A Study on the Pattern Distribution of Yin-Yang Ren [음양인] (Used on Questionnaire) (음양인 유형분류에 관한 연구 (설문지를 중심으로))

  • Objectives : Based on the analysis of Yin-Yang[음양] characteristics and symptoms, each person is classified into Yin-Yang. Also the validity of the result is statistically analized. Methods : From Feb. to May. 2003, the data were collected through a questionnaire given to 690 patients. The questionnaire was composed of 34 items which were about personality, habit, sweat, response to coldness, thirst, bowel, urine, physical shape, and menstruation for women only. SD(Semantic Differential Technique) used for each item, each item is measured as a contrast of two opposite symptoms. Reliability analysis was used to select items and categories. Based on means of items in each category the Yin-Yang index was developed. The validity of Yin-Yang index was investigated using classification and clustering analysis. In statistical analysis, SPSS V10.0.7 PC was used. Results : The obtained results are summarized as follows: 1) We constructed Yin-Yang index based on the middle point of the sum of categorical means. Then we classified each person into Yin or Yang. 2) To investigate the validity of the distribution of personal Yin-Yang degree, the crosstabulation of results from clustering and classification was used. The hit ratio for classification was much higher than Maximum Chance Criterion($C_{max}$), and concurrence in crosstabulation was successful. Therefore we can infer that the distribution of Yin-Yang was valid. Conclusions : Based on Yin-Yang characteristics and symptoms, we was analyzed personal degree of Yin-Yang, and confirmed the validity of its distribution. Therefore this index can be used further for Bian-Zheng [변증] and classification of the constitution.

Study on Reliability and Validity of the 'Qi Blood Yin Yang Deficiency Questionnaire' (기혈음양허손 변증 설문지의 신뢰도 및 타당도 기초연구)

  • The study was performed to determine the reliability and validity of the 'Qi Blood Yin Yang Deficiency Questionnaire (Qi Blood Yin Yang DQ)' for the 100 subjects with chronic fatigue. After 100 subjects respond to the 'Qi Blood Yin Yang DQ', Korean medical doctor classified the subjects into 4 groups such as Qi deficiency group, Blood deficiency group, Yin deficiency group and Yang deficiency group. 100 subjects were retested in the same way after 3 weeks. The reliability and the validity of the questionnaire were inspected. Internal consistency of the 'Qi Blood Yin Yang DQ' was excellent (Cronbach alpha 0.916). Test-retest reliability was good (intra-class correlation coefficient 0.699). Construct validity analyzed by exploratory factor analysis produced 4 factors, which were selected from eigenvalues that are greater than 1.0. The 'Qi Blood Yin Yang DQ' consisted of 4 factors. The factor 1, 2, 3 and 4 showed 'Qi deficiency', 'Yin deficiency', 'Yang deficiency' and 'Blood deficiency' respectively. The 'Qi Blood Yin Yang DQ' restructured in this study may provide a fundamental questionnaire and a further study is required for a more advanced, standardized and statistically proven questionnaire.

Yin-Yang Temperament and Psychopathological Behavior in Middle School Students (중학생에서 음양 기질과 문제 행동의 관련성)

  • Objectives: The Yin-Yang temperament and the Sasang Personality Questionnaire (SPQ) were reported to have relations with psychopathological problems in adolescents. The purpose of this study was to examine how domains of Yin-Yang temperament influence on problem behaviors in Korean middle school students. Methods: Subjects of 681 Korean middle school students completed the SPQ for measuring Yin-Yang temperament and the Youth Self-Report (YSR) for measuring problem behaviors. The differences of problem behaviors between Yin and Yang temperament groups were tested with T-test and profile analysis. The associations of the SPQ subscales on problem behaviors was also examined with Pearson's correlation and regression analysis. Results: Yang temperament was related to the externalizing problem while Yin temperament to the internalizing problem. The Emotionality and Behavior dimension of Yin-Yang temperament were found to be a factor of susceptibility and resilience for problem behaviors, respectively. Three dimensions of Yin-Yang temperament have different influences on externalizing and internalizing problems depending on the sex and Yin-Yang temperament groups of subjects. Conclusions: The Yin-Yang temperament would be useful for predicting and preventing the problem behaviors of middle school students. This study would be useful for screening the psychopathological problems and helping Korean adolescents to develop healthy character.

