Correlation Between the Activities of Daily Living Assessment and Gross Motor Function Measures in Children with Spastic Cerebral Palsy

  • Purpose: This study was to identify the whether there is any correlation between Gross Motor Function Measures (GMFM) and Functional independence Measurement for children (WeeFIM) in order to identify a relevance of daily life motor with gross motor function of children with cerebral palsy. Methods: Subjects were instructed to evaluated by an occupational therapist using WeeFIM (reliability 0.83-0.99) and by a physical therapist using GMFM (reliability 0.93). 10 male children and 13 female children among 23 children being diagnosed as cerebral palsy were targeted for this study. A correlation between GMFM and WeeFIM was analyzed with Kendall correlation. Results: Participants showed significant correlation between GMFM and WeeFIM in all categories, except in the GMFM's walking running jumping category, standing category of GMFM and self care, sphincter control, communication category of WeeFIM. Lying rolling and sitting and crawling kneeling area of GMFM showed a significant correlation with self care(r=0.35, 0.39, 0.4), sphincter control (r=0.45, 0.43, 0.61), mobility transfer (r=0.44, 0.36, 0.64), locomotion (r=0.41, 0.35, 0.74), communication (r=0.4, 0.44, 0.51), and social cognition (r=0.43, 0.51, 0.64) area of WeeFIM (p<0.05). Standing of GMFM and mobility transfer (r=0.74), locomotion (r=0.47) and social cognition (r=0.4) area of WeeFIM showed a significant correlation (p<0.05). Conclusion: These findings indicated significant correlation between activity of daily living (ADL) and gross motor function of children with cerebral palsy. However, because there are a few results that are not significant, both assessment tools should be used for evaluations so that treatment can be achieved with an accurate assessment and establish a therapeutic plan.

Psychometric Properties of the WeeFIM in Korean Children With Cerebral Palsy

  • The WeeFIM is an outcome measure used worldwide to evaluate the functional abilities of children. The aim of this study was to evaluate the psychometric properties of WeeFIM in Korean children with cerebral palsy (CP) using the Rasch model. The mean age of the participants (92 boys and 53 girls) was 10.6 years (SD=2.3, range 5~15 years). The Winsteps software was used for analyzing the internal construct validity and reliability of WeeFIM. For analyzing the internal validity the motor and cognitive area items of the WeeFIM were analyzed both together and separately. When all 18 items were analyzed 4 were considered to be misfits; upper extremity dressing, lower extremity dressing, toileting, and comprehension. When only the 13 motor items were analyzed, toileting, bladder management, and bowel management were considered misfits. In addition, only comprehension was considered as a misfit among the 5 cognitive items. The most difficult motor items were stair climbing, and bathing. The simple ones were eating, bowel management, and bladder management. The most difficult cognitive item was problem solving, and the simplest one was comprehension. The person separation indexes and reliability for combined and divided instruments were reported as excellent. These results demonstrated the applicability of WeeFIM to Korean CP children with satisfactory reliability and validity. Further studies should include young children with CP and compare item difficulty among the different types of CP. In addition, the Korean normative data of nondisabled children should be used to compare the cultural differences between Korea and other countries.

Correlation between Manual Ability Oassification System and Functional Evaluation in Children With Spastic Cerebral Palsy (경직형 뇌성마비 아동의 손 기능 분류 체계와 기능적 수행도 평가 간의 상관)

  • The purpose of this study was to investigate the relationship among functional evaluation systems, the Manual Ability Classification System (MACS), the Gross Motor Function Classification System (GMFCS), and the functional status (WeeFIM) in children with spastic cerebral palsy and to provide the foundation data about MACS for evaluation system of hand function in children with spastic cerebral palsy. For this, sixty children with spastic cerebral palsy were employed in this study. The sixty children were evaluated by using the MACS for their hand function and by using the GMFCS for their motor function. The functional status were assessed by using the Functional Independence Measure of Children (WeeFIM). There were a significant correlation between the MACS and the GMFCS (r =.659, p <.05). The good correlation between the MACS and WeeFIM was found (r = -.576, p <.05). The functional status according to the hand function level evaluated by using the MACS were different significantly (p <.05). The MACS in practice will provide usefulness for assessment of hand function in children with spastic cerebral palsy.

