종양 발생 과정에 관여되고 있는 분자 생물학적 기전을 직접 공격해 보자고 하는 치료 방침은 암 치료에 있어서 아주 유망한 치료방법의 하나로 인정되고 있다. Epidermal growth factor receptor (EGFR) 수용체에 여러 ligands가 결합하게 되면 발암 단계에서부터 암의 진행 과정과 전이 과정 그리고 방사선에 대한 저항성과 관련된 여러 가지중요한 신호전달체계를 활성화시킨다. 특히 진행된 두경부 암 환자들에서 EGFR이 과발현 된 경우에는 매우 불량한 예후를 나타내고 있기 때문에 이러한 signaling pathway의 selective targeting을 위한 많은 임상 시도가 이루어지고 있다. 현재까지 알려진 표적치료 항암제로는 크게 EGFR에 대한 monoclonal antibody와 tyrosin kinase Inhibitors로 대별될 수 있는데 이와 같은 약제들은 여러 xenograft에서 고무적인 실험 결과들이 입증되어 곧 바로 임상 현장에서 적용되고 있다. 그러나 기대와는 달리 EGFR Inhibitor 단독으로 치료한 초기 임상연구 결과들을 보면 극히 소수의 환자에서만 미미한 효과를 나타내고 있고, 방사선 치료와의 병용치료에서도 괄목할만한 항암 효과를 보여주지 않고 있다. 그럼에도 불구하고 많은 실험적 데이터로부터 여러 가지 생물학적 이점이 밝혀져 있고 또 미래 지향적인 치료법의 하나로 각광을 받고 있기 때문에 현재 많은 연구자들은 어떤 환자 군에서 이러한 표적 치료가 도움이 될 것이며, 방사선 치료 또는 항암 치료와는 어떤 방식으로 조합할 것인지, 또 그 순서는 어떻게 할 것이며, 또 환자 선정에 있어 reliable marker는 무엇인지, 어떻게 체내에서 신호 전달체계의 효과적인 차단을 확인할 수 있겠는지, 또한 multiple targ리ed therapy가 필 요하도록 하는 targeted agent에 대 한 Intrinsic 또는 acquired resistance의 기전은 무엇인지 등등, 현재 당면하고 있는 많은 문제점을 규명하고자 노력하고 있다. 특히 EGFR-signaling pathway를 표적으로 하는 표적 지향적 방사선 치료를 위한 translation research의 적절한 모델이 되고 있는 두경부 암 환자에서 이러한 제반 문제점을 해결하기 위해서는 더 많은 임상 연구와 함께 well-Integrated laboratory clinical research program이 필요할 것으로 생각된다 또한 EGFR antagonist 외에도 anglogenlc pathway나 cell-cycle pathway를 표적으로 하는 새로운 약제들이 계속 개발되고 있고 이에 관한 연구가 활발히 진행 중이다. 따라서 이 고찰에서는 두경부 암 환자에서 이러한 약제들을 방사선 치료와 병용하였을 때의 임상 연구 결과들을 재검토해 보고 부가적으로 EGFR blockade에 따르는 내성 문제 그리고 방사선 치료를 병용하면서 여러 표적을 동시에 차단시키는 multiple-targeted therapy의 개발 현황을 간략히 소개하고자 한다
CD11c와 CD80 및 CD86과 같은 보조 수용체는 주로 수지상 세포에서 발현되는 세포 표지 인자이다. 본 연구에서는 KG-1, HL-60, NB4 및 THP-1 세포와 같은 여러 종류의 백혈병 세포를 이용하여 이들 세포에 재조합 Flt-3 리간드를 처리하였을 때 수지상 세포의 표면 인자인 CD11c의 발현에 어떠한 변화가 있는가를 조사하였다. KG-1 세포뿐만 아니라 NB4세포와 HL-60 세포에서도 Flt-3 수용체가 발현됨을 확인하였으나 THP-1 세포에서는 이들 수용체가 발현되지 않았다. KG-1 세포를 Flt-3 리간드나 granulocyte macrophage-colony stimulating factor (GM-CSF)와 tumor necrosis factor (TNF)-$\alpha$를 섞은 배양액에서 배양하였을 때 세포 증식은 억제되었으며 CD11c 발현은 현저히 증가되었다. 그러나 Flt-3 리간드를 처리한 KG-1세포에서는 GM-CSF와 TNF-$\alpha$를 처리한 세포에서와는 다르게 major histocompatibility complex (MHC)-I 및 MHC-II의 발현은 증가되지 않았다. Flt-3 리간드는 HL-60 세포와 NB4 세포의 CD11c 발현도 증가시켰으나 THP-1 세포에서는 아무런 영향이 없었다. CD11c의 발현과 비교하여 CD11b의 발현은 Flt-3 리간드에 의하여 KG-1 세포에서는 약하게 증가하였으나 NB4 세포와 HL-60 세포에서는 증가되지 않았다. KG-1 세포를 Flt-3 리간드로 처리하였을 때 extracellular signal-regulated kinase-1/2 (ERK-1/2)와 p38-mitogen-activated protein kinase (p38-MAPK)의 단백질 인산화가 증가되었으며 Flt-3 리간드에 의한 CD11c 발현의 증가는 MEK의 억제제인PD98059에 의하여 사라짐을 확인하였다. 본 연구 결과는 Flt-3 수용체 리간드의 처리에 의하여 $CD34^+$ myelomonocyte분화 단계인 KG-1 세포와 promyelocyte 분화 단계의 백혈병 세포에서 수지상 세포와 유사한 세포 형으로 분화된다는 것을 보였고 Flt-3 수용체 리간드에 의한 이들 백혈병 세포의 수지상 세포유사 세포로의 분화는 ERK-1/2의 활성화에 의하여 일어날 수 있음을 보여 준다.
