요부 근력운동이 복부비만자의 요통지수, 요부 유연성, 복부비만도에 미치는 영향 (The Effects of Pain, Lumbar Flexibility and Abdominal Obesity of Patient with Low Back Pain and Abdominal Obesity after Lumbar Strengthening Exercise)

  • Purpose : The purpose of this study is to investigate that the Effects of Pain, Lumbar Flexibility and Abdominal Obesity of Patient with Low Back Pain and Abdominal Obesity after Lumbar Strengthening Exercise. Methods : The selected subjects of this study were 17 patients who had diagnosis on low back pain and body mass index was more than $25kg/m^2$ and lumbar length was more than 90cm. They divided into a control group with 10 patients doing lumbar stabilization exercise and an experimental group with 7 patients ding simple lumbar strengthening exercise in order to compare degrees of oswestry disability index and lumbar stability, lumbar length was measured before the exercise, in 4 weeks and 8 weeks after exercises respectively. Result : lumbar strengthening exercise on patients with low back pain and abdominal obesity affected improving function of lumbar and decrease of pain with abdominal obesity. Conclusion : Then lumbar stabilization exercise than simple strengthening exercise affected decrease of Visceral fat tissue on abdominal obesity.

비만 청소년에서 복부 내장지방량을 반영하는 비만지표에 대한 연구 (The Evaluation of Simple Indices of Abdominal Visceral Fat Area Among Korean Obese Adolescents)

  • Objectives : The purpose of this study was to determine what is useful index of abdominal visceral fat area among Korean obese adolescents. Methods : 35 obese adolescents who had visited Kirin Oriental Hospital from August 2004 to May 2006 were included in the study. Weight, height, waist and hip circumferences were measured. Abdominal visceral fat areas at L4-5 level were measured by computed tomography. The subjects were divided into two groups, according to sex. In each group, we investigated the correlation among waist circumference, waist/hip ratio, waist/height ratio, obesity degree and abdominal visceral fat area and evaluated that which one of these kinds of indices is the most useful predictor for visceral fat area in Korean obese adolescents. Results : There were significant correlation between waist circumference, waist/hip ratio, waist/height ratio and obesity index in both group. In boys waist/hip ratio had correlation with abdominal visceral fat area and in girls obesity index had significant correlation with abdominal visceral fat area. Conclusions : Our study suggests that waist/hip ratio in boys, obesity degree in girls can be a relatively good parameter for predicting abdominal visceral fat area.

분변 감입을 동반한 무증상의 심한 소아변비 치험례 (A Case Report of Fecal Impaction in a Child without Abdominal Symptoms)

  • Objectives The purpose of this study is to report the case of fecal impaction in a child without any abdominal symptoms treated by taking herbal medicine. Methods We examined a 7-year-old girl who had normal bowel movement and defecation per day, and had no particular abdominal symptoms. Abdominal radiography was taken, and unexpected severe fecal impaction was observed throughout the abdominal cavity, filled with intestinal gas and feces. According to the subject's parents, she had no generalized symptoms, such as abdominal pain or distension, and had on a regular diet and normal bowel movement daily. She was treated with herbal medicine (Daeseunggi-tang) for 23 days. While she was on the therapy, numbers, doses, bowel movements, and radiography were checked and recorded. Results During the treatment, her stool was softened, and fecal impaction was relieved as showed by abdominal radiography. Conclusions We have identified that there are cases where subjects have no symptoms of abdominal pain, despite presence of severe fecal impaction. In addition, it was found that Daeseunggi-tang is effective in fecal impaction in childhood.

뇌졸중 환자의 배근력과 호흡기능의 상관관계 (The Correlation between Abdominal Muscle Strength and Respiratory Function in Stroke Patients)

  • Purpose: This study aimed to investigate the correlation between abdominal muscle strength and measures of respiratory function in stroke patients. Methods: The study participants comprised 17 (male: 12, female: 5) stroke patients hospitalized at W rehabilitation hospital in Busan, South Korea. Abdominal muscle strength was assessed using a digital manual dynamometer for 5 seconds contacting the sternal notch of the participants to bend the trunk. Respiratory function (forced vital capacity, forced expiratory volume in one second, forced expiratory volume in one second/forced vital capacity, and peak expiratory flow) was assessed using a spirometer. The collected data were analyzed using Pearson's correlation analysis, and the significance level was set 0.05. Results: A statistically significant correlation was found between abdominal muscle strength and forced vital capacity, forced expiratory volume in one second, and peak expiratory flow. However, abdominal muscle strength and forced expiratory volume in one second/forced vital capacity were not significantly correlated. Conclusion: This study demonstrated that there is a relationship between abdominal muscle strength and respiratory function. Exercise programs to strengthen the abdominal muscles are therefore necessary to improve respiratory function in stroke patients.

