Short-term Associations of Air Pollution with Postneonatal Infant Death in Seoul, Korea, 1999-2003

  • Objective to assess whether exposure to air pollutants is associated with postneonatal infant death, using a timeseries methodology, between 1999 and 2003 in Seoul, Korea.. Methods We investigated the short-term effects of air pollution for 548,725 live births during the study period. The daily count of postneonatal infant deaths from all causes and from SIDS (sudden infant death syndrome) by birth order was analyzed by a Generalized Additive Poisson model, with controlling for the effects of seasonal trends, air temperature, relative humidity, barometric pressure, and day of the week as covariates. Results During the study period, we observed 699 deaths from all causes and 47 deaths from SIDS. We did not find any significant associations between daily mortality and ambient levels of air pollutants except for CO and $NO_2$. The estimated relative risk of postneonatal infant death from all causes was 1.17 (95% CI=1.04-1.32) and 1.16 (95% CI=1.03-1.29) by IQR (interquartile range) for CO and $NO_2$ respectively. Also, we observed no clear trend of the mortality effects of air pollution by birth orders. Conclusion In conclusion, our findings suggest that air pollution, in general, influenced adversely postneonatal infant death from all-cause and SIDS although it was not statistically significant. This study may support that the rationale.

양육관련변인과 어머니-영아 상호작용행동간의 관계 (Relations between Parenting-Related Variables and Mother-Infant Interactive Behaviors)

  • Relations between mother-infant interactive behaviors and parenting-related variables were analyzed in a sample of 72 infants (35 boys and 37 girls, average age=31 mo.) and their mothers in Ulsan, Korea. Parents' views on children, parenting stress, fathers' participation in parenting and social support were measured using questionnaires and mother-infant interactive behaviors were observed using the 3-bags test. Among the parenting-related variables, parents' views on children were related significantly with mothers' emotional expressions & infant's participation with mothers. Social support was correlated with mother's behaviors, including positive rewards and emotional expressions. Mother-infant interactive behaviors were closely correlated with one another: Mothers' positive behaviors such as overall reactivity and cognitive stimulation were correlated positively with infants' positive behaviors, including sustained attention and positive affect. Future studies will provide us with greater insights into the mechanisms underlying the effects of these parenting-related variables on infant behavior and development.

아두이노와 압력센서를 이용한 스마트 유아차에 관한 연구 (Study on Smart Infant Vehicle with Arduino and Pressure Sensor)

  • 본 논문에서는 아이들을 태우는 유아차로부터 생길 수 있는 각종 안전사고를 예방 그리고 보다 수월한 유아차 사용을 위한 연구를 진행하였다. 유아차가 보호없이 주행하는 것을 막기 위하여 압력센서와 서브모터를 활용하여 유아차 바퀴에 브레이크 기능을 탑재하였으며 압력센서와 LCD를 활용하여 안전벨트 결속의 유무를 판단하여 유아가 유아차에서 떨어지는 것을 방지할 수 있도록 설계하였다. 추가로 LCD와 LED를 활용하여 일정 온습도를 넘어가면 경고등이 켜지도록 설계하여 유아가 유아차를 사용할 때 쾌적한 환경에 있을 수 있도록 설계하였다.

교사 직무만족도 및 영아기질과 교사-영아 관계 (Child Care Teacher's Job Satisfaction and Infant Temperament Associated with teacher-student Relationship in the 2-year-olds class)

  • 본 연구는 영아 보육시설에서의 교사 직무만족도 및 영아 기질에 따라 교사-영아 관계가 차이가 있는가를 알아보고, 이를 통해 영아 보육에 관한 기초 자료를 제공하는 것을 목적으로 실시하였다. 연구대상은 충주지역의 27개 보육시설에서 영아반 교사 50명과 교사가 담당하는 영아 175명이었다. 연구결과는 첫째, 영아반 보육교사의 직무만족도는 교사의 연령과 근무하는 시설유형에서 부분적으로 차이가 나타났다. 원장과의 관계에 대한 직무만족도는 연령이 높은 교사가 연령이 낮은 교사들보다 더 높았고, 반면, 보수에 대한 만족도는 연령이 낮은 교사들이 높은 교사들보다 높게 나타났다. 영아 성별과 월령에 따른 영아 기질은 차이가 없었다. 교사-영아관계는 성별에 따른 차이가 있어서 여아가 남아의 경우보다 친밀한 관계를 유지하였으며, 어린 영아들이 교사와 더욱 의존적인 관계를 가지는 것으로 나타났다. 둘째, 교사 직무만족도는 교사-영아 관계에서 친밀성과 의미 있는 차이가 있었다. 직무만족도가 높은 교사는 친밀한 교사-영아 관계를 갖는 것으로 나타났다. 갈등과 의존성의 부정적인 교사-영아 관계는 교사의 직무만족도에 따라 차이가 나타나지 않았다. 셋째, 영아 기질에 따른 교사-영아 관계는 차이가 없는 것으로 나타났다. 따라서 기질은 교사-영아 관계에 직접적인 영향을 주지는 않는 것으로 나타났다.

