Effects of Preferred Arch Height and Hardness of the Insole on Static Arch Height and Ankle Stability (인솔의 아치높이 및 경도 선호도가 정적 아치 높이 및 발목 안정성에 미치는 영향)

  • Objective: The purpose of this study was to investigate the differences in static arch height and ankle stability according to the preference for insole height and hardness in the arch area. Method: The study participants were 20 adult males (age: 22.7 ± 1.8 yrs., height: 175.3 ± 4.3 cm, body weight: 72.5 ± 7.7 kg). First, the arch heights of all subjects were measured in static postures (sitting and standing). The inversion and eversion movements of the ankle joint were analyzed during walking (1.3 m/s & 1.7 m/s) and running (2.7 m/s & 3.3 m/s). The variables (static arch height, and inversion and eversion angle of ankle joint) were compared by classifying groups according to the preference for the height and hardness of the arch of the insole. First, it was divided into a high arch insole preference group (HAG, n=8) and a low arch insole preference group (LAG, n=12) according to the preference for the arch height of the insole. Second, it was divided into a high hardness insole preference group (HHG, n=7), medium hardness insole preference group (MHG, n=7), and low hardness insole preference group (LHG, n=6), according to the preference for the arch hardness of the insole. Results: First, the range of motion (ROM) of inversion-eversion at the ankle joint during walking was statistically smaller in HAG than in LAG (p<.05). Second, the arch height change of HHG was statistically greater than that of MHG and LHG (p<.05). Conclusion: In the case of flexible flat feet with a large change in arch height, providing a high hardness arch insole that can disperse foot pressure can improve comfort. It was found that people with high medial and lateral sway of the ankle joint preferred a low arch insole, but it is necessary to differentiate and compare the insole heights of the arch part in detail. In addition, in the case of fast motion such as running, the preference for the arch height and hardness of the insole was not related to the static arch height and ankle stability.

The Moderating Effects of Age and Gender on the Relationship between Values and Communication styles of Korean Adults (한국 성인의 가치와 의사소통 방식 간의 관계에서 연령과 성별의 조절효과)

  • This study examined the moderating effects of age and gender on the relationship between values and communication styles of Korean adults. Five hundred adult men and women across the country responded the questionnaires regarding cultural universal values (openness to change, self-enhancement, conservatism, and self-transcendence), cultural-specific values (collectivism, conformity to norms, emotional self-control, family recognition through achievement, and humility), high-context communication style, and low-context communication style. The results of this study are as follows. First, as a result of exploring the factors influencing the communication style, self-enhancement, emotional self-control, and self-transcendence significantly predicted the high-context communication style. Whereas openness to change, self-enhancement, conformity to norms, emotional self-control, and gender significantly predicted the low-context communication style. Second, age moderated the relationship between self-enhancement and high-context communication style. The high-context communication style significantly increased when the level of self-enhancement was high and the age was younger. Third, age and gender moderated the relationship between conformity to norms and high-context communication style. In the case of males with high conformity to norms and younger age, the high-context communication style significantly increased. Fourth, gender moderated the relationship between collectivism and low-context communication. As collectivism increased, men tended to increase low-context communication styles, while women tended to decrease it. Fifth, gender moderated the relationship between humility and low-context communication. In the case of women with high humility, their low-context communication style was significantly lowered. The implications and limitations of the results of this study were discussed.

Taxonomic Status of Acheilognathus sp. HR (Cypriniformes: Acheilognathidae) Found in the Dalcheon River, a Tributary of Hangang River, Korea (한강 지류 달천에서 발견된 Acheilognathus sp. HR (Cypriniformes: Acheilognathidae)의 분류학적 위치)

  • To investigate the taxonomic status of undescribed species Acheilognathus sp. HR from Korean Acheilognathidae discovered from the Dalcheon River, a tributary of the Hangang River, molecular phylogenetic and morphological characteristics were compared and analyzed with previous studies. As a result of molecular phylogenetic analysis, A. sp. HR formed the same genetic clade as the five subspecies of Acheilognathus tabira, but formed a separate monophyletic group based on the unique genotype, showing clear differences. As a result of morphological analyses, the dorsal fin color in males is grayish and the nuptial coloration of the outer edge of the anal fin is white. The outer edges of the dorsal and anal fins are convexly rounded. A black blotch is present on the dorsal fin of the juvenile, but there is a black blotch absent on the dorsal fin of the small adult female. In the counts, the number of branched dorsal rays is 12~13. In the measurements, the length of the barbels is short and the body depth is deep. Therefore, the A. sp. HR of Hangang River is considered at the level of a distinct species distinguished from each other by the five subspecies of A. tabira by molecular phylogenetic, morphological, and limited distributional characteristics.

