용담사간탕(龍膽瀉肝湯)이 Estradiol Valerate로 유발된 흰쥐의 다낭성 난소 발달과 수태에 미치는 영향 (Effects of Yongdamsagan-tang on the Progression of the Estradiol Valerate-induced Polycystic Ovaries and on the Conception in Rats)

  • Objectives: This study was designed to investigate the effects of Yongdamsagantang on the polycystic ovary(PCO) induced by estradiol valerate(EV) in rats. Methods: After administrating Yongdamsagan-tang to PCO induced rats, we measured the weight of body, ovaries, adrenal glands, and uterus of rats. The observation through naked eye and histopathological observation of ovaries were evaluated. Also, the number of follicle and corpora lutea and content of androstenedione(ADD) and total estrogen were evaluated. The expressions of nerve growth factor(NGF) and corticotropin releasing factor(CRF) were analyzed by immunohistochemistry. The breeding rate and number of implantation with normal male rats were evaluated. Results: - The weight(mg) of ovaries in YST treated group($73.8{\pm}7.6$) was significantly increased(p<0.001) compared with control group($54.3{\pm}4.5$). - The number of mature follicles in YST treated group($7.3{\pm}2.4$) was significantly increased(p<0.01) compared with control group($3.5{\pm}1.2$). - The number of atretic follicles in YST treated group($9.0{\pm}1.5$) was significantly decreased(p<0.01) compared with control group($13.4{\pm}3.8$). - The number of cystic follicles in YST treated group($3.1{\pm}1.1$) was significantly decreased(p<0.01) compared with control group($6.0{\pm}2.0$). - The number of corpora lutea in YST treated group($3.8{\pm}2.1$) was significantly increased(p<0.001) compared with control group($0.3{\pm}0.7$). - The expression of NGF-immunoreactive cells in the ovarian granulosa cells in YST treated group was lesser observed than control group. - The expression of NGF-immunoreactive cells in the adrenal cortex in YST treated group was lesser observed than control group. - The breeding rate in YST treated group(100 %) was significantly increased (p<0.05) compared with control group(50 %). - The number of implantation in YST treated group($6.4{\pm}4.7$) was significantly increased(p<0.05) compared with control group($1.4{\pm}2.6$). Conclusions: We concluded that Yongdamsagan-tang activates the maturation of follicles, normal ovulation, breeding rate and number of implantation. Therefore, this may be effective for the treatment of anovulation, amenorrhea and sterility of PCOS patients.

NF-${\kappa}B$ 조절(調節)을 통한 충울자(茺蔚子)의 염증억제효과 (Leonuri Fructus Ameliorates Acute Inflammation via the Inhibition of NF-${\kappa}B$-mediated Nitric Oxide and Pro-inflammatory Cytokine Production)

  • Leonuri Fructus, a semen of Leonuri Herba, has been used for the treatment of menstrual disorders such as amenorrhea, dysmenorrhea and leukorrhea and for the remedy of hyperemia. The present study was conducted to evaluate the anti-inflammatory effects of the Leonuri Fructus extract (Leonurus japonicus Houtt. EtOH extract; LJE) in vivo and in vitro. In vitro study, the MTT assay for cell viability was conducted to determine the non-cytotoxic concentration of LJE treatment in media. The levels of NO were measured with Griess reagent. Pro-inflammatory cytokines were detected by ELISA method. The inflammation-related proteins of this study were detected by immunoblot anlaysis. The increases of NO production and iNOS expression were detected in LPS-treated cells compared with control, but LJE attenuated the increases of NO and iNOS by LPS. LJE reduced the production of TNF-${\alpha}$ and IL-$1{\beta}$ induced by LPS stimulation. LJE suppresses the signaling pathways of NF-${\kappa}B$ and MAPKs in LPS-induced macrophage cells. In vivo study, carrageenan-induced hind paw acute edematous inflammation rat model was used for evaluation of anti-inflammatory activity of LJE. LJE significantly inhibited the increases of hind paw swelling, skin thicknesses and inflammatory cell infiltrations, and decreased the numbers of mast cell induced by carrageenan injection. These results suggest that LJE has an anti-inflammatory therapeutic potential, which is mediated through modulating NF-${\kappa}B$ activation and MAPK phosphorylation. Inhibition of the rat paw edema induced by carrageenan is considered as direct evidence that LJE may be a useful source to treat inflammation.

