Evidence Based Practice in Long Term Care Settings

  • Purpose: The purpose of this manuscript is to discuss the need for use of evidence based practice (EBP) in LTC, the current use of evidence in long term care facilities and what we know about adoption of the use of EBP in LTC. Methods: Literature review and reporting of findings from the M-TRAIN study that was a quasi-experimental design to test the effectiveness of an intervention to increase the use of EBPs for urinary incontinence and pain in 48 LTC facilities. Results: Barriers to adopting EBPs include lack of available time, lack of access to current research literature, limited critical appraisal skills, excessive literature to review, non-receptive organizational culture, limited resources, and limited decision-making authority of staff to implement change. Strategies to promote adoption of EBP include the commitment of management; the culture of the home; leadership; staff knowledge, time, and reward; and facility size, complexity, the extent that members are involved outside the facility, NH chain membership, and high level of private pay residents. Findings from the M-TRAIN add, stability of nurse leader and congruency between the leaders perception of their leadership and the staff's perception of the leadership. Conclusion: There is clear evidence of the need and the benefits to residents of LTC and to the health care system yet adoption of EBP continues to be slow and sporadic. There is also evidence for the process of establishing best evidence and many resources to find the available EBPs. The urgent need now is finding ways to best get the EBPs implemented in LTC. There is growing evidence about best methods to do this but continued research is needed. Clearly, residents in LTC deserve the best care possible and EBPs represent an important vehicle by which to do this.

대형마트 신선편의식품 소비자의 구매 후 행동에 따른 식품 내·외부 온도 및 미생물학적 오염 변화 (Changes in Internal and External Temperature and Microbiological Contamination depending on Consumer Behavior after Purchase of Fresh-Cut Produces)

  • 본 연구에서는 신선편의식품을 구매한 뒤 집으로 귀가해서 냉장고에 보관하거나 섭취하는데 소요되는 시간과 그에 따른 신선편의식품 내.외부의 온도변화와 미생물학적 변화를 관찰하였다. 시판 중인 신선편의 식품에 대해 위생지표세균인 일반세균, 대장균군 및 대장균, 병원성 미생물인 B. cereus, S. aureus, Salmonella spp., L. monocytogenes를 분석하였다. 설문조사 결과 신선편의식품을 구매 후 귀가 또는 섭취에 소요되는 시간이 최대 3시간 소요되는 것으로 확인되어, 차량 트렁크 내에서 최대 3시간 동안 보관한 결과 차량 트렁크 내부 최고 온도가 봄(3월) 19℃, 여름(7월) 44℃, 가을(9월) 31.3℃, 겨울(2월) 17.6℃로 각각 확인되었으며, 차량 트렁크에서 보관한 신선편의식품의 품온은 최대 봄 18.5℃, 여름 42℃, 가을 29.2℃, 겨울 16.8℃로 확인되었다. 차량 트렁크에 최대 3시간 보관한 신선편의식품의 일반세균수는 봄 3.41 log CFU/g, 여름 4.32 log CFU/g, 가을과 겨울은 각각 3.81 log CFU/g, 3.36 log CFU/g으로 확인되었다. 그 외의 대장균군 및 대장균, B. cereus, Salmonella spp., L. monocytogenes균은 검출되지 않았지만 S. aureus는 여름과 가을철에는 신선편의식품을 1시간만 차량에 보관하더라도 검출되었고, 봄과 겨울은 2시간 이상 차량 트렁크에 보관하였을 때 검출되었다. 이동용기로 흔히 사용되는 종이박스와 스티로폼 박스 내부가 알루미늄필름으로 코팅된 종이 박스를 이용하여 얼음 첨가여부에 따른 이동용기 온도변화 실험과 이동 중 S. aureus의 증식여부를 확인한 결과 스티로폼 박스에서 내부 온도변화가 가장 낮게 유지되었고, 시간 경과에 따른 미생물의 증식도 가장 적게 나타남을 확인할 수 있었다. 이상의 결과로 볼 때 신선편의식품 소비자들에게 구매 후 안전한 섭취를 위해 보관 온도나 시간에 대한 가이드라인과 이동용기를 이용한 신선편의식품의 온도관리에 대한 정보를 제공하여 식품의 안전성을 높이는 홍보가 필요할 것으로 판단된다.

