Fine Needle Aspiration Cytology of Metastatic Epithelial-Myoepithelial Carcinoma of the Scalp - A Case Report - (두피에 전이한 상피-근상피암종의 세침흡인 세포학적 소견 - 1예 보고 -)

  • Epithelial-myoepithelial carcinoma is an uncommon, low grade malignant epithelial neoplasm and metastasis is exceedingly rare. This article highlights the fine needle aspiration cytology(FNAC) of a case of metastatic epithelial-myoepithelial carcinoma of the scalp. A 51-year-old female presented with the left parietotemporal scalp mass two months after the lett parotidectomy for epithelial-myoeplthelial carcinoma. FNAC from the scalp mass showed a biphasic population of ductal epithelial and myoeplthelial origin. These epithelial aggregates were numerous and formed a distinct three dimensional architecture in the background of numerous naked nuclei. The three dimensional architectures were predominantly composed of tightly cohesive eosinophilic ductular epithelial cells which tended to aggregate, overlap, and form tubules. Clear myoepithelial cells in three dimensional tissue fragment were inapparent and a few were attached to the periphery of the fragments. A few myoepithelial cells with clear abundant vaculoated cytoplasm were found In the foamy background. The cytological diagnosis was metastatic epithelial-myoepithelial carcinoma. The histologic findings of the scalp mass were those of typical epithelial-myoepithelial carcinoma. Cytologic distinction of epithelial-myoepithleial carcinoma, pleomorphic adenoma, and adenoid. Cytologic carcinoma may be very difficult but careful attention to clinical features and cellualr details can classify these neoplasms correctly.

  • Intelligent systems(i.e., artificial intelligence), particularly deep learning, are machines able to mimic the cognitive functions of humans to perform tasks of problem-solving and learning. This field deals with computational models that can think and act intelligently, like the human brain, and construct algorithms that can learn from data to make predictions. Artificial intelligence is becoming important in radiology due to its ability to detect abnormalities in radiographic images that are unnoticed by the naked human eye. These systems have reduced radiologists' workload by rapidly recording and presenting data, and thereby monitoring the treatment response with a reduced risk of cognitive bias. Intelligent systems have an important role to play and could be used by dentists as an adjunct to other imaging modalities in making appropriate diagnoses and treatment plans. In the field of maxillofacial radiology, these systems have shown promise for the interpretation of complex images, accurate localization of landmarks, characterization of bone architecture, estimation of oral cancer risk, and the assessment of metastatic lymph nodes, periapical pathologies, and maxillary sinus pathologies. This review discusses the clinical applications and scope of intelligent systems such as machine learning, artificial intelligence, and deep learning programs in maxillofacial imaging.

  • Medical Gateway(MG), that integrates medical devices with its unique data format in various hospital information systems, provides effectiveness in the cost of the management unlike existing systems that uses PC as a gateway to hospital information systems because MG enables to interface medical devices with itself. However, in spite of using MG for integrating medical devices, we need additional programs or systems to interwork with EMR system of each hospital in accordance with the configuration of DB. In this paper, we design and implement the format of medical information and server system for interworking MG with EMR system. The proposed system makes easy exchanging medical data between MG and EMR system. Thus, the proposed system can contribute to a national project to export abroad advanced hospital setup converged with IT.

Presentation of Neurolytic Effect of 10% Lidocaine after Perineural Ultrasound Guided Injection of a Canine Sciatic Nerve: A Pilot Study

