비골 골절에서 외측 비골 절골술의 적용 (Application of Lateral Osteotomy in Nasal Bone Fracture)

  • Purpose: Who may dare to state that optimal choice of treating nasal bone fracture is closed reduction? Few decades of authors' experience in nasal bone fracture has lead to believe that more active and assertive approach in nasal bone fracture by performing simultaneous lateral osteotomy may be applied in proper indications to acquire more accurate reduction and cosmetically satisfying result. Methods: From May 2008 to October 2009, among 241 nasal bone fracture patients, 20 patients underwent simultaneous lateral osteotomy with nasal bone fracture reduction. Followed by rigid septal correction, nasal cavity is packed to stabilize the fracture segment for safer osteotomy. Through intranasal incision, in selected cases of difficult reduction or for cosmetic purposes, various types of lateral osteotomy was performed corresponding to the fracture anatomy, conditions of the nasal cavity. Postoperative nasal packing was retained for one week and nasal dorsum splint for 3 weeks. Results: Lateral osteotomy was utilized for difficult cases of closed reduction, for correction of wide nose, hump and deviation in 9, 5, 2, and 4 cases, respectively. Patient satisfaction was scaled 90% in satisfaction and moderate in 10% (2 cases), complaining of mild nasal tip deviation. Physicians detected 2 cases of apparent deformity with patient recognition; one patient with mild step deformity at the osteotomy site and the other patient with minimal implant mobility. Conclusion: By accompanying profound understanding of the fracture anatomy, more active and assertive approach in nasal fracture reduction can be coincide with simultaneous lateral osteotomy to reduce the rate of secondary deformity and to obtain more cosmetically satisfying result.

3D 얼굴 성형 제작 시스템 구현 (Design and Implementation of 3D Facial Aesthetic Surgery System)

  • 본 논문은 3D 그래픽 기술을 이용하여 성형 시술 전후의 입체 얼굴을 제작하여 얼굴 성형 시술자의 만족도를 높이는 응용프로그램 시스템에 관한 구현 및 연구이다. 사용자의 실물에 최대한 유사한 원형 3D 입체 얼굴을 Pre-Processing 과정 및 Face-Part-Making 과정으로 구분하여 각 과정에 필요한 기술들을 사용하여 구현하였다. 또한 각 얼굴 부위를 담당하는 변형 Controller를 구현하여 제작된 입체 얼굴의 변화를 볼 수 있는 성형 제작 시스템을 구현하였다. 본 논문에서 구현한 얼굴 성형 제작 시스템은 현재 성형외과 등에서 활용되고 있는 2D 시스템에 비하여 효율성과 정확성, 만족도 등에서 보다 좋은 결과를 보여주고 있다.

치유체험프로그램이 만족도에 미치는 영향에 관한 연구: 체험비용과 체험시간을 중심으로 (A Study on the Effect of Healing Experience Program on Satisfaction: Focused on Experience Cost and Experience Time)

