Objective : To define optimal method that calculate the safe direction of cervical pedicle screw placement using computed tomography (CT) image based three dimensional (3D) cortical shell model of human cervical spine. Methods : Cortical shell model of cervical spine from C3 to C6 was made after segmentation of in vivo CT image data of 44 volunteers. Three dimensional Cartesian coordinate of all points constituting surface of whole vertebra, bilateral pedicle and posterior wall were acquired. The ideal trajectory of pedicle screw insertion was defined as viewing direction at which the inner area of pedicle become largest when we see through the biconcave tubular pedicle. The ideal trajectory of 352 pedicles (eight pedicles for each of 44 subjects) were calculated using custom made program and were changed from global coordinate to local coordinate according to the three dimensional position of posterior wall of each vertebral body. The transverse and sagittal angle of trajectory were defined as the angle between ideal trajectory line and perpendicular line of posterior wall in the horizontal and sagittal plane. The averages and standard deviations of all measurements were calculated. Results : The average transverse angles were $50.60^{\circ}{\pm}6.22^{\circ}$ at C3, $51.42^{\circ}{\pm}7.44^{\circ}$ at C4, $47.79^{\circ}{\pm}7.61^{\circ}$ at C5, and $41.24^{\circ}{\pm}7.76^{\circ}$ at C6. The transverse angle becomes more steep from C3 to C6. The mean sagittal angles were $9.72^{\circ}{\pm}6.73^{\circ}$ downward at C3, $5.09^{\circ}{\pm}6.39^{\circ}$ downward at C4, $0.08^{\circ}{\pm}6.06^{\circ}$ downward at C5, and $1.67^{\circ}{\pm}6.06^{\circ}$ upward at C6. The sagittal angle changes from caudad to cephalad from C3 to C6. Conclusion : The absolute values of transverse and sagittal angle in our study were not same but the trend of changes were similar to previous studies. Because we know 3D address of all points constituting cortical shell of cervical vertebrae. we can easily reconstruct 3D model and manage it freely using computer program. More creative measurement of morphological characteristics could be carried out than direct inspection of raw bone. Furthermore this concept of measurement could be used for the computing program of automated robotic screw insertion.

Objective : Bone mineral density (BMD) is an important consideration during fusion surgery. Although dual X-ray absorptiometry is considered as the gold standard for assessing BMD, quantitative computed tomography (QCT) provides more accurate data in spine osteoporosis. However, QCT has the disadvantage of additional radiation hazard and cost. The present study was to demonstrate the utility of artificial intelligence and machine learning algorithm for assessing osteoporosis using Hounsfield units (HU) of preoperative lumbar CT coupling with data of QCT. Methods : We reviewed 70 patients undergoing both QCT and conventional lumbar CT for spine surgery. The T-scores of 198 lumbar vertebra was assessed in QCT and the HU of vertebral body at the same level were measured in conventional CT by the picture archiving and communication system (PACS) system. A multiple regression algorithm was applied to predict the T-score using three independent variables (age, sex, and HU of vertebral body on conventional CT) coupling with T-score of QCT. Next, a logistic regression algorithm was applied to predict osteoporotic or non-osteoporotic vertebra. The Tensor flow and Python were used as the machine learning tools. The Tensor flow user interface developed in our institute was used for easy code generation. Results : The predictive model with multiple regression algorithm estimated similar T-scores with data of QCT. HU demonstrates the similar results as QCT without the discordance in only one non-osteoporotic vertebra that indicated osteoporosis. From the training set, the predictive model classified the lumbar vertebra into two groups (osteoporotic vs. non-osteoporotic spine) with 88.0% accuracy. In a test set of 40 vertebrae, classification accuracy was 92.5% when the learning rate was 0.0001 (precision, 0.939; recall, 0.969; F1 score, 0.954; area under the curve, 0.900). Conclusion : This study is a simple machine learning model applicable in the spine research field. The machine learning model can predict the T-score and osteoporotic vertebrae solely by measuring the HU of conventional CT, and this would help spine surgeons not to under-estimate the osteoporotic spine preoperatively. If applied to a bigger data set, we believe the predictive accuracy of our model will further increase. We propose that machine learning is an important modality of the medical research field.
