean Adequacy Ratio(MAR, shows the general quality of dietary intake) was 0.86±0.12, but the NAR of iron was 0.62±0.27, which was extremely low compared to those of other nutrients. Therefore, concerning the subjects of this research, the MAR value alone does not provide enough ground to conclude that the subjects dietary quality is satisfactory. Nutrients and Salt Consumption of Hypertension Patients According to Treatment Status (고혈압환자에서 치료상태에 따른 영양소 및 나트륨 섭취 양상)

  • High blood pressure is an important determinant of the incidence of coronary heart disease, stroke, congestive heart failure, renal failure, and peripheral vascular disease. Recommendations for control of high blood pressure emphasize lifestyle modification, including weight control, reduced sodium intake, increased physical activity. Subjects who were normotensive (n=19, 47.2±9.0 y, BP l16/81 mmHg) ,treatment hypertensive (n=33, 54.2±6.9 y, BP 132/85 mmHg) and non-treatment hypertensive (n=14, 50.1±11.0 y, 149/94 mmHg) recruited. Anthropometric assessment (height weight waist circumference, hip circumference, fat%, fat mass, and lean body mass) and dietary assessments (using 3-days food records, daily nutrient intakes were inuysed by CAN PRO 2.0 were carried out. Blood and 24-hour urine were collected). Test of recognition for salt taste threshold were performed. In non-treatment hypertensive male subjects, weight, %IBW, BMI, and waist circumference were significantly higher than those of normotensive and treatment hypertensive subjects (p<0.05) .Food habits were not significantly different among the three groups. Intakes of vitamin A, vitamin B,, and vitamin B, were significantly higher in normotensive group (p<0.05). Intakes of sodium and salt taste recognition threshold were the highest in normotensive group and the lowest in treatment hypertensive group (p<0.05). Blood levels of lipids and minerals were not significantly different among the three groups. Urinary calcium level of normotensive group were significantly higher than that of treatment hypertensive and non-treatment hypertensive groups (p<0.05). These results indicate that continuous management of hypertension by drug and non-drug treatment affects salt taste recognition threshold and reduced the consumption of sodium. However, dietary sodium intake exceed recommended sodium intake to prevent and treat hypertension. It is necessary to develop the lifestyle modification program that may have beneficial effects on hypertension treatment.

A Comparative Study on the Growth & Developmental Status of Premature and Full Term Infants During the First 3Years (미숙아와 정상아의 영유아기 성장발달상태 비교연구)

  • The problems of growth & development due to maladjustment are gradually increasing while need for the treatment of children's diseases is decreasing. The level of developmental deficiency or delay correlates with neonatal birth weight and also with gestational age, i.e. degrees of prematurity. There-fore, developmental defects and potential risk factors' are more Common in premature infants than in full term infants. The purpose of this study is to define the difference in the growth at developmental status between premature and full term infants, and to define the relation between the developmental status and the physical growth during the first 3 years' Data were collected from January 10, 1985 to April 6, 1985 at 3 hospitals including St. Mary's Hospital, and through home visiting. The subjects of this study consisted of 79 Premature infants (G.A. <37wks. & B.W. <2.5kg) and 94 full term infants (G.A.≥37 wks. & B.W.≥2.5kg). The study method used was a questionnaire, anthropometric assessment and DDST for normative data of growth & development. The collected data were analyzed using descriptive statistics, chi-square test and t-test. The results of the study were as follows: Hypothesis: 1 : That the prematures will differ from the full term infants in the physical growth status during the first 3 years was partially supported (p<0.02) : The prematures reached up the full term infants in the physical growth status in the first 6 months. And, the first hypothesis was supported (P<0.01) : There are more cases which is below‘the Korean children's physical. growth standards’in prematures than in full term infants. Hypothesis 2 : That the prematures will differ from the full term infants in the developmental status during the first 3 years was supported (P< 0.001);‘Normal’developmental status due to DDST was less in prematures than in full term infants. And, the second hypothesis was Partially supported (P<0.02) : The developmental status of the pre-matures was different from that of the full term infants within the first 3 months by analysis of passed items in DDST, Hypothesis 3 : That the prematures' developmental status will relate to their physical growth during the first 3 years was supported (P<0.001) : If the prematures' developmental status is in delayed status, then, their physical growth status is also in delayed status. This study shows that the prematures differed significantly from the full term infants in the growth at developmental status during their infancy. This means that the nurse can foster the growth & development of the prematures by supportive care during their infancy. Further longitudinal study is needed to verify these findings for the environmental factors.

