Evaluation of Urinary Antigen Test for Rapid Diagnosis of $Streptococcus$ $pneumoniae$ in Community-Acquired Pneumonia Patients

  • We evaluated the performance of the NOW $Streptococcus$ $pneumoniae$ urinary antigen test, standard culture and polymerase chain reaction for detecting $S.$ $pneumoniae$. The urinary antigen test of pneumonia patients represented sensitivity at 72% and specificity at 79%. The results of PCR were targeting for autolysin ($lyt$A), pneumolysin ($ply$), and spn9828. The $lyt$A sensitivity and specificity stood at 56% and 87% respectively while $ply$ sensitivity reported 83% and specificity was 47%, sensitivity and specificity of spn9828 stood at 83% and 73% respectively. The results of urinary antigen test and three genes were all statistically meaningful within $P$ <0.05. When the urinary antigen test of $S.$ $pneumoniae$ was positive, the three kinds of genes were also likely to be positive. According to the result of urinary antigen test, the results of PCR presented a meaningful difference ($P$ <0.05). Especially, the urinary antigen test of $S.$ $pneumoniae$ was likely to be positive ($P$ <0.05) when more than two genes were positive in PCR results.

  • Identification of escherchia coli K1 polysaccharide antigen isolated from urine specimens of urinary tract infections in children were performed from of 1992 to 1993 in Kyoto, Japan. The serotypes of E. coli were categorized that O1:H7, O2:H6, O2:H7, O16:H6, O18:H7, O18:H ̄, and O135:H44 among 14 strains isolated from urine specimens of urinary tract infections in children by the serological test. And, one strain (O18:H ̄, isolation rate: 7.1%) of E. coli K1 polysaccharide antigen among 14 strains were isolated from urine specimens of urinary tract infections in children by the bacteriophage test.

Association between Metabolic Syndrome and Lower Urinary Tract Symptoms, Prostate Specific Antigen, and Prostate Volume: Single C enter Study (대사증후군과 하부요로증상, PSA 및 전립샘 용적과의 연관성 : 단일기관 연구)

  • The purpose of this study was to investigate the influence of metabolic syndrome (MS) on the lower urinary tract symptoms (LUTS), prostate specific antigen (PSA), and prostate volume in Korean men. We analyzed the data from 2654 men over the age of 40 who visited our health promotion center for regular health checkups. Of the total 2654 men, mean age, PSA level, International Prostate Symptom Score (IPSS), and prostate volume were 54.6 years, 1.21ng/ml, 6.2 points, and 27.8ml, respectively. All examinees were divided into MS group (46.5%, 1235 men) and non-MS group (53.5%, 1419). Age and prostate volume were significantly higher in the MS group. The patients were divided into three groups according to their ages: 40's, 50's, and over 60 years old. Prostate volume of the MS group in the younger age groups (40-49 years and 50-59 years) was significantly larger than that of the non-MS group. However, no difference was revealed in the age group of 60-69 years. No significant differences were found in the PSA level and LUTS between the MS and non-MS groups. In the multivariate regression analysis, central obesity was the strongest risk factor for the enlargement of prostate over 30ml among the metabolic components.

  • The FimH subunit of type 1-fimbriated Escherichia coli has been determined as a major cause of urinary tract infection. To produce a possible vaccine antigen against urinary tract infection, the fimH gene was genetically linked to the Itxa2b gene, which was then cloned into the pMAL -p2E expression vector. The chimaeric construction of pMALfimH/Itxa2b was transformed into Escherichia coli TB1 and its N-terminal amino acid sequence was analyzed. (omitted)

  • An electrochemical immunoassay based on osmium-hippuric acid (HA) conjugate films onto the electrode is presented for the detection of urinary HA. This is the first report on the use of the oxidative electropolymerization of 5-amino-1,10-phenanthroline (5-$NH_2$-phen) for immobilizing an antigen, osmium-conjugated HA. As a redox mediator, [Os(5-amino-1,10-phenanthroline)$_2$(4-aminomethylpyridine-HA)Cl]$^{+/2+}$ (Os-phen-HA) was successfully synthesized and electropolymerized onto the screen-printed carbon electrodes (SPCEs). The interaction between osmium-HA conjugate films and antibody-HA ($anti$-HA) was performed by cyclic voltammetry (CV) and differential pulse voltammetry (DPV). The electrical signals were linearly proportional to urinary HA in the range of 0.1-5.0 mg/mL, which is sufficient for use as an immunosensor using a cutoff concentration of 2.0 mg/mL in urine samples. The proposed electrochemical immunoassay method can be extended to various applications for detecting a wide range of different small antigens in the health care area.