터너증후군의 핵형과 표현형간의 연관성 (Correlation between Karyotype and Phenotype in Turner Syndrome)

  • 목 적 : 터너증후군 환자들에게는 다양한 핵형과 표현형이 나타나지만 우리나라에서는 그 연관성에 대한 연구가 미미한 실정이다. 그리하여 본 연구에서는 터너증후군으로 진단받은 환자들의 염색체 이상, 임상양상, 동반 질환에 대해 조사하였다. 대상 및 방 법 : 경북대학교병원에서 터너증후군으로 진단받은 환자 92명을 대상으로 염색체 핵형을 분류하였으며, 그 중 62명을 대상으로 임상 양상 및 동반 질환을 조사하였다. 결 과 : 핵형이 45,X인 환자는 54.3%였고. 섞임증 및 구조 이상이 나머지를 차지하였다. 섞임증의 경우 45,X에 비하여 Turner stigmata의 빈도가 낮았다. 46,X,del(Xp) 및 45,X/46,X,del(Xq)에서는 모두 골격 이상이 나타난 반면, 46,X, del(Xq)에서는 나타나지 않았다. 46,X,del(Xp)에서는 성적 유치증이 나타나지 않았지만, 46,X,del(Xq) 및 45,X/46,X,del(Xq)의 경우에는 이차 성징 지연이 지연 및 무월경이 나타났다. 46,X,i(Xq) 및 45,X/46,X,i(Xq)의 경우 이차 성징이 발현되지 않았고 모두 일차 무월경을 보였다. 그 외에 장완의 isochromosome이 있는 경우 청력 장애 및 갑상선 질환이 더 빈번하게 나타났다. 결 론 : 터너증후군 환자들의 핵형과 표현형 사이의 연관성을 조사하는 작업은 성염색체에 위치하는 유전자자리를 예측하는 정보를 얻을 수 있다는 점에서 중요하다고 생각한다.

정신분열병 환자에서 비정형 항정신병 약물이 프로락틴과 테스토스테론 농도에 미치는 영향 (Effects of Atypical Antipsychotics on Serum Prolactin and Testosterone Levels in Schizophrenic Patients)

  • 본 연구의 목적은 치료 용량의 비정형적 항정신병 약물(risperidone, olanzapine)의 성 호르몬(prolactin과 testosterone)에 대한 효과를 알아보기 위해 총 91명의 정신분열병 환자들의 혈청에서 prolactin과 testosterone의 농도를 측정하였다(haloperidol 28명, 4~20mg/day ; risperidone 31명, 2~6mg/day ; olanzapine 32명, 5~20mg/day). 그 결과 남자 환자에서 olanzapine 복용자의 혈청 prolactin 농도가 risperidone이나 haloperidol 복용자의 혈청 농도보다 낮았으나 risperidone 복용자와 haloperidol 복용자의 혈청 prolactin 농도는 상호 유의한 차이가 없었다. 여자 환자에서는 olanzapine 복용자의 혈청 prolactin 농도가 risperidone이나 haloperidol 복용자의 혈청 농도보다 낮았으며 risperidone 복용자의 prolactin 농도는 haloperidol 복용자보다 낮았다. 또한 남자, 여자 환자 모두에서 haloperidol, risperidone, olanzapine을 각각 복용한 사람들의 혈청 testosterone 농도는 유의한 차이가 없었다. 위의 결과로 저자는 치료 용량에서 risperidone은 혈청 prolactin 농도를 haloperidol 만큼이나 증가시키지만 olanzapine의 경우 혈청 prolactin 농도에는 영향을 끼치지 않으며 비정형적 항정신병 약물은 testosteron의 농도에는 영향을 끼치지 않는다는 것을 알 수 있었다.

Establishment of a library of fragments for the rapid and reliable determination of anabolic steroids by liquid chromatography-quadrupole time of flight-mass spectrometry

  • Anabolic steroids have similar structures to testosterone, both of which promote the growth of muscle mass and increase strength. However, the side effects of anabolic steroid use may lead to heart attacks or strokes. Additionally, the excessive use of steroids inhibits the production of the sex hormones in the body via a negative feedback loop, which results in testicular atrophy in males and amenorrhea in females. Currently, the method of choice used to test for the presence of anabolic steroids is GC-MS. However, GC-MS methods require chemical derivatization of the steroid sample to ensure compatibility with the analytical method; therefore, analysis of many different samples is difficult and time consuming. Unlike GC-MS, the liquid chromatography-quadrupole-time of flight mass spectrometry (LC-Q-TOF-MS) method is suitable for many samples. Twenty-two different anabolic steroids were analyzed by LC-Q-TOF-MS with various collision energies (CE). Accurate mass spectral data were obtained using a Q-TOF-MS equipped with an electro-spray ionization source and operated in the positive MS/MS mode for several classes of steroids that are often the targets of testing. Based on the collected data, fragmentation pathways were carefully elucidated. The high selectivity and sensitivity of the LC-Q-TOF-MS instrument combined with these fragmentation pathways offers a new approach for the rapid and accurate screening of anabolic steroids. The obtained data from the 22 different anabolic steroids will be shared with the scientific community in order to establish a library to aid in the screening of illegal anabolic steroids.

