Physical Fitness, Leisure Time Physical Activity, and Serum Lipid Levels in Middle-Aged Male Workers (중년 남성 근로자에서 신체 적합도, 여가중 신체 활동과 혈중 지질 농도)

  • This is a cross-sectional study to evaluate the relationships between physical fitness, leisure time physical activity, and serum lipid levels in middle-aged male workers. Physical fitness was measured by a step test score, and leisure time physical activity was self-reported on a questionnaire. Serum total cholesterol was negatively related to physical fitness(r=-0.27), and positively to obesity index(r=0.27). But leisure time physical activity was related to total cholesterol negatively(r=-0.20) only in subjects whose total cholesterol levels were above 170mg/dl. High density lipoprotein(HDL) cholesterol was positively related to physical fitness(r=0.15), negatively to obestiy index(r=-0.22), and positively to weekly alcohol consumption(r=0.14). Total cholesterol/HDL cholesterol ratio was related to physical fitness(r=-0.23), obesity index(r=0.32), total cigarette index (r=0.13), weekly alcohol consumption(r=-0.13), and vegetable preference(r=0.13). Physical fitness was also related to leisure time physical activity(r=0.19) and obesity index(r=-0.18). In multiple linear regression models, physical fitness(beta=-0.23) and obesity index(beta=0.18) were significantly associated with total cholesterol, obesity index(beta=-0.25) with HDLcholesterol, and obesity index(beta=0.30), physical fitness(beta=-0.16) and vegetable preference (beta=0.14) with total cholesterol/HDL cholesterol ratio. In conclusion, as physical fitness has a stronger relationship with serum lipid levels than leisure time physical activity, and the association between physical fitness and leisure time physical activity is modest, physical fitness should be added as an important variable in addition to activity in future epidemiologic studies.

The Welfare Systems in Sweden and Korea with a Focus on the Demographic Transition (인구변천 과정에서 본 한국과 스웨덴의 복지 상태 비교)

  • The Swedish welfare state has been the model for others to emulate the archetypical example of state intervention. The state interventions are presented in the form of legal acts. These social welfare acts can be classified according to the demographic transition theory. According to the Bogue's theory, the demographic transition in Sweden took place in four stages : the pre-transitional stage before 1810; the early transitional stage from 1810 to 1860; the mid-transitional stage from 1860 to 1930; the late transitional stage from 1930 to now. As we look into the social welfare acts in Sweden, the relief of the poor was the major concern of the early transitional stage, the care of workers was the major concerns of the mid-transitional stage and the care of the families was the major concerns in the late transitional stage. The Korea's transition period can be devided as follows; the pre-transitional stage before 1960; the early transitional stage from 1960 to 1969; the mid-transitional stage from 1970 to 1987; and the late transitional stage from 1987 to now. In Korea, the major concern of the early transitional stage was the care of the officials and the workers; in the mid-transitional stage the care of the aged and the handicapped were the major concerns. And in the late transitional stage the expanding of the welfare clients was the major concern. If we compare the results of both countries, the relief of the poor, the care of the workers and the care of the families will be the major concerns in Korea, because the social welfare acts in Korea are extended to specific groups and not to the whole population. The acts related to these social issues have been arranged in 120 years in Sweden. But Korea had to do the same work in 27 years. So the burden of making those social acts will be four times heavier. If we want to extend the benefits of the social system to the general population, we need to look at the design and approach of the swedish model. The reason why swedish social acts constitute an international model has more to do with the uniqueness of its design and approach. First of all, it is characteristic by its universalism, secondly by its emphasis on social services and thirdly by its productivitism. Also the swedish welfare state supported by a high-tax system called the earnings-related welfare system. In order to achieve an effective welfare state, we Koreans should pay attention to the relief of the poor, the care of the worker and the families. We should also focus on a good system design and prepare appropriate budgets.

Survey on Menu Preferences of Adults for Korean Food Made from Korean Traditional Sauces (장류를 이용하여 조리하는 한식 메뉴에 대한 성인 기호도 조사)

