The aim of this systematic review was to collect evidence on the following 10 technical aspects of glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA): screw insertion angles; screw orientation; screw quantity; screw length; screw type; baseplate tilt; baseplate position; baseplate version and rotation; baseplate design; and anatomical safe zones. Five literature libraries were searched for eligible clinical, cadaver, biomechanical, virtual planning, and finite element analysis studies. Studies including patients >16 years old in which at least one of the ten abovementioned technical aspects was assessed were suitable for analysis. We excluded studies of patients with: glenoid bone loss; bony increased offset-reversed shoulder arthroplasty; rTSA with bone grafts; and augmented baseplates. Quality assessment was performed for each included study. Sixty-two studies were included, of which 41 were experimental studies (13 cadaver, 10 virtual planning, 11 biomechanical, and 7 finite element studies) and 21 were clinical studies (12 retrospective cohorts and 9 case-control studies). Overall, the quality of included studies was moderate or high. The majority of studies agreed upon the use of a divergent screw fixation pattern, fixation with four screws (to reduce micromotions), and inferior positioning in neutral or anteversion. A general consensus was not reached on the other technical aspects. Most surgical aspects of baseplate fixation can be decided without affecting fixation strength. There is not a single strategy that provides the best outcome. Therefore, guidelines should cover multiple surgical options that can achieve adequate baseplate fixation.
Background: Condylar dislocation can arise as a complication in patients who required mandibular and/or condylar reconstruction and were operated on with fibula free flap (FFF) using surgical guides designed using simulation surgery. Surgeons should be aware of the complications in these present cases when planning and performing reconstructions as well as predicting prognoses. Cases presentation: Two cases showed condylar dislocation in mandibular reconstruction using a FFF fixed with a reconstruction plate. Three cases showed condylar dislocation in mandibular reconstruction using a fibula free flap fixed with a mini-plate. Conclusion: Despite the lack of clinical symptoms in these cases following mandibular reconstruction using an FFF, the mandibular condyle was severely displaced away from the glenoid fossa. A surgeon must have sufficient time to consider the use of a long flap with thickness similar to that of the mandible, ways to minimize span and bending, and methods of fixation. The patient, moreover, should be educated on condylar dislocation. Customized CAD/CAM-prototyped temporomandibular condyle-connected plates may be a good alternative even if virtual simulation surgery is to be performed before surgery. These considerations may help reduce the incidence of complications after mandibular reconstruction.
The Haptic Exploration Laboratory at The Johns Hopkins University is currently exploring many problems related to haptics (force and tactile information) in human-machine systems. We divide our work into two main areas: virtual environments and robot-assisted manipulation systems. Our interest in virtual environments focuses on reality-based modeling, in which measurements of the static and dynamic properties of actual objects are taken in order to produce realistic virtual environments. Thus, we must develop methods for acquiring data from real objects and populating pre-defined models. We also seek to create systems that can provide active manipulation assistance to the operator through haptic, visual, and audio cues. These systems may be teleoperated systems, which allow human users to operate in environments that would normally be inaccessible due to hazards, distance, or scale. Alternatively, cooperative manipulation systems allow a user and a robot to share a tool, allowing the user to guide or override the robot directly if necessary. Haptics in human-machine systems can have many applications, such as undersea and space operations, training for pilots and surgeons, and manufacturing. We focus much of our work on medical applications.
The purpose of this study were to investigate 1) Values placed upon born hereditary appearance and a made up appearance, 2)examined for points of body and clothing and 3)discrepancy in opinions about Coordination system of 3D Virtual Model This study chose qualitative research approach in-depth interviews were from December 12, 2002, to February 20, 2003. The subjects of the study were 12 women aged in twenties 12 women aged over forty. The results of this study were as follows. 1. Women aged in their twenties defined appearance as follows. there are important a made up appearance, Appearance is looking at point of view from body shape to face, makeup, hair style, body image, cosmetic surgery, clothing and is looking at the whole point of view from hair to tiptoe. This seems to include attitude, personality, behavior, images, and feeling. 2. Regarding body image Women aged in their twenties prefer a slender figure. Women aged in their twenties exerts all possible efforts to have an attractive body through dress well. 3. Regarding body image Women aged in their twenties prefer coordinate to system of 3D virtual model.

The reconstruction of the mandible is a complex procedure because various cosmetic as well as functional challenges must be addressed, including mastication and oral competence. Many surgical techniques have been described to address these challenges, including non-vascularized bone grafts, vascularized bone grafts, and approaches related to tissue engineering. This review summarizes different modifications of the free vascularized fibula graft, which, since its introduction by Hidalgo in 1989, has become the first option for mandibular reconstruction. The fibula free flap can undergo various modifications according to the individual requirements of a particular reconstruction. Osteocutaneous flaps can be harvested for reconstruction of composite defects. 'Double-barreling' of the fibula can, for instance, enable enhanced aesthetic and functional results, as well as immediate one-stage osseointegrated dental implantation. Recently described preoperative virtual surgery planning to facilitate neomandible remodeling could guarantee good results. To conclude, the free fibula bone graft can currently be regarded as the "gold standard" for mandibular reconstruction in case of composite (inside and outside) oral cavity defects as well as a way of enabling the performance of one-stage dental implantation.
