Incidence of Peripheral Cyanosis in Patients with Therapeutic Hypothermia after Cardiac Arrest in the Acute Care Unit of a Tertiary General Hospital (일개 상급종합병원 응급중환자실 내 심정지 후 저체온요법을 적용한 환자의 말초 청색증 발생 현황)

  • Purpose : This study aimed to identify the incidence of peripheral cyanosis and the characteristics and clinical results of patients with therapeutic hypothermia after a cardiac arrest. Methods : Data were collected from April to June 2021 via the electrical medical records of 95 patients with therapeutic hypothermia for 72 hours after a cardiac arrest admitted to an acute care unit at a tertiary hospital between January 1, 2016, and December 31, 2019. The data were analyzed using descriptive statistics and a t-test, Mann-Whitney U test, Chi-squared test, Fisher's exact test, and logistic regression using SPSS/WIN. Results : The incidence of peripheral cyanosis was 20%. In the peripheral cyanosis group, peripheral vascular disease, fibrinogen, vasopressor, infection, disseminated intravascular coagulation, acute physiology, chronic health evaluation II score on the second hospital day, nursing intervention, and mortality on the seventh hospital day were higher. The level of fibrinogen and use of vasopressors affected the occurrence of peripheral cyanosis. Conclusion : Considering the influencing variables, careful observation is necessary for patients with high fibrinogen levels and vasopressor use. These results provide basic data to recognize the need for nursing intervention for peripheral cyanosis and encourage nurses to deliver them during therapeutic hypothermia.

Development and Evaluation of a Community based Multifaceted Cognitive Training Program for the Elderly with Mild Cognitive Impairment (경도인지장애 노인을 위한 지역사회 기반 다면적 인지훈련 프로그램 개발 및 효과평가)

  • Purpose: The purpose of this study is to develop and evaluate the effects of a multifaceted cognitive training program on cognitive function, depression, and instrumental activities of daily living (IADL) in community dwelling elders with mild cognitive impairment. Methods: A non-equivalent control group pretest-posttest design was used. The subjects were recruited from a community health center in D metropolitan city and were assigned to the experimental or control group. Weekly 50-minute session intervention was delivered to the experimental group over 12 weeks. 8 weeks and 12weeks after intervention, the changes in cognitive function, depression, and IADL in the groups were measured and compared. Data were analyzed with Mann-Whitney U test, Wilcoxon Signed Rank test, and post-hoc test with Bonferroni correction using SPSS/WIN 23.0. Results: When compared with their counterparts (n=15), the elders in the experimental group (n=12) showed significant improvement in cognitive function and depression at week 12. The mean scores of IADL at week 12 were not significantly different between the groups. Conclusion: The results indicate that this multifaceted cognitive training program is effective in improving cognitive function, depression, and avoiding deterioration of IADL among elders with mild cognitive impairment.

Mothers' Perception on Antibiotic Use for Their Children Under Six Years Old (6세 이하 아동 어머니의 항생제 사용인식)

  • Purpose: The objective of this study was to investigate mothers' perception of antibiotic use for their children. Methods: A cross-sectional survey was conducted of a convenient sample of 210 mothers who visited the two pediatric hospitals in D city. Descriptive statistics, t-test, analysis of variance, Kruskal-Wallis test, Mann-Whitney U test, and Pearson correlation coefficients with SPSS WIN 24.0 were used to analyze the data. Results: The perception of mothers on antibiotic use for their children was relatively low (3.28/5) and showed a statistically significant difference based on the mothers' educational background (${\chi}^2=15.30$, p<.001). About 57-77% of participants discontinued or reduced the dosage of the antibiotics if their children's symptoms were relieved, and about 60% of them perceived that antibiotics treat viral infections. Mothers' adherence to specific doses of antibiotics in their children was positively correlated with their knowledge and beliefs (r=.17, p=.014) and their attitude regarding the use of antibiotics (r=.17, p=.014). Only 1.4% of the participants had educational experience regarding the correct use of antibiotics. Conclusion: Based on the results, it can be concluded that an educational program for mothers should be developed and to verify its effects in order to induce proper use of antibiotics among mothers.