  • Purpose: This study was an investigation of the effects of the bridge exercise with the sensory feedback of combined abdominal drawing-in on transverse abdominal and balance in patients with stroke. Methods: Forty subjects were randomly assigned into two groups. Subjects in the bridge exercise group (BG, n = 20) or feedback drawing-in bridge exercise group (FDBG, n = 20) were studied for 30 minutes each, twice daily, for four weeks. Outcomes were measured using affected weight distribution (AWD), anterior limit of stability (ALOS), posterior limit of stability (PLOS), timed up-and-go test (TUG), the Berg balance scale (BBS), and transverse abdominis thickness (TRA) before and after the four-week intervention period. Results: There were significant effects in the FDBG pre-intervention and post-intervention in AWD, ALOS, PLOS, TUG, BBS, and TRA. Conclusion: The results of this study suggest that the bridge exercise with sensory feedback combined with abdominal drawing-in could be beneficial for patients with stroke in terms of transverse abdominal and balance.

Multifidus Thickness Comparison between the Effectiveness of Abdominal Bracing and Maximum Expiration Maneuvers in Lumbo-Pelvic Upright Sitting Posture

  • Background: Trunk flexor-extensor muscles' co-activation and upright posture are important for spinal stability. Abdominal bracing and maximal expiration are being used as exercises to excel torso co-contraction. However, no study has on comparison of the effect of this exercise on multifidus in the upright sitting posture. Objectives: This study aims to verify the effectiveness of abdominal bracing and expiration maneuvers in lumbo-pelvic upright sitting. Design: Cross-sectional study. Methods: Eighteen healthy women were recruited for this study. The multifidus muscle thickness of all subjects was measured in three sitting conditions (lumbo-pelvic upright sitting, lumbo-pelvic upright sitting with abdominal bracing, and lumbo-pelvic upright sitting with maximum expiration) using ultrasound. One-way repeated measure analysis of variance was used for the evaluation. Results: Compared to lumbo-pelvic upright sitting, lumbo-pelvic upright sitting with abdominal bracing and lumbo-pelvic upright sitting with maximum expiration were associated with significantly increment of muscle thickness. There was no significant difference in muscle thickness between lumbo-pelvic upright sitting with abdominal bracing and lumbo-pelvic upright sitting with maximum expiration. Conclusion: Abdominal bracing and maximum expiration could be beneficial to increasing lumbar multifidus thickness in lumbo-pelvic upright sitting.

Traumatic abdominal wall hernia with hemoperitoneum caused by blunt injury: laparoscopic exploration with mini-laparotomy repair. A case report

  • Traumatic abdominal wall hernia is a very rare clinical entity. Herein, we report the case of a patient who was transferred from a local clinic to the emergency department because of left lower abdominal pain. Initially, an intra-abdominal hematoma was observed on computed tomography and no extravasation was noted. Conservative treatment was initiated, and the patient's symptoms were slightly relieved. However, though abdominal pain was relieved during the hospital stay, bowel herniation was suspected in the left periumbilical area. Follow-up computed tomography showed traumatic abdominal wall hernia with hemoperitoneum in the abdomen. We performed a laparoscopic exploration of the injury site and hernia lesion. The anterior abdominal wall hernia was successfully closed.