A Comparative Study of Symptoms in Patients with Hwabyung according to Yin-Yang Personality Types (음양 성격 유형에 따른 화병 환자의 증상 비교 연구)

  • Objectives: To compare symptoms and characteristics among patients diagnosed with Hwabyung according to their yin-yang personality types. Methods: A structured clinical interview was conducted to diagnose Hwabyung, followed by a cross-sectional study involving a total of 118 Hwabyung patients (95 females). Various measures including the Sasang Personality Questionnaire (SPQ), Hwabyung Comprehensive Test (HCT), HwaByung Scale (HBS), State-Trait Anger Expression Inventory (STAXI), Beck Depression Inventory-II (BDI-II), Beck Anxiety Inventory (BAI), Eogul Scale (ES), and Patient Health Questionnaire-15 (PHQ-15) were administered. Results: SPQ-total showed significant negative correlations with HBS-personality and STAXI-control, and a significant positive correlation with STAXI-out. SPQ-behavior showed significant positive correlations with HBS-symptom, STAXI-trait, STAXI-in, STAXI-out, BDI-II, BAI, ES, and PHQ-15. SPQ-cognition exhibited significant negative correlations with HBS-personality, STAXI-control, and ES. SPQ-emotion had significant negative correlations with HBS-personality and STAXI-in and a significant positive correlation with STAXI-out. SPQ-Yin demonstrated a significantly higher HBS-personality than SPQ-Yang. SPQ-Yang exhibited significantly higher STAXI-out than SPQ-Yin, whereas SPQ-Yin showed significantly higher STAXI-control and STAXI-in than SPQ-Yang. Although differences in depression, anxiety, resentment, and physical symptoms between yin and yang were not statistically significant, variations in interpretation criteria were observed. Conclusions: Anger expression patterns and physical/psychological symptoms among Hwabyung patients are different based on their yin-yang personality types. These findings can inform the prediction of symptoms in Hwabyung patients according to their yin-yang personality types and aid in establishing appropriate treatment plans.

The Relationships between Sex-Role Identity and Yin-Yang Clothing Behavior of Mothors and Daughters (어머니와 딸의 성역할 정체감과 음양의복행동간의 관계)

  • The purposes of this study were to investigate the relationship between mothers and daughters in sex-role identity and yin-yang clothing behavior as well as the relationship between sex-role indentity and yin-yang clothing behavior Sex-role identy was measured by Bem Sex-Role Inventory (1974) modified and supplemented for this study. For the measurement of yin-yang clothing behavior, researcher developed a questionnaire. Data were collected from 422 women, both college women and her mothers. For the statistical analysis, frequency, distribution, t-test, one-way analysis of variance, Duncan's multiple range test, $x^2-test$, Pearson's correlation coefficients, analysis of covariance. multiple regression analysis were used. The results of this research can be summarized as follows: 1 There was significant difference in yin-yang clothing behavior according to home environmental variables such as family type, mother's employment. 2. There was not significant difference in mother's yin-yang clothing behavior according to mother's sex-role identity. But positive significant relationship was found between daughter's sex-role identity and daughter's yin-fang clothing behavior. 3. Mother's sex-role identity had influence on daughter's sex-role identity And mother's yin-yang clothing behavior had influence on daughter's yin-yang clothing behavior, too. 4. Mother's fin-yang clothing behavior and daughter's sex-role identity directly affected daughter's yin-yang clothing behavior, but mother's sex-role identity indirectly affected daughter's yin-yang clothing behavior.