Reliability and Validity of the Korean Translation of the Pediatric Evaluation of Disability Inventory in School-Aged Children With Cerebral Palsy

  • The purposes of this study were to examine the reliability and validity of Korean translation of Pediatric Evaluation of Disability Inventory (PEDI-K) in school-aged children with cerebral palsy (CP). The PEDI-K, Functional Independence Measure for Children (WeeFIM) and Gross Motor Function Classification System (GMIFCS) were completed in 104 school-aged children with CP by therapists. The internal consistency of the PEDI-K was calculated by Cronbach's alpha (${\alpha}$) for assessing reliability. Concurrent validity was evaluated by correlation with the subsets of WeeFIM. Discriminant validity was assessed by comparing GMFCS levels with tests of the PEDI-K. The results showed that internal consistency was good (Cronbach's ${\alpha}$ ranged from .97~.98). Concurrent validity was demonstrated. The correlation with WeeFIM was high in the Functional Skills (self-care, r=.74~.94; mobility, r=.59~.91; social function, r=.65~.93) and in the Caregiver Assistance (self-care, r=.75~.94; mobility, r=.63~.90; social function, r=.78~.96). Discriminant validity was demonstrated on significant decreases in domain scores with increasing GMFCS levels. Reliability and validity have been demonstrated on the PEDI-K. This study extends usage of PEDI-K in clinical activities and research.

Evaluation of Balance and Activities of Daily Living in Children with Spastic Cerebral Palsy using Virtual Reality Program with Electronic Games (전자게임을 이용한 가상현실프로그램이 경직성 뇌성마비 아동의 균형과 일상생활활동에 미치는 영향)

  • The aim of this study were to examine the effects of virtual reality program on balance and activities of daily living in children with spastic cerebral palsy (CP) using the Pediatric Balance Scale (PBS) and the Functional Independence Measure for Children (WeeFIM), and to measure relationship between the PBS and the WeeFIM. For this, A total of 20 spastic CP classified as the Gross Motor Function Classification System (GMFCS) I and II were employed. The Participant's were allocated randomly to 2 groups: a virtual reality group (n=10) and the control group (n=10). Both groups received muscle strengthening exercise for 3 sessions, 30 minutes per week over a 12 week period. The virtual reality group practiced additional virtual reality program. The virtual reality group showed significant increases in balance (p<0.05) and activities of daily living (p<0.05). There were a significant correlation between the PBS and the WeeFIM (p<0.05). Application of the virtual reality program to treat the spastic CP will be feasible and suitable. And the PBS was a useful tool to predict activities of daily living in the spastic CP.

The Effect of Home-based Program For Children With Sensory Modulation Disorder (감각조절장애 아동을 위한 가정 프로그램의 효과)

  • Objective : To explore the effectiveness of sensory integration program of home-based outcomes in child with sensory modulation disorder. Methods : This study used the simple case study. The subjects were three children diagnosed as developmentally delayed who 14 month and 26 month males and a 15 month female. After initial evaluation, parents were educated on sensory diet and Wilbarger protocol method for 30 minutes twice for home-based treatment and asked to make out daily treatment planning. To measure improvement of children, we used for the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999). Results : After the home-based program, the scored of the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999) were higher rather than initial evaluation after the home-based program. Conclusion : The home-based program is effective for children with sensory modulation disorder but parents had to be educated by therapist specific evaluation and treatment in sensory integration.