메시지 프레이밍과 조절 초점에 따른 졸음운전 예방 OOH 광고 효과에 관한 연구 (A Study on the Effects of Drowsy Driving Prevention OOH Advertising Depending on Message Framing and Regulatory Focus)

  • 졸음운전 예방 OOH 광고는 차량 통행이 이루어지는 장소 및 시점에서 노출된다는 점에서 효과적인 졸음운전 예방 메시지 전달 수단이 될 수 있다. 본 연구는 대학생 200명을 대상으로 한 실험 연구를 통해 졸음운전 예방 행동의도에 대한 졸음운전 예방 OOH 광고의 메시지 프레이밍과 조절 초점의 영향을 조사했다. 연구 결과, 졸음운전 예방 OOH 광고의 메시지 프레이밍과 조절 초점은 졸음운전 예방 행동 의도에 대해 상호작용효과를 갖는 것으로 나타났다. 긍정적 프레이밍 메시지의 경우 조절 초점 성향에 따라 졸음운전 예방 행동 의도에 유의한 차이를 보이지 않았다. 반면 부정적 프레이밍 메시지의 경우 향상 초점보다 예방 초점 성향일 때 졸음운전 예방 행동 의도가 더 높았다. 본 연구의 결과는 메시지 전략 측면인 메시지 프레이밍과 수용자 개인 성향 측면인 조절 초점을 함께 조명함으로써 관련 분야의 연구자와 실무자에게 수용자의 성향을 고려한 메시지 전략 개발에 대한 이론적 및 실무적 함의를 제공한다.

발사환경에 대한 인공위성 전장품의 구조진동 해석 (Structural Vibration Analysis of Electronic Equipment for Satellite under Launch Environments)

  • The impulse between launch vehicle and atmosphere can generate a lot of noise and vibration during the process of launching a satellite. Structurally, electronic equipment (KOMPSAT 2, RDU : Remote Drive Unit) of a satellite consists of aluminum case containing PCB (Printed circuit boards). Each PCB has resistors and IC (Integrated circuits). Noise and vibration of wide frequency band are transferred to the inside of fairing, subsequently creating vibration of the electronic equipment of the satellite. In this situation. random vibration can cause malfunctioning of the electronic equipment of the device. Furthermore, when tile frequency of random vibration meets with natural frequency of PCB. fatigue fracture nay occur in the part of solder joint. The launching environment, thus. needs to be carefully considered when designing the electronic equipment of a satellite. In general. the safety of the electronic equipment is supposed to be related to the natural frequency, shapes of mode and dynamic deflection of PCB in the electronic equipment. Structural vibration analysis of PCB and its electronic components can be performed using either FEM(Finite Element Method) or vibration test. In this study. the natural frequency and dynamic deflection of PCB are measured by FEM, aud the safety of the electronic components of PCB is being evaluated according to the results. This study presents a unique method for finite element modeling and analysis of PCB and its electronic components. The results of FEA are verified by vibration test. The method proposed herein may be applicable to various designs from the electronic equipments of a satellite to home electronics.

