Retrospective clinical and radiographic evaluation of restored endodontically treated teeth

  • Objectives: The aim of this study was to perform a clinical and radiographic analysis of endodontically treated teeth (ETT) restored with cast metal posts (CMPs) or prefabricated glass fiber posts (GFPs) and crowns. Materials and Methods: Fifty ETT were restored with 25 CMPs and 25 GFPs at a private dental clinic between 2001 and 2016. The restorations consisted of 12 all-ceramic crowns, 31 metal-ceramic crowns, and 7 composite resin crowns. Demographic data, type of teeth, type of post-and-core system, time of placement, crown restorations, the number of proximal contacts, the type of antagonist, and reports of any complications after post-and-core placement were recorded for each patient. Assessments were performed at baseline (radiographic) and follow-up (radiographic and clinical). Data were analyzed by the McNemar test, the Pearson χ2 test, and Kaplan-Meier survival curves (α = 0.05). The mean follow-up was 67.6 months. Results: No significant difference was observed for any of the radiographic parameters when the baseline and final radiographs were compared. In the clinical evaluation, anatomical form (p = 0.009) and occlusion (p = 0.001) showed significant differences according to the type of crown restoration; specifically, metal-ceramic and all-ceramic crowns outperformed composite resin crowns. Conclusions: CMPs and GFPs showed favorable results for restoring ETT after 6 years of follow-up. All-ceramic and metal-ceramic crowns showed higher survival rates and better clinical outcomes.

In vitro study of compressive fracture strength of Empress 2 crowns cemented with various luting agents

  • All-ceramic restorations have had a more limited life expectancy than metal ceramic restorations because of their low strength. Their relatively lower strength and resistance to fracture have restricted the use of all-ceramic crowns to anterior applications where occlusal loads are lower. But there has been increasing interest in all-ceramic restorations because patients are primarily concerned with improved esthetics. Many efforts have been made to in prove the mechanical properties of dental ceramics. This study was designed to elucidate the influence of the luting agent on the strength of the Empress 2 crown (staining technique) cemented on human teeth. Seventy extracted human permanent molar teeth were chosen. Teeth were prepared for Empress 2 crowns with milling machine on a surveyor. A dental bur was placed in the mandrel that was positioned so that the long axis of the bur was perpendicular to the surveyor base. Dimensions of the Empress 2 crown preparation were $6^{\circ}$ taper on each side, $1.5{\pm}0.1mm$ shoulder margin, and 4mm crown height. The luting cements used in this study were as follow: 1. Uncemented 2. Zinc phosphate cements (Confi-Dental) 3. Conventional glass ionomer cement : Fuji 1 (GC) 4. Resin-modified glass ionomer cements : Fuji plus (GC) 5. Adhesive cements : Panavia F (Kuralay), Variolink II (Vivadent), Choice (Bisco). Fracture test using Instron. The crowns were loaded in compressive force to evaluate the effect of these cements on the breaking strength of these all-ceramic crowns. A steel ball with a diameter of 4mm was placed on the occlusal surface and load was applied to the steel ball by a cylindrical bolt with a crosshead speed of 0.5mm per minute until fracture occurred. The fractured surface was examined using Scanning Electron Microscopic Image (SEM) to discover the correlation between fracture strength and bonding capacity. Within the limitation of this in vitro study design, the results were as follows : 1. fomentations significantly increased the fracture resistance of Empress ceramic crowns compared to control. Uncemented (206.9 N): ZPC (812.9 N): Fuji 1 (879.5 N): Fuji Plus (937.7 N): Choice (1105.4 N): Variolink II (1221.1 N): Panavia F (1445.2 N). 2. Resin luting agent, treated by a silane bond enhancing agents, yielded a significant increase in fracture resistance. In some of the Panavia F group, a fracture extended into dentin. 3. According to SEM images of fractured Empress crowns, the stronger the bond at both interfaces(crown and die), the more fracture strength was acquired.