영아용 몬테소리 수행평가 도구 개발 - 6~24개월 영아를 중심으로 - (The Development of Infant Montessori Performance Assessment Scales - Centered on 6 to 24 Month Infants -)

  • The purpose of this study was to test the reliability and validity of the Infant Montessori Performance Assessment Scales developed by the authors. Subjects of the study consisted of 132 Montessori teachers and 50 infants who were attending Montessori home school throughout the whole country, using census method. Infant's age ranged from 6 to 24 months. Data were analysed by using SAS 8.2 PC program. Statistical methods employed were frequency, Cronbach's alpha, Kappa coefficient, test-retest correlation, construct validity, and concurrent validity. The Cronbach's alpha of 6 sub-scales included physical, creativity, practical life, sense, language, and cognitive education, which ranged from .70 to .86. And the! correlation of the test/re-test was .72. The correlation between the 6 sub-scales of Infant-Montessori Performance Assessment Scales and the total scores of 6 items ranged from .06 to .84. Therefore, the construct validity of Infant Montessori Performance Assessment Scales was verified. The Kappa coefficient of inter-rater reliability was .76. The correlation between the Infant Montessori Performance Assessment Scales and the Standardized Korean Creativity Traits Checklist(K-CTC) and the Korean Child Social Maturity Scales showed non-significant levels of .16 and .12 respectively. Conclusively, Infant Montessori Performance Assessment Scales developed by the authors were verified through the above reliability and validity tests. Specifically the Infant Montessori Performance Assessment Scales showed the relationship of the convergent and divergent validity with the Korean Child Social Maturity Scales and the Standardized Korean Creativity Traits Checklist, respectively.

모-태아 상호작용 증진 프로그램이 임부 및 영아의 수면-활동 양상에 미치는 효과 (Effects of a Mother-fetus Interaction Promotion Program on Sleep and Activity in Mothers and Infants)

  • Purpose: This study was done to evaluate effects of a mother-fetus interaction promotion program on maternal and infant sleep/activity regulation, and examine the correlation between maternal and infant sleep/activity. Methods: Thirty-four mother-infant dyads were recruited from an OB-GY clinic with postpartum care center, and assigned to the intervention group (17) or control group (17). Data were collected from January 18 to August 5 2005. A programmed education focused on mother-fetus interaction was provided to the intervention group in the 3rd trimester. Maternal sleep/activity record was completed at 32-36 weeks gestation and infant sleep/activity record was completed for one month after birth. Results: During the 3rd trimester, there were no significant differences in maternal sleep/activity between the groups, except for frequency of mothers' night awakening and urination at night. There were no significant differences in infant sleep, but significant difference was found in feeding times between the groups. Correlation of maternal sleep in the 3rd trimester and infant waking times was significant. Conclusion: The results of this study indicate that maternal sleep/activity during pregnancy influences infant sleep/activity at 3-4 weeks, and that nursing interventions to regulate maternal sleep/activity provided to primiparas can be beneficial to the regularity of infant sleep/activity patterns.

모자동실이 초산모의 영아에 대한 태도와 돌보기 자신감에 미치는 영향 (The Effect of Rooming-in on Maternal Attitude and Self Confidence for Infant Care among Primiparas)

  • The purpose of this study was to compare maternal attitude and self confidence for infant care of primiparas of rooming-in and not rooming-in. The subjects were 128 primiparas who had delivered at eight general hospital in Seoul. 67 primiparas were in three rooming-in facilities and 61 primiparas were in five not rooming-in facilities. The data were collected from primiparas using Cohler's Maternal Attitude Scale and Pharis' Self Confidence Scale at postpartal 1 or 2 weeks. The results of this study were as follows : 1. The mean of maternal attitude was 100.32 and the range was from 82 to 138. The score of primiparas in rooming-in(100.94) was higher than those of not rooming-in(97.43). There was a significant difference in maternal attitudes between rooming-in group and not rooming-in group(P=.001). There were no significant differences in maternal attitude according to age and infant sex. But types of feeding were related to maternal attitude (P=.017). 2. The mean of self confidence for infant care was 119.55 and the range was from 58 to 173. The mean of primiparas in rooming- in(123.10) was higher than those of not rooming-in (115.86). There was a significant difference in self confidence for infant care between rooming-in group and not rooming- in group (P=.040). No significant differences existed in self confidence for infant care according to age, infant sex, and types of feeding. 3. The rate of breast feeding was 64.2% in rooming-in group and 34.4% in not rooming-in group at postpartal 1 or 2 weeks. There was a significant difference in breast feeding between the two groups(P=.004). In conclusion, rooming-in facilities provided primiparas with more positive maternal attitude and greater self confidence for infant care and increased the rate of breast feeding.

