A Case Report of Autism Spectrum Disorder Treated by Korean Medicine (한방치료로 호전된 자폐스펙트럼장애 1례 보고)

  • Objectives The purpose of this study is to report a case of one autism spectrum disorder child who was treated by Korean medicine treatment. Methods The subject was a male child with autism spectrum disorder. This patient was treated with oriental herbal medicine and acupuncture. The improvement was observed by K-CARS. Results Korean medicine treatment relieved an autism spectrum disorder child's symptoms. For example, emotional excitement, hyperactivity disorder and repetition behavior are improved. K-CARS score at the initial stage of the treatment was 48 points, which can be considered as severe autistic. After 27 months of the treatment, the K-CARS was 26 points which is not autistic. There was no side effect reported. Conclusions This study showed that Korean medicine can be an effective treatment option for autism spectrum disorder.

Review of Traditional Chinese Medicine Clinical Studies on Tuina Treatment of Children's Autism Spectrum Disorder (소아 자폐스펙트럼 장애의 추나 치료에 대한 중의학 임상 연구 고찰)

  • Objectives The purpose of this study is to investigate the effect of Tuina treatment for children with Autism Spectrum Disorder (ASD), and to propose guidance to future studies or application in clinical setting. Methods The articles published from year 2000 to May 2021 by China National Knowledge Infrastructure (CNKI) were searched by setting up field as '中醫學', '中藥', '中西醫結合' and by setting up keyword as '小儿自閉症', '儿童自閉症', '自閉症譜系障碍', 'Autism', 'Autism spectrum disorder' that corresponds to participant and '推拿', '導引', '按摩', 'Tuina', 'massage'. Results Nine selected articles were analyzed and summarized by authors and year published, characteristics, diagnostic criteria, treatment methods and contents, treatment duration, evaluation criteria and results of the study. The articles were also reviewed by reported side effects, and evaluated the quality of the study according to Cochrane Handbook's Risk of Bias 2 (RoB 2) assessment of the Randomized Control Trial (RCT) articles. All studies used Tuina treatment to achieve effective results for pediatric autism spectrum disorder. Conclusion Pediatric Tuina treatment (TM) is a non-invasive treatment that does not stimulate patients with unstable ASD but increase patient adherence, and can be safely and simply operated. Therefore, pediatric Tuina treatment is worth utilizing as a clinical basis for further development of ASD treatment.

  • Autism is a life-long neurodevelopmental disorder characterized by qualitative abnormalities in reciprocal social interactions and patterns of communication. Patients with autism are difficult to manage during dental treatment. Thus they need special consideration like physical restraint, conscious sedation or general anesthesia. A 5-year-old male dental patient with autism was reffered to dental treatment under conscious sedation using intramuscular midazolam that creats anterograde amnesia. Dental procedure using midazolm which cause anterograde amnesia can be effective treatment strategy in autism patient.

Review of Clinical Studies for Oriental Treatment of Autism in Traditional Chinese Medicine (자폐증 한방 치료에 대한 최신 임상 연구 고찰 - 중의학 논문 중심으로 -)

  • Objectives The purpose of this review is to investigate the clinical studies of oriental treatment for Autism in China and seek the better methods to study and treat Autism in Korea. Methods We searched for the study at CNKI by keywords, 'Autism', '孤獨症', '自閉症', in the last 5years. Results Selected 14 clinical studies were divided into 6 case reports and 8 control studies. The subjects of clinical study were the average 4-9 years old children. It was described that the symptoms were improved after treatment in 6 case reports. 8 control studies reported that the experimental group were better effective than the comparison group. Frequently used method for treatment was Scalp acupuncture. Conclusions It is promising that we could develop our own treatment and studies for autism in Korea by using Scalp acupuncture, based on these clinical studies.

Sensibility Estimation Analysis about Treatment Circumstance of Children with Autism (자폐성 장애아동 치료환경에 대한 감성평가분석)

  • This study verified a hypothesis "treatment circumstance that considers character of children with autism has an effect on emotion stability", and I set the final aim to suggest emotional design elements according to treatment program and also to construct basic data about study of treatment circumstance of children with autism. So I conduct a survey of teachers of nine centers for the disabled about "satisfaction about treatment circumstance, importance of design elements of treatment circumstance that increase children's with autism stability, sensibility estimation about treatment circumstance" and committed statistical analysis. As the results, emotional design elements that have to be improved are the wall finishing material, the color of the wall, and the artificial lighting. The color of the wall has an effect on importance of treatment circumstance and emotional design elements influence 47% to satisfaction of treatment circumstance. It get the positive result about the hypothesis "treatment circumstance that considers character of children with autism has an effect on emotion stability".

Use of Midazolam Intranasal Spray for Dental Treatment of Autism Patients (자폐증 환자의 행동조절을 위한 Midazolam Intranasal Spray 사용사례)

  • Autism patients in general have bad oral hygiene. It is hard for autism patients to get dental treatment as poor communication. Therefore, they may have to be treated by deep sedation or general anesthesia in numerous cases. However, this process requires induction with mask, so it is not easy to do for disobliging autism patient. Midazolam is a water-soluble benzodiazepine, has been used in pediatric dentistry or dentistry for the handicapped because of rapid onset. Midazolam can be administered through oral, rectal, intramuscular,intravenous, and intranasal (IN) routes. IN route of midazolam may be considered as effective way to allay for uncooperative autism patients before general anesthesia. In this case report, two autism patients required dental treatment. Intranasal spray of midazolam before general anesthesia was safe and effective procedure of behavioral management.