  • This study was performed to investigate menu preferences of adult customers for Korean food made from Korean traditional sauces. A total of 962 valid responses were used for data analysis. Statistical analyses were conducted using the SPSS package program (ver 20.0). The results of this study are summarized as follows: overall preferences were highest for braised spareribs, followed by braised short ribs, grilled beef ribs, and bulgogi. On the other hand, overall preferences were lowest in eggplant namul, followed by squid and radish soup, and grazed lotus roots and burdock. Female's overall preferences were significantly higher than male's for nine menu items, including japchae and seasoned acorn starch curd with vegetables, whereas male's overall preferences were significantly higher than female's for 19 menu items, including grilled deodeok and frozen pollack stew. Moreover, the menu item that was most significantly preferred was soybean sprouts soup in subjects aged under 20 years and Korean meatballs in subjects in their twenties. Subjects in their forties showed significantly higher preferences for 15 menu items, including braised hairtail, grilled deodeok, and seasoned and grilled yellow corvina compared to other age groups. Furthermore, menu preferences of production service workers were significantly higher than other groups for frozen pollack stew, loach stew, grilled dried pollack, steamed dry pollack, dried pollack soup, eggplant namul, and seasoned and grilled yellow corvina, whereas menu preferences of official professional workers and students were significantly higher than those of production service workers for braised short ribs, grilled beef ribs, seasoned and simmered chicken, mixed noodles, Korean meatballs, stir-fried rice pasta with vegetables, spicy soft bean curd stew, japchae, mung bean jelly mixed with vegetables and beef, bibimbap, and stir-fried squid. The results show that menu preferences of adults customers differed depending on gender, age, and occupation. In conclusion, the results of this study should provide foodservice managers with information about menu planning for target customers of commercial or non-commercial foodservice cafeterias and recipe development of low-sodium Korean foods.

Employment Support for the Low-income Elderly in the OECD Countries: Implications for Senior Employment Policy (OECD 국가의 저소득 고령자 고용지원정책 : 노인일자리사업에 주는 함의)

  • The Korean government has implemented the senior employment policy as a direct job creation policy since 2004. A realistic discussion of policy alternatives and orientation for this has been given little attention even though senior employment policy has been carried out for the last 10 years and it will be expanded next year. This study tries to examine active labor market policy especially focusing on direct job creation programs and policies for the disadvantaged low-income elderly in OECD countries, and then it suggests some developmental alternatives for senior employment policy based on the study's results. The main results from this analysis are summarized in two points. Firstly, except pension policies, employment policy for older workers in the OECD countries is highly proportional to the tackling of objective factors reducing the demand for older workers (wage subsidies, reduced social security contribution rate etc). And the strategies of improving employability have not been relatively important and direct job creation policy has been marginal. Secondly, employment support policies for the low-income elderly can be divided into three types: support for the low-income elderly, alleviating early retirement and support for full employment according to the criteria which are determined by policy objectives and the social economic index. Korea's employment support policies belong to the type of direct job creation among them. This seems to be due to the fact that the rate of elderly poverty is extremely high and an income security system has not been developed in Korea. However, the policy objective is still uncertain. Therefore, this policy needs to set up clear objectives and establish a proper system for the achievement of its goals. If we focus on the strength of its employment characteristics, we need to modify the policy's plan in the perspective of labor market policy. But if we intend to keep both of the current objectives, it is better for this policy to be divided into two parts: social participation and income supplements. Or it also may be a solution to transform the system into an employment service, a training system which supports participants to move into unsubsidized jobs such as SCSEP in the U. S.

Performance State and Improvement Countermeasure of Primary Health Care Posts (보건진료소(保健診療所)와 업무실태(業務實態)와 개선방안(改善方案))

  • This study was performed to investigate the performance state and improvement countermeasure of Primary Health care Posts(PHPs). The operation reports of PHPs(1996 330 PHPs, 1999 313 PHPs) located in Kyongsangbuk-Do and data collected by self-administered questionnaire survey of 280 community health practitioners(CHPs) were analyzed. The major results were as follows: Population per PHP in 1999 decreased in number compared with 1996. But population of the aged increased in number. The performance status of PHP in 1999 increased compared with 1996. A hundred forty one community health practitioners(50.4%) replied that the fiscal standing of PHP was good. Only 1.4% replied that the fiscal standing of PHP was difficult. For the degree of satisfaction in affairs, overall of community health practitioners felt proud. The degree of cooperation between PHP and public health institutions was high and the degree of cooperation of between PHP and private medical institutions was high. The degree of cooperation between PHP and Health Center was significantly different by age of CHP, the service period of CHP, and CHP's service period at present PHP. Over seventy percent of CHPs replied that they had cooperative relationship with operation council, village health workers, community organization. CHPs who drew up the paper on PHP's health activity plan were 96.4 % and only 11.4% of CHPs participated drawing up the report on the second community health plan. CHPs who grasped the blood pressure and smoking status of residents over 70% were 88.2%, 63.9% respectively and the grasp rate of blood pressure fur residents were significantly different according to age and educational level of CHP. CHPs received job education in addition continuous job education arid participated on research program in last 3 years were 27.5%, respectively. CHPs performed the return health program for residents in last 3years were 65.4%. Over 95% of CHPs replied that PHPs might be necessary and 53.9% of CHPs replied that the role of PHPs should be increased. CHPS indicated that major reasons of FHPs lockout were lack of understanding for PHP and administrative convenience, CHPs were officials in special government service governors intention of self-governing body. CHPs suggested number of population in health need such as the aged and patients with chronic disease, opinion of residents, population size, traffic situation and network in order as evaluation criteria for PHP and suggested results of health performance, degree of relationship with residents, results of medical examination anti treatment, ability for administration and affairs in order as evaluation criteria for CHP. CHPs replied that the important countermeasures for PHPs under standard were affairs improvement of PHPs and shifting of location to health weakness area in city. Over 50% of CHPs indicated that the most important thing for improvement of PHPs was affairs adjustment of CLIP. And CHPs suggested that health programs carried out in priority at PHP were management of diabetes mellitus and hypertention. home visiting health care, health care for the aged. The Affairs of BLIP should be adjusted to satisfy community health need and health programs such as management of diabetes mellitus and hypertention, home visiting health care, health care for the aged should be activated in order that PHPs become organization reflecting value system of primary health care.

