Novel Role of Dipterocarpus tuberculatus Roxb. as a Lipogenesis Inhibitor and Lipolysis Stimulator in 3T3-L1 Adipocytes (3T3-L1 지방세포에서 lipogenesis 저해제와 lipolysis 촉진제로서 Dipterocarpus tuberculatus Roxb.의 새로운 역할)

  • The pharmacological efficacy of Dipterocarpus tuberculatus Roxb. has been verified in only several fields including photoaging, inflammation, hepatotoxicity, acute gastritis and osseointegration. To identify the novel functions of Dipterocarpus tuberculatus Roxb. on anti-obesity, inhibitory effect on lipid accumulation and stimulatory effect on lipolysis were investigated in MDI (3-isobutyl-1-methyl-xanthine, dexamethasone, and insulin) stimulated 3T3-L1 adipocytes treated with methanol extracts of Dipterocarpus tuberculatus Roxb. (MED). Lipogenic targets, including lipid accumulation, level of lipogenic transcription factors, and expression of lipogenic regulators, were downregulated in MDI-stimulated 3T3-L1 adipocytes treated with MED without any significant cytotoxicity. Also, MED treatment inhibited the mRNA levels of adipogenic targets including peroxisome proliferator-activated receptor (PPAR)γ and CCAAT-enhancer binding protein (C/EBP) α, as well as lipogeic targets including adipocyte fatty acid binding protein 2 (aP2) and fatty acid synthetase (FAS) in MDI-stimulated 3T3-L1 adipocytes. A similar decrease patterns were detected in Oil red O stained lipid droplets of MED treated MDI-stimulated 3T3-L1 adipocytes. Furthermore, several lipolytic targets, such as cAMP concentration, concentration of free glycerol, expression level of lipases, including ATGL, perilipin and HSL, were upregulated in MDI-stimulated 3T3-L1 adipocytes treated with MED. These results show that MED has a novel role as a lipogenesis inhibitor and lipolysis stimulator in MDI-stimulated 3T3-L1 adipocytes.

Gastric Emptying Time in Acute and Chronic Hepatitis B Patients (급성 및 만성 B형 간염환자의 위배출시간에 관한 연구)

  • Anorexia, nausea, and vomiting are one of the most frequent symptoms in viral hepatits patients. These may be due to poorly detoxified substances by dysfunctioned hepatocytes or by gastritis, but the pathophysiology is not totally understood. The symptoms interfere with adequate nutrient intake and are managed by metoclopramide, which accelerates gastric emptying. Thus delayed gastric emptying may well be a contributing factor to such symptoms. To determine such a relationship, we measured gastric emptying time in 11 normal subjects,9 acute (AVH), and 12 chronic B viral hepatitis (CVH) patients. All were males with a mean age of 23 years. An egg was labeled with 0.5 mCi of $^{99m}Tc-sulfur$ colloid, fried, then eaten between 2 slices of bread with 100 cc of water. Anterior and posterior images were taken at 20 minute intervals over a 2 hour period. A geometric mean of activity pertaining to the gastric region was measured, and $T_{1/2}$ was calculated from the time activity curve. $T_{1/2}$ for normal the group was $57.8{\pm}6.3$ minutes while that for the AVH and CVH group was $58.2{\pm}8.2$ (p=0.40) and $64.1{\pm}10.5$ (p=0.09), respectively. There was 1 AVH Patient and 4 CVH patients with prolonged $T_{1/2}$. Anorexia and nausea was seen in 71% and 46% of the patients, respectively. 80% and 60% of the patients with prolonged $T_{1/2}$ had anorexia and nausea, respectively.

A Study of Clinical Manifestations of Gastrointestinal Symptoms in Children with Henoch-Schönlein Purpura (소아 Henoch-Schönlein Purpura 환아에서 위장관 증상 유무에 따른 임상 소견에 대한 연구)