  • Background: Phenol and alcohol have been used to ablate nerves to treat pain but are not specific for nerves and can damage surrounding soft tissue. Lidocaine at concentrations > 8% injected intrathecal in the animal model has been shown to be neurotoxic. Tests the hypothesis that 10% lidocaine is neurolytic after a peri-neural blockade in an ex vivo experiment on the canine sciatic nerve. Methods: Under ultrasound, one canine sciatic nerve was injected peri-neurally with 10 cc saline and another with 10 cc of 10% lidocaine. After 20 minutes, the sciatic nerve was dissected with gross inspection. A 3 cm segment was excised and preserved in 10% buffered formalin fixative solution. Both samples underwent progressive dehydration and infusion of paraffin after which they were placed on paraffin blocks. The sections were cut at $4{\mu}m$ and stained with hemoxylin and eosin. Microscopic review was performed by a pathologist from Henry Ford Hospital who was blinded to which experimental group each sample was in. Results: The lidocaine injected nerve demonstrated loss of gross architecture on visual inspection while the saline injected nerve did not. No gross changes were seen in the surrounding soft tissue seen in either group. The lidocaine injected sample showed basophilic degeneration with marked cytoplasmic vacuolation in the nerve fibers with separation of individual fibers and endoneurial edema. The saline injected sample showed normal neural tissue. Conclusions: Ten percent lidocaine causes rapid neurolytic changes with ultrasound guided peri-neural injection. The study was limited by only a single nerve being tested with acute exposure.

A Study on the Guidance Signage System of Outpatient in General Hospital using Spatial Configuration Theory - View from G.D.Weisman's Way-finding Influence Factors (공간구조론을 적용한 종합병원 외래부 유도사인 배치 및 평가에 관한 연구 - G.D.Weisman의 길찾기 요소를 중심으로)

  • Purpose: Signs that are installed at unnoticeable places or that disconnect before the destination can bring errors of location information delivery. Therefore, this study aims to find out the spatial relation between structure of space and signs in the perspective of visual exposure possibility, operating arrangement and assesment by applying spatial structure theory. Methods: Effectiveness of organization of guidance signs was evaluated after the four way-finding factors(Plan Configuration, Sign System, Perceptual Access, Architectural Difference) that G.D.Weisman suggested were interpreted by spatial structure theory(J-Graph analysis, Space Syntax, Visual Graph Analysis) under the premise that it is closely related to the structure of space. Results: 1) Because the south corridor that connects each department of outpatient division is located in the hierarchy center of the space, and walking density is expected to be high, guidance signs need to be organized at the place with high integration value. 2) The depth to the destination space can be estimated through J-Graph analysis. The depth means a switch of direction, and the guidance signs are needed according to the number. 3) According to visibility graph analysis, visual exposure can be different in the same hierarchy unit space according to the shape of the flat surface. Based on these data, location adjustment of signs is possible, and the improvement effect can be estimated quantitatively. Implications: Spatial structure theory can be utilized to design and evaluate sign systems, and it helps to clearly understand the improvement effect. It is desirable to specify design and estimation of sign systems in the order of J-Graph analysis${\rightarrow}$Space Syntax Theory${\rightarrow}$visibility graph analysis.

Energy Consumptions and Daylight Illumination levels of a Multi-beded Patient Room according to the Window Shapes and Shading (창의 형태 및 차양 계획에 따른 다인 병실의 에너지소비량과 주광조도의 평가 및 분석)

  • Window and shading designs have a great influence on energy consumption and daylighting in buildings. As far as energy is concerned, small window area is advantageous. But it is not good to the patient healing in hospital. So it is important to find out the optimum window shape which is favorable for both energy consumption and patient healing. In this study, annual energy consumption and daylight illumination levels were analyzed according to the window shapes and shading devices for a multi-beded patient room in hospitals. The simulations were conducted for 19 different cases by COMFEN 4.0 computer simulation program. The results of this paper are as follows. First, window to wall area ratio and shading devices have great influences on annual energy consumption. But it is a problem in that they decrease significantly daylight level in bed room. Second, considering the same energy consumption, reducing the width of window rather than the hight of window is desirable for the secure of daylight level. Third, increase of the number of horizontal shade is not desirable in south face of the building for the energy consumption and daylight level. Fourth, sun shade is not necessary in north face of the building for the energy consumption and daylight level.