Influence of porcelain veneering on the marginal fit of Digident and Lava CAD/CAM zirconia ceramic crowns

  • PURPOSE. Marginal fit is a very important factor considering the restoration's long-term success. However, adding porcelain to copings can cause distortion and lead to an inadequate fit which exposes more luting material to the oral environment and causes secondary caries. The purpose of this study was to compare the marginal fit of 2 different all-ceramic crown systems before and after porcelain veneering. This study was also intended to verify the marginal fit of crowns originated from green machining of partially sintered blocks of zirconia (Lava CAD/CAM system) and that of crowns obtained through machining of fully sintered blocks of zirconia (Digident CAD/CAM system). MATERIALS AND METHODS. 20 crowns were made per each system and the marginal fit was evaluated through a light microscope with image processing (Accura 2000) at 50 points that were randomly selected. Each crown was measured twice: the first measurement was done after obtaining a 0.5 mm coping and the second measurement was done after porcelain veneering. The means and standard deviations were calculated and statistical inferences among the 2 groups were made using independent t-test and within the same group through paired t-test. RESULTS. The means and standard deviations of the marginal fit were $61.52{\pm}2.88{\mu}m$ for the Digident CAD/CAM zirconia ceramic crowns before porcelain veneering and $83.15{\pm}3.51{\mu}m$ after porcelain veneering. Lava CAD/CAM zirconia ceramic crowns showed means and standard deviations of $62.22{\pm}1.78{\mu}m$ before porcelain veneering and $82.03{\pm}1.85{\mu}m$ after porcelain veneering. Both groups showed significant differences when analyzing the marginal gaps before and after porcelain veneering within each group. However, no significant differences were found when comparing the marginal gaps of each group before porcelain veneering and after porcelain veneering as well. CONCLUSION. The 2 all-ceramic crown systems showed marginal gaps that were within a reported clinically acceptable range of marginal discrepancy.

A new manufacturing method of all ceramic restoration using alumina tape and presentation of clinical cases

  • In an effort to facilitate fabrication procedure of all ceramic crowns, a novel preparation method for all ceramic crown using alumina tape was developed. The alumina tape having a uniform thickness was cast by using Doctor blade method. The physical properties of newly introduced alumina tape has biaxial flexure strength of $500\sim600MPa$. The value of toughness is $3.18\sim3,28MPa.m^{1/2}$ which corelates with fracture and the linear shrinkage rate of the alumina tape is 0.44% during core production. The marginal fitness of the alumina tape all-ceramic restoration with $90{!`}$shoulder margin had average marginal discrepancy at $78.3{\S}$ > and average marginal gap at $44.4{\S}$ >. At the marginal preparation of $135{!`}$deep chamfer, the average marginal discrepancy at $82.1{\S}$ > and the average marginal gap at $40.2{\S}$ > had been reported. This fabrication procedure of all ceramic crowns with alumina tapes is easier and less technical sensitive for dental technicians. After restoration with new all ceramic crowns we followed the patients 2 years later, there were no complications as porcelain fractures or periodontal disease. We had good esthetic clinical results with new all ceramic crowns.

Evaluation of marginal fidelity of copy-milled and CAD/CAM all ceramic crowns

  • Statement of the problem. The interest in all-ceramic restorations has increased as more techniques have become available. With the introduction of machinable dental ceramics and CAD/CAM systems or Copy-milling systems there is a need for evaluating the quality levels of these new fabrication techniques. Purpose. This study was to evaluate the fitting accuracy of machined all-ceramic crowns made out of an industrially prefabricated feldspathic porcelain. Material and Methods. Three master models with different cutting depth (0.8mm/1.0mm/1.2mm)were produced using a palladium-silver alloy. A total of 36 working dies, 12 of each form, was used for the modellation of prototype resin copings and 36 additional crowns, 12 of each cutting depth, were produced by using the $CEREC^{(R)}2$ system for all crowns. The maginal fit of all 72 crowns was then evaluated on their respective master die at 54 circularly staggered points of measurement per crown under a fixation pressure of 30 N by using a computerized video image system. Results. The medians of the copy-milled $CELAY^{(R)}$ crowns ranged from 29 to $36{\mu}m$. The highest value for the marginal gap was found in group B (cutting depth 1.0mm) at $107{\mu}m$. The median for the $CEREC^{(R)}2$ crowns was found between 43.5 and $70{\mu}m$. The maximum values for all three groups ranged from $181{\mu}m$ to $286{\mu}m$. With $286{\mu}m$ the highest value for marginal gap was found in group C. the Kruskal-Wallis test and multiple comparisons analysis procedure revealed a significant influence of the production technique on the marginal fit in all three groups (p<0,02). Conclusion. 1. The $CELAY^{(R)}$ system is capable to produce all-ceramic crowns with a significantly better marginal fit than the $CEREC^{(R)}2$ system. 2. As far as premolar crowns produced with the $CEREC^{(R)}2$ system are concerned, the cutting depth has a significant influence on fitting accuracy. 3. The production of crowns with an acceptable marginal fit is possible with both systems. However, adhesive luting is recommended for milled feldspathic porcelain crowns.