Lipid accumulation product is a predictor of nonalcoholic fatty liver disease in childhood obesity

  • Background: Lipid accumulation product (LAP) is associated with the presence and severity of nonalcoholic fatty liver disease (NAFLD) in adults. Purpose: Here we evaluated the ability of LAP to predict NAFLD in obese children. Methods: Eighty obese children (38 girls; age 6-18 years) were included. Anthropometric measurements and biochemical values were obtained from the patients' medical records. LAP was calculated as [waist circumference (WC) (cm) - 58]×triglycerides (mmol/L) in girls; [WC (cm) - 65]×triglycerides (mmol/L) in boys. The minLAP and adjLAP were described (3% and 50% of WC values, respectively) and the total/high-density lipoprotein cholesterol index (TC/HDL-C) was calculated. NAFLD was observed on ultrasound, and patients were divided into 3 groups by steatosis grade (normal, grade 0; mild, grade 1; moderate-severe, grade 2-3). The area under the curve (AUC) and appropriate index cutoff points were calculated by receiver operator characteristic analysis. Results: LAP was positively correlated with puberty stage (rho=0.409; P<0.001), fasting insulin (rho= 0.507; P<0.001), homeostasis model assessment of insulin resistance (rho=0.470; P<0.001), uric acid (rho=0.522; P<0.001), and TC/HDL-C (rho=0.494; P<0.001) and negatively correlated with HDL-C (rho=-3.833; P<0.001). LAP values could be used to diagnose hepatosteatosis (AUC=0.698; P=0.002). The LAP, adjLAP, and minLAP cutoff values were 42.7 (P=0.002), 40.05 (P=0.003), and 53.47 (P= 0.08), respectively. For LAP, the differences between the normal and mild groups (P=0.035) and the normal and moderate-severe groups were statistically significant (P=0.037), whereas the difference between the mild and moderate-severe groups was not (P>0.005). There was a statistically significant difference between the normal and mild groups for adjLAP (P=0.043) but not between the other groups (P>0.005). There was no significant intergroup difference in minLAP (P>0.005). Conclusion: LAP is a powerful and easy tool to predict NAFLD in childhood. If LAP is ≥42.7, NAFLD should be suspected. This is the first study to assess LAP diagnostic accuracy for childhood obesity.

A Study of Health-related Habits, Dietary Behaviors and the Health Status of the Middle-aged and the Elderly Living in the Chonju Area (II) (전주지역 중, 노년층의 생활습관과 건강상태 조사(II))

  • This study investigated the anthropometric and biochemical indices, and the health and nutritional factors influencing the two indices among 194 middle-aged and elderly subjects (108 middle-aged and 86 elderly) residing in a medium sized city for more than 10 years. In the examination of their dietary habits, 8.3% of the middle-aged subjects and 14.0% of the elderly subjects had two meals a day, and more female subjects had two meals per day. Of the subjects who ate meals at regular times, 75.0% were middle-aged and 79.1% were elderly, and the degree of irregularity of meals was greater for female subjects. The study of the dietary behavior of the subjects indicated that 71.3% and 66.3% of the middle-aged and elderly, respectively responded that the amount of food in each meal was sufficient. The subjects ate alone comprised 19.7% of the middle-aged females and 31.5% of the elderly females. The prevalence of smoking among the subjects was 28.1% for the middle-aged, 18.8% for the elderly male and 7.4% for the elderly females. The percentage of the subjects who drank alcohol was 34.4% of the middle-aged males and 13.2% of the middle-aged females. Slightly less than half of the subjects exercised more than once a week, with the male subjects showing a higher rate than the female subjects. The average body mass indices (BMI) were 24.5 and 24.6 for the middle-aged male and female, respectively, and 22.6 and 24.0 for the elderly male and female, respectively. BMI assessment showed that underweight subjects (BMI < 20) comprised 3.7% of the middle-aged, 14.0% of the elderly, and that 40.7% of the middle-aged and 24.4% of the elderly were overweight (25 < BMI < 30) , and 0.9% of the middle-aged and 1.2% of the elderly were classified as obese (BMI 30) . A waist/hip ratio (WHR) greater than 0.8 was found in 89.5% of the middle-aged females and 90.7% of the elderly females, showing high abdominal fat deposition in the majority of females. The average systolic blood pressure of females was 121.1 ± 17.1 mmHg for the middle-aged and 129.6 ± 21.3 mmHg for the elderly subjects. The systolic blood pressures showed a significantly difference between the two age groups. Those defined as anemic subjects based on hemoglobin values comprised 13.0% of the middled-aged group and 16.3% of the elderly group. There was a tendency for higher fasting glucose levels among the elderly subjects. An increase in total plasma cholesterol levels with age was shown. The female subjects had higher cholesterol levels than the males'The study of the correlation between the daily habits and health status showed that the amount of food eaten at each meal, the frequency of eating out, and the use of dietary supplements appeared to influence BMI, WHR, the plasma triglyceride and plasma cholesterol levels; omitting one meal had a positive correlation with the systolic blood pressure and plasma cholesterol. These results suggest that desirable dietary habits and concerns for health are contributing factors for maintaining good health, as indicated by normal blood lipid levels.

