입원 노인환자의 부적절 약물사용현황 및 용량적절성 평가 (Assessment of Inappropriate Medication Use and Dosage in Elderly Patients)

  • This study aimed to evaluate the patterns of inappropriate medication use and inappropriate dosage in elderly patients in Korea. A retrospective study was performed for the elderly 65 years or older admitted from January 2007 to December 2007 in a medical center, Seoul, Korea. Potentially inappropriate medication (PIM) use in the elderly was evaluated using Beers criteria. Eighteen drugs out of Beers criteria were included in the formulary of the institute. Inappropriate dosage was set using Beers criteria, CMS (the Centers for Medicare& Medicaid Services) guideline, Geriatric Dosage Handbook. As results, the patients with PIM were 2,172 during the study period. The commonly used inappropriate medications were drugs for the nervous system (n=1237, 44.78%), the alimentary System (n=663, 24.54%) and the cardiovascular system (n=494, 18.28%). The elderly patients with prescription of inappropriate dosage were 10% out of patients with PIMs. The commonly inappropriate dosage drugs were digoxin (n=75, 27.27%), diazepam (n=70, 22.55%) and ferrous sulfate (n=66, 24.00%). Logistic regression analysis showed the number of PIM, days of hospital stay as predictors related to inappropriate dosage use. In conclusion, CNS drugs were frequently prescribed as PIM and inappropriate dosages were identified. It is needed to develop a means of decreasing adverse drug events in elderly.

실수형 유전-퍼지를 이용한 정수장 응집제주입제어에 관한 연구 (A Study on the Coagulant Dosage Control in the Water Treatment Using Real Number Genetic-Fuzzy)

  • The optimum dosage control is presumably the goal of every water treatment plant. However it is difficult to determine the dosage rate of coagulant, due to nonlinearity, multivariables and slow response characteristics, etc. To deal with this difficulty, the real number genetic-fuzzy system was used in determining the dosage rate of the coagulant. The genetic algorithms are excellently robust in complex optimization problems. Since it uses randomized operators and searches for the best chromosome without auxiliary informations from a population which consists of codings of parameter set. To apply this algorithms, we made the real number rule table and membership function from the actual operation data of the water treatment plant. We determined optimum dosages of coagulant(LAS) using the fuzzy operation and compared them with the dosage rate of the actual operation data.

Comparison of Flocculati on-Spectrophotometry and Streaming Current Detector Method to the Control of Flocculants for the Removal of Humic Acid

  • Flocculation-spectrophotometry and streaming current detector( SCD ) method were investigated and compared in order to determine the optimum dosages of synthetic cationic polymers of different charge density and molecular mass for the removal of humic acid. The optimum dosage for each of the polymers was determined with the dosage at which the lowest absorbance of humic acid was shown for the formal. and was determined with the dosage required during charge neutralization of humic acid for the latter It was in good agreement between both methods and there is a strong inverse correlation between the optimum dosage and charge density of the polymers, with highly charged polymer giving the lowest optimum dosage, pointing out the importance the charge neutralization. By flocculation-spectrophotometry, it was found that the absorbance of humid acid with the amount of each of the polymers dosed, changes sharply for polymers of high charge density, but changes rather broadly for polymers of low and middle charge density, Both methods showed that a stoichiometric correlation exists between the optimum dosage of each of the cationic polymers and the negatively charged humic acid.

인지부하이론을 적용한 약물계산훈련용 스마트폰 어플리케이션의 개발 및 효과 (Development and Effectiveness of a Drug Dosage Calculation Training Program using Cognitive Loading Theory Based on Smartphone Application)

  • Purpose: This study was done to develop and evaluate a drug dosage calculation training program using cognitive loading theory based on a smartphone application. Calculation ability, dosage calculation related self-efficacy and anxiety were measured. Methods: A nonequivalent control group design was used. Smartphone application and a handout for self-study were developed and administered to the experimental group and only a handout was provided for control group. Intervention period was 4 weeks. Data were analyzed using descriptive analysis, ${\chi}^2$-test, t-test, and ANCOVA with the SPSS 18.0. Results: The experimental group showed more 'self-efficacy for drug dosage calculation' than the control group (t=3.82, p<.001). Experimental group students had higher ability to perform drug dosage calculations than control group students (t=3.98, p<.001), with regard to 'metric conversion' (t=2.25, p =.027), 'table dosage calculation' (t=2.20, p =.031) and 'drop rate calculation' (t=4.60, p<.001). There was no difference in improvement in 'anxiety for drug dosage calculation'. Mean satisfaction score for the program was 86.1. Conclusion: These results indicate that this drug dosage calculation training program using smartphone application is effective in improving dosage calculation related self-efficacy and calculation ability. Further study should be done to develop additional interventions for reducing anxiety.