The Stability of Yin-Yang Temperament in Middle School Students (중학생에 있어서 음양 기질의 안정성)

  • Objectives The Yin-Yang construct was shown to have stable internal reliability or consistency using Sasang Personality Questionnaire (SPQ) throughout the various ages, however the study of stability over specific period of time was only conducted using adults. The purpose of the study was to examine the stability across the time using middle school students whose physical and psychological characteristics were most rapidly changing. Methods The 122 middle school students (65 boys and 57 girls) completed the SPQ at 1st year and 2nd year with one-year time difference. Cronbach's ${\alpha}$ and Pearson's correlation analysis were employed in order to show the internal consistency and stability over one-year interval after considering sex difference. Results The internal consistency of SPQ-T, SPQ-B, SPQ-C, SPQ-E using Cronbach's ${\alpha}$ were .61, .72, .28, .28 in 2016 and .62, .72, .26, .46 in 2017. The one-year test-retest reliability of SPQ-T, SPQ-B, SPQ-C, SPQ-E using Pearson's correlation analysis were .56, .61, .31, .23 in boys and .58, .63, .64, .29 in girls. Conclusion The current study showed the stability of Yin-Yang temperament across the one-year time suggesting that the Yin-Yang temperament is quite stable and measurable concept and the SPQ would be considered as an objective and reliable personality assessment.

A Review of Studies Using Syndrome Differentiation Questionnaire in Cancer Patients (암 환자 대상 변증 설문지 활용 현황에 대한 문헌고찰)

  • Objective: The aim of this review is to analyze the studies using syndrome differentiation questionnaire in cancer patients. Methods: We searched electronic databases including Pubmed, google scholar, Cochrane library, CNKI, KISS, RISS and OASIS. Key words used for searching were cancer, Korean medicine, pattern identification, and questionnaire. Studies using a symptom differentiation questionnaire to cancer patients were selected. Results: 35 studies were enrolled. A total of 17 questionnaires was used. Most of the types of included studies were observational studies, followed by randomized controlled trials (RCTs) and validation studies. The purposes of using questionnaires were rrelation analysis, outcome measurement, evaluating adverse events, subgroup analysis, and questionnaire development. The most used questionnaire was Body Constitution Questionnaire (BCQ), and it was used 8 times, Questionnaire for the Sasang Constitution Classification II (QSCC II) was used 5 times, Constitution in Chinese Medicine Questionnaire (CCMQ), TCM-Symptom Complex Differentiation Questionnaire (TCM-SCDQ), Yin Deficiency Questionnaire were used 4 times, and Qi Blood Yin Yang Deficiency Questionnaire was used twice. BCQ is a questionnaire diagnosing and evaluating yang deficiency, yin deficiency, and blood stasis. It has high reliability, validity, and optimal cut-off value. Conclusion: BCQ is the most used syndrome differentiation questionnaire in cancer-related studies. So, BCQ could be recommended in syndrome differentiation-related cancer studies.

Development of a Guideline for the Application of a Diagnostic Tool for Menopausal Syndromes Based on the Use of Systemic Review and Delphi Method (체계적 문헌고찰과 델파이 기법을 활용한 갱년기장애 변증(辨證)진단 도구 개발을 위한 기초 연구)