Clinical Trial Study on Acupuncture for Autism Based on the Current Traditional Chinese Medical Journals (자폐증의 침(鍼)치료에 대한 임상 연구 고찰 -최근 중의학 임상 연구를 중심으로-)

  • Objectives : The purpose of this study is to investigate the current acupuncture treatment of autism. Methods : 13 Chinese clinical studies published in the last five years were selected for analyzing. Results : Treatment methods were used; massage of acupoints, electro acupuncture, scalp acupuncture, the general acupuncture and special acupuncture treatments for autism as Jin's three needling, Intelligence-increasing acupuncture. Commonly used acupoints were sishencong(四神總), shenting(神庭), benshen(本神), yintang(印堂), naohu(腦戶), naokong(腦空), bohui(百會), neiguan(內關), yanyu1qu(言語1區), yanyu2qu(言語2區), yanyu3qu(言語3區). 10 studies were showed the effects of acupuncture with total improvement rate and 7 of them were higher than 80%. Also, most of the studies showed the acupuncture treatment was extremely effective in improving autism children's speaking ability. Conclusions : Acupuncture have been shown as an effective treatment on autism. Further studies about various treatment using by acupuncture and the evaluation standard would be needed.

  • Autism or autism spectrum disorder (ASD) is a behavioral syndrome characterized by persistent deficits in social interaction, and repetitive patterns of behavior, interests, or activities. The gene encoding Methyl-CpG binding protein 2 (MeCP2) is one of a few exceptional genes of established causal effect in ASD. Although genetically engineered mice studies may shed light on how MeCP2 loss affects synaptic activity patterns across the whole brain, such studies are not considered practical in ASD patients due to the overall level of impairment, and are technically challenging in mice. For the first time, we show that hippocampal MeCP2 knockdown produces behavioral abnormalities associated with autism-like traits in rats, providing a new strategy to investigate the efficacy of therapeutics in ASD. Ketamine, an N-Methyl-D-aspartate (NMDA) blocker, has been proposed as a possible treatment for autism. Using the MeCP2 knockdown rats in conjunction with a rat model of valproic acid (VPA)-induced ASD, we examined gene expression and ASD behaviors upon ketamine treatment. We report that the core symptoms of autism in MeCP2 knockdown rats with social impairment recovered dramatically following a single treatment with ketamine.

Review of Pathogenesis, Pattern Differentiation, Treatment Principle, Formula of Traditional Chinese Medicine for Autism Spectrum Disorder (자폐 스펙트럼 장애의 한의학적 병인, 변증, 치법, 처방에 대한 문헌적 고찰 -중의학 논문을 중심으로-)

  • Objectives The purpose of this study is to provide evidence of pathogenesis, pattern differentiation, treatment principle, formula of traditional Chinese medicine for autism spectrum disorder by reviewing journal articles published in China. Methods We searched literatures dated up to 8 January, 2018 in China National Knowledge Infrastructure (CNKI), and evaluated pathogenesis, pattern differentiation, treatment principle, formula of traditional Chinese medicine for autism spectrum disorder. Results Thirty four studies were selected for analysis. These studies included pathogenesis, pattern differentiation, treatment principle, formula of traditional Chinese medicine for autism spectrum disorder. The most common pattern differentiations were the pattern of kidney essence depletion, pattern of phlegm confounding the orifices of the heart, pattern of dual vacuity of the heart and spleen, pattern of effulgent heart-liver fire, pattern of liver failing to course freely. The most common treatment principles were supplementing the kidney, transquilizing, fortifying the spleen, transforming phlegm, opening the orifices, and calming the liver. The most commonly-used formulas were Yukmijihwangtang (六味地黃湯), Modified Ondamtang (加味溫膽湯), Guibitang (歸脾湯), Yangsimtang (養心湯), Jwaguihwan (左歸丸), Cheonmagudeungeum (天麻鉤藤飮), and Danchisoyosan (丹梔逍遙散). Conclusions This study shows the latest trend of pathogenesis, pattern differentiation, treatment principle, formula of traditional Chinese medicine for autism spectrum disorder. Further study is needed to solidify these findings.

A Comparison between Korean and Chinese Clinical Studies for the Treatment of Autism Spectrum Disorder (자폐 스펙트럼 장애에 대한 한국과 중국의 한방치료 연구 동향 비교)

  • Objectives The purpose of this study is to figure out the recent trend of the treatment for autism spectrum disorder (ASD) by comparing Korean and Chinese clinical studies. Methods National Digital Science Library (NDSL), Oriental medicine Advanced Searching Integrated System (OASIS), Research Information Sharing Service (RISS) and Korean Traditional Knowledge Portal (KTKP) were used to search Korean studies which were published from January, 2011 to May, 2017. Also Chinese National Knowledge Infrastructure (CNKI) and Wanfang data were used to search Chinese studies which were published from the same period. Key words of 'Autism' and 'Autism spectrum disorder' were used. Results 3 Korean studies and 21 Chinese studies were selected and analyzed to find out the most commonly used diagnostic criteria, treatments, including herbal medicine and acupuncture, and treatment assessment procedures. Conclusions As a result of comparing Korean and Chinese clinical studies for the treatment of ASD, both Korean and Chinese medicine treatments showed their effectiveness. However, there were some differences between two countries' clinical trends. In order for this study to be helpful, more highly evidenced clinical studies should be followed.
