A Study to define the range of sample size for setting Intensive Care Unit(ICU) facility guidelines - Focused on analysis of guidelines and present of ICUs facilities by general hospitals size (중환자실 시설기준 작성을 위한 기본시설 수준 설정 연구 - 국내·외 시설기준 및 국내 종합병원 병상규모 별 시설 현황분석을 기초로 하여)

  • Purpose : This study is a basic research to establish guidelines for healthcare facility in Korean healthcare market. As a first step, it is a main purpose for making beginning point of ICU guideline to set a basic level of ICU facility size and quality by analysis current ICU facilities and existed domestic guidelines : law, criteria of healthcare insurance review and assessment service, credit of Korea institute for healthcare accreditation. Methods : First of all, the all requirements of existed guidelines are put together and summarized. The summary is compared with that of US, UK, and Australia to know its quality difference with international market. In addition, all hospitals in Korean market are classified based on total bed number and ICU bed number to know the most occupied facility size range in the market. Second of all, by comparing 15 ICUs' current setting of the public general hospital, a general condition of ICU facility would be extracted based on function, floor area per bed, services Results : 72.8% of hospitals in Korea are belonging below 500 beds hospital. Among them 200-299 beds hospitals occupied 35.3% and are shown as the most occupied hospital size. As 15 public general hospitals are analyzed, it is clear that the more bed size and services the more ICU area per bed. As a result it is sure that the 300~500 bed may be a clear line as a general ICU condition whose function and bed number relatively consistent in the range. Implications : to keep the qualified medical environment and contemporary hospital trend, the guideline as a minimum requirement keep naturally out from current healthcare settings and should reflect their limit to reconcile with the new trend in the market.

  • Purpose: This paper aims to explore approaches for obtaining KOLAS certification for masks developed as protective equipment for use during evacuation processes in the event of a radiation disaster involving residents within a radiation emergency planning zone. Method: Various reports, papers, and data from the KOLAS accreditation bodies' websites were examined for this study. Result: Although domestic radiation disaster preparedness measures have been established to enhance resident protection, the distribution of protective equipment is limited to thyroid protection drugs. Supplementary support items like masks are necessary to prepare for radiation disasters. Currently, there is no KOLAS-accredited certification body for radiation protection masks. Conclusion: For masks that have established performance certification criteria, a dual process is required for KOLAS certification. This involves obtaining an official test report as an industrial respirator mask, receiving certification, and then obtaining a general test report based on internal standards.

  • In recent globalization era, Korean architectural education and profession are emerging as the fields with serious problems. There are two major reasons. One is an internal factor caused by Ministry of Education's full scale shift from department to faculty system. Due to the drastic change in this system, most of schools have serious difficulties to deliver the proper professional education. And the other is an external factor. In WTO era, Union of International Architects has adopted $\ulcorner$UIA Accord on Recommended International Standards of Professionalism In Architectural Practice$\lrcorner$. According to the education section of the Accord, it requires 5 years of professional education in the accredited school. So, if it is formally adopted in the Beijing Congress, which is scheduled to be held in June 1999, we may face very serious problems in architectural education. As mentioned above, with the present education system it is believed not easy to meet their new criteria. Within the present system, which does not have any mandatory relationship with professional licence, students might go on to diverse fields such as architectural design, structural engineering, construction management and building equipment engineering. However, if UIA Accord and Engineering Accreditation are required at the same time, it will not be easy to meet their new criteria with present architectural education system. The goal of this paper is to suggest the models to meet the standards of international community while keeping the strengths of existing systems.

Robust Reference Intervals for Serum Kappa and Lambda Free Light Chains from a Multi Centre Study Population from Hyderabad, India: Myeloma Diagnostic Implications

