A Case Study of the Tic Disorder Patient with Nausea and Vomiting Treated by Korean Medical Treatment with Temporomandibular Joint Balancing Therapy (오심 구토를 동반한 틱 장애 환자에 대해 턱관절균형요법을 병행한 한방 치험례)

  • Objectives: The purpose of this study is to report the effect of Korean medical treatment with Temporomandibular Joint Balancing Therapy (TBT) on Tic disorder patient with nausea and vomiting. Methods: In this study, Tic disorder patient with nausea and vomiting was treated by Korean medical treatment (acupuncture, herb medicine, etc.), including Temporomandibular Joint Therapy from Feb 28th, 2023 to Jul 10th, 2023. Yale Global Tic Severity Scale (YGTSS) and visual analogue scale (VAS) were used for measuring the Tic disorder and accompanying symptoms. Results: After 32 sessions treatment, the YGTSS decreased from 67 to 0, and VAS associated with Tic disorder, nausea and vomiting also decreased 6 to 0 and 8 to 0 respectively. Conclusions: These results showed that Korean medical treatment with Temporomandibular Joint Balancing Therapy could improve Tic symptoms with nausea and vomiting. But further studies will be needed.

The Clinical Observation on 11 Cases of patient with terminal stage of Gastric cancer (말기(末期) 위암(胃癌)환자 11례에 대한 증례보고)

  • Objective : Gastic cancer has become major cancers which cause nausea, vomiting. Especially patients with terminal stage of gastric cancer may suffer from nausea, vomiting and other symptoms that can keep patients from taking medicine or food. In those cases, there may be no use of taking herbal medicine to treat or palliate symptoms. So we wanted to know the potential efficiency of Acupuncture and Moxibustion whether they could control the symptoms of terminal stage of Gastric cancer without herbal medicine. Methods : Under the assumption that Acupuncture and Moxibustion may be effective for palliating nausea, vomiting on terminal stage of Gastric cancer, the following points were administrated SaGwan(Hapkok($LI_4$), Taechung($LR_3$)), Chok-Samli($ST_{36}$), Kongson($SP_4$), Naegwan($PC_6$) for Acupuncture, Chungwan($CV_{12}$) for Moxibustion. This observation was carried out on 11 patients with terminal stage of Gastric cancer. We reviewed medical records, specifically intake/output check with vomiting, nausea. Results : After therapy of Acupunture and Moxibustion, there were 22% of complete responses, 46% of major responses and 32% of failures. Therapy resulted in 2 cases of goodness, 4 cases of fairness, 5 cases of badness as satisfaction degree. Unfortunately 2 cases of badness expired. Conclusion : We have concluded that Acupunture and Moxibusiton therapy were effective to palliate the nasea, vomiting of terminal Gastric cancer. So if Gastric cancer develop difficulties of taking medicine with patients, to consider using the methods of Acupunture and Moxibution is worthy to palliate the nausea, vomiting and so on.

Evaluation of Antiemetic Therapy for Breakthrough Nausea and Vomiting in Patients with Hematopoietic Stem Cell Transplantation (조혈모세포 이식 환자의 돌발성 구역·구토에 대한 항구토제 사용 현황)

  • Background: The patients receiving hematopoietic stem cell transplantation (HSCT) are known to have a high incidence of breakthrough nausea and vomiting due to the conditioning regimen. The purpose of this study was to evaluate the adequacy of antiemetic therapy for breakthrough nausea and vomiting in patients receiving HSCT and to propose an effective treatment regimen. Methods: We retrospectively reviewed the electronic medical records of 109 adult patients. The collected data were used to identify (1) antiemetic and dosing regimens prescribed for controlling breakthrough nausea and vomiting, (2) the rate of patients who developed breakthrough nausea and vomiting, and (3) the percent of antiemetics prescribed on the day of symptom onset. Based on the National Comprehensive Cancer Network guideline, we assessed the suitability of antiemetics for breakthrough nausea and vomiting, and prescription timing. Results: All patients were prescribed pro re nata antiemetics. About 40.0%, 41.4%, and 18.6% of patients were using one, two, and three or more additional drugs for breakthrough nausea and vomiting, respectively. The most frequently administered drugs were intravenous metoclopramide (43.8%) and granisetron patch (36.2%). Breakthrough nausea and vomiting occurred in 87 patients (79.1%) and they developed symptoms 320 cases. About 220 cases (68.8%) were treated with additional antiemetics on the day of symptom onset and the rate of symptom resolution was only 10.3% (9 patients). Conclusion: The breakthrough nausea and vomiting in patients receiving HSCT occurred very frequently and was hard to control, thus requiring more rapid and aggressive treatments.

