Wrap-around Free Flap in Acute Thumb Injury (족무지 유리피부편을 이용한 급성 수무지 손상의 치료)

  • Firstly the wrap-around free flap from the great toe was transferred successfully by Morrison and O'Brien in 1980 for reconstruction in chronic case of amputated thumb distal to the MP joint. Since then, significant bone peg resorption and grafted bone fracture have been reported in most of cases after operation. We have performed thumb reconstructions within 2 weeks after injury in 11 cases which included acute crushing injuries or failed replantation of thumb at Guro Hospital from September, 1983 to October, 1993. The results obtained from this study are as follows: 1. There were 8 males and 3 females and the mean age was 28.8 years old ranged from 3 years old to 50 years old. 2. The most common injury mechanism was machinary injury in 10 of 11 cases. 3. We have performed thumb reconstructions by using wrap-around free flap from the great toe without bone graft in 6 cases and with bone graft in 5 cases within 2 weeks after injury. 4. During 6 years and 1 month mean follow-up period, there were bone resorptions in 3, grafted bone fracture in 1, infections in 2 and soft tissue necrosis in 3 cases. 5. We obtained satisfactable results in respect of function and cosmesis and urged to use this flap for reconstruction in acute thumb injury.

A Study on the Efficiency of CDMA Cellular System Simulation with Wrap Around Technique (Wrap Around 기법에 의한 CDMA 셀룰라 시스템 시뮬레이션 효율에 관한 연구)

  • Most of CDMA system algorithm must be verified by simulating the cellular system consisted of many other tier cells. But it takes long time to simulate such a huge CDMA cellular system. In this paper, we propose an effective time saving simulation scheme based on wraparound technique. And we study that how our remaining cell interference compensation method works on calculating of the total interference, SIR, outage probability, and cell capacity. We made a conclusion that by using proposed scheme, we can get the same result of maximum configuration cells with only one tier real cells and that we can minimize the simulation time.

  • Spinal extradural arachnoid cyst (SEAC) is a rare disease and has surgical challenges because of the critical surrounding anatomy. We describe the rare case of a 58-year-old woman who underwent extradural cyst total excision with dural repair and presented with refractory cerebrospinal fluid (CSF) leakage even though two consecutive surgeries including dural defect re-repair and lumbar-peritoneal shunt were performed. The authors covered the sacral defect using bilateral gluteus maximus muscle flap in tongue in groove and wrap around pattern for protection of visible sacral nerve roots and blockage of CSF leakage point. With the flap coverage, the disappearance of cyst and fluid collection was confirmed in the postoperative radiological finding, and the clinical symptoms were significantly improved. By protecting the sacral nerve roots and covering the base of sacral defect, we can minimize the risk of complication and resolve the refractory fluid collection. Our results suggest that the gluteus muscle flap can be a safe and effective option for sacral defect and CSF leakage in extradural cyst or other conditions.

Cost Analysis of Wrap Silage Production in the Paddy Field for Forage Crop Cropping System (답리작 사료작물의 랩사일리지 생산비 분석)

  • Mechanized operation model and mechanical cultivation technology for winter barley, rye, Italian ryegrass and sudan grass wrap silage production system at the paddy field for cropping system was developed. Also, a series of experiment were performed and lots of data were collected and analyzed to develope mechanical technology, coverage area, and optimum size of the farm (break-even point) for wrap silage production system. The coverage area for winter barley or rye wrap-silage production system is determined around 61.9, 73.4, 77.5, 88.2 ha in the case of drill seeding and different ripening species by tractor power 50, 75, 100, 130 ps, respectably. The break-even point of the farming size is analyzed as 20 ha and its production cost is estimated around 367, 383, 430, 443 won/TDN-kg in the case of winter barley wrap-silage by tractor power 50, 75, 100, 130 ps, respectably. The break-even point of the farming size is analyzed as coverage area and its production cost is estimated around 237, 215 won/TDN-kg in the case of winter barley wrap silage and sudan grass by the tractor power 50, 100 ps, respectably.