The mediating effect of optimism between grit and learning flow of nursing students (간호대학생의 그릿과 학습몰입과의 관계에서 낙관성의 매개효과)

  • Purpose: The purpose of this study was to investigate the mediating effect of optimism on the relationship between grit and learning flow in nursing college students. Methods: Structured self-reported questionnaires were used to measure grit, optimism and learning flow. The study was conducted on 200 nursing students in P, U and J cities between September 1 and September 20, 2020. The data were analyzed using a t-test, one-way ANOVA, Pearson's correlation coefficients and hierarchical multiple linear regression with SPSS/WIN 23.0. Results: Significant relationships were found between learning flow and grit (r=.60, p<.001), between learning flow and optimism (r=.42, p<.001), and between grit and optimism (r=.42, p<.001). Additionally, optimism had a partial mediating effect on the relationship between grit and learning flow (Z=3.11, p<.001). Conclusion: These results indicate that interventions to increase the level of grit along with optimism is necessary in order to increase the level of nursing college students' learning flow.

The Related Factors on Cervical Cancer Screening Intention among Married Immigrant Women based on the Health Belief Model (결혼이민여성의 자궁경부암 검진에 대한 건강신념, 문화적 장애성 및 자궁경부암 검진 의도의 영향 요인)

  • Purpose: The purpose of this study is to identify the factors of health beliefs, cultural barriers, and intentions of cervical cancer screening behaviors in married immigrant women and provide information for the development of intervention programs. Methods: The subjects were 207 married immigrant women living D and S cities, and G and Y counties. The data were collected from April to June 2019, using a self-report structured questionnaire that was translated into English, Chinese, Vietnamese, and Korean, and analyzed by the SPSS/WIN 24.0 program. Results: As a result of this study, it was found that the intention of cervical cancer screening for married immigrant women were high when they had a job (β=-.17, p=.014), experience of Pap testing within the past year (β=-.28, p<.001), experience of cervical cancer prevention education (β=-.18, p=.008), and a higher perceived sensitivity (β=.18, p=.016). All of these variables together explained 22% of the intention of cervical cancer screening behaviors in immigrant women married to Korean men. Conclusion: In order to increase the cervical cancer screening behaviors in married immigrant women, intervention strategies to increase perceived susceptibility and decrease cultural barriers for immigrant women should be developed.

Factors Influencing Nursing Practice for Physical Restraints among Nurses in the Intensive Care Unit (중환자실 간호사의 신체보호대 간호수행의 영향요인)

  • Purpose : This study aimed to identify the factors influencing physical restraint-related practice among nurses working in the intensive care unit (ICU). Methods : The participants consisted of 169 ICU nurses in three general hospitals in B and U cities. Data were collected from December 2021 to January 2022 through a structured self-administered questionnaire. Demographic characteristics, physical restraint-related practice, Person-centered Critical Care Nursing (PCCN), work environment, and attitudes toward physical restraint use were measured. For data analyses, hierarchical multiple regressions were conducted using SPSS/WIN 25.0. Results : Clinical careers in the ICU, better work environments, higher PCCN levels, and more positive attitudes toward physical restraint use were associated with a better practice of physical restraints, which together explained 35.5% of the total variance of the outcome. Conclusion : Our findings suggest that to promote a safe physical restraint-related practice among ICU nurses, it is important to improve the nursing environment, prepare guidelines for applying PCCN, and provide education for endorsing positive attitudes toward the use of physical restraints.

Effect of Moxibustion at Junggeuk(CV3), Singwol(CV8) on Women's Urinary Incontinence and Quality of Life (중극(中極), 신궐(神厥) 뜸요법이 여성의 요실금과 삶의 질에 미치는 효과)

  • Objectives : This study is to examine the effects of moxibustion at Junggeuk(CV3), Singwol(CV8) on women's urinary incontinence and quality of life. The study is a research based on the non-equivalent control group pretest-posttest design. Methods : The subjects of the study were 45 women who were using either of 2 health clinics located in Ulsan. They were sampled in accordance with predetermined standards and then divided into the experimental and control groups, respectively consisting of 22 and 23 members. In treating those women through moxibustion, this researcher applied 2 sheets of 'shingigu' to each of the women on 40 min, per time basis. This experiment was conducted 6 times in total for 2 weeks on every two day basis. Data from all of the measurements were statistically processed and analyzed using SPSS/WIN 12.0. Demographic characteristics of the subjects were examined and indicated in frequency and percentage. The homogeneity test of the two groups was conducted using $x^2$-test, Fisher's exact test, t-test and Mann-Whitney U Test. Hypotheses set for the study were verified through t-test and Mann-Whitney U Test. Results : The experimental group who take moxibustion will be lower in the severeness of urinary incontinence than the control group who do not was supported(p<0.001). 2. The experimental group who take moxibustion will be higher in score for life quality than the control group who do not was supported(U=42.00, p<0.001). Conclusions : These findings suggest that moxibustion can be an effective intervention for women having urinary incontinence because it treats the symptom with no adverse effect and it is non-invasive and easily applicable.