The goal of total knee replacement (TKR) surgery is to replace patient's knee joint with artificial implants in order to restore normal knee joint functions. Since mismatched knee implants often cause a critical balancing problem and short durability, designing a well-fitted implant to a patient's knee joint is essential to improve surgical outcomes. We developed a software system that three-dimensionally (3D) simulates TKR surgery based upon 3D knee models reconstructed from computed tomography (CT) imaging. The main task of the system was to extract precise 3D anatomical parameters of a patient's knee that were directly used to determine a custom fit implant and to virtually perform TKR surgery. The virtual surgery was simulated by amputating a 3D knee model and positioning the determined implant components on the amputated knee. The test result shows that it is applicable to derive surgical parameters, determine individualized implant components, rehearse the whole surgical procedure, and train medical staff or students for actual TKR surgery. The feasibility and verification of the proposed system is described with examples.
본 논문은 3D 그래픽 기술을 활용하여 사용자의 실제 모습과 같은 3D 얼굴 모델을 구현하고, 구현된 얼굴 모델을 활용하여 성형 시술 전과 후를 가상으로 비교함으로 성형수혜자의 만족도를 높이는 응용프로그램인 3D 가상 얼굴 성형 시스템에 대한 연구 및 구현이다. 사용자의 실물과 최대한 유사한 원형 3D 얼굴 모델링 제작을 위한 3D 모델링 기법과 피부를 표현하는 맵핑 기법, 그리고 데이터베이스 시스템을 사용한 얼굴 데이터의 저장 및 사용자 맞춤 가이드 시스템을 구현한다. 또한, 세부적인 미세 조정을 위한 변형 Controller를 구현하여 얼굴의 미세한 변화를 나타낼 수 있다. 본 논문에서 구현한 3D 얼굴 성형 시스템은 성형외과 등에서 사용되고 있는 이미지를 활용한 시스템에 비하여 정확성, 편리성, 만족도가 높은 결과를 보여주고 있다.
시간, 공간, 금전적인 문제 때문에 실습이 쉽지 않은 학문들이 있다. 특히 의료교육은 장소와 시간의 제약이 특히 심하며 cadaver(카드버:인체해부 실습용 시체)를 통해 이루어지기 때문에 어려움이 많다. 그 대안으로 다양한 가상 수술방법이 존재하지만 장비가 고가이거나 구하기 쉽지 않기 때문에 어려운 문제가 발생한다. 이러한 문제점의 해결 방법으로 3D-Virtual Reality(3D가상현실)를 활용하여 실제와 유사한 환경을 제공하고자 한다. 일반적으로 컴퓨터의 Interface(인터페이스)로 키보드와 마우스를 사용하지 않고 Oculus Rift(오큘러스 리프트)와 Leap Motion(립 모션)를 사용함으로써 더욱더 현실감을 느낄 수 있어 실습교육비용을 절감함으로써 실습교육에 대한 효과를 극대화 할 수 있다. 본 논문에서는 두개의 Device(디바이스)인 Oculus Rift(오큘러스 리프트)와 Leap Motion(립 모션)를 이용하여 3D가상 수술 시스템을 구축하고 실습할 수 있는 제안으로 의료교육인 수술교육에 대치하여 효율성을 높이는 방식을 제안하였고, 이는, 다른 Simulation(시뮬레이션)이 활용 가능한 학문분야에 까지 확장할 수 있을 것으로 사려된다.
This paper explores design and control methods of a surgical robot for total hip replacement surgery which can be easily maneuvered by a surgeon Like an advanced surgical tool. The 3-DOF in-parallel surgical robot is fixed directly onto patient's femur by the bone clamp during surgery. With the master/slave combined surgical robot, the surgeon can directly control the motion of the surgical robot with surgeon's experience and judgment during operation. For the easiness of operation, the master/slave combined robot is controlled using admittance control paradigm. And for the precise operation, the robot motion is restricted at the surgical boundary using virtual hard wall display.
소동물의 슬개골 수술은 인간의 뼈보다 약하여 수술의 난이도가 어렵기 때문에, 소 동물의 슬개골 탈구 치료 수술 과정의 학습은 수의사에게 매우 중요하다. 소 동물의 슬개골 수술 훈련은 사체를 이용한 훈련을 하고 있으나 소 동물의 사체가 부족하기 때문에 시청각 자료나 수술 참관으로 수술을 학습하고 있어 수의학과 학생 및 전문 수의사들에게 충분한 훈련 환경이 제공되지 못하는 상황이다. 본 논문에서는 이러한 문제를 해결하기 위하여 소동물의 슬개골 수술을 위한 햅틱 시뮬레이터를 제안한다. 제안한 시뮬레이터는 슬개골 드릴링 수술에 적용시켰으며 소동물의 드릴링에 대한 Force Feedback을 모델링하고 햅틱 인터페이스를 제공하고, 사용자가 몰입하여 수술 경험을 할 수 있는 워크 벤치를 제공하였다. 본 논문에서 제안된 슬개골 햅틱 시뮬레이터가 제공하는 드릴링 과정에서 햅틱 피드백에 대한 사용자 평가 결과 유의미한 결과를 얻을수 있었다.