  • Purpose: This study was undertaken to evaluate the factors correlated with the clinical course and outcome in patients of Henoch-Sch${\ddot{o}}$nlein Purpura. Methods: The medical records of 104 children diagnosed with Henoch-Sch${\ddot{o}}$nlein Purpura (HSP) from January 1996 to April 2006 were reviewed retrospectively. The patients were divided into two groups: patients with Gastrointestinal (GI) symptoms and those without GI symptoms. When there were joint, scrotum, and renal symptoms except for skin lesion in whole HSP, those patients were excluded. The history of acute infection, duration of admission, treatment requirement, recurrence of HSP, CBC, stool occult blood test, abdominal ultrasonographic findings and GI endoscopic findings were reviewed. Results: Among 104 patients, patients with GI symptoms included 66 cases (63.5%), those without GI symptoms accounted for 38 cases (36.5%). GI symptoms included: abdominal pain in 57 cases (54.8%), vomiting 21 cases (20.2%), GI bleeding 5 cases (4.8%), nausea 3 cases (2.9%) and diarrhea 3 case (2.9%). Positive GI symptoms and GI mucosal lesions on GI endoscopy had a statistically significant correlation with increased admission duration, treatment requirement, recurrence of HSP, and positive stool occult blood. Six cases with small intestinal wall thickening were noted on abdominal ultrasonography. Six cases of hemorrhagic gastritis and hemorrhagic duodenitis, 3 cases of duodenal ulcer, 3 cases of hemorrhagic gastritis and duodenal ulcer, 2 cases of hemorrhagic duodenitis and colitis, and 1 case of colitis were noted on GI endoscopy. Conclusion: These results suggest that GI endoscopic examination may be helpful for the diagnosis and treatment of children with HSP.

  • Objective : Gastritis is a major complication of gastrointestinal disease. Lonicera japonica is used in folk medicine to treat different diseases such as exopathogenic wind-heat, epidemic febrile diseases, sores, carbuncles and some infectious diseases. Therefore, this study examined the effects of Lonicera japonica water extract (LJE) on HCl/ethano-linduced acute gastric ulceration and anti-oxidants properties. Methods : LC-ESI-IT-TOF MS was employed for rapid identification of major compound from LJE. The antioxidant activities were evaluated through total polyphenol and flavonoid contents and radical scavenging assays and superoxide dismutase (SOD)-like activity. SD rats were randomly divided into five different groups including the normal group, ulcer group, positive group (20 kg/mg of omeprazole, ip), and experimental groups (100 kg/mg and 500 kg/mg of LJE, ip). Results : 4,5-Dicaffeoyl quinic acid, loganic acid, secologanic acid, sweroside, loganin, vogeloside were identified based on the detection of the molecular ion with those of literature data. The LJE was possessed free radical scavenging activities such as DPPH (IC50=189.7 ㎍/㎖), ABTS (IC50=164.5 ㎍/㎖), and SOD-like activity (IC50=405.02 ㎍/㎖). Macroscopic and histological analyses showed LJE treated group were significantly reduced to an extent that it allowed leukocytes penetration of the gastric walls compared with the ulcer group. In addition, an ulcer inhibition rate and prostaglandin E2 levels were increased in rats treated with LJE. Conclusion : The present study has demonstrated the antioxidantive and gastroprotective effect of LJE, these findings suggested that LJE has the potential for use in treatment of gastric disorders.

A Prospective Study on Emergency Room Utilization in Children with Nonsurgical Gastrointestinal Disorders (비외과적 소화기질환 환아들의 응급실 이용양상에 대한 전향적 조사연구)

  • Purpose: There have been few reports about common gastrointestinal diseases in children visiting emergency room. The aim of this study was to present basic data and their meanings about emergency room utilization in children with nonsurgical gastrointestinal disorders. Methods: The authors prospectively studied 1,228 consecutive children with gastrointestinal diseases, amongst 6,179 nonsurgical pediatric patients who visited the emergency room of Seoul Red Cross Hospital from Jan. 1st 1998 to Dec. 31st 1999. Results: 1) First visit was 60.7% of total visits and 30.7% were between 1 and 3 years of age while 80.4% were below 6 years of age. Male patients were predominant by a ratio of 1.3:1. 2) The peak month of visits was December (12.1%), and the peak time of visits was between 8:00 pm and midnight (35.9%). Average length of stay at emergency room of the total patients were 0.86 hour. 3) Five major diseases were acute gastroenteritis (44.3%), fecal impaction &/or constipation (21.3%), acute gastritis (16.4%), intussusception (4.6%), and infantile colic (4.3%) in order. 4) 19.6% of the total patients were hospitalized. Conclusion: There were differences in various distributions regarding each nonsurgical gastrointestinal disease entity in children visiting emergency room even though distributions of the total patients in our study were not so different from those in previous reports by others.