The Usefulness of Extradural Anterior Clinoidectomy for Low-Lying Posterior Communicating Artery Aneurysms : A Cadaveric Study

  • Objective : To confirm the usefulness of the extradural anterior clinoidectomy during the clipping of a low riding posterior communicating artery (PCoA) aneurysm through cadaver dissection. Methods : Anatomic measurements of 12 adult cadaveric heads (24 sides total) were performed to compare the microsurgical exposure of the PCoA and internal carotid artery (ICA) before and after clinoidectomy. A standard pterional craniotomy and transsylvian approach were performed in all cadavers. The distance from the ICA bifurcation to the origin of PCoA (D1), pre-anterior clinoidectomy distance from the ICA bifurcation to tentorium (D2), post-anterior clinoidectomy distance from the ICA bifurcation to tentorium (D3), pre-anterior clinoidectomy distance from the tentorium to the origin of PCoA (D4) and post-anterior clinoidectomy distance from the tentorium to the origin of PCoA (D5) and the distance of the ICA obtained after anterior clinoidectomy (D6) were measured. We measured the precise thickness of the blade for the Yasargil clip with a digital precision ruler to confirm the usefulness of the extradural anterior clinoidectomy. Results : Twenty-four sites were dissected from 12 cadavers. The age of the cadavers was 79.83±6.25 years. The number of males was the same as the females. The space from the proximal origin of the PCoA to the preclinoid-tentorium (D4) was 1.45±1.08 mm (max, 4.01; min, 0.56). After the clinoidectomy, the space from the proximal origin of the PCoA to the postclinoid-tentorium (D5) was 3.612±1.15 mm (max, 6.14; min, 1.83). The length (D6) of the exposed proximal ICA after the extradural clinoididectomy was 2.17±1.04 mm on the lateral side and 2.16±0.89 mm on the medial side. The thickness of the Yasargil clip blade used during the clipping surgery was 1.35 mm measured with a digital precision ruler. Conclusion : The proximal length obtained by performing an external anterior clinoidectomy is about 2 mm, sufficient for proximal control during PCoA aneurysm surgery, considering the thickness of the aneurysm clips. In a subarachnoid hemorrhage, performing an extradural anterior clinoidectomy could prevent a devastating situation during PCoA aneurysm clipping.

Coronal Three-Dimensional Magnetic Resonance Imaging for Improving Diagnostic Accuracy for Posterior Ligamentous Complex Disruption In a Goat Spine Injury Model

  • Objective: The purpose of this study was to investigate whether three-dimensional (3D) magnetic resonance imaging could improve diagnostic accuracy for suspected posterior ligamentous complex (PLC) disruption. Materials and Methods: We used 20 freshly harvested goat spine samples with 60 segments and intact surrounding soft tissue. The animals were aged 1-1.5 years and consisted of 8 males and 12 females, which were sexually mature but had not reached adult weights. We created a paraspinal contusion model by percutaneously injecting 10 mL saline into each side of the interspinous ligament (ISL). All segments underwent T2-weighted sagittal and coronal short inversion time inversion recovery (STIR) scans as well as coronal and sagittal 3D proton density-weighted spectrally selective inversion recovery (3D-PDW-SPIR) scans acquired at 1.5T. Following scanning, some ISLs were cut and then the segments were rescanned using the same magnetic resonance (MR) techniques. Two radiologists independently assessed the MR images, and the reliability of ISL tear interpretation was assessed using the kappa coefficient. The chi-square test was used to compare the diagnostic accuracy of images obtained using the different MR techniques. Results: The interobserver reliability for detecting ISL disruption was high for all imaging techniques (0.776-0.949). The sensitivity, specificity, and diagnostic accuracy of the coronal 3D-PDW-SPIR technique for detecting ISL tears were 100, 96.9, and 97.9%, respectively, which were significantly higher than those of the sagittal STIR (p = 0.000), coronal STIR (p = 0.000), and sagittal 3D-PDW-SPIR (p = 0.001) techniques. Conclusion: Compared to other MR methods, coronal 3D-PDW-SPIR provides a more accurate diagnosis of ISL disruption. Adding coronal 3D-PDW-SPIR to a routine MR protocol may help to identify PLC disruptions in cases with nearby contusion.