  • In order to solve the shortage of roughage supply for dairy farm in Korea, the mechanized model system for the production of winter cereal wrap silage in the fellow paddy field was developed in the previous studies. In this study, storage properties after long term storing of the winter cereal wrap silage were investigated. Also, the qualities of the wrap silage which was stored during 4 months were evaluated by sensory testings, analyzing chemical properties(moisture content, pH and organic acids) and investigating the increase of feeding value. The result of this study were summarized as follows; 1. After 300 hours storage, the acid level of the silage decreased slowly to 4.6 -4.8 pH. Also, the temperature was stabilized around 27-28$^{\circ}C$. The silage having below 5.0 pH is a desirable level for the most of good silage. 2. According to the sensory test and chemical property analyzation it was evaluated as the first class silage like a com silage. 3. Also, when the wrap silage are fed to the cattle, the cattle produced milk more 25% than that of conventional feed. 4. As an overall conclusion, winter cereal wrap silage in the fallow paddy field cm be one of the major roughage source in replacement of com and other conventional silage to dairy cattle in Korea.

  • This paper presents a method to convert 3D CAD model to an appropriate analysis model using wrap-around, smooth-out and thinning operators that have been originally developed to realize the multi-resolution modeling. Wrap-around and smooth-out operators are used to simplify 3D model, and thinning operator is to reduce the dimension of a target object with simultaneously decomposing the simplified 3D model to 1D or 2D shapes. By using the simplification and dimension-reduction operations in an appropriate way, the user can generate an analysis model that matches specific applications. The advantage of this method is that the user can create optimized analysis models of various simplification levels by selecting appropriate number of detailed features and removing them.

Achilles tendon reconstruction with a half-width Achilles graft and wrap-around fascial flap

  • A 70-year-old man complained imbalance while walking, inability to perform ankle flexion, and could not stand on tip-toe 3 months after injury. The ankle looked swollen with loss of Achilles contour and obvious gait disturbance. Magnetic resonance imaging shows a 5-cm Achilles tendon gap. Subsequently, surgery was performed to solve the neglected Achilles tendon rupture. Patient was put under general anesthesia with a regional block. Using a nontourniquet technique, a reconstructive procedure was performed using a half-width autologous Achilles tendon graft, which was attached to the calcaneal prominence with wire in a double strand Bunnell fashion. As for the proximal stump, double core Bunnell/modified Kessler suturing was carried out to suture the graft to Achilles stump. To increase the vascularization, an ipsilateral gastrocnemius fascial flap with a distally based-pedicle was harvested to wrap around the tendon graft. At a 6-month follow-up, the patient was able to stand on tip-toe and had also regained a normal gait.

Functional Evaluation of Thumb Reconstruction according to the Level of Amputation (절단부위에 따른 수무지 재건의 기능적 평가)

  • In 1980 Morrison and O'Brien reported their experiences about the reconstruction of amputated thumb using wrap-around neurovascular free flap from the great toe with a nonvascularized iliac bone graft. From then it has been considered to be a good reconstructive procedure for the amputated thumb, but it's indication has been limited distal to the metacarpophalangeal(MP) joint. We have performed 37 cases of wrap-around free flap from the great toe for the reconstruction of thumb amputated at distal or proximal to the MP joint and investigated their functional results according to the level of amputation. Level of amputation was distal to the MP joint in 25 cases and proximal to it in 12 cases. Pinching and grasping power, two point discrimination and the amount of opposition to the other fingers were compared to the uninjured hand. Pinching and grasping power were not significantly different according to the level of amputation but the amount of two point discrimination was significantly high in the cases amputated proximal to the MP joint. The opposition of reconstructed thumb to the other fingers was completely possible in all cases amputated distal to the MP joint. In 12 cases amputated proximal to the MP joint of the thumb, opposition was completely possible in 6 cases in which the iliac bone block was fixated in the position of $30^{\circ}$ flexion and $45^{\circ}$ internal rotation but in 6 cases in the fixation of $30^{\circ}$ flexion and $30^{\circ}$ internal rotation, the opposition of reconstructed thumb to the ring and little fingers were impossible in 5 cases and only to the little finger in 1 case. In this study, we concluded that even if amputation proximal to the MP joint, it is no more contraindication of the wrap-around free flap procedure for thumb reconstruction, however in these cases we recommend iliac bone block fixation in the position of $30^{\circ}$ flexion and $45^{\circ}$ infernal rotation for the better functional outcome.

  • In this study, a LIDAR laser scanner was used to scan a rock slope around Mt. Gwanak and to produce point cloud from which directional information of rock joint surfaces shall be extracted. It was analyzed using two different algorithms, i.e. Ball Pivoting and Wrap algorithm, and four sampling intervals, i.e. raw, 2, 5, and 10 cm. The results of Fuzzy K-mean clustering were analyzed on the stereonet. As a result, the Ball Pivoting and Wrap algorithms were considered suitable for extraction of rock surface orientation. In the case of 5 cm sampling interval, both triangulation algorithms extracted the most number of the patch and patched area.