17α-Hydroxylase 결핍에 인한 남성가성반음양 1례 (A Case of Male Pseudohermaphroditism due to 17α-Hydroxylase Deficiency)

  • 46,XY 핵형인 남성에게서 $17{\alpha}$-hydroxylase의 결핍이 있을 경우 성호르몬의 생성장애로 인해 남성가성반음양에 의한 여성 표현형을 보이게 된다. 코티졸이 결핍되면 부신피질자극호르몬의 증가 및 이로 인한 광불무신피질호르몬의 합성이 증가되어 저레닌성 고혈압, 저칼륨혈증이 발생한다. 41세 된 여자 환자가 원발성 무월경과 고혈압을 주소로 내원하였다. 46,XY 핵형을 보였으며, 호르몬 검사상, 혈중 에스트라디올, 테스토스테론, 레닌, 코티졸이 감소되어 있으며 부신피질자극호르몬 및 deoxycorticosterone이 증가되어 있어 $17{\alpha}$-hydroxylase 결핍에 의한 남성가성반음양으로 진단되었다. 이 질환의 치료로 복강경을 이용한 양측 고환적출술을 시행하였고, 부신피질호르몬과 항고혈압 재제, 에스트로젠을 투여하였다. 본 연구에서는 위 증례에 대하여 간단한 문헌고찰과 함께 보고하는 바이다.

삼릉(三稜) 추출물의 간세포 보호 및 미토콘드리아 보호 효과 (Mitochondria protection of Sparganii Rhizoma against oxidative stress in heptocytes)

  • Objectives : Sparganii Rhizoma is frequently used in traditional herbal medicine for treatment of blood stasis, amenorrhea and functional dyspepsia and has been reported to exhibit anti-oxidant, anti-proliferation and anti-angiogenesis peoperties. In this study, we investigated the cytoprotective effect and underlying mechanism of Sparganii Rhizoma water extract (SRE) against oxidative stress-induced mitochondrial dysfunction and apoptosis in hepatocyte. Methods : To determine the effects of SRE on oxidative stress, we induced synergistic cytotoxicity by co-treatment of arachidonic acid (AA) and iron in the HepG2 cell, a human derived hepatocyte cell line. Results : Treatment of SRE increased relative cell viability and altered the expression levels of apoptosis-related proteins such as Bcl-xL, Bcl-2 and procaspase-3. And SRE also inhibited the mitochondrial dysfunction and excessive reactive oxygen species production induced by AA+iron. In addition, SRE activated of AMP-activated protein kinase (AMPK), a potential target for cytoprotection, by increasing the phosphorylation of AMPKα at Thr-172. Morever, SRE increased phosphorylation of acetyl-CoA carboxylase, a direct downstream target of AMPK. Conclusion : These results indicated that SRE has the ability to protect against oxidative stress-induced hepatocyte damage, which may be mediated with AMPK pathway.

폐경기 여성에서 호르몬 상태와 우울 및 불안 성향간의 상관관계 연구 (A Study on the Correlation of Hormonal Status with Depression - Anxiety Traits in Menopausal Women)

  • 저자들은 1994년 3월부터 1995년 10월까지 중앙대학교 부속병원 폐경기 클리닉에 내원했었던 환자중 폐경군 83명과 대조군 73명을 BDI, STAI로써 우울과 불안정도를 측정하였고, 혈중 E2, LH, FSH 수치를 측정하여 결과는 다음과 같았다. 폐경 여성의 평균 연령은 45.3세였으며 BDI와 STAI 점수 비교에서 폐경군이 대조군에 비해 유의(p<0.01)하게 높아 폐경군이 더 높은 우울 및 불안 성향을 보였으나 폐경 여성이 불안 및 우울감을 더 유발한다는 가설을 뒷받침 하지는 못하였다. 혈중 호르몬의 비교에서 대조군과 폐경군간에 E2치와 FSH치에서 양군간에 유의한 차이(p<0.01)가 있었다. 그리고 대조군에서 STAI, BDI 접수와 호르몬간의 상관관계에서 유의한 상관관계는 없었으며 폐경군에서도 STAI, BDI 접수와 호르몬간의 상관관계에서 유의한 상관관계는 없었다. 즉 폐경기 호르몬의 상태와 우울 및 불안과의 상관관계는 없었다. 폐경기의 심리적 장애의 원인을 생물학적, 사회문화적, 또는 심리적 요인 중 단일 요인으로 성명하기보다 상호 복합적으로 규명해야 하겠다.