Cephalometric analysis of skeletal Class II malocclusion in Korean adults (한국 성인 골격성 II급 부정교합자의 측모두부규격 방사선 계측학적 연구)

  • This study was performed to evaluate horizontal and vertical characteristics according to lateral cephalometry of adult Korean skeletal Class II patients using a selected horizontal and vortical reference planes of Koreans. 60 males and 60 females consisting of freshman of Yonsei University from 1996 to 1997 and patients with history of orthognatic surgery at the Dental Hospital of Yonsei University with a skeletal Class II profile were chosen and compared with 70 males and 70 females with normal occlusion. The skeletal Class R group had the following conditions : 1. Profile composed of a retrognathic mandible or protrusive maxilla; 2. Class II molar and canine key; 3. ANB-greater than $4^{\circ}; 4$. Wits appraisal-greater than 1.0mm; Cephalometric analysis consisted of 22 skeletal, 25 soft tissue, 12 dental measurements. The results were as follows. 1. There was no considerable vortical measurement difference between the skeletal Class II malocclusion group and the normal occlusion group in skeletal analysis. But, some variations were found between the two groups in soft tissue analysis. 2. Mandibular length of the skeletal Class II malocclusion group was smaller than that of the normal occlusion group. Mandible was more posteriorly positioned in the Class II malocclusion group than in the normal occlusion group. 3. The length and antero-posterior position of the maxilla were not different between the Class II malocclusion and the normal occlusion group. 4. The antero-posterior position of the nose, upper lip and maxillary soft tissue, and nasolabial angle were not different between the two groups. 5. Mandibular soft tissue of the Class H malocclusion group was more posteriorly positioned than that of the normal. 6. The vertical measurements of the incisors(U1-HP, L1-HP) were bigger in the Class II malocclusion group than in the normal, but those of the molars(U6-HP, U6-MP) showed no significant difference between the two groups. 7 Classifying the skeletal Class II malocclusion group according to the antero-posterior position of both jaws, normally positioned maxilla and retruded mandible was 43.3%, both normally positioned maxilla and mandible 28.3%, both retruded maxilla and mandible 20.0%..

A Study on Horizontal Reference Planes in Lateral Cephalogram in Korean Children (한국 아동의 측모두부 수평 기준선에 관한 연구)

  • Various types of horizontal reference planes are used for diagnosis, treatment planning and evaluation of treatment results. But these reference Planes lack accuracy and repro-ducibility, and are mainly for Caucasian. Unlike the adult patients who have completed growth, the horizontal reference planes for growing children may change continuously during growth. Therefore this must be considered in selecting the horizontal reference plane. The purpose of this study was to Investigate the angle formed by the Sella-Nasion(SN) plane and Frankfort-Horizontal(FH) plane and evaluate the angle formed by FH plane and other horizontal reference planes in relation to different skeletal maturity and malocclusion types. 540 subjects with no orthodontic treatment history were chosen, and hand -wrist X-rays and lateral cephalometric X-rays were taken. According to SMA(Skeletal Maturity Assessment) of hand-wrist X-rays, the subjects were classified into 3 skeletal maturity groups : SMI 1-4 for group A, SMI 5-7 for group B and SMI 8-11 for group C. A second classification was made according to cephalometric analysis of lateral cephalograms. The subjects were classified into 3 malocclusion groups : Skeletal Class I, II and III malocclusion group. 10 measurements were evaluated. The results were as follows. 1. The angle formed by the SN plane and FH plane showed no difference among skeletal maturity groups, malocclusion groups, and between .sexes. 2. The angles formed by the SN plane and FH plane were $8.27^{\circ}{\pm}2.31^{\circ}$ for males and $8.59^{\circ}{\pm}2.24^{\circ}$ for females. The average value for females and males was $8.42^{\circ}{\pm}2.28^{\circ}$. 3. The angle formed by the FH plane and palatal plane was almost constant showing no difference among skeletal maturity groups, malocclusion groups, and between sexes($1.09^{\circ}{\pm}3.21^{\circ}$).

Relationships between Nailfold Plexus Visibility, and Clinical Variables and Neuropsychological Functions in Schizophrenic Patients (정신분열병 환자에서 손톱 주름 총 시도(叢 視度) (Nailfold Plexus Visibility)와 임상양상, 신경심리 기능과의 관계)

  • Objectives:High nailfold plexus visibility can reflect central nervous system defects as an etiologic factor of schizophrenia indirectly. Previous studies suggest that this visibility is particularly related to the negative symptoms of schizophrenia and frontal lobe deficiency. In this study, we examined the relationships between nailfold plexus visibility, and various clinical variables and neuropsychological functions in schizo-phrenic patients. Methods:Forty patients(21males, 19 females) satisfying the DSM-IV criteria for schizophrenia and thirty eight normal controls(20 males, 18 females) were measured for Plexus Visualization Score(PVS) by using the capillary microscopic examination. For the assessment of psychopathology, process-reactivity, premorbid adjustment, and neuropsychological functions, we used Positive and Negative Syndrome Scale(PANSS), Ullmann-Giovannoni Process-Reactive Questionnaire(PRQ), Phillips Premorbid Adjustment Scale(PAS), Korean Wechsler Adult Intelligence Scale(KWIS), Continuous Performance Test(CPT), Wisconsin Card Sort Test (WCST), and Word Fluency Test. We also collected data about clinical variables. Results:PVS was correlated with PANSS positive symptom score and composite score negatively. There were no correlations between PVS and PRQ score, PAS score and neuropsychological variables respectively. Conclusions:This study showed that nailfold plexus visibility was a characteristic feature in some schizophrenic patients, and that higher plexus visibility was associated with the negative symptoms of schizophrenia. There was no association between plexus visibility and neuropsychological functions.

