중대재해사례를 통한 굴삭기 안전사고 원인분석 (Analysis of Cause of Excavator Safety Accidents according to the Accident Case Study)

  • 최근 건설업의 대형화, 복잡화됨에 따라 안전사고가 증가하고 있다. 특히, 건설장비에 의한 사고 발생수가 증가하고 있다. 이런 사고의 예방을 위해서는 발생원인을 분석할 필요가 있다. 본 논문은 한국산업안전공단의 건설재해사례를 통해 안전사고 원인을 분석한다. 연구의 범위는 굴삭기 장비로 한정하였다. 본 연구의 결과를 요약하면 다음과 같다. 1) 굴삭기 재해 발생 형태는 협착${\cdot}$감김, 낙하${\cdot}$비래, 충돌${\cdot}$접촉 순으로 분석되었따. 2) 재해원인은 작업반경 내에 근로자출입, 주용도 이외의 용도로 사용함, 작업시작 전 저멈불량${\cdot}$ 작업방법불량 순으로 분석되었다. 이 연구결과는 차후 연구의 기초를 정립하고, 안전사고를 줄이는 기대효과를 가질 것이다.

개인 및 사고원인 특성이 재난안전인식에 미치는 영향분석 (Analysis of the Effect of Individual and Accident Cause Characteristics on Disaster Safety Perception)

  • The purpose of this study is to diagnose the level of disaster safety perception (disaster safety importance, accident possibility, personal safety, and regional safety) of local residents, and to analyze the impact of individual and accident cause characteristics. The analysis method used multiple regression analysis, and the main analysis results are as follows. First, disaster safety importance and accident possibility were higher as residents who had experience in safety education and were willing to visit the safety experience center in the province. Second, disaster safety importance was higher as the cause of the accident was the lack of response 119 and police. And accident possibility was higher as the cause of the accident was the lack of prior treatment by the state and local governments. Third, personal safety and regional safety were higher for men, and especially personal safety was lower as residents with children. The results of this study can be used as basic data for establishing regional customized disaster safety policies based on the perception of local residents, who are policy consumers.

화학사고 유발 사고대비물질 관리 개선 방안 (Improvement Strategy for Management of Accident Preparedness Substances Causing Chemical Accidents)

  • Chemical accidents occur by a variety of causes and aspects. Accident preparedness substances (APS) are defined by toxic chemicals highly likely to cause accidents due to their high acute toxicity, explosiveness or likely to cause severe damage where a chemical accident occurs among the chemicals. APS is designated and announced by Presidential Decree. However, chemical accidents occurred for recent 7 years were arisen mainly from non-accident preparedness substances, and only 24 species were included as chemical accident trigger among total 69 APS. In addition, APS were designated in 2014 and the list has not been updated since then although it needs to be amended in several aspects. Accordingly, this study analyzes the necessity of modification of APS management, and presents improved plans.

  • Recently, in the whole fields of industry including shipping business, the conception and importance of Human Factor is very highly and frequently emphasized. But the domestic crew disaster including injury and illness in our shipping business shows higher accident rate than other shipping countries. Therefore the developing of measurec to prevent/reduce the accident is highly required as the marine accident including crew disaster cause enormous loss of property and human life in size and scale. But, because the domestic data regarding the crew accident are relatively insufficient and rare, the developing of these measures has many difficulties. Therefore, this study is to make the concerning data for the useful reference by showing the trend and current situation of crew accidents, assorted by the crew's rank, type of the accident, injured part of the body, cause of the accident, type of ships and type of the works when the accident occurred, by using the each ocean-going shipping company's recent 5 years('95∼'99) data in korea.

  • The purpose of this study is to attempt a Analysis of a pointed-end Equipment Arm Safety-Accident for Fault Tree Analysis. Three major techniques were used first problem is Z-Model by which accident Analysis & prevention of a pointed-end Industry can be made, Fault Tree Analysis(FTA) bywhich quantification of a pointed-end Equipment accident Analysis can be made it 5 years in past and the third, manual-written by which minimal cut set to accident can be Identified. A example has been made of issue point a pointed-end Equipment that the Arm in loader happen to Injuries. According to the Analysis lack of safety knowledge, unsafety-behavior seem to be the primal cause of accident. Comparision of the accident cause to actual report demonstratesthat the FTA a efficient tool for Industrial Accident prevention.