Power Aware Routing Protocol in Multimedia Ad-hoc Network Considering Hop Lifetime of Node

  • The purpose of this research is to extend Ad-hoc network system lifetime with the proposed routing protocol which has considered hop lifetimes of the nodes while guaranteeing QoS in the establishment process of Ad-hoc network communication paths. Based on another power aware routing system that proposed in the advanced research [1], we are proposing an alternative power aware routing system in which nodes' hop lifetimes are compared in order to extend the lifetime of an Ad-hoc network system and delay factors have been considered for the assurance of QoS. The research of the routing protocol in this paper, which aims to maximize the system survival time considering power consumption status during the path searching in MANET and pursues the mechanism that controls hop delays for the same reason, can be applied to the study of WSN. The study concerning such phenomena is essential so that the proposed protocol has been simulated and verified with NS-2 in Linux system focusing on the lifetimes of the hops of the nodes. Commercialization of smart devices and arrival of the ubiquitous age has brought about the world where all the people and things are connected with networks. Since the proposed power aware method and the hop delay control mechanism used to find the adequate communication paths in MANET which mainly uses batteries or in WSN, they can largely contribute to the lifetime extension of the network system by reducing power consumptions when utilized for the communications attempts among soldiers during military operation, disaster areas, temporary events or exhibitions, mobile phone shadow areas, home networks, in-between vehicle communications and sense networks, etc. This paper presents the definitions and some advantages regarding the proposed outing protocol that sustain and extend the lifetime of the networks.

대구지역 일개 초등학교를 대상으로 한 소아 수면문제에 대한 예비연구 (Preliminary Study of Children's Sleep Problems in an Elementary School in Daegu)

  • Objectives: The purpose of this study was to investigate the prevalence of sleep problems in Korean elementary school children and the differences in sleep problems elated to their school grade and gender. Methods : One public elementary school in Dae-gu, Korea as randomly selected. And, parents who agreed to participate this study were asked to complete a modified version of Tucson Children's Assessment of Sleep Apnea screening questionnaire (TuCASA). Results: The most common sleep problem was 'falling asleep in vehicle (44.2%)' and the prevalence rate of this problem differed by school grade. The prevalence rate of falling asleep while watching TV before 8 P.M. (8.0%), daytime fatigue (21.9%) and nocturnal enuresis during the past 6 months (5.3%) were significantly differed by school grade. The prevalence rate of falling asleep while doing home work, falling sleep during a lesson, snoring and bruxism were 12.6%, 1.0%, 26.7% and 13.2% respectively. Although these rates did not show any differences between grades, snoring as more in boys and daytime fatigue was more in girls. Conclusion: This study demonstrates that many elementary school children may have various sleep problems. Lower grade elementary schoolers had sleep problems which related to euro-developmental factors, whereas higher grade elementary schoolers had sleep problems related to sleep deprivation and stress. Finally, more female schooler showed signs of fatigue or sleepiness than male schoolers.

학령기 아동의 사고예방 교육안 개발 및 평가 (Development and Evaluation of Injury Prevention Education Proposal for Elementary School Children)

  • This study was to develop and evaluate injury prevention education proposal which will helpful and can be utilized directly on the first line spot for elementary school children. Education proposal development and evaluation process was 1) Construction of 10-times education proposal contents proper to schooler 2) Testify validity through 3 pediatric nursing professor and 4 elementary school nurse 3) Pretest was done from March, 2002 to July on 3-6th grade 313 elementary school students 4) Through correction and revision after pretest evaluation meeting, final injury prevention education proposal was developed 5) After 10-times injury education, evaluation was carried out about the degree of help in education contents and general constitution of injury education to total subjects of educated children. Injury prevention education proposal consists of 10 times and each subjects are followings. 1st is 「introduction of injury prevention education and the importance of injury prevention」 2nd is「safety in and around home」, 3rd is 「injury prevention in school」, 4th is 「prevention of violence」, 5th is 「motor vehicle safety」, 6th is 「water safety」, 7th is 「prevention of fire and burns」, 8th is 「toy and product safety」, 9th is 「sports and recretional activities safety」and the final 10th is 「injury prevention caused by animals」. In the evaluation, the degree of help in education contents showed it helped to children averaged 1.66 and general constitution showed averaged 2.17 that children satisfied about developed injury prevention proposal. This study expected to provide systematic and concrete guidance in injury prevention education for elementary school children.