R-type $Ca_v2.3$ high voltage-activated $Ca^{2+}$ channels in peripheral sensory neurons contribute to pain transmission. Recently we have demonstrated that, among the six $Ca_v2.3$ isoforms ($Ca_v2.3a{\sim}Ca_v2.3e$), the $Ca_v2.3e$ isoform is primarily expressed in trigeminal ganglion (TG) nociceptive neurons. In the present study, we further investigated expression patterns of $Ca_v2.3$ isoforms in the dorsal root ganglion (DRG) neurons. As in TG neurons, whole tissue RT-PCR analyses revealed the presence of two isoforms, $Ca_v2.3a$ and $Ca_v2.3e$, in DRG neurons. Single-cell RT-PCR detected the expression of $Ca_v2.3e$ mRNA in 20% (n=14/70) of DRG neurons, relative to $Ca_v2.3a$ expression in 2.8% (n=2/70) of DRG neurons. $Ca_v2.3e$ mRNA was mainly detected in small-sized neurons (n=12/14), but in only a few medium-sized neurons (n=2/14) and not in large-sized neurons, indicating the prominence of $Ca_v2.3e$ in nociceptive DRG neurons. Moreover, $Ca_v2.3e$ was preferentially expressed in tyrosine-kinase A (trkA)-positive, isolectin B4 (IB4)-negative and transient receptor potential vanilloid 1 (TRPV1)-positive neurons. These results suggest that $Ca_v2.3e$ may be the main R-type $Ca^{2+}$ channel isoform in nociceptive DRG neurons and thereby a potential target for pain treatment, not only in the trigeminal system but also in the spinal system.
We employed human SK-MEL-28 cells as a model system to identify cellular proteins that accompany N-(4-methyl)phenyl-O-(4-methoxy)phenyl-thionocarbamate (MMTC)-induced apoptosis based on a proteomic approach. Cell viability tests revealed that SK-MEL-28 skin cancer cells underwent more cell death than normal HaCaT cells in a dose-dependent manner after treatment with MMTC. Two-dimensional electrophoresis in conjunction with matrixassisted laser desorption/ionization-time of flight (MALDI-TOF) mass spectrometry analysis or computer matching with a protein database further revealed that the MMTC-induced apoptosis is accompanied by increased levels of caspase-1, checkpoint suppressor-1, caspase-4, NF-kB inhibitor, AP-2, c-Jun-N-terminal kinase, melanoma inhibitor, granzyme K, G1/S specific cyclin D3, cystein rich protein, Ras-related protein Rab-37 or Ras-related protein Rab-13, and reduced levels of EMS (oncogene), ATP synthase, tyrosine-phosphatase, Cdc25c, 14-3-3 protein or specific structure of nuclear receptor. The migration suppressing effect of MMTC on SK-MEL-28 cell was tested. MMTC suppressed the metastasis of SK-MEL-8 cells. It was also identified that MMTC had little angiogenic effect because it did not suppress the proliferation of HUVEC cell line. These results suggest that MMTC is a novel chemotherapeutic and metastatic agents against the SK-MEL-28 human melanoma cell line.
Background: Inoperable and metastatic hepatocellular carcinoma (HCC) is associated with a poor prognosis and low chemotherapeutic efficiency. Sorafenib is an oral multi-kinase inhibitor exerting its effects via the RAF/MEK/ERK pathway, vascular endothelial growth factor receptor (VEGFR) and platelet derived growth factor receptor beta (PDGFR-${\beta}$) tyrosine kinases. Randomized studies have shown a significant contribution of sorafenib to life expectancy and quality of life of cancer patients. The aim of the present study is to evaluate the efficacy and side effects of sorafenib therapy in Turkey. Materials and Methods: Data for 103 patients (82 males, 21 females) receiving sorafenib therapy in 13 centers from February 2008 to December 2012 were evaluated. Median age was 61 years and median ECOG performance status was 1 (range: 0-2). 60 patients (58%) had hepatitis B, 15 patients (15%) had hepatitis C infection and 12 patients (12%) had a history of alcohol consumption. All of the patients had Child scores meeting the utilization permit of the drug in our country (Child A). Results: A total of 571 cycles of sorafenib therapy were administered with a median of four per patient. Among the evaluable cases, there was partial response in 15 (15%), stable disease in 52 (50%), and progressive disease in 36 (35%). Median progression-free survival was 18 weeks and median overall survival was 48 weeks. The dose was reduced only in 6 patients and discontinued in 2 patients due to grade 3-4 toxicity, 18 patients (17%) suffering hand-foot syndrome, 7 (7%) diarrhea, and 2 (2%) vomiting. Conclusions: This retrospective study demonstrated better efficacy of sorafenib therapy in patients with advanced HCC compared to the literature while progression-free survival and overall survival findings were comparable. The side effect rates indicate that the drug was tolerated well. In conclusion, among the available treatment options, sorafenib is an efficient and tolerable agent in patients with inoperable or metastatic HCC.
