A Study on Characters of DITI, HRV and Body Composition Analysis in Amenorrhea (무월경 환자의 DITI와 HRV, 체성분 분석을 통한 특성 연구)

  • Purpose: The purpose of this study is investigate the characters of DITI, HRV and Body composition analysis in Amenorrhea. Methods: We studied 47 patients visiting ○○ hospital from 1st October 2007 to 19st September 2009. The subjects were categorized in two groups, amenorrhea group(26) and normal group(21). We studied the difference of DITI, HRV and Body composition analysis between two groups by Mann-whitney test using SPSS for windows(version 12.0). Results: there is not statistically significant difference of DITI, HRV and BMI between amenorrhea group and normal group. But there is statistically significant difference of percent body fat and waist-hip ratio between amenorrhea group and normal group. Conclusion: Percent body fat and Waist-Hip ratio of amenorrhea groups is lower than normal groups. Percent body fat and Waist-Hip ratio can be diagnosis index.

A Case Report about Pregnancy and Delivery with Primary Amenorrhea by Oriental Medicine in Oocyte Donation (원발성 무월경 환자의 난자공여 시술시 한약치료를 병행한 임신 및 출산 1례)

  • Objectives: The aim of this case is to report the effects of oriental medicine on one patient with primary amenorrhea for endometrial preparation and implantation. Methods: A patient with primary amenorrhea had symptoms of sleep disorder, diarrhea, colpoxerosis. For preparing endometrium and implantation in oocyte donation after one previous failure, she was treated by twice a day herb medication for 75 days. And we observed the effects of treatments by improvement of symptoms and following up endometrial proliferation ultrasonography. After implantation, for maintaining pregnancy and live birth, she was also treated by twice a day herb medication for 45 days. Results: After treatments, Symptoms of sleep disorder, diarrhea, colpoxerosis were improved and the thickness of endometrium was prepared for implantation in oocyte donation. So she was pregnant and gave birth to a healthy baby 36 weeks later. Conclusions: This case shows that oriental medicine has its effective implementation for the implantational surroundings on patients with primary amenorrhea in oocyte donation programs.

Factors of Occurrence of Amenorrhea and Climacteric Symptoms in Breast Cancer Patients Underwent Chemotherapy (항암화학요법을 받은 유방암판자의 무월경 발생과 갱년기 증상 관련요인)

  • Purpose: This study aimed to investigate the factors of occurrence of amenorrhea and the severity of climacteric symptoms in breast cancer patients who underwent chemotherapy. Method: Women diagnosed with breast cancer without metastasis or recurrence, had surgery followed by chemotherapy, and had menses at the time of surgery were recruited from S hospital located in Seoul. A total of 99 women aged 31 thru 55 years participated and filled out a structured questionnaire including the Functional Assessment Cancer Therapy-Breast plus Endocrine Symptom when they visited the clinic for follow-up. Result: In 88 women amenorrhea occurred within an average of 2 months since beginning chemotherapy, and menstruation was resumed in only 11 women. About 98% of women aged over 40 experienced a cessation in menses thus age was an apparent factor of amenorrhea (Exp(B)=.76, p<.05). Presence of chronic disease (${\beta}=.25$, p<.05) and body weight change (${\beta}=.30$, p<.01) were significant factors influencing the severity of climacteric symptoms. Conclusion: Nurses need to have clinical evidences of menstrual changes due to breast cancer treatment. Information about premature menopause and climacteric symptoms should be provided according to women's health conditions so that they cope better during their survival.

  • Purpose: To report the effect of Korean medical treatments on 1 patient with secondary amenorrhea after ovarian surgery for bilateral endometriomas diagnosed as premature ovarian failure. Methods: The patient who had secondary amenorrhea was treated with Korean medical treatments such as herbal medication, herbal acupuncture and moxibustion therapy, and so forth. Results: After the Korean medical treatments, the patient could have her periods again. And high FSH level was decreased to normal. Conclusions: The case report shows that Korean medical treatment can be an effective option for treating secondary amenorrhea after ovarian surgery for bilateral endometriomas diagnosed as premature ovarian failure.

A Case Report about Herbal Medicine Treatment's a Single Woman Patient with a Low Level of AMH in Progress of Oocyte Cryopreservation (난소기능저하 미혼여성의 난자동결보존 시 한약복용을 병행한 증례보고)

  • Objectives: The aim of this case is to report the effects of herbal medicine on a single woman patient with a low level of AMH (anti-$M{\ddot{u}}llerian$ Hormone) in progress of Oocyte Cryopreservation. Methods: A patient with a low level of AMH had symptom of secondary amenorrhea. For preparing oocyte cryopreservation after a long time of secondary amenorrhea, she was treated by twice a day herb medication for 10 months. And we observed the effects of treatments by improvement of symptoms and following up endometrium ultrasonography. After oocyte cryopreservation, for maintaining her menstruation, she was also treated by twice a day herb medication for two and a half months. Results: After treatments, symptom of amenorrhea was improved and the thickness of endometrium was increased as well as AMH in progress of oocyte cryopreservation. So 20 oocytes could be cryopreserved. Conclusions: This case shows that herbal medicine can be a concurrent method for a single woman patient with secondary amenorrhea in progress of oocyte cryopreservation.