Seasonal Variation of Nutritional Intake and Quality in Adults in Longevity Areas (고령인구 비율이 높은 지역 성인 및 노인의 계절별 영양소 섭취실태)

  • The purpose of this study was to estimate seasonal variation of nutritional intake and quality in adults in longevity areas. Dietary survey was given to 469 subjects over age 20 living Bukjeju-gun, Yecheon-gun, and Sunchang-gun of Korea, using 24-hour recall method every 4 seasons over one-year period. The mean daily intakes (%RDA) of 4 seasons were 1313.3 ㎉ (72.0%) for energy, 47.3 g (82.2%) for protein, 20.2 g for fat, 228.0 g for carbohydrate, 12.8 g for dietary fiber. The differences were hardly significant among the seasons. Mean daily intakes of most of the vitamins and minerals were lower than RDA except vitamin C and folate, especially %RDAs of Ca, vitamin D, vitamin B$_2$, vitamin E were less than 60% of RDA. In general, nutrient intake were high in spring compared to other seasons except vitamin C which was high in fall and winter. Mean daily intakes of cholesterol were 151.7 mg and 124.3 mg in males and females, respectively. The differences was significant between the two sexes but not among the seasons. PUFA : MUFA : SFA ratio of the subject was 1.0 : 1.3 : 1.2. Average CPF ratio of energy intake was 72.7 : 14.4 : 12.9, and energy intake ratio from carbohydrate was low in spring, in contrast energy intake ratios from protein was significantly high in spring. Mean adequacy ratio (MAR), an index of overall nutritional quality was 0.64 for female and 0.71 for male. The indices of nutritional quality (INQ) were over 1 for most of nutrients except 0.73 of Ca, 0.87 of vitamin A, 0.69 of vitamin B$_2$, and 0.65 of vitamin E. Both MAR and INQ were significantly different among sex and seasons, values were higher in males than in females and were higher in spring with the exception of vitamin C. In conclusion, subjects in longevity areas did not consume enough nutrients quantitatively as well as qualitatively, especially Ca, vitamin A, vitamin B$_2$, and vitamin E. Also mean daily intakes of most of vitamins and minerals were insufficient in females, and were significantly different among season. Therefore we must consider a counterplan to augment nutrition intake for them. In addition, it seems to be essential to micro nutrients to the food composition database to estimate dietary intakes more accurately.

Effects of Age and Sex on the Pharmacokinetics of Tacrolimus during Pediatric Kidney Transplantation: A Single Center Study (소아 신장이식 환자에서 연령 및 성별에 따른 타크롤리 무스의 약동학적 차이에 관한 단일기관 연구)

  • Purpose: The pharmacokinetics of tacrolimus, one of the most widely used immunosuppressive drugs, are known to vary by sex, age, and ethnicity during pediatric transplantation. This study assessed the pharmacokinetic characteristics and associated factors of tacrolimus in Korean children receiving a kidney transplant. Methods: We retrospectively reviewed the pharmacokinetic data (therapeutic dose, trough level, clearance, and half-life) of 9 children who were given tacrolimus as one of their initial immunosuppressive drugs after kidney transplantation. In addition, we compared the findings to data from 10 adult kidney transplant recipients. Results: The mean age of our pediatric patients was 13.9 years, and the maleto- female ratio was 4:5. The mean dose of tacrolimus was $0.19{\pm}0.14$ mg/kg/day. The mean dose of tacrolimus for males was $0.23{\pm}0.12$ mg/kg/day, which was significantly higher than the dose for females ($0.16{\pm}0.14$ mg/kg/day). The trough level was not significantly different between both groups. The clearance rate of tacrolimus for males was also significantly higher than females. Although the dosage of tacrolimus for patients over the age of 12 years was lower ($0.18{\pm}0.13$ vs. $0.21{\pm}0.16$ mg/kg/day) and the trough level was higher ($8.2{\pm}4.5$ vs. $7.2{\pm}4.2$ mg/mL) than that for patients under the age of 12 years, there was no significant difference between them. However, there were significant differences between children and adults in dose, clearance, and half-life of tacrolimus. Conclusion: Out study suggests that the pharmacokinetics of tacrolimus tends to vary with sex and age. Therefore, large-scale prospective studies are required to verify the proper therapeutic dosage of tacrolimus in Korean children.