The Relationships between the Empowerment, ursing Performance, Job Satisfaction and Turnover Intention of Long-term Care Hospital Nurses (노인요양병원 간호사의 임파워먼트와 간호업무성과, 직무만족 및 이직의도와의 관련성)

  • The purpose of this study is to explore the relationships between the empowerment, nursing performance, job satisfaction and turnover intention of long-term care hospital nurses. The subjects were 258 nurses at a long-term care hospital in B and U city. The data were collected from November 23, 2012 to January 21, 2013. Data were analyzed by SPSS win 20.0 program. The mean score of empowerment was 3.22, nursing performance was 3.78, job satisfaction was 3.25 and turnover intention was 2.87. Empowerment was correlated with nursing performance and job satisfaction positively, but correlated with turnover intention negatively. As a result of multiple regression, empowerment has 50.3% of the explanatory power on job satisfaction, 13.4% on nursing performance, and 21.9% on turnover intention. The significant predictive factors of nursing performance and job satisfaction were information and support in empowerment. In addition, turnover intention was resources in empowerment. These results suggest that empowerment improvement programs should be focused on developments to improve the quality of nursing.

Level of physical activity and influencing factors of the first and second year nursing students (간호대학 저학년생의 신체활동정도와 영향요인)

  • This study was conducted to investigate the level of physical activity and identify factors influencing physical activity of first and second year nursing students. The participants were 234 female first and second year nursing students at one nursing college located in U city. Data were collected from September 5 to 15 in 2016 and analyzed by descriptive statistics, t-test, ANOVA, $Scheff{\breve{e}}$ test, Pearson's correlation coefficient, and stepwise multiple regression using SPSS/WIN 22.0. The average level of physical activity measured by the Korean version of the Godin-Shephard Liesure-Time Exercise Questionnaire was 9.09(range 0-80). According to the physical activity group classified by leisure score index, there were 25 students(10.7%) in the active group, 31 students (13.2%) in the moderately active group, and 178 students(76.1%) in the insufficiently active group. Factors influencing the level of physical activity were intention to participate in physical activity and habit to physical activity, which together explained 21.2% of the total variance in physical activity. Therefore, this study suggests that it is necessary to develop and apply a physical activity program enhancing intention and habit to physical activity for first and second year nursing students.

Effect of Elderly People's Health Promotion Program on Physical, Physiological and Psychological Parameter for in Senior Center Participation of the Elderly (노인 건강증진프로그램이 경로당 이용 노인의 신체적, 생리적 및 심리적 지수에 미치는 영향)

  • The purpose of this study was to examine the effects of an elderly people's health promotion program performed in a senior center located in J-si. Most interesting were the effects on the physical, physiological, and psychological parameters for senior center participation of the elderly. A nonequivalent control group pre-posttest design was employed. A total of 41 people living in J-si were assigned to either an experimental group (n=21) or control group (n=20). The experimental group was provided with the eight-week elderly people's health promotion program from May 1 to September 2 in 2019. The data were analyzed using SPSS/WIN 24.0 using descriptive statistics, 𝒙2-test, Fisher's exact test, and t-test. The results indicated a significant difference between the experimental group and control group with respect to the scores of the physical parameters, including right grip strength (t=-3.01, p=.004) and flexibility of the waist (t=-2.11, p=.09); the physiological parameters, including the systolic blood pressure (t=-3.02, p=.004); and psychological, including depression (t=2.42, p=.020), hopelessness (t=5.22, p<.001), and quality of life (t=4.28, p<.001). Therefore, the elderly people's health promotion program with senior center participation of the elderly is a useful nursing intervention program that can improve the right grip strength, flexibility waist, reduce the systolic blood pressure, depression and hopelessness, and improve the quality of life.