다른 스캐닝 방법으로 제작된 리튬 디실리케이트 단관의 변연 및 내면 적합도 평가 (Evaluation of the marginal and internal fit of lithium disilicate crowns fabricated by different scanning methods)

  • Purpose: This study was to compare adaptations of lithium disilicate CAD/CAM crowns fabricated with different scanning systems. Methods: This study selected the mandibular right first molar as an abutment for experiments and produced 10 working models. Lithium disilicate crowns appropriate for each abutment were produced by using a CEREC$^{(R)}$ CAD/CAM system. The independent t-test was then used to compare and analyze the data obtained from the two groups(${\alpha}$=0.05). Results: Mean(SD) adaptation were $86.93(22.82){\mu}m$ for the InS group, $88.42(26.77){\mu}m$ for the ExS group. They were no statistically significant differences between groups for adaptation(p>0.05). Conclusion: Within the limitations of this study, the results suggest that the accuracy of all investigated optical scanner were satisfactory for clinical use. Further assessment and improvement of the lithium disilicate ceramic for the fabrication of FPDs is evidently still required.

Wear of primary teeth caused by opposed all-ceramic or stainless steel crowns

  • PURPOSE. This study was conducted to evaluate the effects of full-coverage all-ceramic zirconia, lithium disilicate glass-ceramic, leucite glass-ceramic, or stainless steel crowns on antagonistic primary tooth wear. MATERIALS AND METHODS. There were four study groups: the stainless steel (Steel) group, the leucite glass-ceramic (Leucite) group, the lithium disilicate glass-ceramic (Lithium) group, and the monolithic zirconia (Zirconia) group. Ten flat crown specimens were prepared per group; opposing teeth were prepared using primary canines. A wear test was conducted over 100,000 chewing cycles using a dual-axis chewing simulator and a 50 N masticating force, and wear losses of antagonistic teeth and restorative materials were calculated using a three-dimensional profiling system and an electronic scale, respectively. Statistical significance was determined using One-way ANOVA and Tukey's test (P<.05). RESULTS. The Leucite group ($2.670{\pm}1.471mm^3$) showed the greatest amount of antagonist tooth wear, followed by in decreasing order by the Lithium ($2.042{\pm}0.696mm^3$), Zirconia ($1.426{\pm}0.477mm^3$), and Steel groups ($0.397{\pm}0.192mm^3$). Mean volume losses in the Leucite and Lithium groups were significantly greater than in the Steel group (P<.05). No significant difference was observed between mean volume losses in the Zirconia and Steel groups (P>.05). CONCLUSION. Leucite glass-ceramic and lithium disilicate glass-ceramic cause more primary tooth wear than stainless steel or zirconia.