The Effect of Computer-Based Cognitive Rehabilitation Program on Visual Perception and ADL in Children with Intellectual Disaility (전산화 인지프로그램(코트라스)이 지적장애 아동의 시지각과 일상생활에 미치는 효과)

  • The purpose of the study was to verify the clinical effect of Computer-based cognitive rehabilitation program for Children for recovering the visual perception and ADL ability in intellectual disability. For this purpose, 3 intellectual disabilities with visual perception and ADL disorders participated in a clinical experiment to verify the effect of Computer-based cognitive rehabilitation program. They all have 5 common conditions in intellectual disability for objective experimental result. All participant were evaluated with Korean-Development Test of Visual Perception-2(K-DTVP-2), Wee Functional Independence Measure(Wee-FIM) before and after the planned intervention sessions. All intervensions were intensively carried out for 20weeks, 1 time a week. The result were as follows. K-DTVP and Wee-FIM scores were improved significantly compared to pre-intervention. Further studies are needed to verify the effect on various group and lots of participants.

Effect of Self Care Training(based on International Classification of Functioning, Disability and Health) on Functional Independence in the Young Children with Spastic Cerebral Palsy (국제 기능 장애 건강분류의 구성요소에 기반을 둔 자기관리 훈련이 경직성 뇌성마비 아동의 기능적 독립성에 미치는 영향)

  • The purpose of this study was to investigate the effect of self-care training based on ICF(International Classification of functioning, Disability and Health) on functional independence in the young children with spastic cerebral palsy. Total of 43 young children(male=25, female=18; age range from 36month to 72month) with spastic cerebral palsy, classified at GMFCS(Gross Motor Function Classification System) levels III-IV. Total of 32sessions of a self-care training (eating, grooming, bathing, toileting) were given 4 times a week for 30minutes from August 1th to September 30th of 2008. Changes in the functional independence after the training obtained by Wee-FIM(Functional Independence Measure for Children). Results were as follows: Functional independence was significantly increased after the training. As a result, a self-care training should be applied as an effective intervention to improve the functional independence in the young children with spastic cerebral palsy.

Utility of Function Classification System in Children with Cerebral Palsy (뇌성마비 아동의 기능적 수준 분류 체계의 유용성)

  • The purpose of this study was to investigate the utility of function classification system in children with cerebral palsy (CP). For this, relationship among the Manual Ability Classification System (MACS), the Gross Motor Function Classification System (GMFCS), and the functional status (WeeFIM) in children with cerebral palsy form September 2008 to August 2010. The participants was 217 children with CP in this study. The 217 children were evaluated by using the MACS for their hand function and by using the GMFCS for their motor function. The functional status were assessed by using the Functional Independence Measure of Children (WeeFIM). The GMFCS have a significant correlation with total score and domains of WeeFIM (p<.05) There were a significant correlation with total score and domains of WeeFIM (p<.05) except no significancy with communication domain in dyskinesia type. The highest number of participants were in level 1 (20.3) and level 5 (40.6%) for GMFCS. For MACS, the highest number of participants were level 2 (48.8%) and level 5 (16.6%). The function classification of GMFCS and MACS in practice will provide usefulness for assessment of function in children with CP.

A Correlation between the Visual Perception Ability and Activities of Daily Living for Children with Spastic Diplegia (경직형 양하지 뇌성마비아동의 시지각 능력과 일상생활동작과의 관련성 연구)

  • Objective : The purpose of this study was to evaluate the correlation between visual perception ability and activities of daily living(ADL) in the children with spastic diplegia who were treated at developmental disabilities children salutary institute. Method : The subject were consisted of 8 children with spastic diplegia(mean age, 10). The visual perception ability was measured by the Developmental Test of Visual Perception(DTVP) and by the Functional Independence Measure for Children(WeeFIM) to evaluate activities of daily living. Collected data analysis were completed by using correlation analysis. Results : Visual perception ability of children with spastic diplegia is the highest in Position in Space(PS) score, and is the lowest in Visual-Motor coordination(VM) score. Average visual perception performance of children with spastic diplegia is the highest in SR(Spatial relationship) score, and is the lowest in VM score. In activities of daily living, eating component were significantly correlated with Visual-Motor coordination. Conclusions : Therefore, accurate evaluation of visual perception ability and visual perception training for children with spastic diplegia will be important to improve patient's activities of daily living skill.