Assessment of Appetite and Nutritional Status in Cancer Patients Undergoing Radiation Therapy : A Prospective Study (방사선치료를 받은 암환자의 식욕상태 및 영양상태 변화양상 평가)

  • This study was conducted to identify appetite and nutritional status of 48 cancer patients who have been irradiated over 150cm2 on chest or pelvic area over the three-month period. The data were gathered 3 phases, Each from initiation to completion of radiotherapy through the questionnaires of anorexia, the anthropometric and biochemical measures were used such as weight, TSF, MAC, MAMC, serum albumin and hemoglobin, TLC. Using SAS program, data were analyzed by percentage, Mean±SD, and two-way repeated measures ANOVA. The results were summarized as follows : 1. Eighty five percent(85%) of the subjects were aged from fifties to sixties. Cancers in the chest area occurred in 100% of men, 56% of the all subjects. The other 44% were pelvic cancer and 71% of the pelvic cancer occurred in women. 2. There were no significant differences in the appetite scores by all groups(characteristics). Changes of the appetite score over time were statistically significant by age, sex, cancer areas staging, treatment modality, and radiation dosage (F=4.0, p=.022; t=6.09, p=.003; t=4.90, p=.009; F=3.28, p=.042; t=5.04, p=.0084; t=4.76, p=.011). The appetite score on the 2nd phase (4 weeks after initiating radiotherapy) decreased from the 1st phase (initiating irradiation), and then increased on the 3rd phase (completing irradiation). 3. There were no significant differences in the body weight and MAMC by all characteristics, and no changes in the body weight and MAMC over time. However there were significant differences of TSF, MAC, level of hemoglobin, level of albumin, and TLC by all characteristics during the three phases. TSF of the men and the chest cancer were lower than those of the women and the pelvic cancer (t=73.20, p=.0001; t=22.91, p=.0001). And there was significant difference by cancer staging(F=3.19, p=.050). But there was no change in TSF over time. MAC of the men and the chest cancer were lower than those of the women and the pelvic cancer each(t=9.23, p=.004; t=17.85, p=.0001). But no change in MAC over time. Levels of hemoglobin had significant differences by age, sex and cancer areas; levels of hemoglobin of older than the fifties, men, and chest area were higher than those on the others(F=3.82, p=.029; t=21.75, p=.0001; t=8.71, p=.005). Levels of albumin were significant differences by sex and cancer areas; levels of albumin on women, and pelvic area were higher than those on the others(t=6.34, p=.015; t=15.23, p=.0003). While the levels of hemoglobin were changed over time, levels of albumin were not changed and within normal limit. TLC of the men was higher than women(t=5.05, p=.029). Changes in the level of hemoglobin over time were statistically significant according to sex, cancer areas, and radiation dosage(t=3.49, p=.035; t=3.36, p=.039; t=4.04, p=.021).

A Study on the Food Habit and Seasonal Difference of Nutrient Intake of Adult Working Women (일부 성인직장여성들의 식습관 및 계절별 영양소섭취상태 조사)