CFD를 활용한 밀폐공간 가스질식사고의 피해 영향 평가 (An Estimation of the Consequence Analysis for Asphyxiation Accident in Confined Space using C.F.D.)

  • Recently, various engineering approaches have been widely used in the accident investigation field to identify the cause of the accident and to predict damage by accident. Computational analysis is the most commonly used method of accident investigation technique. This technique is mainly used to identify the mechanism of the accident generation and to determine the cause when it is difficult to reproduce the situation at the time of the accident or when it is impossible to perform a reproduction experiment. In this study, The computational fluid dynamics analysis for nitrogen asphyxiation accident generated by defect of building structural between diffusion outlet and cooling tower was performed to determine the inflow path of the suffocation gas, death possibility by concentration of suffocation gas and predicted the time of death due to the accident using 3D modeling and FLACS program. We can quantify diffusion concentration of asphyxiation gas and predict mechanism of death occurrence by accident and evaluate the consequence Analysis through this study. In the future, This method can be widely used in the field of gas safety by improving the reliability and validity of the analysis.

  • Universal Design involves designing products and spaces so that they can be used by the widest range of people possible. Universal Design evolved from Accessible Design, a design process that addresses the needs of people with disabilities. This paper introduces the universal design concept through analysis of causes of railway accident within the urban railway station. Railway accident analysis was performed by the internal facilities. We analyzed the standard of accident type, accident cause and accident subject such as elevator, escalator, stairs etc.

전문 건설공사 재해원인 분석 기법에 관한 연구 (Analysis Techniques for Accident Causes of Subcontract Work at Construction Site)

  • There are many accident causes related to even an accident. As well four(4) stepped causes of one accident at construction sites are analyzed in the study. First, eleven(11) cause factors are set up at each step such as policy level, management, indirect, direct level which are discussed and determined by field experts with 15year experience or more. Therefore, one direct cause occurred in construction site can be connected to the previous cause than in direct and management, and policy problem of previous management. These questionnaires results are analyzed with three different methods such as weighing level by Delphi technique, correlation analysis, critical pass method. Three different methods show their characteristic to see which subcontract work is more dangerous or not. Subcontract or at a construction site can be use the above three different cases as they need at their site in order to make more effective countermeasures.

화학사고 원인분석을 통한 예방대책 수립에 관한 연구 (A Study on Prevention Measure Establishment through Cause Analysis of Chemical-Accidents)

  • Even if several chemical accident prevention systems such as PSM(Process Safety Management), RMP(Risk Management Plan), etc. have been carried out, many chemical accidents have still occurred at industrial plants in Korea. We describe the status of chemical industry and the trend of occurrence of chemical accidents in Korea. And this paper analyzes the recent chemical accidents in eight ways. These ways include chemical accident forms, ignition sources, sources of chemical equipment, human vs equipment/material causes, worker's working situation, employee scale, hazardous substances, week & time, fatalities of manufacture & contractor's workers. Finally we proposes the four representative prevention measures brought to result of cause analysis by accident statistics.

화학실험실 사고 Data를 이용한 근본원인분석 Map 개발 및 원인 분석 (Cause Analysis and Development of Root Cause Analysis Map using Data of Chemical Laboratory Accidents)

  • 화학실험실 사고의 근본원인분석 Map을 개발하기 위하여 석유화학 사고 근본원인분석 Map을 참고하여 3단계 사고 요인 트리(Tree)로 구성된 Map을 작성하였다. 원인 인자 도표(Cause Factor Charting) 방식을 적용하여 실험실 사건 사고 211건을 1~5단계까지 사고 원인을 조사하고, 그 사고의 원인을 EXCEL 프로그램에 입력하였다. 그 후, 그 사고요인들을 유형과 각 단계별로 분류하여, 근본원인분석 Map 초안(draft)을 작성하였다. 또한, 연구실 사건 사고 211건의 근본원인이 근본원인분석 Map초안에 적절한지 재확인하였다. 향후 실험실에서 발생할 수 있는 사고의 원인들을 보완함으로써, 화학실험실에 관한 RCA Map이 개발되었다. 본 연구에서 제시한 근본원인분석 Map을 기반으로 발생빈도를 고려하여 사고 원인을 1~5단계로 나누어 분석한 결과, 3단계 원인은 관리시스템 35.%, 모니터링 12.2%, Human Factor Eng. 15.1%, 교육훈련 12.1% 등의 순으로 나타났다.