  • 본 연구는 치유농업 분야에서 치유체험프로그램이 만족도에 미치는 영향에 관한 연구이다. 농촌 체험프로그램을 개발 하는데 있어서 치유 체험프로그램을 구성하는 어떤 요인이 만족도에 영향을 미치는지와 치유체험프로그램의 마케팅관점에서 어느 정도의 참여시간과 참여비용이 치유체험프로그램의 만족도에 미치는지에 대한 영향을 실증분석하고자 한다. 치유 체험프로그램의 체험비용과 체험시간이 만족도에 미치는 영향에 대하여 연구해 봄으로써, 치유체험프로그램의 개발 방향성을 제시하고자 한다. 이를 위하여 체험비용과 체험시간을 매개변수로 하는 치유체험프로그램이 만족도에 미치는 영향을 실증분석함으로써 농촌 체험프로그램의 개발 시 우선적으로 고려해야 할 부분에 대한 이론적 기반을 제시한다. 체험프로그램의 하위요인으로 오락적 체험, 교육적 체험, 일탈적 체험, 심미적 체험이 있으며, 매개변수로 체험시간, 체험비용이 있으며, 종속변수로 만족도를 두었다. 또 한 설문대상자의 인구통계학적 변수를 통제변수로 설정하여 연구결과의 신뢰성을 확보하였다. 조사대상은 치유체험프로그램에 관심이 있거나 인지하고 있는 불특정 다수를 대상으로 수집한 설문지 중 유효한 314명을 대상으로 실증분석을 시행하였다. SPSS v22.0을 사용하였으며, 매개 효과를 검정하기 위하여 Baron & Kenny(1986)의 3단계 검증법과, Andrew F. Hayes(2018)의 SPSS PROCESS Macro Model No.4 검증 방법을 적용하여 분석결과에 대하여 비교함으로써 매개 효과에 대한 신뢰성을 확보하였다. 연구결과 교육적 체험(𝛽=.134, t=1.759*)이 체험비용에 정(+)의 영향을 미치는 것으로 나타났으며, 심미적 체험(𝛽=.144 t=1.684*)이 체험시간에 정(+)의 영향을 미치는 것으로 나타났다. 또한 교육적 체험(𝛽=.239, t=4.112***)이 만족도에 정(+)의 영향을 미치는 것으로 나타났으며, 심미적 체험(𝛽=.330 t=4.921***)이 만족도에 정(+)의 영향을 미치는 것으로 나타났다. 체험시간은 심미적 체험과 만족도 사이에서 부(-)의 비일관적 매개 효과를 미치는 것으로 나타났다. 즉 심미적 체험이 만족도에 미치는 총 효과는 𝛽=.330 t=4.921***이며, 체험시간을 매개변수로 투입 시 직접효과(𝛽=.349 t=5.241***)는 총효과에 비해 𝛽=.019 만큼 증가한 반면, 간접효과(𝛽=-.019)로 부(-)의 매개 효과를 미치는 것으로 나타났다.

가치관과 의복의 만족도, 과시성, 유행 및 한복에 관한 연구 - 대전 지방의 여대생과 어머니를 중심으로 - (A Study of General Values and Satisfaction, Exhibitionism, Fashion of Clothing and Korean Clothing)

  • The purpose of this study were; (1) to investigate the relationship between the general values and 3 aspects of clothing behaviors. (2) to investigate attitudes toward Korean clothing between students and their mothers. General values were measured by the Allport-Vernon-Lindzey Study of Values(Part 1 only), which determines relative prominence of six basic values. Three aspects of clothing behaviors were assessed by statements dealing with satisfaction, exhibitionism, and fashion. Korean clothing measures consisted of statements dealing with positive and negative attituded toward psychological and practical aspects. The samples consisted of 200 students(student group) and 100 mothers (mother group) in Daejon. The data were analyzed by correlation and t-values. The results obtained through this study were as follows: 1) Positive relationship were found between: (1) Aesthetic value and the exhibitionism of clothing, (2) Aesthetic value and the fashion of clothing. Negative relationships existed between (1) Theoretical value and the exhibitionism of clothing, (2) Economic value and the fashion of clothing, (3) Social value and the fashion of clothing. 2) Significant differences were found between two groups(students and their mothers) : Economic, aesthetic, and political values as well as exhibitionism and fashion in clothing behaviors. 3) In general, highly positive attitude toward Korean clothing was found on psychological aspect, while highly negative attitude was found on practical aspect. Significant differences were found between two groups; (1)‘Graceful’of psychological aspect and‘economic’,‘inexpensive’of practical aspect in positive attitudes toward Korean clothing, (2)‘Unsuitable to current life style’,‘not modern’and‘not beautiful’of psychological aspect in negative attitudes toward Korean clothing.