  • To assess the food habits and the seasonal differences of nutrient intakes and diet qualities of adult working women aged 30 - 49y in Busan, dietary survey was conducted in summer and in winter by a questionnaire and two-day food record. Anthropometric assessment was also investigated in two seasons. 91.8% of those skipped breakfast in the main. 44.3% had irregular meals. The mean daily energy intake was 1725.8 kcal with 63.3% of energy intake being supplied by carbohydrates, 14.7% by protein, 22.3% by fat in summer and 1598.4 kcal with 62.1% of energy intake being supplied by carbohydrates, 15.6% by protein, 22.1% by fat in winter. Over 70% of iron intake came from plant origin in two seasons. The mean intakes of energy, calcium, iron and vitamin A in summer and energy, calcium, iron, vitamin A and vitamin B2 in winter were below Recommended Dietary Allowance (RDA) for Koreans. As well as insufficiency in iron, the bioavailability of iron is considered to have been low because most of iron intake came from plant origin in two seasons. For calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter, proportions of subjects with intake levels less than 75% of RDA were over 40% in summer and over 50% in winter, respectively. The nutrient adequacy ratios (NAR) were below 0.75 for calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter. NARs of iron (p<0.05), vitamin A (p<0.01) and vitamin B2 (p<0.001) in winter were significantly lower than those in summer. The mean adequacy ratios (MAR), an index of overall dietary quality were 0.85 in summer and 0.80 in winter. The MAR in winter was significantly lower than that in summer (p < 0.05). The indexes of nutritional quality (INQ) were below 1 for calcium and iron in summer and calcium, iron, vitamin A and vitamin B2 in winter. The intake (p<0.05) and NAR (p<0.05) of vitamin B2 showed positive significant correlations with height in winter. In conclusion, nutrient intake and diet quality of adult working women were different between the summer and the winter. So nutritional education programs for summer and winter are needed for adult working women.

Association between Myofascial Pain Syndrome and the Assessment of Pain and the related Function tests in female telephone directory assistance operators using VDT (VDT 사용 여성 전화교환원들의 근막동통증후군과 동통 및 기능평가 검사와의 관련성)

  • The Association between myofascial pain syndrome and some tests was assessed in a cross-sectional study of 904 female telephone operators using video display terminals(VDTs). 105 cases were diagnosed as Myofascial pain syndrome with symptom questionnaire, laboratory examination and physician's physical examination and 550 controls were defined to show only musculoskeletal symptoms using NIOSH symptom criteria. Data on demographics, musculoskeletal symptom and visual analogue scale(VAS) were obtained by questionnaire. Anthropometric measurements, pressure pain threshold, Grip & Pinch strength and laboratory test were conducted. No significant difference between case and control at comparing of demographics, occupational history and body measurements were observed. But, Grip strength, pressure pain threshold and VAS showed the statistical difference between case and controt. Age, pressure pain threshold of Rt. Upper trapezius and VAS were associated with myofascial pain syndrome in multivariate logistic regression analysis. This results suggest that, to diagnose of myofascial pain syndrome, the consideration of the objective aspects among patients' subjective symptom complaints through the VAS and pressure pain threshold is required.

Effects of Medical Nutrition Therapy on Dietary Quality, Plasma Fatty Acid Composition and Immune Parameters in Atopic Dermatitis Patients (아토피 피부염 환자에서 12 주간의 심층 영양관리에 따른 식사의 질과 혈중 지방산조성 및 면역지표에 관한연구)

  • Atopic dennatitis (AD) is one of the major public health problem. It has been reported that the prevalence of AD in children and adults are 10-20% and 1-3%, respectively. Westernization of food habits, urbanization, and environmental pollution are contributing factors toward the recent rise in prevalence. Excessive dietary restriction leads to chronic malnutrition in atopic dermatitis patients. The purpose of this study was to investigate the effects of medical nutrition therapy (MNT) on quality of diet and blood immune parameters in atopic dermatitis patients. The 19 atopic dermatitis patients (7 men and 12 women) admitted to K University Medical Center were studied. During the 12 weeks of intervention, the subjects were given MNT by a dietitian for 30-45 minutes every other week. MNT was comprised with general dietary therapy, intake of balanced meals, emphasis on n-3 fatty acid contents in foods, and food allergies. Anthropometric and dietary assessment and blood analysis were taken at baseline and after 12 weeks of MNT. After 12 weeks of MNT, the subjects' dietary qualities, including dietary diversity score (DDS), meal balance score (MBS) and dietary variety score (DVS) were significantly increased (p < 0.05). According to significantly increased intake of EPA and DHA, dietary n-6/n-3 fatty acid ratio decreased to the recommended level for the atopic dermatitis patients (p < 0.05). These changes of dietary fatty acid consumption were reflected erythrocyte fatty acid composition. After 12 weeks of MNT, serum levels of IgE and IL-4 levels were significantly decreased, however, the levers of INF-\{gamma}, WBC, lymphocyte and TLC were not changed. As a conclusion, the individualized MNT improved the quality of diet in atopic dermatitis patients thereby influenced RBC fatty acid composition and IgE and IL-4 levels.