Analysis of Biomechanical Changes According to Mechanical Alignment of the Lower Limbs when Gait with a Material Handling (중량물 취급 보행 시 하지의 역학적 정렬에 따른 생체역학적 변화 분석)

  • Objective : Walking with a Material handling is an activity frequently undertaken by agricultural workers in Korea, due to the nature of their work. This study aimed to investigate differences in biomechanical variables according to the mechanical alignment of the lower limbs when walking with a heavy load, and to use this as basic data in the design of various working environments to reduce the skeletomuscular burden on the knee joint. Method : The study subjects comprised of 22 right-foot dominant adult men and women aged between 20 and 23 years. The subjects were divided into a varus or valgus group according to the mechanical alignment of the lower limb by using radiographic findings. The subjects walked without any load and with a load of 10%, 20%, or 30% of their body weight held in front of them. The Kwon3d XP program was used to calculate biomechanical variables. Results : The flexion/extension moment of the knee joint showed a decreasing trend with increased load, irrespective of the mechanical alignment of the lower limb, while the varus group did not show normal compensatory action when supported by one leg at the point of maximum vertical ground reaction force. In addition, in terms of the time taken, subjects showed no difficulties in one-foot support time up to 20%/BW, but at 30%/BW, despite individual differences, there was an increase in single limb. The increased load resulted in a decrease in the ratio of standing phase to ensure physical stability. The valgus group showed a trend of increasing the stability of their center of mass with increasing load, through higher braking power in the early standing phase. Conclusion : In conclusion, although there was no statistical difference in biomechanical variables according to the mechanical alignment of the lower limbs, the varus group showed a more irregular walking pattern with a Material handling than the valgus group, partially proving the association between lower limb alignment and walking with a Material handling.

Blood Aluminum Concentrations among Residents of the South-east Costal Area of Korea (우리나라 일부 지역 주민들의 혈중 알루미늄 농도)

  • Objectives: Aluminum is well known as a potent neurotoxicant. There are many reports that aluminum can be toxic to humans and to animals. However, there are only few studies on the assessment of aluminum exposure among humans in Korea. Therefore, the aim of this study was to evaluate current aluminum concentrations among the adult population in regions of Korea. Methods: We selected 439 adults aged 20-89 years from the Busan-Ulsan-Gyeongnam region and certain other sites. Blood aluminum concentrations were analyzed using inductively-coupled plasma mass spectrometry (ICP/MS). The geometric means (GM) and 95% confidence intervals (CI) of aluminum concentrations were calculated, and we also confirmed the proportion of excess of the aluminum reference value. Results: Total GM (95% CI) of aluminum levels was 14.26 (13.43-15.14) ppb. Levels among males (15.58 [14.09-17.22] ppb) were higher compared to among females (13.51 [12.54-14.55] ppb), and levels increased with subject age from 40 years and over. Three point one nine percent of the subjects exceed the reference value of 50 ppb. Lastly, aluminum concentration has a log-normal distribution with lnN (x; 2.89, 0.642), x>0. Conclusion: In the present study, we evaluated the distribution of blood aluminum concentrations among the normal population in Korea, and we found some adults exceeding the reference levels. However, in order to compare the results with other studies, an extended study including measurement of serum aluminum level is required. In addition, further research on various population groups, including occupational exposure of workers, is required.