약물계산 오류예방을 위한 간호사의 역량과 투약안전과 관련된 병원조직풍토간의 정준상관관계 (Canonical Correlation between Drug Dosage Calculation Error Prevention Competence of Nurses and Medication Safety Organizational Climate)

  • Purpose: The purpose of this study was to investigate the relationship between drug dosage calculation error prevention competence and medication safety organizational climate. Methods: We surveyed 207 nurses from 15 hospitals. An assessment survey was designed to assess the medication safety organizational climate which consisted of four subcategories including medication safety cultures, medication safety initiatives, medication error communication, and medication error management competence. The drug dosage calculation error prevention competence contains two subcategories; Dosage calculation habits and ability. The data were collected from July to August 2011. Descriptive statistics, t-test, ANOVA, partial Pearson correlation coefficient, canonical correlation were used. Results: Organizational climate was related to dosage calculation error prevention competence with two significant canonical variables. The first canonical correlation coefficient was .53 (Wilks' ${\lambda}$=0.71, df=8, p<.001) and that of the second was .21 (Wilks' ${\lambda}$=0.96, df=3, p=.027). The first variate indicated higher perception of medication safety cultures, safety initiatives, error communication and error management competence were related to better dosage calculation habits. The second variate showed higher perception of medication safety cultures and lower medication error management competence were related to higher calculation ability. Conclusion: Continuous supporting strategies for medication safety organizational climate should be implemented to improve drug dosage calculation habits.

신규 졸업 간호사의 약물계산오류의 위험에 영향을 미치는 요인 (Predictors of Drug Dosage Calculation Error Risk in Newly Graduated Nurses)

  • Purpose: This study was to identify predictors of drug dosage calculation error risk in newly graduated nurses. Methods: A total of 115 newly graduated nurses who passed their employment examination, but didn't work for hospital yet, were recruited from a university hospital. The data were analyzed by descriptive statistics, $X^2$-test, t-test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression using the SPSS 18.0 program. Results: The mean score of 'drug dosage calculation ability' was $0.81{\pm}0.16$ and the mean score of 'certainty of calculation' was $2.95{\pm}0.60$ out of a 5 point scale. The error risk of drug dosage calculation was positively related to anxiety for drug dosage calculations (r=.388, p<.001), but negatively related to interest and confidence in mathematics (r=-.468, p<.001), confidence related to dosage calculations (r=-.426, p<.001). The main predictors of error risk related drug calculations in newly graduated nurses were identified as interest and confidence in mathematics (${\beta}$=-.468, p<.001). This factor explained about 21.9% of the variance in error risk of drug dosage calculation. Conclusion: The strategies used to decrease the error risk related drug dosage calculation such as improving interest and confidence in mathematics should be developed and implemented.

마황(麻黃)의 적정 용량에 대한 고찰(考察) (A study on proper dosage of Ma-huang(麻黃))

  • Objective : The purpose of this study is to find out the proper dosage of Ma-huang for clinical use of Shanghanlun(傷寒論) Methods : To achive the purpose of this study, web-databases(pubmed, naver, google) were searched with the keywords including 'dose of Ma-huang Ephedra Ephedrine','dosage of Ma-huang Ephedra Ephedrine', and 'water extract of Ma-huang Ephedra'. The searched 30 papers and articles were reviewed. Results & Conclusions : 1. Proper dosage of Ma-huang 1) Adult: up to 9-12g/day 2) Adolescent: up to 6g/day 3) Hypertension disorder patient: up to 6g/day 4) lactating women: up to 6g/day 5) child: <2 years 0.7-2.5g/day, $${\geq_-}2$$ years 2.6-6g/day (Different from body weight) Although administration of Ma-huang to hypertension disorder patient, lactating women, child is safe on the paper, It is not recommended to these people because Ma-huang is one of toxic herbs. 2. Dosage form of Ma-huang There's no safety paper about pill or powdered Ma-huang(麻黃). There's not pill or powdered prescription of Ma-hunag in Shanghanlun(傷寒論), either. So it is recommended to administrate water exetract of Ma-huang.
