  • In mass casualty situation due to radiological accidents, it is important to start aggressive management with rapid triage decisions. External contamination needs immediate decontamination and internal contamination should be treated with special expertise and equipment to prevent the rapid uptake of radionuclides by target organs. Acute radiation syndrome shows a sequence of events that varies with the severity of the exposure. More severe exposures generally lead to more rapid onset of symptoms and severe clinical findings. After the massive exposure, various systems of the body reflect their severe damages that can lead to death within hours or up to several months. The disease progression has classically been divided into four stages: prodromal, latent, manifest illness, and recovery or death. Three characteristic clusters of symptoms including the hematopoietic syndrome, the gastrointestinal syndrome and the cerebrovascular syndrome are all associated with the acute radiation syndrome. The standard medical management of the patients with a potentially survivable radiation exposure includes good medical, surgical and supportive measures. Specific treatment with cytokines and bone marrow transplantation should be considered. The management of internal contamination is much the same as the treatment of poisoning. The standard decontamination should be applied to reduce uptake, and the chelating agents can be administered to enhance the clearance of radioisotopes. Radioactive iodine ($^{131}I$) as one of the nuclear fission products can increase the incidence of thyroid cancer in children. Potential benefit of potassium iodide prophylaxis is greater especially in neonates, infants and small children.

  • Inhalation of toxic gases can lead to pneumonitis. It has been known that methane gas intoxication causes loss of consciousness or asphyxia. There is, however, a paucity of information about acute pulmonary toxicity from methane gas inhalation. A 21-year-old man was presented with respiratory distress after an accidental exposure to methane gas for one minute. He came in with a drowsy mentality and hypoxemia. Mechanical ventilation was applied immediately. The patient's symptoms and chest radiographic findings were consistent with acute pneumonitis. He recovered spontaneously and was discharged after 5 days without other specific treatment. His pulmonary function test, 4 days after methane gas exposure, revealed a restrictive ventilatory defect. In conclusion, acute pulmonary injury can occur with a restrictive ventilator defect after a short exposure to methane gas. The lung injury was spontaneously resolved without any significant sequela.

  • Phosphamidon(PMD) is orgnophosphate insecticide broadly using in agriculture. In order to study PMD toxicity in the testis, histopathological change and apoptosis were assessed following acute and chronic oral administration in rats and chickens. In acute studies, histopathological changes included necrosis and desquamation of spermatogenic cells, multinucleated giant cells in the lumen of seminiferous tubules, and necrotic cells and the giant cells in the epididymal lumen. Atrophy of seminiferous tubule was seen in the chronic exposure with low doses. The toxic effects of PMD in chronic exposure including clinical signs and histopathological changes were more pronounced in chickens than rats. Apoptosis assessment was performed by TUNEL method and Hoechst staining. TUNEL-positive apoptotic cells were found in spermatocytes of seminiferous tubules, testicular apoptosis was more prominent following acute exposure than control and chronic exposure. Above mentioned result noticed that PMD causes apoptotic death and effects directly the spermatocytogenesis.

  • The acute and genetic effect of formaldehyde on mice through inhalation route was studied. The Riley's chamber with one stack of cage was used for the exposure and the micronucleus test was performed under unprecedently maximum exposure concentration. LC50's of formaldehyde in mice by whole body exposure for 4 hours were 105.5 ppm with 95% confidence interval of 72.6 ppm and 143.2 ppm for male, and 159.2 ppm with 95% confidence interval of 116.5 ppm and 272.7 ppm for female. Cinicial symptoms by acute exposure were salivation, lacrimation, and abnormal respiration.

