Efficacy of Thermal Therapies in Masseter Area - Thermographic Study - (악안면부에 대한 수종 온냉요법시의 체열변화에 관한 비교연구)

  • The purpose of this study was to assess the efficacy of several thermal therapies using ice pack, moist-hot pack and ultrasound, separately and concomitantly and to obtain the background information on the vascular changes after thermophysical therapies. The author had used 15 healthy subjects were examined and the subjects were divide into 5 groups : ice pack, moist-hot pack, ultrasound, ice pack and moist-hot pack, ice pack and ultrasound. Observation were made immediate before and 0,5,10,20,30,45,60,90 minutes after treatment. Thermography was performed in an Agema 870 thermovisio with 0.1$^{\circ}C$ difference of gradual temperature shift. The results were as follows : 1. Using ice pack only, the surface temperature of the masseter region was increased lapse of time, and most remarkably 90 minutes after the treatment. 2. Using moist-hot pack only, the surface temperature of the region was remarkably increased immediately after the treatment, but decreased lapse of time. 3. Using moist-hot pack with ice pack, the surface temperature of the face was remarkably increased immediately after the treatment, and decreased lapse of time, Hyperthermia was maintained for a longer time as compared with the group of moist-hot pack only. 4. Using ultrasound only, the surface temperature of the region was increased gradually, and most remarkably 30 minutes after the treatment, but decreased in the course of time. 5. Using ultrasound combined with ice pack, the surface temperature of the region was gradually decreased until 30 minutes after the treatment, and decrease to some extend at 45 minute. And then a gradual increase observed over the remaining period of the experiment. 6. Hyperthermia were maintained for a long time in the groups using ice pack combined with moist-hot pack and ultrasound as compared with the other groups. Our data suggest that ice pack can promote the efficacy of other thermal therapies.

  • Ultrasound is known to be harmless to the human body and is widely used in obstetrics and gynecology to confirm the diagnosis and development status of fetus. Diagnosis Although long - term use of ultrasound may cause changes in body temperature, studies on the uterine temperature changes due to ultrasound have been lacking. The purpose of this study was to investigate the change of temperature according to ultrasonic scanning time using a self - produced uterine model phantom. Ultrasound equipment and a 4MHz convex probe were used to construct the uterine model phantom similar to the human uterus using acrylic and pig uterus, which are tissue equivalents. Three probe type thermometers were installed to measure the inside of the acrylic water tank, the uterus, and the atmospheric temperature. The temperature of the uterine phantom was ascertained by measuring the temperature of the subject for 6 hours, 361 times. In this study, the possibility of human body temperature elevation due to ultrasound could be confirmed and this study will be used as the basic data of ultrasonic heat absorption study.

Musculoskeletal Ultrasound Findings of Knee Pain Patients: A Retrospective Observational Study (근초음파 진단기기를 활용한 슬관절 통증 환자의 초음파 소견: 후향적 관찰연구)

  • The study aimed to understand the knee condition of patients with knee joint pain who visited an Korean Medicine hospital by performing musculoskeletal ultrasound. From July 1, 2021 to October 31, 2021, information on the gender, age, diagnosis, and ultrasound findings of knee joint pain patients were retrospectively collected. To examine the condition of the knee joint, images were taken in a total of three postures. All images were analyzed by an examiner. 7 cases of osteoarthritis, 1 case of medial collateral ligament rupture, and 1 case of quadriceps tendon calcification were collected. It was able to check the condition of articular cartilage, lipoma arborescens, and the formation of osteophytes through musculoskeletal ultrasound. The implementation of musculoskeletal ultrasound can enable Korean medicine doctors to give more active treatment for patients through accurate and scientific diagnosis. Based on more evidence, it is thought that the use of medical devices by Korean medicine doctors will need to be made in the future.

Multistage Transfer Learning for Breast Cancer Early Diagnosis via Ultrasound (유방암 조기 진단을 위한 초음파 영상의 다단계 전이 학습)

  • Research related to early diagnosis of breast cancer using artificial intelligence algorithms has been actively conducted in recent years. Although various algorithms that classify breast cancer based on a few publicly available ultrasound breast cancer images have been published, these methods show various limitations such as, processing speed and accuracy suitable for the user's purpose. To solve this problem, in this paper, we propose a multi-stage transfer learning where ResNet model trained on ImageNet is transfer learned to microscopic cancer cell line images, which was again transfer learned to classify ultrasound breast cancer images as benign and malignant. The images for the experiment consisted of 250 breast cancer ultrasound images including benign and malignant images and 27,200 cancer cell line images. The proposed multi-stage transfer learning algorithm showed more than 96% accuracy when classifying ultrasound breast cancer images, and is expected to show higher utilization and accuracy through the addition of more cancer cell lines and real-time image processing in the future.

