Comparative Evaluation of Erythrocyte Sedimentation Rate by the Test 1 Analyzer and Westergren method in Patients with Anemia (빈혈환자에서 자동 ESR 기기인 Test 1과 Westergren법의 비교)

  • Erythrocyte sedimentation rate(ESR) measurements were performed in a total of 191 blood samples from patients below a hematocrit of 35% by both the Test-1(SIRE Analytical Systems, Udine, Italy) and the Westergren method, endorsed as the reference method for ESR by the International Council for Standardization in Hematology(ICSH). The corrected Westergren ESR values were also obtained applying the formula of Fabry (corrected ESR = ESR measured x 15/55-Hct). Linear regression analysis showed a close correlation (r=0.85) between the two methods both before and after ESR correction in total samples. There was no significant correlation difference between two methods in both two groups with hemoglobin $${\geq_-}9g/dl$$ and <9g/dl before and after ESR correction (r=0.873, r=0.827 respectively before correction and r=0.867, r=0.830 respectively after correction). Also, for two groups with hematocrit $${\geq_-}24%$$ and <24%, no significant difference was observed (r=0.859, r=0.792 respectively before correction and r=0.782, r=0.842 respectively after correction). However, for samples with <60mm of Westergren ESR before correction, a better correlation coefficient was obtained than samples with $${\geq_-}60mm$$(r=0.701, r=0.541 respectively). In corrected Westergren ESR, also samples that were obtained with <40mm showed better correlation than samples with $${\geq_-}40mm$$ (r=0.690, r=0.347 respectively). In conclusion, we found a good correlation between Test-1 and Westergren measurements in patients with anemia but an expert group discussion is required to solve discrepancy between two methods in blood samples with very high ESR.

The effect of Westergren tube angle, temperature and osmobility with the different percent of NaCl solution on erythrocyte sedimentation rate (적혈구 침강속도에 대한 Westergren tube 각도, 온도 및 NaCl 농도가 미치는 영향)

  • Erythrocyte sedimentation rate is influenced by plasma protein, red cell itself and physiological conditions and it is clear that the measurement of erythrocyte sedimentation rate varies with technique and various environmental factors. The effect of temperature, angle of test tube, and osmobility with different percent of NaCl solution on erythrocyte sedimentation rate on sheep was determined by modified Westergren method. In sheep, as the angle of Westergren tube was decreased from $90^{\circ}$ to $45^{\circ}$, erythrocyte sedimentation rate was increased in both diluted plasma and NaCl solutions. As temperature was increased from $4^{\circ}C$ to $20^{\circ}C$ at $45^{\circ}$ angle. erythrocyte sedimentation rate was increased. In ruminants, this modified Westergren methods, erythrocyte 1 : plasma 9 instead of erythrocyte 4 : plasma 6(whole blood), enable them to have meanings as dog therefor, using this method, clinical can determine the erythrocyte sedimentation rate of ruminants for diagnosis. Because erythrocyte sedimentation rate was changed according to the angle of Westergren tube, temperature as erythrocyte diluted with NaCl, this study detected that the change of temperature, the angle of Westergren tube could change erythrocyte sedimentation rate by effecting red cell itself. The increase of osmobility owing to change of NaCl percent resulted in the decrease of rapid erythrocyte sedimentation rate. So this fact indicate that deformibility and the change of red cell volume have meaning in the change of erythrocyte sedimentation rate.

Evaluation of Automated ESR Measuring System, $SEDIsystem^{TM}$ ($SEDIsystem^{TM}$을 이용한 적혈구 침강속도 측정의 평가)

  • The ESR is one of the oldest laboratory test still in use. Although it lacks specificity in diagnosis, it can be effective for monitoring disease activity and following-up. The Westergren method is used for reference method, however coefficient of variation has been described 0.8% to 22.9% according to the literature. Since the ESR was invented in 1921, measurement technique has developed and automated measurement is introduced. We analyzed one hundred forty-three patient samples using $SEDIsystem^{TM}$ automated ESR measuring system and compared with modified Westergren and Wintrobe methods. Comparison between $SEDIsystem^{TM}$ and modified Westergren for ESR measurement yields the following regression equation; y = 0.863x - 1.69 (r=0.830), $SEDIsystem^{TM}$ and Wintrobe y = 1.14x - 14.7 (r=0.789), respectively. We repeated measurement to evaluate reliability, results are not significant in statistically. In conclusion, $SEDIsystem^{TM}$ automated ESR measurement correlated with modified Westergren and Wintrobe methods, reveal reliable results after 4 hours and can report rapidly for large samples. Thus, these results indicate that $SEDIsystem^{TM}$ automated ESR measurement may be useful tool for clinical practice.