  • Recently, the small-scale local governments of japan are enlarging and enriching the service of health, medical and welfare facilities for the elderly . Intensive arrangement of the three types of facilities is one of the effective ways especially for the small-scale local governments. The service network between facilities benefits by the intensive arrangement. Benefits include the network of the medical service, the share of care information, the share of space and equipment. It can be a notable feature in the intensive arrangement that the elderly were taken from welfare facilities (especially dayscare center) to hospital of high movement frequency quickly. Instead of EV path as possible, It is desirable to stand close between facilities of high movement frequency. For large area, the heavy snow made low daycare ratio. Therefore, it is desirable to construct a branch office at a long distance.

  • Recently, Rural village of japan are enlarging and enriching the service of health, medical and welfare facilities for the elderly. Concentrated arrangement of the three types of facilities is one of the effective ways especially for the small-scale local governments. This paper focus on a questionnaire survey of employees who work in the facilities to evaluate service network. Using this survey data, it is used to make a plan of intensive arrangement. As a result, their satisfaction with concentrated arrangement was considerable high. In particular, they expressed satisfaction with service network between Medical and Welfare. Due to increasing outpatients, the concentrated arrangement has contributed to hospital funds and given emotional security to the aged in welfare facilities. In order to promote service network effect, there are many opinions that management of health facilities(local governments) is needed.

A Study on the Evacuation Procedure Analysis Model of General Hospital Considering Patients Types (환자의 유형을 고려한 종합 병원의 피난 절차 분석 모델에 관한 연구)

  • Purpose: This study aimed to present an analysis model evaluating evacuation performance considering patient types and procedural evacuation in the medical facility. The user group of the medical facility, including users challenged in evacuation behavior, entails the risk of many casualties. Therefore, it is necessary to plan an evacuation procedure that considers the evacuation characteristics of users. Methods: Through the review of precedent studies, the evacuation procedure of the medical facility, the classification of patient types, and the evacuation procedure was set as conditions and variables for the analysis. The result caused by a variety of conditions and variables were explored. Results: 1) The total evacuation completion time and congestion time were shortened at the procedural evacuation. Moreover, it derived many users from evacuating at the initial phase. 2) The proposed model can provide a basis for proposing a space planning direction that considers the possibility of not carrying out the evacuation plan. 3) It supports safe evacuation by identifying variables that reduce overcrowding by comparing the congestion time of overcrowded spaces. 4) The analysis model can identify the overcrowded space through the evacuation route and suggest the basis for architectural improvements that reduce overcrowding. Implications: The study results can be used to analyze the performance of evacuation procedures and support the establishment of evacuation procedures and building plans for safe evacuation for medical facilities.

Epithelial-mesenchymal Transition and Its Role in the Pathogenesis of Colorectal Cancer

  • Epithelial-to-mesenchymal transition (EMT) is a collection of events that allows the conversion of adherent epithelial cells, tightly bound to each other within an organized tissue, into independent fibroblastic cells possessing migratory properties and the ability to invade the extracellular matrix. EMT contributes to the complex architecture of the embryo by permitting the progression of embryogenesis from a simple single-cell layer epithelium to a complex three-dimensional organism composed of both epithelial and mesenchymal cells. However, in most tissues EMT is a developmentally restricted process and fully differentiated epithelia typically maintain their epithelial phenotype. Recently, elements of EMT, specially the loss of epithelial markers and the gain of mesenchymal markers, have been observed in pathological states, including epithelial cancers. Increasing evidence has confirmed its presence in human colon during colorectal carcinogenesis. In general, chronic inflammation is considered to be one of the causes of many human cancers including colorectal cancer(CRC). Accordingly, epidemiologic and clinical studies indicate that patients affected by ulcerative colitis and Crohn's disease, the two major forms of inflammatory bowel disease, have an increased risk of developing CRC. A large body of evidence supports roles for the SMAD/STAT3 signaling pathway, the NF-kB pathway, the Ras-mitogenactivated protein kinase/Snail/Slug and microRNAs in the development of colorectal cancers via epithelial-tomesenchymal transition. Thus, EMT appears to be closely involved in the pathogenesis of colorectal cancer, and analysis refered to it can yield novel targets for therapy.