수단-목적 사슬이론에 따른 체험용 대여한복업체의 가치구조 (Value Structure of Experiential Rental Hanbok Companies Based on Means-end Chain Theory)

  • This study explores the value structure of experiential rental Hanbok companies using the means-end chain theory. For this, soft laddering was conducted for nine experiential rental Hanbok companies located in Jeonju Hanok Village. Next, the value structure was created. The results are as follows. First, the elicited values are in the following order: "creation of wealth," "promotion of Korean culture," "aesthetic value," "self-actualization," "community development," and "transmission of tradition." Second, most attributes are connected to "creation of wealth." Third, "promotion of Korean culture" and "community development" are similar to Holden's instrumental values. Fourth, some attributes linked to "aesthetic value" express what the experiential rental Hanbok companies consider to be beautiful. Fifth, the companies pursue "company satisfaction" through "consumer satisfaction" and "company differentiation," which leads to "self-actualization" and "aesthetic value." Sixth, it seems that "transmission of tradition" can be achieved through "expertise/expert training," indicating that some companies think expertise is necessary for the transmission of Hanbok and do it. This study is significant because it is a new attempt to understand the core values of experiential rental Hanbok companies and provides insights into their growth.

A Study on Workwear Prototype Development: Based on the Functional, Expressive, Aesthetic (FEA) Model

  • This study aims to develop workwear prototypes by applying a systematic approach considering the characteristics of workwear. A case study was conducted before this study to derive workwear's four characteristics: 'Ergonomic Pattern-Making, Certified Fabric, Specialized Color, and Customized Details.' a prior study proposed the integrated framework combining these characteristics with the FEA model. The new framework identified that these characteristics are considered in terms of functionality, expression, and aesthetics; it can increase workers' satisfaction and meet the market demand without concentrating on only particular aspects. Before prototype production, the requirements for each characteristic of workwear were analyzed through theoretical research of previously published related papers. The study primarily gathered workwear requirements data and sources from consumer satisfaction surveys and investigations into the wearing conditions of work clothes. When considering all aspects of pattern-making, fabric, color, and detail in functionality, 'comfort movement, body protection, improved work efficiency' were identified as necessary. Expressive requirements were fundamental, including 'reflecting the wearer's preferences, expressing a sense of belonging and identity. It was clear that incorporating design elements and applying current trends to the aesthetic requirements of work clothes was necessary. Four prototypes comprised two top and bottom sets and two overalls using these requirements. The framework was used throughout the entire process of planning, producing, and evaluating prototypes, and through this, the results fulfilled the requirements. This study is significant because it produced workwear prototypes using an integrated approach that considered functional, expressive, and aesthetic aspects.

서양복식에 나타난 양적과장의 미의식에 관한 연구(I) -고대부터 근대복식을 중심으로- (The Aesthtic Consciousness of Voluminous Enlargement in the Western Costume - From Ancient to the Modern Times -)

  • Opposed to following the contours of the human body, the voluminous enlargement in costume, which characterizes the distinguished enlargement in space rather than the contour of human body, mean the enlargement aspect involving the vertical protrusion and the expansion of shape and volume as well as the extension of length. The costume enlargement as a different method of expression is a symbol showing a meaning of something and an aesthetic expression containing man's will. This voluminous enlargement of costume, as an aesthetic expression, has different formative characteristicsand immanent meanings according to ideals and thoughts as well as social and cultural background of each age Accordingly, the aesthetic consciousness also differaccording to the change of the times. To study the aesthetic consciousness of costume's voluminous enlargement, focous had been given to milieus that show comparatively conspicuous voluminous enlargement Periods that have been subjected to this study include costumes of the ancient Egypt, the Gothic period in the Middle age, the Renaissance, the Baroque Rococo of the recent times, and the modern era (Empire, Romantic, Art Nouveau. etc) With focus given to the principle of design obtained through this study were used to analyze the aesthetic characteristics, Futhermore, based on the spirit of the times and the socio-cultural symbolism, research on immanent meanings, as supported by objectivity and universal validity, was also made, the enlargement beauty of costume had been placed under the aesthetic category and, by interpreting the analogies of presented in aesthetic consciousness, the true nature of the voluminous enlargement in costume had been traced. Immediate breast reconstruction following nipple-sparing mastectomy in an Asian population: Aesthetic outcomes and mitigating nipple-areolar complex necrosis