체험적 안전교육이 일개 도서지역 중학생의 안전행동과 기본생활습관에 미치는 효과 (The Effects of Safety Behavior and Standard Life Habit on Experiencial Safety Education for one Island Middle School Students)

  • Purpose:This thesis was performed to evaluate the effect of the experience safety education on the safety behavior and standard of habit to the middle school students of one islands. Methods:The participants of this study included 43, located in the adjoining region in S county. While one of the class, composed of 19 students was designated as an experimental group, and the other class, composed of 24 students, was compared as a control group. Used program in this study was safety education program, 'Safe School, Safe Life' which was developed by Korea Occupational Safety & Health Agency. This program has emphasis on the virtual case rather than lecture style education. Results:The findings in this research were as follows. Indoor safety behavior was significant difference between the two groups. On the other hand, the effect on outdoor safety behavior was not difference. In playground case, active strength was needed without continuous attention. The effect of the experience safety behavior education was not shown in the area of traffic rules, pedestrian safety, and vehicle safety. But home safety behavior was effective. Education program on the manners of standard life habit gives positive results. But in the area of rules, this program was not effective, since students had tendency to emphasize the rigid scale rather than manners. Conclusion:These findings in the study give us the necessity of experience safety education program to prepare various situations of everyday life and to reinforce safety behavior and improve standard life habit.

복합재료 교량 바닥판의 주행속도에 따른 동적응답 평가 (Estimation of Dynamic Response of Advanced Composite Material Decks for Bridges Application under Various Vehicle Driving Velocities)

  • 복합 신소재의 건설 분야 적용은 확대되고 있는 추세이고, 높은 비강도 비를 지닌 복합 신소재 교량 바닥 판의 개발은 국내외에서 활발히 진행되고 있다. 본 연구의 목적은 이동 하중을 받는 비등방성 복합재료 적층판의 동적 응답을 정식화하고, 유한요소법을 사용하여 적층에 따른 동적거동 특성을 분석하는 것이다. 수치 해석 모델에 대해서 이동하중의 속도를 증가시키면서 동적 확대 계수를 계산하였다. 또한 적층형식 및 순서, 섬유 보강 각도 등의 변화에 따른 동적 거동 특성을 분석하였다. 본 연구 프로그램의 타당성을 확보하기 위해 휨과 자유 진동 해석에 관한 기존 문헌 결과와 비교하여 검증하였다. 또한 이동 하중에 의한 동적 해석에 대해 모우드 중첩법과 Newmark 직접 적분법을 사용하였다. 이러한 이동 하중과 적층 수, 적층 순서 및 섬유 보강 각도에 따른 수치 해석 결과는 완전 복합 신소재 교량 바닥 판을 개발하는데 있어서 중요한 기초 자료로 제시될 수 있을 것이다.

소방용 AVL 시스템 개발 (Development of an AVL System for Fire Fighting Services)

  • 무선 통신과 모바일 단말기의 발달로 언제 어디서든 무선 통신을 할 수 있게 되었다. 이로 인하여 AVL 시스템의 활용의 폭이 넓어져 상황관제, 물류관리, 택배, 택시, ITS(Intelligent Transportation Systems) 등에 활용되고 있다. 본 논문에서는 구조, 구급 등과 같은 소방 활동에서 활용하기 위한 AVL 시스템을 설계 및 구현하고자 한다. 세부적으로 AVL 관련 기존 연구 및 시스템을 분석한 후에 긴급구조, 구급에 이용할 수 있는 AVL 시스템을 설계하고 구현한다. 이를 위하여 먼저 AVL 서버와 클라이언트를 구현하여 무선 통신을 이용하여 서버와 클라이언트의 안정적인 통신을 실현하고자 한다. 그리고 AVL 시스템을 이용하여 출동차량이 신속하게 재난 위치까지 찾아갈 수 있도록 설계하며, 차량의 위치정보를 이용하여 본부 상황실에서 차량 위치를 실시간으로 확인하여 효율적인 관제가 이루어질 수 있도록 개발한다.