수종의 전부도재관 코어의 두께에 따른 파절강도의 비교 연구 (COMPARATIVE STUDY OF FRACTURE STRENGTH DEFENDING ON THE THICKNESS OF SOME ALL CERAMIC CORES)

  • Statement of problem : The increased awareness of esthetics in dentistry has brought the esthetic consideration in prosthetic restorations . Dental ceramics offer better esthetics for use of prosthetic restorations. Unfortunately, dental ceramic materials are not always the most suitable candidate materials since their inherently brittle nature. In recent years, basic research in ceramic science has led to the recognition or several approaches to strengthen and to enhance esthetics of ceramics. Several all ceramic systems use ceramic core and porcelain build up structures . Ceramic cores influence to strength of all ceramic crowns . So the strength of ceramic cores is important to all ceramic crowns. Purpose : The purpose of this study is to estimate the flexural strength of ceramic cores in some all ceramic systems. Material and method : A biaxial flexure test was conducted on three groups(Cergo, Empress 2, In-Ceram). Each group consisted of 30 discs of nearly identical dimension with a 0.5mm, 1.0mm, 1.5mm thickness and 12mm in diameter. The fracture load was recorded by Instron. Analysis of valiance(ANOVA) and Tukey's tests were performed using SAS statistical software. Results : 1.5mm thickness of specimens were significantly stronger than 0.5mm and 1.0mm thickness of specimens in Cergo and In-Ceram. But each sepecimen group of Empress 2 was no significantly strength by thickness. In order of In-Ceram, Empress 2 and Cergo has significantly stronger strength in the same thickness. Conclusion : In-Ceram is the strongest ceramic material in 3 materials. All the materials can be used according to the required characters.

치아 부위에 따른 지르코니아 도재관의 파절강도 (Fracture strength of zirconia ceramic crowns according to tooth position)

  • 연구목적: 본 연구는 상악과 하악의 각 치아부위에 $Lava^{TM}$ All-Ceramic System을 이용하여 지르코니아 전부도재관을 제작하고 각각의 파절강도를 알아보고자 하였다. 연구방법:중절치와 견치는 절단연의 삭제량을 2.0 mm, 소구치와 대구치는 교합면의 삭제량을 1.5 mm, 그리고 축면 경사도는 $8^{\circ}$로하여모두8군의 실험군을 설정하였다. 금속 다이와 지르코니아 도재관을 제작한 후 레진시멘트 (Rely $X^{TM}$ Unicem)를 이용하여 합착하고 만능시험기상에서 치아 장축에 $30^{\circ}$경사지게 하여 중절치와 견치에 제작된 지르코니아 도재관은 절단연 중앙 부위에 하중을 가하였고, 소구치와 대구치에 제작된 지르코니아 도재관은 기능교두의 협설측 경사면 중앙부위에 하중이 가해지도록 한 후 파절강도를 측정하였다. 결과:1. 지르코니아 도재관의 평균 파절강도는 하악 제1대구치에서 2963 N으로 가장 높게 나타났으며, 하악 중절치에서 1035 N으로 가장 낮게 나타났다. 2. 지르코니아 도재관과 IPS Empress 도재관과의 평균 파절강도는 모든 치아에서 지르코니아 도재관이 더 크게 나타났다. 3. 지르코니아 도재관의 파절 양상은 하중이 가해진 중앙 부위에서 치아 장축으로 수직적인 파절상을 보였다. 4. 상악과 하악의 동일 부위에 제작된 지르코니아 도재관의 파절강도는 소구치에서 상악이 하악에 비하여 낮은 것으로 나타났으며 (P< .05), 다른 부위에서는 유의한 차이가 없었다 (P> .05). 결론: 본 실험의 결과로 지르코니아 도재관은 임상에서 활용하기에 만족할 만한 강도를 갖고 있음을 알 수 있었다.

전부 주조관 제작을 위한 내화모형재 분리에 관한 연구 (A Study about the Separation of the materials Used in Refractory Cast Dies For Making All Ceramic Crowns)

  • I attempted to suggest a better method to separate a refractory cast in order to solve the problems caused by bonding between refractory casts and ceramics in the firing-processing of all ceramic crowns as a part of dental prosthodontics. I emphasize the advantages and effectiveness of easy separation after firing by using the separating materials about the refractory cast dies. 1. We must not usse HF(55%) that is used as the chemical separating methods. 2. We decrease such methods as grinding, blasting, bur, and point 3. We can save time, costs and manual labors by using this methods. 4. This method regulates the expansion and constriction of heat because of the chemical reaction of the separating materials. 5. This method contribute to the breaking down without modification by minimizing the chemical and mechanical damages of the marginal and interior parts.