Diagnostic Strategy of Primary Site in Metastatic Bone Tumor (전이성 골종양에서 원발병소의 진단)

  • We carried out a prospective study of the effectiveness of a diagnostic strategy in thirty consecutively seen patients who had skeletal metastasis. The diagnostic strategy consisted of the recording of a medical history, physical examination, routine laboratory analysis, plain radiography of the involved bone and chest, whole-body technetium-99m-phosphonate bone scintigraphy, abdominal ultrasound, computed tomography of the chest, abdomen and pelvis, fiberbronchoscopy and fibergastroscopy. After this evaluation, a biopsy of the most accessible osseous lesion was done in twenty four patients. On the basis of the our diagnostic strategy, we were able to identify the primary site of the malignant tumor in nineteen patients(63%). The laboratory values were non-specific in all patients. The history and physical examination revealed the occult primary site of the malignant tumor in one patient(3.3%) who had carcinoma of the breast. Plain radiographs of the chest established the diagnosis of carcinoma of the lung in three patients(9.9%). Computed tomography of the chest identified an additional three primary carcinoma of the lung(9.9%). Fiberbronchoscopy identified an additional one primary carcinoma of the lung(3.3%). Abdominal ultrasound established the diagnosis in three patients(9.9%). Computed tomography of the abdomen and pelvis established the diagnosis in four patients(13.2%). Fibergastroscopy established the diagnosis in two patients(6.6%). Examination of the biopsy tissue established the diagnosis in one patient(3.3%). So we recommend to perform plain radiographs of chest, abdominal ultrasound, chest C-T, abdomino-pelvic C-T, fiber-bronchoscopy, fibergastroscopy sequentially.

Emerging Role of Hepatobiliary Magnetic Resonance Contrast Media and Contrast-Enhanced Ultrasound for Noninvasive Diagnosis of Hepatocellular Carcinoma: Emphasis on Recent Updates in Major Guidelines

  • Hepatocellular carcinoma (HCC) can be noninvasively diagnosed on the basis of its characteristic imaging findings of arterial phase enhancement and portal/delayed "washout" on computed tomography (CT) and magnetic resonance imaging (MRI) in cirrhotic patients. However, different specific diagnostic criteria have been proposed by several countries and major academic societies. In 2018, major guideline updates were proposed by the Association for the Study of Liver Diseases, European Association for the Study of the Liver (EASL), Korean Liver Cancer Association and National Cancer Center (KLCA-NCC) of Korea. In addition to dynamic CT and MRI using extracellular contrast media, these new guidelines now include magnetic resonance imaging (MRI) using hepatobiliary contrast media as the first-line diagnostic test, while the KLCA-NCC and EASL guidelines also include contrast-enhanced ultrasound (CEUS) as the second-line diagnostic test. Therefore, hepatobiliary MR contrast media and CEUS will be increasingly used for the noninvasive diagnosis and staging of HCC. In this review, we discuss the emerging role of hepatobiliary phase MRI and CEUS for the diagnosis of HCC and also review the changes in the HCC diagnostic criteria in major guidelines, including the KLCA-NCC practice guidelines version 2018. In addition, we aimed to pay particular attention to some remaining issues in the noninvasive diagnosis of HCC.

  • Image segmentation for object to extract data from ultrasound acquired is an essential preprocessing step for the effective diagnosis. Various image segmentation methods have been studied. In this study, interactive image segmentation method by graph cut algorithm is proposed to develop a variety of applications of vascular ultrasound imaging and diagnostics. General imaging and vascular ultrasound imaging segmentation by entering constrain condition such as foreground and background. In the future it will be able to develop new ultrasound diagnostics.