이 연구의 목적은 골격 성숙도를 평가하기 위해 3번째 손가락의 중간 지골을 이용하는 방법과 경추를 이용하는 방법 사이의 체질량 지수 백분위 수에 따른 일치도를 조사하는 것이다. 후향의 차트 분석을 통하여 체질량 지수 자료, 수완부 방사선 사진, 측방 두부 방사선 사진이 같은 날에 채득된 경우를 조사 대상으로 하였다. 조사 대상은 체질량 지수 백분위 수에 따라 4개 군으로 나뉘었다. 2가지 방법 사이의 체질량 지수 백분위 수 군에 따른 일치도는 가중치 카파 통계량을 계산하여 측정되었다. 전체 군에 대해 2가지 방법 간에 좋은 일치도를 보였지만 성별 및 체질량 지수 백분위 수 군에 따라서 일치도가 달랐다. 남자는 과체중 군에서 가장 큰 일치도를 보였으나 여자의 경우 저체중에서 비만으로 감에 따라 일치도가 더 증가하는 경향을 보였다. 어린이와 청소년의 성장을 평가할 때 성별과 체중 상태가 골령을 분석하는 2가지 방법의 일치도에 영향을 줄 수 있기 때문에 소아치과 의사는 이를 고려해야 한다.

Objective : The most common complication of anterior cervical discectomy and fusion (ACDF) is cage subsidence and maintenance of disc height affects postoperative clinical outcomes. We considered cage subsidence as an inappropriate indicator for evaluating preservation of disc height. Thus, this study aimed to consider patients with complications such as reduced total disc height compared to that before surgery and evaluate the relevance of several factors before ACDF. Methods : We retrospectively reviewed the medical records of 40 patients who underwent stand-alone single-level ACDF using a polyetheretherketone (PEEK) cage at our institution between January 2012 and December 2018. Our study population comprised 19 male and 21 female patients aged 24-70 years. The minimum follow-up period was 1 year. Twenty-seven patients had preoperative bone mineral density (BMD) data on dual-energy X-ray absorptiometry. Clinical parameters included sex, age, body mass index, smoking history, and prior medical history. Radiologic parameters included the C2-7 cobb angle, segmental angle, sagittal vertical axis, disc height, and total intervertebral height (TIH) at the preoperative and postoperative periods. Cage decrement was defined as the reduction in TIH at the 6-month follow-up compared to preoperative TIH. To evaluate the bone quality, Hounsfield unit (HU) value was calculated in the axial and sagittal images of conventional computed tomography. Results : Lumbar BMD values and cervical HU values were significantly correlated (r=0.733, p<0.001). We divided the patients into two groups based on cage decrement, and 47.5% of the total patients were regarded as cage decrement. There were statistically significant differences in the parameters of measuring the HU value of the vertebra and intraoperative distraction between the two groups. Using these identified factors, we performed a receiver operating characteristic (ROC) curve analysis. Based on the ROC curve, the cut-off point was 530 at the HU value of the upper cortical and cancellous vertebrae (p=0.014; area under the curve [AUC], 0.727; sensitivity, 94.7%; specificity, 42.9%) and 22.41 at intraoperative distraction (p=0.017; AUC, 0.722; sensitivity, 85.7%; specificity, 57.9%). Using this value, we converted these parameters into a bifurcated variable and assessed the multinomial regression analysis to evaluate the risk factors for cage decrement in ACDF. Intraoperative distraction and HU value of the upper vertebral body were independent factors of postoperative subsidence. Conclusion : Insufficient intraoperative distraction and low HU value showed a strong relationship with postoperative intervertebral height reduction following single stand-alone PEEK cage ACDF.
본 연구는 경추 CT 검사 후에 영상을 재구성하는 방법으로, 사위 관상면(oblique coronal) 재구성 각도에 따라서 신경공 골성 협착을 가장 잘 표현할 수 있는 재구성 각도를 제시하고자 한다. 경추의 정중시상면과 구추관절에서 척수 중심을 지나는 선을 그어 만들어지는 각도를 측정하였다. 신경공에서 척수 중심을 지나는 선을 그어, 정중시상면과 이루는 각도를 측정하여 비교하였다. 경추 4-5 레벨에서는 평균 9.2°, 경추 5-6 레벨에서는 평균 9.9°, 경추 6-7 레벨에서는 평균 8.4° 로 구추관절각보다 돌기사이관절각이 크게 측정되었으며, 통계적으로 평균에 유의한 차이가 있었다(p<0.01). 또한, 하부 경추로 갈수록 각도가 증가하는 것을 알수 있었다. 하부 경추(C5-6, C6-7)의 신경공과 척수 중심이 이루는 각은 55~60° 사이이며, 여기에 90°가 되도록 사위 관상면 영상을 재구성하면 신경공협착 정도가 잘 관찰된다. 기존 CT 스캔 영상을 이용한 재구성 영상이므로 추가적인 방사선 피폭을 받지 않으면서, 경추 신경공 골성 협착을 진단하는데 큰 의의가 있다.