A Study on Physiological Index, Anxiety and Depression by the Severity of Lower Urinary Tract Symptoms in Patients with Benign Prostatic Hyperplasia (전립선비대증 환자의 하부요로증상의 중증도에 따른 생리학적 지수 및 불안, 우울)

  • Purpose: The purpose of this study was to investigate the physiological index, anxiety and depression by the severity of lower urinary tract symptoms in patients with benign prostatic hyperplasia. Methods: This research was conducted from the 4th to the 27th of May in 2016 on112 patients with benign prostatic hyperplasia. The data were analyzed using a chi-square test, ANOVA, and Pearson Correlation Coefficients. Results: The results demonstrated a difference depending on the dysuria period of each lower urinary tract symptom, marital status, occupation and perceived health state of the patient. The physiological index by lower urinary tract symptoms showed a difference in the maximum flow rate, amount of post-void residual urine and Prostate-Specific Antigen (PSA), and anxiety and depression factors also revealed a difference. Lower urinary tract symptoms showed a positive correlation to the amount of post voided residual urine and PSA, a negative correlation to the maximum flow rate and also indicated a positive correlation to depression. Conclusion: The results of the study belonging to the moderate and severe categories were high when the result was based on the categorization of subjects with benign prostatic hyperplasia with lower urinary tract symptoms. Considering that depression levels increase as the symptom intensifies and the anxiety score is also high with mild symptoms, psychological support intervention is needed when a pattern of benign prostatic hyperplasia appears.

O serotypes of Uropathogenic Escherichia coli Isolated in Korea (비뇨기 병원성 대장균의 O 항원형 동정)

  • The O serotypes of uropathogenic Escherichia coli isolated in Korea were studied using a complete set of rabbit O antisera raised with reference O antigen type strains of E. coli. The distribution of "O" serotypes found in this survey was grossly similar with the prevalence of "O" types observed in other parts of the world, and some differences were also noted. A total of 31 "O" serotypes were identified and the most frequent serotype associated with urinary tract infections was O75(11.5%), which was followed by O6(7.4%), O10 and O40(5.7%, respectively).0 and O40(5.7%, respectively).

  • This paper describes an electrochemical immunoassay for simple, fast and quantitative detection of a urinary hippuric acid which is one of major biological indicator in toluene-exposed humans. The electrochemical system of immunoassay was based on the directly osmium complex conjugated with hippuric acid. With the competition between free hippuric acid (HA) and the osmium-hippuric acid conjugate (Os-HA) to bind with antibody hippuric acid (Anti-HA) coated onto gold nanoparticles, the electrical signals were proportional to urinary hippuric acid (HA) in the range of 0.01-5 mg/mL which is enough range to be used for in-field or point-of-care (POC) diagnosis. The proposed electrochemical method can be extended to the applications to detect a wide range of different small molecules in the field of health care.

MST1R as a potential new target antigen of chimeric antigen receptor T cells to treat solid tumors

  • Although chimeric antigen receptor T cell (CAR-T) is a promising immunotherapy in hematological malignancies, there remain many obstacles to CART cell therapy for solid tumors. Identifying appropriate tumor-associated antigens (TAAs) is especially critical for success. Using a bioinformatics approach, we identified common potential TAAs for CAR-T cell immunotherapy in solid tumors. We used the GEO database as a training dataset to find differentially expressed genes (DEGs) and verified candidates using the TCGA database, obtaining seven common DEGs (HM13, SDC1, MST1R, HMMR, MIF, CD24, and PDIA4). Then, we used MERAV to analyze the expression of six genes in normal tissues to determine the ideal target genes. Finally, we analyzed tumor microenvironment factors. The results of major microenvironment factor analyses showed that MDSCs, CXCL1, CXCL12, CXCL5, CCL2, CCL5, TGF- β, CTLA-4, and IFN-γ were significantly overexpressed in breast cancer. The expression of MST1R was positively correlated with TGF- β, CTLA-4, and IFN-γ. In lung adenocarcinoma, MDSCs, Tregs, CXCL12, CXCL5, CCL2, PD-L1, CTLA-4, and IFN-γ were significantly overexpressed in tumor tissues. The expression of MST1R was positively correlated with TGF- β, CTLA-4, and IFN-γ. In bladder cancer, CXCL12, CCL2, and CXCL5 were significantly overexpressed in tumor tissues. MST1R expression was positively correlated with TGF- β. Our results demonstrate that MST1R has the potential as a new target antigen for treating breast cancer, lung adenocarcinoma, and bladder cancer and may be used as a progression indicator for bladder cancer.