  • The International Myeloma Working Group considers the serum free light chain (SFLC) assay to be an adjunct to traditional tests. Apart from the FLC ratio, the absolute values of individual free light chains also are gaining importance as they appear to be more relevant in certain clinical settings. Automated assays are available for their determination. As laboratories put new test systems into use catering to different disease populations, they are required by accreditation and certification bodies to verify or establish performance specifications, including reference intervals (RIs) representative of their population. Our aim was to establish local RIs for SFLC in a multicentre representative healthy population using a robust method. There was no significant relationship between SFLC levels and age, gender and creatinine levels. The 95% RI for ${\kappa}SFLC$ was 4.81 to 33.86mg/L, for ${\lambda}$ SFLC was 5.19 to 23.67mg/L and for ${\kappa}/{\lambda}SFLC$ was 0.36 to 2.33, significantly higher than the values given by the manufacturer. The ${\kappa}/{\lambda}$ SFLC ratio at 2.23, covering 100% of the data, showed 72% sensitivity (95% CI=39.0 - 94.0), 100% specificity (95% CI=71.5 - 100.0), 100% PPV (95% CI=21.5 - 100.0), 95% NPV (95% CI=75.4 - 99.9), and 79% accuracy (95% CI=56.0 - 93.0). In the patient group, kit RI for ${\kappa}/{\lambda}$ SFLC ratio classified 45.5% (n=5) as positive vs 9.1% (n=1) positive by the study RI, while the kit RI for kappa FLC classified 90.9% (n=10) as positive vs 54.5% (n=6), indicating increased probability of false positive test results with the kit RI when applied to our patient population. Appropriate and specific reference intervals and criteria values result in fewer false-positive and false-negative results which means fewer wrong or missed diagnoses.

  • Librarian certification in Korea is issued by the LIS Departments, the Academy of Librarianship, and the Academic Credit Bank system; however, it has been pointed out that there is a limit to how much these nurture high quality librarians, because the education quality and contents varies from education institutions and there is no verification method for certification issued by educational programs. Therefore, this study investigated certification systems of academic or training programs that are conducted at home and abroad, analyzed how the certification systems are oriented, what the purpose and criteria of the certification systems are, and what the content of assessment is. As a result of this investigation, several areas needing change were identified which if adopted can improve the system. These included making amendments to the library laws related to the certification system, substantially revising the relevant enforcement ordinance, making changes to the selection of the Certification authority, establishing certification standards and procedures, developing contents related to document examination and due diligence audits, rating the effects of the certification system, and setting standards. Improving the Librarian Certification System has been discussed over the past 20 years and should not be delayed any longer because of the university structural reform of the current government, the rapid rise of the qualified librarians, decreasing employment due to the human resources supply and demand imbalances, all of which has resulted in a survival crisis of four-year Department of Library and Information Science.

  • We have developed standards based on international criterions for the quality control of dose tested by the measurement institutions of individual exposure doses through improving the reliability of data on the exposure dose of individuals working in radioactive environment and securing the accuracy and reliability of individual dose measurements. Laws related to radiation dose applied to domestic institutions refer to ANSI N13.11.1993, but currently, in U.S. and some other countries the measurement of radiation doses is based on ANSI N13.11.2001 that reduced test categories and tightened the standards. We made efforts to simplify the standards and to reduce the number of dosimeters required in experiment, and avoided preventing or hindering the use of future technologies not approved under the current law such as glass dosimeter and optical stimulation dosimeter. The Quality Management Manual of Radiation Dosimetry Service, Assessment Manual of Radiation Dosimetry Service Accreditation Program, and the Personnel Dosimetry Performance. Criteria for Testing are documents applicable in supervising laboratories.

  • Since the Child Friendly Cities Initiative(CFCI), a UNICEF-led initiative, was first introduced to Seongbuk-gu in 2013, more than half of the districts of Seoul are making efforts to achieve the accreditation of the Child Friendly City(CFC). At this point, when an initiative is transformed from a special policy of a few districts into a general policy of many local governments, we need to examine and check on how friendly urban parks are to children. This study focused on neighborhood parks that tend to be less friendly to children as compared to children's parks and looked into the current status, because neighborhood parks are also well used by users of all ages including children. The evaluation criteria was developed based on the review of domestic and international guidelines of child friendly parks and neighborhood parks. Futhermore, field studies, user questionnaire, and a participatory design workshop were introduced to analyze the status of child-friendly environment of neighborhood parks. Findings are as follows. First, vague definitions of child cause the inconsistency of child-related policies. Second, Neighborhood parks are not conceived as a public space for children. Third, the consideration of youth is relatively low. Fourth, adjacent area of a park turns out not to be child friendly as well.