  • Acute Cd exposure produces hepatotoxicity, whereas chronic Cd exposure produces nephrotoxicity, hematotoxicity, immunotoxicity and bone damage. Previous experiments suggest that the low-molecular-weight, metal-binding protein metallothionein (MT) in liver protects against liver injury, but is responsible for the kidney injury observed after chronic Cd exposure.(omitted)

  • Indigenous species means a species that is likely, due to historical presence, to occur at a specified site for some portion of its life span. Therefore, indigenous species can be useful as an indicator to assess environmental risk caused by hazardous chemicals in a specific site. So far a few toxicity studies using freshwater species which are indigenous to Korea have been carried out. In this study, a freshwater shrimp (Neocardina denticulata) indigenous to Korea was used for acute toxicity test of heavy metals. Neocardina denticulata were exposed to cadmium chloride $(CdCl_2)$, copper chloride $(CuCl_2)$ and zinc chloride $(ZnCl_2)$ using automatic flow-through system for 96 hours. The 96h LC50s were calculated as 0.043 $(0.042{\sim}0.045)mg\;CdCl_2/L,\;0.104(0.098{\sim}0.113)mg\; CuCl_2/L\;and\;2.021\;(1.633{\sim}2.594)mg\;ZnCl_2/L$. When compaired with some international standard species such as medaka(Oryzias latipes), Neocardina denticulata had high sensitivity. Therefore, this study suggested that Neocardina denticulata have possibilities for a sensitive test species to test heavy metal toxicity in aqua-system.

  • Childhood acute lymphoblastic leukemia (ALL) is one of the most common hematologic malignancies, accounting for one fourth of all childhood cancer cases. Exposure to environmental factors around the time of conception or pregnancy can increase the risk of ALL in the offspring. This study aimed to evaluted the role of prenatal and postnatal exposure to high voltage power lines on the incidence of childhood ALL.This cross-sectional case control study was carried out on 22 cases and 100 controls who were born and lived in low socioeconomic families in Isfahan and hospitalized for therapeutic purposes in different hospitals from 2013-2014.With regard to the underlying risk factors, familial history and parental factors were noted but in this age, socioeonomic and zonal matched case control study, prenatal and childhood exposure to high voltage power lines was considered as the most important environmental risk factors of ALL (p=0.006, OR=3.651, CI 95%, 1.692-7.878). As the population was of low socioeconomic background, use of mobiles, computers and microwave was negligible. Moreover prenatal and postnatal exposure to indoor electrically charged objects was not determined to be a significant environmental factor. Thus, pre and post natal exposure to high voltage power lines and living in pollutant regions as well as familial influence could be described as risk factors of ALL for the first time in a low socioeconomic status Iranian population.

  • Whole-body exposure to high-dose radiation causes injury involving multiple organs that depends on their sensitivity to radiation. This acute radiation syndrome (ARS) is caused by a brief exposure of a major part of the body to radiation at a relatively high dose rate. ARS is characterized by an initial prodromal stage, a latent symptom-free period, a critical or manifestation phase that usually takes one of four forms (three forms): hematologic, gastrointestinal, or cardiovascular and neurological (neurovascular), depending upon the exposure dose, and a recovery phase or death. One of the most important factors in treating victims exposed to radiation is the estimation of the exposure dose. When high-dose exposure is considered, initial dose estimation must be performed in order to make strategy decisions for treatment as soon as possible. Dose estimation can be based on onset and severity of prodromal symptoms, decline in absolute lymphocyte count post exposure, and chromosomal analysis of peripheral blood lymphocytes. Moreover, dose assessment on the basis of calculation from reconstruction of the radiation event may be required. Experience of a criticality accident occurring in 1999 at Tokai-mura, Japan, showed that ARS led to multiple organ failure (MOF). This article will review ARS and discuss the possible mechanisms of MOF developing from ARS.

Exposure to environmental factors around the time of conception or pregnancy can increase the risk of ALL in the offspring. This study aimed to evaluate the influence of prenatal and postnatal exposure to high voltage power lines on the incidence of childhood ALL. It also examines the role of various factors such as environmental factors and alpha-amylase as a marker in the development of leukemia. This cross-sectional case control study was carried out on 22 cases and 100 controls who born and lived in low socioeconomic families in Tehran and were hospitalized for therapeutic purposes in different hospitals ofrom 2013-2014. With regard to the underlying risk factors; familial history and parental factors were detected as risk factors of ALL but in this age, socioeonomic and zonal matched case control study, prenatal and childhood exposure to high voltage power lines was considered as the most important environmental risk factor (p=0.006, OR=3.651, CI 95% 1.692-7.878).