영아기 어머니의 산후 우울 정도와 모성역할 자신감, 양육 스트레스, 영아기질에 관한 연구 (Postpartum Depression and Maternal Role Confidence, Parenting Stress, and Infant Temperament in Mothers of Young Infants)

  • Purpose: The purposes of this descriptive survey study were to describe postpartum depression level and to identify the influence of postpartum depression on maternal role confidence, parenting stress and infant temperament, in order to develop a base for nursing intervention programs for mothers who have postpartum depression. Method: Data were collected from 183 mothers of infants who visited one public health center in Goyang city. The instrument used for this study was a self-report questionnaire to identify the postpartum depression (BDI), maternal role confidence, parenting stress (PSI) and infant temperament (WBL). Results: Of the mothers 42.1% were in the normal range for postpartum depression, 33.3% in the mild group and 24.6% were in the moderate to severe group. There were significant differences in parenting stress, and infant temperament according to level of postpartum depression. There were correlations between maternal role confidence (r=-0.13), infant temperament (r=-0.20), parenting stress (r=0.51) and postpartum depression, and weak negative correlation between parenting stress and infant temperament (r=-0.30). Conclusion: Postpartum depression in the infant period when the mother's role is very important and the effect is not only on the mother's role but also on the infant's growth. It is essential to assess and provide immediate care to mothers who have postpartum depression.

Knowledge on Sudden Unexplained Infant Death-related Safe Sleep Practices and Infant Cardiopulmonary Resuscitation in Pediatric Nurses

  • Purpose: Sudden unexplained infant death (SUID) is a major contributor to infant mortality, and pediatric nurses have the responsibility to educate parents on SUID-reducing strategies. This study was conducted to measure pediatric nurses' knowledge of SUID-related safe sleep practices (K-SSSP) and infant cardiopulmonary resuscitation (K-ICPR). Methods: In total, 136 pediatric nurses were administered a survey including K-SSSP (13 items), K-ICPR (5 items), confidence in K-SSSP education (1 item; 5 points), and other factors relating to SUID experiences or education. Results: The correct answer rates of the K-SSSP and K-ICPR were 62.6% and 62.5%, retrospectively. The mean score for confidence in K-SSSP education was 2.6±0.9. Only 18 nurses (13.2%) responded that they educated parents on the content of the K-SSSP, while 76 nurses had received education on SUID. Positive relationships were observed between K-SSSP scores and higher education, between K-ICPR scores and having own child(ren) and clinical experience, and between confidence in K-SSSP education and higher education or having one's own child(ren). Nurses caring for newborns performed more SUID education than nurses working in other units. Conclusion: There is a profound need to implement a systemic educational program on SUID and strategies to reduce SUID for pediatric nurses.

모아상호작용과정의 진행양상에 관한 연구 (A Study on the Change of Primipara's Mother - Infant Interactional process)

  • One of the important tasks for new parents, especially mothers, is to establish warm, mutually affirming interpersonal relationships with the new baby in the family, with the purpose of promoting the health development of the child and the well-being of the whole family. Nurses assess the quality of the behavioral characteristics of the maternal-infant interaction. The purpose of this longitudinal descriptive correlational study is to investigate the changes of the mother - infant interactions from postpartum 1 day to postpartum 8weeks of the transition to parenthood. The aim was to contribute to the development of theoretical under standing on which to base care toward promoting the quality of maternal - infant interaction. Data were collect ed directly by the investigator and at rained from Jul, 1, 1990 to Jun 8, 1991, Subjects were a random sample of 44 mothers, 44 who had a normal delivery (but with out other perinatal complications) at four general hospitals in Seoul. Instruments used were the Stainton Parent-infant Interaction Scale (1981). The first observations were made in the delivery room, followed by day 1, day 2, day 3 and 2 weeks, 4 weeks, 6 weeks and 8 weeks after, birth, for a total of 8 contacts. Observations in the hospital were made during the hour prior to scheduled feedings. The infant was placed beside the mother. Later contacts were made at home. Data analysis was don by computer using as SPSS program and included, Paired t-test, t-test and Pearson Correlation coefficient ; the results were as follows. 1. The daily maternal-infant interaction score for the initial contact ofter birth to 8 weeks postpartum had the lowest average score 5.21 and the highest 8.02(in a range of 0-10). This subject group of mothers needed Extra nursing supporting to promote their maternal-infant interaction. 2. The daily scores for the maternal-infant interaction tended to rise, showing a gradual improvement over the period of eight weeks. However, there were significantly different increases in maternal-infant interaction only from the first to second day (p=0.000) and from the fourth to sixth weeks ofter birth.(p=0.000) 3. When the seven items of maternal-infant interaction were evaluated items of maternal-infant inter action were evaluated separately, "Care taking for the baby" had the highest average score, 1.60(in a range of 0-2) and "Speaks to baby" the lowest, 0.8. All items, suggested the subjects' need of nursing intervention to promote maternal-infant interaction 4. There were positive correlations between certain general characteristics, namely, both a higher economic status (p=0.027) and breast feeding (p=0.021) and maternal-infant interaction.