A Comparative Study on the Pattern of Outpatient Department Utilization at a Tertiary Level Hospital before and after Implementation of the Patient Referral System (의료전달체계 실시 전후의 3차 진료기관 외래환자 이용양상 비교)

  • This study was conducted to assess the effects of implementation of the patient referral system started July 1st, 1989. A comparison on the pattern of outpatient services of the Departments of Internal Medicine, General Surgery, and Pediatrics at the Yeungnam University Hospital was conducted for each one year period before and after implementation of the patient referral system. The pre-implementation period was from July 1, 1988 to June 30, 1989 and the post-implementation period was from July 1, 1989 to June 30, 1990. The information used for this study was obtained from official forms, prepared by the Yeungnam University Hospital, and submitted to the Korean Medical Insurance Cooperatives. After implementation of the patient referral system, the number of outpatient cases in the Department of Internal Medicine decreased 36.1% from 9,669 cases to 6,181 cases a year. Cases in the Department of General Surgery decreased 23.7% from 1,864 cases to 1,422 cases a year. The number of cases in the Department of Pediatrics decreased 36.9% from 3,372 cases to 2,128 cases a year. After implementation of the patient referral system, the average age of cases in the Departments of Internal Medicine and General Surgery was 52.5 and 49.7 years old, respectively. This was a significant increase in comparison with the pre-implementation period. After implementation of patient referral system, the proportion of new outpatients in the Department of Internal Medicine decreased from 24.1% to 14.6%, the Department of General Surgery from 36.0% to 23.4%, and the Department of Pediatrics from 15.5% to 8.3%. The number of visits per case decreased significantly in the Department of Internal Medicine(from 1.74 to 1.61), but there was no significant change in the Departments of General Surgery and Pediatrics. The length of treatment per case increased significantly in all three departments(from 16.1 days to 19.3 days in the Department of Internal Medicine, from 12.0 days to 15.2 days in the Department General Surgery, and 8.9 days to 11.2 days in the Department of Pediatrics). The number of clinical tests per case increased significantly in the Department of Internal Medicine (from 2.2 to 2.5), in the Department of Pediatrics(from 0.8 to 1.1) and increased in the Department of General Surgery(from 6.4 to 6.6). The average medical cost per case decreased from 43,900 Won to 42,500 Won in the Department of Internal Medicine, while the cost increased from 75,900 Won to 78,500 Won in the Department of General Surgery and from 12,700 Won to 13,500 Won in the Department of Pediatrics. In case-mix, the chronic degenerative disease(i. e. hypertension, diabetes mellitus, angina pectoris, malignant neoplasm, and pulmonary tuberculosis) ranked higher and acute infectious diseases and simple cases(i. e. gastritis and duodenitis, haemorrhoids, anal fissure, carbuncle, acute URI, and bronchitis) ranked lower after implementation of the patient referral system compared to before implementation.

Protective Effect of Canavalia gladiata on Gastric Inflammation Induced by Alcohol Treatment in Rats (알코올성 위염 동물 모델에서 작두콩 추출물의 보호 효과)

  • The objective of this study was to investigate the protective effect of extracts from Canavalia gladiata (CGE) on gastric inflammation induced by alcohol treatment in SD rats. Rats were divided into four groups: G1 (normal group), G2 (gastric inflammation induced by alcohol), G3 (gastric inflammation induced by alcohol with lansoprazole pretreatment), G4 (gastric inflammation induced by alcohol with 250 mg/kg b.w. CGE pretreatment), G5 (gastric inflammation induced by alcohol with 500 mg/kg b.w. CGE pretreatment). After the oral administration of 40% alcohol and samples for seven days, acute gastritis was induced with 70% alcohol and 0.15 M HCl. After 1 h of alcohol administration, the animals were sacrificed. Groups pretreated with lansoprazole or CGE showed an attenuation of gastric mucosal injury, including decreases in sub-epithelial loss, hemorrhages, and gastric juice secretion induced by administration of alcohol. The oral administration of CGE (500 mg/kg b.w.) significantly decreased the levels of TBARS. To examine molecular factors that regulate inflammation, the protein expression of NF-${\kappa}B$ and COX-2 were measured through immuno-histochemistry. Compared with the normal group (G1), the expression of NF-${\kappa}B$ and COX-2 were clearly increased in G2. COX-2 and NF-${\kappa}B$ were expressed even higher in groups pretreated with CGE compared to G2. In conclusion, our data show that Canavalia gladiata has inhibitory and protective effects on gastric inflammation induced by alcohol treatment in SD rats.