Child Nutrition Survey in Rural Health Project Areas (농촌보건사업지역(農村保健事業地域)의 아동영양(兒童營養) 실태조사(實態調査))

  • The main purposes of the nutrition and clinical surveys were to provide baseline information on the nutritional status of pre-school children in rural health demonstration project areas of the Korea Health Development Institute (KHDI) for nutrition guidance services for the MCH target group. The survey covered a total of 222 pre-school children and 135 mothers in Okgu Gun, Cholla Pukto Province from August 10 to August 17, 1979. The survey results are summarized as follows: 1) Family Environment Seventy percent of the households had more than three children, and the mean family sire was 6. Sixty-nine percent of the mothers and 47% of the fathers of the surveyed households were educated at or below the primary school level. The majority, 70% of the mothers, were aged between 20 years and 35 years. 2) Anthropometric Measurements and Hemoglobin Value 4.5% of the children were lower than 80% weight for age of the Korean standard, and 5.4% were lower than 85% arm circumference for age of the Jelliffe's standard resectively, and those were suffering from protein-energy malnutrition. Angular stomatitis were observed on 66.2% of the subjects. Mean hemoglobin value was 11.1g/100m1, and 44.2% of the subjects were categorized as anaemia. 3) Food and Nutrient Intake of animal foods was very low, ranging from 2.9 to 17%. Consumption of eggs was less than 2% of total food intake, and intake of legumes was also very scanty, between 0.8 to 3.7%. These data present evidence of very poor protein intake, quality as well as quantity. Energy intake of children was 60.0 to 64.4% of the recommended allowance, and mean protein intake only met 47.4% of the recommendation. Low intake of vitamins except thiamin were also found. 4) Mother's Nutrition Knowledge Eighty-five percent of the mothers were entirely ignorant regarding the 'five basic food group' which is most important fact on food and nutrition guidance. Mean knowledge score from 14 basic questions about food and nutrition was as low as 5.1. There was a significant positive correlation between mother's educational level and nutrition knowledge score. 5) Family Planning Variable There were significant correlation among maternal, family planning variables, and some of the nutritional and physical measurements. The study revealed that the mother's educational level and nutrition knowledge score are more crucial factors than the family planning variables on effecting food intakes on children. Recommendation : According to the results of the surveys, there were high incident rates of nutritional anaemia and angular stomatitis among pre-school children, and most of rural women had very limited knowledge about food and nutrition. As a main part of the health education activities, the community health workers should provide nutrition education to the village mothers to improve the nutrional status of young children in rural areas. Nutrional promotion at the primary health care level should be mainly based on appropriate nutrition education.

Association of Family Values with Depressive Mood in Korean Married Women: The 4th Korean Longitudinal Survey of Women and Families Panel (기혼여성의 우울감과 가족가치관과의 관련성 연구: 제4차 여성가족패널조사(2012) 자료분석)

  • Background: Family values of a married woman may be related to her own depressive mood. Since depressive mood of a married woman is likely to exert a negative influence, in terms of mental health, on her, her family members, and the whole society's, it may be very important to explore the relationship between family values in married women and their depressive mood. Methods: In this study, we analyzed nationally representative 5,818 married women aged 20 years or older from the 4th panel data of 2012 Korean Longitudinal Survey of Women and Families. As for variables of interest, we constructed three family values variables: family-oriented view of marriage, individualistic view of marriage, and traditional view of marital roles. Then we employed multivariate logistic regression analyses to explore the relationship between family values and depressive mood, adjusting for family and socio-demographic factors. Results: In total, 804 married women (18.4%) had experienced depressive mood. All of the three family values variables were significant in their relationships with depressive mood. The women categorized as 'very weak' in family-oriented view of marriage were more likely to experience depressive mood than the women categorized as 'very strong' (odds ratio [OR], 1.98; 95% confidence interval [CI], 1.53-2.55). By contrast, the women categorized as 'very weak' in individualistic view of marriage (OR, 0.43; 95% CI, 0.33-0.55) and in traditional view of marital roles (OR, 0.68; 95% CI, 0.51-0.92) were less likely to experience depressive mood than their respective counterpart women categorized as 'very strong.' Conclusion: In Korea, married women's values towards marriage itself and roles between wives and husbands had significant associations with their depressive mood. This suggests that in order to improve mental health in married women, we need to take social and cultural dimensions into consideration along with public health interventions.

Mediation Effect of Family Resilience between Family Stress and Social Problem Solving Ability of Women's Social Worker in Middle Age (중년기 여성 사회복지사의 가족스트레스와 사회적 문제해결능력 간 가족탄력성의 매개효과)

  • This study examined the mediation effect of the family resilience between the family stress and social problem solving ability of the women social worker and welfare specialist, considering that women's middle age needs the resilience including the adaptation, reestablishment in the family life cycle. The sampling target was the 328 women social workers in the middle age, and it performed the Sobel Test for the mediation effect verification after enforcing the multiple regression analysis, next, it is same. Firstly, the economic problem was most highly severe among the family stress subfactor. Secondly, there was altogether the part mediation effect of the family resilience' subfactors, ie, communication processes, belief systems and organizational patterns between the family stress and social problem solving ability. In conclusion, if the family stress which the middle aged women social worker confronted was accurately analyzed and the family is intervened so that the family resilience functions properly, the social problem solving ability would be able to be improved.