  • Purpose: To report the effect of oriental medicine on 2 patients with secondary amenorrhea diagnosed as hypothalamic-pituitary failure. Methods: The patients who had secondary amenorrhea were treated with the oriental medical treatments such as herbal medication, herbal acupuncture and moxibustion therapy, and so forth. Results: After the oriental medical treatments, the patients could have their periods again, their menstrual condition was improved. Conclusion: This case report shows that the oriental medical therapy is effective for treating secondary amenorrhea diagnosed as hypothalamic-pituitary failure.

A Clinical Study on Differentiation of Syndromes of Amenorrhea or Oligomenorrhea with DSOM (한방진단시스템 DSOM을 이용한 무월경 및 희발월경의 변증진단 연구)

  • Purpose: In oriental medicine, doctors have mainly made diagnosis and treatment with amenorrhea or delayed menstrual period based on overall analysis of symptoms and signs patients have. We think patients with amenorrhea or delayed menstrual period to have symptoms that are classified into one differentiation of syndromes, and then studied to make the index. This study has been carried out to investigate pattern identifications and classify symptoms according to them. Methods: We examined 52 patients who visited Dong-eui university oriental medical center from June 2005 to February 2009 for undergoing treatment for amenorrhea or delayed menstrual period and made OB & GY questionnaires up Results: We investigated whether the patients had symptoms concerned with symptom types by analyzing the result of DSOM(Diagnos System of Oriental Medicine, hearafter DSOM). It came out 51 cases among 52 with pathogenesis that was related to the symptom types. The symptom types of were surveyed into Insufficiency of Kidney and Liver(肝腎不足), Insufficiency of Qi and Blood(氣血虛弱), Insufficiency of Yin & Dryness of blood(陰虛血燥), stagnation of Qi and Blood(氣滯血瘀), phlegm and damp(痰濕阻滯), coldness of Blood(血寒), Heat of Heart(心火). Conclusion: As a result of the investigation, one case did not have symptoms of differentiation of syndromes of amenorrhea or delayed menstrual period. 29 cases had 1 or 2 differentiation of syndromes. 21 patients had such complex symptoms of diverse differentiation of syndromes that it was difficult to diagnose a differentiation of syndromes in clinical survey. It is necessary to put the priority among the differentiation of syndromes in diagnosis in the future.

Minor Mullerian Anomalies and Oligomenorrhea (경미한 $M{\ddot{u}}llerian$ 기형과 희발월경의 상호관계에 관한 연구)

  • This study was undertaken to demonstrate the relationship between oligomenorrhea/amenorrhea and minor mullerian anomalies. Hysterosalpingograms were taken in total 139 patients including 62 infertile patients with normal menstrual intrerval, 47 infertile patients with oligomenorrhea or amenorrhea and 30 tubal reanastomosis candidates with normal menstrual interval. The results were summarized as follows: 1. In unselected infertile patients, the occurrence rate of minor mullerian anomalies was 38%. 2. The occurrence rate of oligomenorrhea/amenorrhea in infertile patients with minor mullerian anomaly was significantly higher than that of infertile patients with normal uterus and the reverse was the ( ) result. 3. There was no significant difference in the occurrence rate of minor mullerian anomalies between infertile patients and tubal reanastomosis candidates with normal menstrual interval. 4. There was no immediate relationship between the degree of fundal anomaly and the duration of the menstrual intervals and/or the duration of the oligomenorrhea or amenorrhea.

Effects of the Obesity Therapy with Korean Herbal Medicine on Menstrual Cycle Disorder: Case Series (한방비만치료가 월경주기 이상에 미치는 영향: 증례보고)

  • Obesity is associated with menstrual cycle disorder. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese female patient with oligomenorrhea or amenorrhea. This is a retrospective observational study performed on obesity patients with oligomenorrhea or amenorrhea. Nine patients were treated with KHM and lifestyle management at least 2 months. During the treatment, body weight, body mass index (BMI), percent body fat (PBF) and menstrual cycle length were checked afterwards. Body weight, BMI, and PBF of the patients were decreased during treatments. Five of 9 subjects recovered to normal menstrual cycle. Two subjects improved menstrual cycle. The other subjects remained at oligomenorrhea or amenorrhea. This study shows that the obesity therapy including KHM could be effective to improve menstrual cycle on obese patient. Further large scale studies are necessary for more evidences.

  • Purpose: The purpose of this study is to assess the effects of oriental medicine on secondary amenorrhea & hypomenorrhea objectively. Methods: By serum hormone assay and history interview, we classified the causes of each patient having secondary amenorrhea & hypomenorrhea. We also diagnosed and treated each patient according to them. And then we estimated the results of treatment by follow-up measurements of serum hormone level. Results: 1. We diagnosed case I as hypothalamic-pituitary disfunction, case II, III as PCOS and case IV as ovarian failure by classifying the causes from western medical scientific angle. 2. We also diagnosed and treated the cases from oriental medical scientific angle. 3. We confirmed the improvement of cases by follow-up measurements of serum hormone level. Conclusion: These results of serum hormone assay showed how medically effective oriental medical therapies of secondary amenorrhea & hypomenorrhea were.