Development of Ultrasound Diagnostic System for Functional Gastrointestinal Disorders using Finite Difference Method (유한차분법을 이용한 기능성 위장 장애 진단용 초음파 시스템의 개발)

  • The disaster from functional gastrointestinal disorders (FGID) has detrimental impact on the quality of life of the affected population. There are, however, rare diagnostic methods for FGID. Our research group identified recently that the gastrointestinal tract well of the patients with FGID became more rigid than that of healthy people when palpating the abdominal regions overlaying the gastrointestinal tract. The objective of the current study is, therefore, to identify feasibility of a diagnostic system for FGID based on ultrasound technique, which can quantify the characteristics above. Two-dimensional finite difference (FD) models (one normal and two rigid models) were developed to analyze the reflective characteristic (displacement) on each soft-tissue layer responded after application of ultrasound signals. Based on the results from FD analysis, the ultrasound system for diagnosis of the FGID was developed and clinically tested via application of it to 40 human subjects with/without FGID who were assigned to Normal and Patient Groups. The results from FD analysis showed that the maximum displacement amplitude in the rigid models (0.12 and 0.16) at the interface between the fat and muscle layers was explicitly less than that in the normal model (0.29). The results from actual specimens showed that the maximum amplitude of the ultrasound reflective signal in the rigid models $(0.2{\pm}0.1Vp-p)$ at the interface between the fat and muscle layers was explicitly higher than that in the normal model $(0.1{\pm}0.0Vp-p)$. Clinical tests using our customized ultrasound system showed that the maximum amplitudes of the ultrasound reflective signals near to the gastrointestinal tract well for the patient group $(2.6{\pm}0.3Vp-p)$ were generally higher than those in normal group $(0.1{\pm}0.2Vp-p)$. These findings suggest that our customized ultrasound system using the ultrasound reflective signal may be helpful to the diagnosis of the FGID.

Comparison of 19-gauge conventional and Franseen needles for the diagnosis of lymphadenopathy and classification of malignant lymphoma using endoscopic ultrasound fine-needle aspiration

  • Background/Aims: Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) using a 19-gauge needle is an efficient sampling method for the diagnosis of lymphadenopathy. This study compared 19-gauge conventional and Franseen needles for the diagnosis of lymphadenopathy and classification of malignant lymphoma (ML). Methods: Patient characteristics, number of needle passes, puncture route, sensitivity, specificity, and accuracy of cytology/histology for lymphadenopathy were analyzed in patients diagnosed with lymphadenopathy by EUS-FNA using conventional or Franseen needles. Results: Between 2012 and 2022, 146 patients met the inclusion criteria (conventional [n=70] and Franseen [n=76]). The median number of needle passes was significantly lower in the conventional group than in the Franseen group (3 [1-6] vs. 4 [1-6], p=0.023). There were no significant differences in cytological/ histological diagnoses between the two groups. For ML, the immunohistochemical evaluation rate, sensitivity of flow cytometry, and cytogenetic assessment were not significantly different in either group. Bleeding as adverse events (AEs) were observed in three patients in the Franseen group. Conclusions: Both the 19-gauge conventional and Franseen needles showed high accuracy in lymphadenopathy and ML classification. Considering sufficient tissue collection and the avoidance of AEs, the use of 19-gauge conventional needles seems to be a good option for the diagnosis of lymphadenopathy.

Transabdominal Ultrasound Assessment of the Polycystic Ovary Syndrome (다낭난소증후군 진단시 복식 초음파의 유용성에 관한 연구)

  • Objective: The objective of the study was to determine the diagnostic performance of transabdominal ultrasound by receiver operating characteristic (ROC) curve analysis, in order to evaluate the usefulness in establishing the diagnosis of polycystic ovary syndrome (PCOS). Methods: Questionnaires were given to 8,793 reproductive women reviewed at Ewha Womans University Mokdong hospital. Ultrasound examinations were performed in 701 women with a transabdominal transducer. Transabdominal ultrasounds were performed in 185 normal control women (normal menstruation without hyperandrogenism or PCO morphology) and 248 PCOS patients according to National Institutes of Health (NIH) PCOS diagnosis criteria. ROC curves were calculated for ovarian volume and follicle number. Results: In normal control group, the mean age were $23.64{\pm}4.26$ years old and the mean ovarian volume and follicle number were $6.03{\pm}1.89\;cm^3$ and $6.49{\pm}1.93$, respectively. The ovarian volume showed an area under the ROC curve (AURC) of 0.761. A ovarian volume decision threshold >$9\;cm^3$ had a sensitivity of 51.0% and a specificity of 91.4% for the diagnosis of PCOS. The follicle number showed an AURC of 0.733. A follicle number decision threshold ${\geq}9$ had a sensitivity of 54.9% and a specificity of 87.0% for the diagnosis of PCOS. A follicle number decision threshold ${\geq}10$ had a sensitivity of 53.2% and a specificity of 90.4%. A follicle number and a ovarian volume did not have a high diagnostic power for screening for PCOS. Conclusion: Our results suggest that transabdominal ultrasound assessment is not effective for the detection of PCOS in young women of reproductive age.