  • In order to determine erythrocyte sedimentation rate (ESR) indicating acute phase inflammation, a Westergren method has been widely used because it is cheap and easy to be implemented. However, the Westergren method requires quite a long time for 1 hour. In this study, a gated recurrent unit (GRU) neural network was used to reduce measurement time of ESR evaluation. The sedimentation sequences of the erythrocytes were acquired by the camera and data processed through image processing were used as an input data into the neural network models. The performance of a proposed models was evaluated based on mean absolute error. The results show that GRU model provides best accurate prediction than others within 30 minutes.

Evaluation of erythrocyte sedimentation rate in racehorse measured by westergren method (경주마(競走馬)에서 Westergren법(法)에 의한 적혈구 침강속도(沈降速度) 측정의 평가(評價))

  • The study was basically carried out to establish the appropriate condition for applying the Westergren method for erythrocyte sedimentation rate(W-ESR) in Throughbred racehorses at recess. To do this, we examined the correlationship among some factors including the kind of anticoagulants, optimal ambient temperature and reading time for W-ESR in healthy racehorses. The difference between the blood samples treated with only 3.8% solution of sodium citrate(SC) and both EDTA and SC as a coagulant, there was not recognized any significant difference(p<0.05) in the levels of W-ESR irrespective of the ambient temperature of $20^{\circ}C$ or $30^{\circ}C$. The best optimal ambient temperature for W-ESR in horses was proved at $30^{\circ}C$ resulting from the tendency of the most reduced dispersion in mean values analyaed from repeatly 4 times to the same blood samles compared with those of $10^{\circ}C$ and $20^{\circ}C$. The optimal reading time was determined as 60 minutes(Y=237.2~4.1X, $r^2=0.998$) and 70 minutes(Y=247.8~4.2X, $r^2=0.999$) under the same temperature of $30^{\circ}C$; the latter showed the better result on the basis of the correlation of packed cell volume(PCV) and ESR values. About 13 healthy racehorses, we compared the real values of W-ESR respectively obtained at 60 minutes and 70 minutes at $30^{\circ}C$ with the anticipated values of PCV by means of the analysis of linear regression equitation. As the result of this, the strong correlation between both of them was confirmed. For practical use of W-ESR in Thoroughbred racehorese, we can recommend the condition of 60 or 70 minutes for the optimal reading time as well as $30^{\circ}C$ for the best ambient temperature.

Red Cell Sedimentation Rates of Reshuffled Packed Cell Volume in Chicken (PCV수치를 변경시킨 닭적혈구 침강속도)

  • The packed cell volume(PCV) of chicken, volume percentage of erythrocytes in whole blood, was reshuffled of 20%, 40% and 60% using autoplasma, and the red blood cell sedimentation rate was measured in Westergren tubes at room temperature($27{\pm}1^{\circ}C$) and low temperature ($8{\pm}1^{\circ}C$). The results obtained were summarized as follows. 1. The sedimentation rates of chicken red blood cell were settled faster at low PCV than higher PCV, i,e., there was a reverse relationship between the sedimentation rate and PCV. 2. The sedimentation rate of chicken red blood cell was accelerated more at high temperature than low temperature. 3. The sedimentation rate of reshuffled chicken red blood cell by time was almost linear for several hours.

  • The packed cell volume(PCV) of Korean native goat, volume percentage of red blood cell in whole blood, was reshuffled of 20%, 40% and 60% using autoplasma, and erythrocyte sedimentation rate was measured in Westergren tubes at room temperature ($27{\pm}1^{\circ}C$) and low temperature ($8{\pm}1^{\circ}C$). The sedimentation rates of red blood cell obtained are summarized as follows. The erythrocyte sedimentation rates of Korean native goat are accelerated more at high temperature than low temperature. The erythrocyte sedimentation rates of reshuffled Korean native goat upon time are almost linear for several hours. The erythrocyte sedimentation rates of Korean native goat are settled faster at low PCV than higher PCV, i. e., there is a reverse relationshif between the erythrocyte sedimentation rate and packed cell volume.