  • Background Nipple-sparing mastectomies (NSMs) are increasingly performed to obtain the best aesthetic and psychological outcomes in breast cancer treatment. However, merely preserving the nipple-areolar complex (NAC) does not guarantee a good outcome. Darkly pigmented NACs and a tendency for poor scarring outcomes are particular challenges when treating Asian patients. Herein, we review the reconstructive outcomes following NSM at Singapore General Hospital. Methods All breasts reconstructed following NSM over an 11-year period from 2005 to 2015 were reviewed. Information was collected from the patients' records on mastectomy indications, operative details, and complications. Patient satisfaction, breast sensation, and aesthetic outcomes were evaluated in 15 patients. Sensation was quantified using the Semmes-Weinstein monofilament test. Results A total of 142 NSMs were performed in 133 patients for breast cancer (n=122, 85.9%) or risk reduction (n=20, 14.1%). Of the procedures, 114 (80.2%) were autologous reconstructions, while 27 (19.0%) were reconstructions with implants. Complications occurred in 28 breasts (19.7%), with the most common complication being NAC necrosis, which occurred in 17 breasts (12.0%). Four breasts (2.8%) had total NAC necrosis. The overall mean patient satisfaction score was 3.0 (good). The sensation scores were significantly diminished in the skin envelope, areola, and nipple of breasts that had undergone NSM compared to non-operated breasts (P<0.05). Half of the subset of 15 patients in whom aesthetic outcomes were evaluated had reduced nipple projection. Conclusions Immediate reconstruction after NSM was performed with a low complication rate in this series, predominantly through autologous reconstruction. Patients should be informed of potential drawbacks, including NAC necrosis, reduced nipple projection, and diminished sensation.

Long-term postoperative satisfaction and complications in nasal bone fracture patients according to fracture type, site, and severity

  • Background: It is difficult to completely fix nasal bone fractures with closed reduction, as it is often accompanied by septal cartilage damage, and this often results in postoperative secondary deformities. Thus, patients are often reluctant to undergo closed reduction surgery. The present study aimed to evaluate aesthetic and functional satisfaction, as well as satisfaction with and complications of closed reduction, according to nasal bone fracture type. Methods: The subjects were patients who underwent closed reduction under general anesthesia from January 2017 to December 2018. Based on the modified Murray classification, patients were classified into five groups according to the fracture site, septal fracture, and deviation. A total of 211 patients were sent a web-based survey on postoperative satisfaction and complications, as well as intention for revision and cosmetic surgery. Sixty-one patients (28.9%) responded. Results: There were no significant differences in aesthetic and functional satisfaction or satisfaction with closed reduction according to the fracture type, site, or severity. Postoperative functional complications developed in 14 of 61 patients (22.95%). With 10 out of 24 (41.67%) patients (p = 0.044), the bilateral fracture with septal fracture or prominent septal deviation type had a higher incidence of complications than the other types. Conclusion: The incidence of complications is higher for bilateral fracture with septal fracture or prominent septal deviation compared to the other nasal bone fracture types. Therefore, long-term follow-up after closed reduction surgery for this fracture type can aid in establishing additional postoperative treatment plans and improving patient satisfaction.

현대 패션에 나타난 고스(goth)이미지의 미적 특성 (The Aesthetic Characteristics of Goth Image in Modern Fashion)

  • The purpose of this study is to analyze the aesthetic characteristics of goth image which have had influence on literature, film, music and art, in modern fashion. The method of this study is to analyze the documentary about gothic and goth, and the fashion magazines since 2000. The results of this study are as follows: First, sensuality shows the excessive exposure of body and inner wear, and emphasizes a resistance of sexual consciousness and a image of independent, active, powerful woman. Second, androgyny shows the goth women wearing a men's cloth and encourages a person to have a perfect being and satisfaction. Third, horror shows the symbol of death and suggests a substance of desire hidden in our mind. Fourth, historicity shows victorian fashion which have a romance of gothic and baroque, not a cult but a modern image. In conclusion, goth image in modern fashion does not show a substance negative and horrorful, but a substance positive as a perfection, satisfaction, a sense of freedom, obliteration of a feeling of uneasiness and powerful woman's image.