흡연자와 비흡연자의 앉은 자세에서 일어서는 동작 시 배근육 활성도 차이 (Differences in the Activation of Abdominal Muscles During Sit-to-Stand between Smokers and Non-smokers)

  • Purpose: This study aimed to determine whether the abdominal muscles, which are the major lumbar stabilizers along with the respiratory muscles, are affected by smoking. We compared abdominal muscle activity between smokers and non-smokers during the sit-to-stand movement. Methods: A total of 28 healthy adult males (14 smokers and 14 non-smokers) in their 20s-30s voluntarily participated in the study. The subjects performed the sit-to-stand movement, and then their abdominal muscle activity was measured. The surface electromyography system was employed to measure the maximal voluntary isometric contraction (MVIC) values of the subjects' rectus abdominis, external oblique abdominal, internal oblique abdominal, and transversus abdominis muscles. Then, the values were quantified into %MVIC. Results: The activity of the rectus abdominis muscle was higher in the smokers than in the non-smokers. Conversely, the activity of the internal oblique abdominal and transversus abdominis muscles, which are deep abdominal muscles, was higher in non-smokers than in smokers, but the difference was not statistically significant. Conclusion: Smoking can inhibit the activity of smokers' deep abdominal muscles and increase the activity of their superficial muscles, thus making it detrimental to their musculoskeletal system.

Systemic Classification for a New Diagnostic Approach to Acute Abdominal Pain in Children

  • Purpose: With previous methods based on only age and location, there are many difficulties in identifying the etiology of acute abdominal pain in children. We sought to develop a new systematic classification of acute abdominal pain and to give some helps to physicians encountering difficulties in diagnoses. Methods: From March 2005 to May 2010, clinical data were collected retrospectively from 442 children hospitalized due to acute abdominal pain with no apparent underlying disease. According to the final diagnoses, diseases that caused acute abdominal pain were classified into nine groups. Results: The nine groups were group I "catastrophic surgical abdomen" (7 patients, 1.6%), group II "acute appendicitis and mesenteric lymphadenitis" (56 patients, 12.7%), group III "intestinal obstruction" (57 patients, 12.9%), group IV "viral and bacterial acute gastroenteritis" (90 patients, 20.4%), group V "peptic ulcer and gastroduodenitis" (66 patients, 14.9%), group VI "hepatobiliary and pancreatic disease" (14 patients, 3.2%), group VII "febrile viral illness and extraintestinal infection" (69 patients, 15.6%), group VIII "functional gastrointestinal disorder (acute manifestation)" (20 patients, 4.5%), and group IX "unclassified acute abdominal pain" (63 patients, 14.3%). Four patients were enrolled in two disease groups each. Conclusion: Patients were distributed unevenly across the nine groups of acute abdominal pain. In particular, the "unclassified abdominal pain" only group was not uncommon. Considering a systemic classification for acute abdominal pain may be helpful in the diagnostic approach in children.

한국 중년 남성에서 복부지방 축적이 혈청 지질 및 지단백 농도에 미치는 영향 (Influence of Total Abdominal Fat Accumulation on Serum Lipids and Lipoproteins in Korean middle-aged men)

  • Anthropometry, computed tomography(CT) at the umbilical level, nutrient intake, blood pressure, serum levels of lipids and lipoproteins and response of glucose, c-peptide, insulin, and free fatty acid(FFA) during oral glucose tolerance test(OGTT) were estimated on 11 normal-weight controls and 35 overweight and obese middle-agd men. The areas of total abdominal, subcutaneous and visceral were determined by CT scanning technique. Total abdominal fat area correlated the most significantly with the levels of serum lipids, lipoproteins and insulin among several obese indices. Compared with normal-weight controls, overweight and obese men with abdominal fat lower than 29000$\textrm{mm}^2$ showed an increase in waist-hip ratio, areas of total abdominal(35%), visceral and subcutaneous fat and C-peptide response area during OGTT, though age, percent ideal body weight, body mass index, % body fat, and all biochemical indices except C-peptide response area were not different between two groups. Overweight and obese men with abdominal fat greater than 29000$\textrm{mm}^2$ showed a higher values in total abdominal fat(85%), serum levels of triglyceride, total-and LDL-cholesterol, the ratio of LDL-to HDL-cholesterol, and response areas of FFA, insulin and C-peptide during OGTT than normal-weight controls. Overweight and obese men with great abdominal fat showed an increase in alcohol ingestion and percent calorie intake per total energy expenditure, compared with normal-weight controls. Our results indicate that obesity and a certain level of total abdominal fat accumulation is required to observe abnormal levels of serum lipids, lipoproteins and insulin in Korean middle-aged men. In addition, increased alcohol and calorie intake and decreased physical activity could partly explain total abdominal fat accumulation in men.