배란장애를 동반한 다낭성 난소인 여성에서 혈중 Inhibin 농도의 증가 (Increased Serum Level of Inhibin in Oligo-amenorrheic Women with Polycystic Ovaries)

  • Normal and abnormal follicular growth and steroidogenesis depend on gonadotropins as well as intraovarian peptides, which may mediate or potentiate gonadotropin action. Inhibin also affect follicular development and steroidogenesis and may play a role in dominant follicle selection and follicular atresia. Therefore, we studied the differences of serum inhibin, gonadotropin and androgen levels in the women with only the ultrasound findings and no disorder, and polycystic ovary (PCO) with ovulatory disturbance. We prospectively analysed forty-three women with PCO. The diagnosis of PCO was based on typical appearance of the ovaries on TVS. Twelve women with regular menstrual cycle and normal ovarian morphology were selected as control. Basal levels of inhibin, luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol $(E_2)$, testosterone (T), androstenedione (ADD), dehydroepiandrosterone-sulfate (DS), prolactin and TSH in serum were determined. There were significant differences in basal LH levels and LH/FSH ratio between the control and the women with PCO. The basal levels of inhibin and $E_2$ in the oligo-amenorrheic PCO (N=34) were significantly higher than those in the control. There was higher negative correlation between the inhibin and T levels in the oligo-amenorrheic PCO, but, not in the regular cycling PCO. Also, there was higher positive correlation between the LH and T levels in the oligo-amenorrheic PCO, but not in the regular cycling PCO. These data presume that the initial event of PCO is elevated pituitary LH secretion. Elevated levels of LH may down-regulate LH receptors on granulosa cells and also cause hypertrophy of the thecal layer. High level of androgen secreted by the hypertrophied thecal layer may stimulate inhibin secretion from granulosa cells and can be converted to estrogen by extraovarian tissues and could serve to augment pituitary sensitivity to GnRH with a resultant secretion of more LH than FSH. Inhibin may inhibit FSH action on granulosa cell in the PCO follicle, impairing follicular development and dominant follicle selection resulted in ovulatory disturbance.

H 감비탕(GB-001)이 폐경 전·후 비만 여성의 체중감량에 미치는 영향에 대한 후향적 연구 (The Retrospective Analysis of Effects of H Gambitang (GB-001) on Weight Loss in Pre- and Post-Menopausal Obese Woman)

  • Objectives: The purpose of this study is to verify the effects of H Gambitang (GB-001) on body composition and basal metabolic rate in pre- and post-menopausal obese woman retrospectively. Methods: The study was conducted on 57 middle-aged obese women. They were divided into two groups; pre-menopausal obesity group (Pr-MOG; n=34) and post-menopausal obesity group (Po-MOG; amenorrhea was continued to 14~48 month, n=23) with no significant difference of age, height, body weight and body mass index. All subjects took herbal medicine 3 times a day, for 6 weeks during treatment period. The weight, height, body fat mass, skeletal muscle mass, and basal metabolic rate of all subjects were measured on first visit. The following measurements were repeated after 2 weeks (2nd), 4 weeks (3rd) and 6 weeks (4th) with Inbody 370 (Biospace) equipment to identify changes of body fat mass, skeletal muscle mass, and basal metabolic rate. Results are represented as mean and standard deviation. Results: Po-MOG showed significantly lower decrease rate in weight reduction on 2~6 weeks while significantly higher decrease rate in skeletal muscle reduction on 4~6 weeks. There were no significant differences between two groups in body fat reduction rate and basal metabolic increasing rate. Conclusions: H Gambitang (GB-001) can be used not only in Pr-MOG but in Po-MOG in weight loss although the effect can be lower in Po-MOG. To prevent skeletal muscle mass loss in Po-MOG, following study on adjusting dose and components of H Gambitang (GB-001) thought to be necessary.