A Case Report of Cholinergic Urticaria Patient Using Oriental Medical Treatment with Melonis Calyx Vomiting Therapy (과체 토법을 병용한 콜린성 두드러기 환자의 한방 치험 1례)

  • Objectives : The purpose of this study is to report the efficacy of using oriental medical treatment with Melonix Calyx Vomiting Therapy for Cholinergic urticaria patient. Methods : We experienced one case of Cholinergic urticaria patient treated with oriental medical treatment and Melonix Calyx Vomiting Therapy(MCVT). MCVT is traditional treatment, which induces vomiting by eating Melonix Calyx powder with water. To evaluate the results of this treatment, we measured the decrease of pruritus, number of wheals, redness. Decrease of pruritus, number of wheals was assessed by Urticaria Activity Score(UAS) and quality of life was assessed by Chronic urticaria quality of life questionnaire(CU-Q2oL). Results : UAS decreased from 6 to 0, and severity score of redness decreased from 3 to 0. CU-Q2oL score decreased from 78 to 23. Conclusions : This study suggests that using MCVT with oriental medical treatment is effective on Cholinergic urticaria.

  • Objectives : The purpose of this study is to report the efficacy of using oriental medical treatment withMelonis Calyxvomiting therapy for sudden sensorineural hearing loss(SSNHL).Methods : We experienced one case of SSNHL treated with oriental medical treatment(acupuncture, herbal medicine) andMelonis Calyxvomiting therapy(MCVT). MCVT is a traditional treatment, which induces vomiting by eatingMelonis Calyxpowder with water. To evaluate the results of this treatment, we measured the hearing recovery, decrease of tinnitus and vertigo. The degree of hearing improvement was evaluated by Siegel's Criteria. Decrease of tinnitus and vertigo was assessed by Visual Analogue Scale(VAS).Results : The pure tone 5 frequency average improved by 39㏈(marked recovery, from 64㏈ to 25㏈). VAS of tinnitus decreased from 10 to 1, and vertigo decreased from 4 to 0.Conclusions : This study suggests that using MCVT with oriental medical treatment is effective on SSNHL.

  • Hyperemesis gravidarum is a severe and intractable form of nausea and vomiting in pregnancy. It is a diagnosis of exclusion and may result in weight loss; nutritional deficiencies; and abnormalities in fluids, electrolyte levels, and acid-base balance. The peak incidence is at 8-12 weeks of pregnancy, and symptoms usually resolve by week 16. Interestingly, nausea and vomiting of pregnancy is generally associated with a lower rate of miscarriage. The cause of severe nausea and vomiting in pregnancy has not been identified. Extreme nausea and vomiting may be related to elevated levels of estrogens or human chorionic gonadotropin. Once the diagnosis is made, treatment consists of mainly supportive care. Until vomiting is controlled, these patients should be maintained as NPO. We treated 1 patient who had hyperemesis gravidarum and visited Kyung-Won Incheon Oriental Hospital by Herbal medication, acupuncture, moxa and nega therapy and got good result from them.