  • Objectives: This study was conducted towards developing a screening tool for syndrome differentiation in the diagnosis of menopause in menopausal and perimenopausal women. Methods: We conducted a literature review of studies on menopausal diagnosis based on syndrome differentiation, and examined well-founded differentiated syndromes and their respective clinical symptoms. Based on the findings, we created a questionnaire through consultations with Oriental medicine experts in physiology, pathology, and diagnostics. Finally, the research team conducted an expert Delphi study on differentiated syndromes and the associated clinical symptoms. Results: Seven differentiated syndromes were selected, including Liver Depression (肝鬱), Kidney Yin Deficiency (腎陰虛), Kidney Yang Deficiency (腎陽虛), Liver and Kidney Yin Deficiency (肝腎陰虛), Kidney Yin and Yang Deficiency (腎陰陽兩虛), Heart-Kidney Noninteraction (心腎不交), and Dual Deficiency of Heart and Spleen (心脾兩虛); 4 disease locations, including liver (肝), heart (心), spleen (脾), and kidney (腎); and 3 disease natures, including Yin Deficiency (陰虛), Qi Stagnation (氣滯), and Blood Deficiency (血虛). In addition, we added 3 supplemental disease natures, including Yang Deficiency (陽虛), Qi Deficiency (氣虛), and Heat (火熱), in consideration of syndrome differentiation categories that may possibly be added in a follow-up clinical questionnaire. Conclusions: This resulted in a total of 7 differentiated syndromes, 4 disease locations, and 6 disease natures. We translated the clinical symptoms of these 17 categories into Korean Hangeul. After consulting with 5 Oriental medicine experts and a psychology expert, we produced a questionnaire for use in diagnosing menopause based on syndrome differentiation. The calculation of scores for the syndrome differentiation screening tool will be confirmed through clinical research based on the results of a review of existing literature.

Definition, Role and Method of Yinyang Pattern Differentiation (음양변증(陰陽辨證)의 정의와 기능 및 판별방법 연구)

  • In order to ensure the fact that eight principle pattern differentiation is used clinically as a basic guideline for Korean medicine practice, the definition, role and method of yin-yang pattern differentiation with its case report were explored at first. Yinyang Pattern Differentiation is a method of discriminating human tendencies or morbidity based on the yin and yang characteristics expressed in living bodies. And yin and yang are the two contrasting characteristics and aspects of the interaction when certain physical conditions that have a lasting effect on the human physiological metabolic function are correlated with the morbidity. Specific methods of yinyang pattern differentiation can be divided into several types of yin and yang indicators. First, time and space factors like day and night, hot and cold seasons, above and below, topographical districts. Second, colors and pulse and their/or relative clearness and muddiness, hardness and softness, moving and resting. Third, diagnose yin and yang patterns through distinguishing the true and false of a fever and cold in an emergency phase such as increase of brain pressure and shock state. Fourth, general characteristics of the propensity and constitution of a subject such as body type, speech, behavior, and physiological metabolism. And for clinical use, these were summarized again as a symptom indicators of physical signs and color, pulse, tongue and questionnaire indicators of propensity, body type, and space-time characteristics. Conclusively, it was confirmed that yinyang pattern differentiation has its own diagnostic significance which is distinct from exterior-interior, cold-heat and deficiency-excess pattern differentiation.

Clinical Observation on Heart Rate Variability and Peripheral Blood Flow by Doppler Ultrasound in Healthy Subjects Stimulated by Cold Stress (정상인의 한냉자극에 대한 심박변이도 및 도플러 초음파 혈류계를 이용한 말초 혈류관찰)

  • Objectives: This study was performed to investigate the effect of cold stress on heart rate variability and peripheral blood flow by Doppler ultrasound in healthy subjects. Methods: We classified the subjects by their Yin or Yang tendency using a Yin-Yang questionnaire. Cold stress was taken on the left hand of subjects for 10 seconds. Heart rate variability and peripheral blood flow were measured on the right radial artery before and after cold stress. Results: The Yang tendency group showed a significant decrease of heart rate after cold stress which was not seen in the Yin tendency group. The Yang tendency group showed a decrease of LF, total power, and increase of HF after cold stress, while the Yin tendency group showed the opposite. However, there were no statistically significant differences of heart rate variability analysis between the results of the two groups before and after cold stress. Recovery time of blood flow velocity by ultrasound Doppler was significantly different between the Yin and Yang tendency groups after cold stress. Conclusion: These results suggest that people with Yin or Yang tendency may show different responses in the autonomic nervous system. Further modified studies may include the responses to various manipulation techniques in acupuncture treatment and the individual responses according to pattern identification in traditional Korean medicine.