Studies on the red blood cell sedimention rates in heteroplasma of chicken and goat (닭 및 산양의 이종plasma내에서 적혈구 침강에 대한 연구)

  • In order to study the marked variation of red blood cell sedimentation rate in some species of animals, the packed cell volume, volume percentage of erythrocytes in whole blood, was reshuffled of 20%, 40% and 60% using heteroplasma of chicken and goat, and the red blood cell sedimentation rate was measured in Westergren tubes at $27{\pm}1^{\circ}C$ and $8{\pm}1^{\circ}C$. The results obtained were summarized as follows: 1. The values of packed cell volume(PCV) of goat and chicken were $40.7{\pm}4.1%$ and $30.2{\pm}2.2%$ respectively. 2. The sedimentation rates of reshuffled red blood cell were settled faster at lower PCV than higher PCV, i.${\acute{e}}$. there was a reverse relationship between the sedimention rate and PCV. 3. Red blood cells of chicken settled quickly, where as those of goat settled very slowly. Chicken red blood cell sedimented rapidly even in goat plasma, and goat red blood cell sedimented slowly in chicken plasma. These findings indicate that the plasma is not the only factor determining the rapid red blood cell sedimentation rate of chicken. 4. The sedimentation rate of reshuffled red blood cell of chicken and goat were accelerated at higher temperature than lower temperature.

Angled Tube Method for Determining Erythrocyte Sedimentation Rate of Cattle (경사관법(傾斜管法)에 의한 우혈액(牛血液)의 적혈구침강율(赤血球沈降率) 측정(測定))

  • The measurement of angled erythrocyte sedimentation rate (ESR), as a replacement for perpendicular ESR, for cattle blood was scrutinized since it has been well known that perpendicular ESR in cattle is too slow to be adopted as an effective clinical test. Samples of blood were taken from 186 Korean native cattle over 2 years old. The results obtained in the experiment were summarized as follows. 1. Average values of perpendicular ESR/24hrs in 15 apparently healthy cattle, as measured by Wintrobe, Westergren and capillary tubes, were $5.8{\pm}2.2$, $11.1{\pm}3.7$ and $10.4{\pm}4.5%$ respectively, which were found to be similar to the values of perpendicular ESR/hr of normal blood of human. 2. The ESR was determined in the tubes held at 90, 75, 60, 45, 30 and 15-degree angles, using 3 types of tubes. For the diagnostic purposes, the best results were obtained from the tubes held at 45-degree angle. 3. The angled ESR values increased as the diameters of the tube-bores decreased. 4. The tube length did not affect the angled ESR(%). 5. The angled ESR values increased with the increased environmental temperature during the ESR measurement. 6. The storage temperature at $5^{\circ}C$, $20^{\circ}C$ and $35^{\circ}C$, of the blood for 24 hours did not affect the angled ESR. 7. Samples of blood were treated with 4 kinds of anticoagulants (heparin, $K_2$-EDTA, double oxalate and sodium citrate) and the ESR was determined at 45-degree angle, using capillary hematocrit tubes. The ESR values were higher in the blood samples treated with sodium citrate than in those treated with other anticoagulants. 8. By using the autologous plasma, the PCV was adjusted to be 5, 10, 20, 30, 40 and 50ml/100ml and the ESR was determined in the capillary hematocrit and Wintrobe tubes held at 45 degrees. In both of the methods the ESRs increased as the values of PCV decreased. The regressions of ESR to PCV in both 45-degree-angled capillary and Wintrobe tubes were curvilinear. For the capillary hematocrit tubes the second degree polynomial $Y=61.9779-2.3533x+0.0228x^2$ (r=0.9999) fits the data. And in the case of Wintrobe tubes the second degree polynomial $Y=27.9767-1.1314x-0.0117x^2$ (r=0.9998) fits the data. 9. The 45-degree angled ESR was determined in the blood of 71 healthy Korean native cows using capillary hematocrit tubes. The average PCV was $35.4{\pm}3.6ml/100ml$. The observed ESR/hr averaged $7.2{\pm}2.7%$, while the corrected ESR/hr to a PCV of 36ml/100ml averaged $6.6{\pm}1.3%$. From these results it was concluded that to obtain the best results the ESR/hr of Korean native cattle should be determined at 45-degree angle at room temperature($20^{\circ}C$) using capillary hematocrit tubes.