  • Objective : Methods of Jang Cong-zheng's treatment related to his theories include of promoting the sweating-vomiting-purgation(汗吐下三法), prickling-bleeding method(瀉血療法), and emotional therapy(情志療法). He succeeded Liu wan-su(劉完素) as promoter of fire-heat pattern theory(火熱論), thereby emphasizing six qi(六氣), especially fire(火) and dampness(濕), as pathogens. He suggested that physician should treat patients whose diseases occurred due to fire(火) and dampness(濕) with promoting the sweating(汗)-vomiting(吐)-purgation(下) methods, which would remove pathogen qi(邪氣) and help circulation of qi and blood, therefore getting them back healthy qi(正氣). Method : I will try to describe the Jang Cong-zheng's treatment in "Ru-Men-Shi-Qin", and I would like to explain the sweating-vomiting-purgation(汗吐下三法), prickling-bleeding method(瀉血療法), and emotional therapy(情志療法). Result : For emotional diseases, he suggested that most should be caused by heart fire(心火) so they should be treated with methods of suppressing heart fire and medication would be using cool and cold herbal drugs which lead to clear heart fire. Conclusion : Theories of Zhang zi-he(張子和) can be applied to nurturing for modern world maintaining and improving health by his care methods. People these days suffer usually from excess intake or unbalanced diet problem, so wastes matter and pathogenic qi can be driven out of the human body by applying the sweating-vomiting-purgation methods. His medical theories could contribute to modernization of traditional medicine this way.

Efficacy of Epidural Droperidol in Reducing Nausea and Vomiting Associated with Postoperative Epidural Morphine (술후 경막외 Morphine으로 인한 오심 및 구토에 대한 경막외 Droperidol의 효과)

  • Background: To determine the effectiveness of continuous epidural infusion of droperidol, combined with epidural morphine, in reducing nausea or vomiting associated with epidural morphine and minimizing the side effects of droperidol, 48 patients undergoing elective thoracic surgery were randomly assigned to one of two study groups. Methods: Patients received continuous infusion of epidural morphine(6.0 mg/day) following a bolus loading dose of 3.0 mg(Group A), or epidural mixture of morphine(6.0 mg/day) plus droperidol(5.0 mg/day) following a bolus loading dose(morphine 3.0mg, droperidol 1.5 mg)(Group B). For the first 48 postoperative hours, the incidence of nausea or vomiting, the need for antiemetic therapy, level of sedation, and adverse effects associated with droperidol were evaluated. Results: The incidence of nausea or vomiting and the number of patients who required antiemetic therapy were significantly less in Group B than in Group A(P<0.05). There were no significant differences between groups with regard to the adverse effects associated with droperidol such as mental depression, respiratory depression and abnormal movements(P=NS). Conclusion: We conclude that simultaneous titration of morphine and droperidol via epidural continuous infusion following epidural bolus injection of the mixture reduces nausea or vomiting associated with epidural morphine while it prevents the side effects of droperidol.

  • PFAPA syndrome is characterized by periodic fevers associated with aphthous stomatitis, pharyngitis, and cervical adenitis and is unusual in infants and children. We report on a case of PFAPA syndrome mimicking cyclic vomiting syndrome in a 42-month-old girl. She had experienced multiple episodes of cyclic vomiting with abdominal pain from age 20 to 30 months. When she was 30 months old, periodic fever with pharyngitis was combined with cyclic vomiting, and when 40 months old, aphthous stomatitis and cervical adenitis were added. These periodic symptoms and signs were not treated with prokinetics or antibiotics. Symptom duration of an episode was 3 days. After cimetidine therapy (150 mg three times daily for 6 months), her febrile and cyclic vomiting episodes ceased. At the time of writing she had not received therapy for 10 months and has remained well without periodic attack.

A Case of Psychogenic vomiting (七情吐) patient who showed loss of the weight due to Maladjustment in a Military life (군대생활 부적응으로 급격한 체중감소를 나타낸 칠정토(七情吐) 환자 치험 1례)

  • Psychogenic vomiting (七情吐) is the disease of vomiting due to seven emotions that joy, anger, melancholy, anxiety, grief, fear and terror being the response of the mind to the environmental stimuli. We experienced a 21year-old man who had a psychogenic vomiting due to maladjustment in a military life, and whose condition was improved through oriental medical treatment. We treated the patient with Herbal medications and Giungoroen (至言高論)-wise saying and lofty opinion). Giungoroen is psychological therapy that promotes patient's recognition of disease and will to cure it through conversation. After being treated, the patient showed that symptoms (vomiting, nausea, abdominal discomfort, insomnia, a depressed mood, a feeling of uneasiness) was improved considerably. Tills result suggests that oriental medical treatment bas good effect on psychogenic vomiting due to adjustment disorder.