Purpose: The purpose of this study was to investigate the effects of lumbar stabilization exercise on balance ability in patients with adolescent idiopathic scoliosis. Methods: This study was one group pretest-posttest design. Twenty young idiopathic scoliosis patients were recruited for this study. They had the lumbar stabilization exercise program for 40 minutes a day, three times per week for 3 weeks. The lumbar trunk muscle endurance test and clinical balance tests (functional forward reach, functional lateral reach, Fukuda 50-stepping, one leg standing with eyes open and eyes closed) were measured before and after the lumbar stabilization exercise. Results: There were significant differences between pre-and post-test in absolute value of the difference between right and left lateral reach, distance of Fukud 50-stepping and one leg standing on the each side with eyes closed(p<.05). The results showed that the lumbar trunk muscle endurance significantly increased(p<.05). Conclusion: In this study indicated that the lumbar stabilization exercise was effective on balance ability and lumbar trunk muscle endurance in patients with young idiopathic scoliosis.
Objective: The purpose of this study is to compare chronic low back pain patients' pain provocation position so as to identify the relevance with lumbar stabilizing muscles atrophy and pain provocation position. Design: Cross-sectional study. Methods: Fifty five chronic low back pain patients were participated in this study. Subjects were eligible for study participation if they were 35-55 years old and had experienced low back pain for more than 3 months. Subjects were questioned about pain and pain provocation test were done. And then they were inspected their cross sectional area (CSA) of lumbar muscles (erector spinae, iliopsoas, and multifidus) by using computed tomography. Analyze the relevance through the result data with painful area, aspect of pain and pain provocation position. Results: CSA of erector spinae showed significant decrease on ipsilateral extension position (p<0.05). Iliopsoas muscle showed significant decrease on contralateral position (p<0.05). Multifidus showed significant decrease on the position of contralateral extension and contralateral flexion (p<0.05). Conclusions: Based on the results of our study, it may be possible to evaluate muscle atrophy by assessing causing position.
The offshore subsea platforms are connected to subsea pipelines to transport gas/oil from wells. The pipe is a multilayered structure of polymer and steel for compensating both flexibility and strength. The pipe also requires reinforcement structures to endure the extreme environmental conditions. A vertebrae structure of bend restrictors is one of the reinforcement structures installed to protect the subsea pipe from excessive bending deformations. In this study, structural behaviors of the subsea pipeline with bend restrictors are investigated by the multi-body contact analysis in Abaqus 6.14-2. Contact forces of each bend restrictor extracted from the multi-body contact analysis can be boundary conditions for topology design optimization in Altair Hyperworks 13.0 Hypermesh-Optistruct. Multiple design constraints are considered to obtain a manufacturable design with efficient material usage. Through the multi-body contact analysis with optimized bend restrictors, it is confirmed that the bending performance of the optimized design is enhanced.
Objectives : Ankylosing Spondylitis, inflamatic disease, appeared symostosis, pain and dyscinesia in spine and sacroilic joint. It repeated an improvement and a depravation. We observed one patient with Ankylosing Spondylitis. Methods : This patient was juged incurable in points of western medical's View. But he was improved by treating oriental medical therapy. The therapies which used in this patient was acupunture, bee venom, korean chuna therapy and kinesiatrics etc. We measured the R.O.M, special test and degree of pain every day. Results : The R.O.M of extension in lumbar ${vertebrae(0_{\circ}\;{\rightarrow}5_{\circ}\;)\;and\;patrick\;sign(++/-\;{\rightarrow}\;-/-)}$ was improved a little. Degree of pain was improved better than admission. Conclusions : This case has demonstrated that the Ankylosing Spondylitis treated by oriental medical therapies proved good results.
Multiple jaw cysts are one of the most constant features of the basal cell nevus syndrome. Basal cell nevus syndrome is inherited as an autosomal dominant trait with variable expressiveness. This syndrome comprises a number of abnormalities such as multiple nevoid basal cell carcinomas of the skin, skeletal abnormalities as bifid rib and fusion of vertebrae, central nervous system abnormalities as mental retardation, eye abnormalities with multiple jaw cysts. The odontogenic keratocysts in patients with this syndrome are often associated with the crowns of unerupted teeth and huge size; on radiographs they may mimic dentigerous cysts. The most important feature of the cyst is its extraordinary recurrence rate. Since recurrence may be long delayed in this lesion, follow-up of any case of odontogenic keratocyst with roentgenograms and clinical examination of basal cell carcinoma are essential for at least five years after surgery. We report the result of 7-year follow up after cyst enucleation associated with basal cell nevus syndrome with the literature of review.