  • The purpose of this research study is to analyze the influence on the necessity of establishment of eco-friendly hospital by evaluating hospital employees' awareness and their satisfaction level of working environment. Questionnaire investigation was conducted in April 2014 throughout five different medium-size hospitals in Seoul, Gyeonggi and Chungcheongbuk provinces. 213 sets of survey were used to conduct Frequency Analysis, T-test, Dispersion Analysis(ANOVA) and Structural Equation Modeling Analysis. The research concluded that employees with higher level of education, working position and longer term of working experience were more aware about Eco-friendly hospital. In addition, male, doctor, and employees with higher level of education and working position were more satisfied with their working environment. And male, employees with higher level of education and working position were more aware about necessity of establishment of eco-friendly hospital. According to the Structural Equation Modeling Analysis, the awareness of Eco-friendly hospital and the satisfaction level of working environment were the varying factors that influence the necessity of establishment of Eco-friendly hospital. Therefore, it is crucial for hospitals that are preparing for establishing Eco-friendly hospital to raise employees' level of understanding of Eco-friendly hospital and their working environment satisfaction level.

The Present Situation and Tasks of University Nursing Education in Korea (한국 간호교육의 현황과 과제)

  • This study was intended to grasp the history of nursing education from the beginning to the present in Korea, and grip and look-out current diversified systems of nursing education on basis of February, 2000 through literatural review and investigation by close telephone interviews. The basic nursing educational institutions in the whole country were total 113, namely, 3 years course, 65 junior colleges of nursing, and 4 years course, 48 colleges of nursing. And there were 3 types of continuing nursing educational system: two of three were transferring to another college for gain bachelor's degree in nursing; RN-BSN programs and university of broadcasting, and the other was the system of independent learning and then examination for BSN. Total nursing graduates from junior college of nursing courses and college of nursing courses were 7,564 on February, 1999. In general graduate school, the number of master courses were 21 and Ph.D courses were 13. And the number of special graduate schools were 21, i.e., graduate school of education were 7, graduate school of administration were 2, graduate school of public health were 11 and graduate school of industry was 1. As the perspective on nursing education, we overviewed changing nursing organizational environment, increasing the system of continuing education, making standards in nursing education and systemization of nursing educational accreditation, specialization of nursing, information system in nursing education and education of graduate school. The summary of the above overviewed subjects were as follows; Every nursing educational institution needed to educate by educational criteria and standard and characteristically run BSN and graduate courses. Specialization in nursing has to develop more and more, therefore advanced education and law should be prepared appropriately. According to the age's and social needs, we have to establish counter-plan for fundamental educational environment. We have to sensitive to rapidly changing information in the era of globalization. In the level of university education, each university needs characterization of educational objectives, goals and contents, and has to replace the shortage of the number of professor. And the regulation of thesis and dissertation examinations need to be reinforced. Education in nursing should consists with specialization. Collaboration among universities will bring efficiency in the nursing education.

Perceptions and Experiences of Acupuncture among Expatriates Living in Korea: A Qualitative Study (국내 거주 외국인의 침 치료 경험과 한의학에 대한 인식: 질적연구)

  • Objectives : In this study, we conducted a focus group interview among expatriates living in Korea, to investigate their experiences and perceptions of acupuncture and Korean Medicine in order to improve the usage of Korean Medicine in the medical tourism market. Methods : The inclusion criteria for the focus group interview was: (1) participants between the age of 20 to 75 years old; (2) those who had lived in Korea for more than 90 days, and (3) those who were able to express their thoughts freely in English. The participants were informed that the interviews will be recorded and transcribed. We ended recruitment once saturation of materials was met. Semi-structured interviews were conducted, and the data was analyzed after each interview. Results : The interviews were held between June to December 2016. Twenty-four participants were recruited but four dropped out and a total of twenty participants successfully finished the interviews. Six focus group interviews were conducted. Analysis revealed that Korean Medicine was relatively less known compared to Traditional Chinese Medicine or Complementary and Alternative Medicine. Participants automatically associated acupuncture with the management of pain or stress and replied that these were the areas that they thought acupuncture would be most effective. Positive experiences with acupuncture and with the acupuncture practitioners were factors that promoted the use of acupuncture whereas lack of awareness, accessibility and accreditation were factors that hindered its use. Other factors that hindered the use of acupuncture was needle phobia and the perception that acupuncture lacks scientific evidence. Conclusions : Awareness on Korean Medicine and acupuncture is low. Participants lack awareness on what diseases acupuncture can treat, the scientific evidence behind the mechanism of action, and the rigorous education system that Korean Medicine doctors must go through to get their license. Rigorous marketing should be encouraged, which includes greater exposure in the media, more honest reviews from patients, and promotion of the scientific evidence base.
