A Long Way from Transfer to Transition: Challenges for Pediatric and Adult Nephrologists

  • Significant advances in the diagnosis and medical care of children with chronic kidney disease (CKD) are major reasons for the better survival rates of children and adolescents with CKD than the survival rates reported in previous decades. These patients are reaching adulthood, and therefore require a transition to adult medical care. This transition phase is well-recognized to be associated with considerably increased morbidities and medical problems, such as non-adherence, graft loss after transplantation, and loss to follow-up. Low adherence increases morbidity and medical complications and contributes to poorer qualities of life and an overuse of the health care system. However, these tragic outcomes may be avoidable through a structured and well-defined transition program. In the last decade, there has been increasing interest to resolve these medical and psychological problems that occur during the transfer of young adult patients from pediatric to adult renal units. The aims of a successful transition from pediatric to adult medical care include enhancing the individual development of better health-competence and stabilizing, or even improving, the state of health. This review will focus on various aspects of the transition phase of adolescents who have CKD or who underwent kidney transplantation from pediatric to adult nephrology care.

Predictors of Stage of Change for Exercise Behaviors among Middle Aged Women based on the Tanstheoretical Model (범이론적 모형(Transtheoretical Model)에 근거한 중년여성의 운동행동 변화단계 예측요인)

  • Purpose: The study was performed to investigate the predictors associated transition with each stage of exercise behavior based to the Transtheoretical Model, and to provide basic data for exercise behavior programs for middle aged women. Methods: The subjects consisted of 434 women residing in B city by convenience sampling. The collected data were analyzed with multiple logistic regression using SPSS/WIN program. Results: The predictors of transition from precontemplation to contemplation were consciousness raising(OR=2.095, 95% CI: 1.305-3.363), Reinforcement management(OR= 1.903, 95% CI: 1.107-3.271), Stimulus control (OR=2.176, 95% CI:1.239-3.820), Menstrual status (OR=5.327, 95% CI: 1.110-25.562). The predictors of transition from contemplation to preparation were Helping relationship(OR= 1.671, 95% CI: 1.065-2.662), Pros(OR=2.500, 95% CI: 1.254-4.982), Perceived Health status (OR=.178, 95% CI: .041-.770). The predictor of transition from preparation to action was Stimulus control(OR=1.918, 95% CI: 1.103- 3.336). The predictors of transition from action to maintenance were Consciousness raising(OR= 1.939, 95% CI: 1.031-3.647), Dramatic relief (OR=2.249, 95% CI: 1.025-4.931). Conclusions: Adequate examination on the factors, which can predict the transitional stages of change exercise behaviors among middle aged women, which is presented in this study. The results of this study will become the pillar of exercise intervention program planning and application.

  • Flight modes of automatic flight control system for light general aviation flight deck. Garmin G1000, were realized using Stateflow. In developing aircraft, it is difficult to verify the operational flight program in particular branch statement because developer can not see any visual logical steam. So, Stateflow has been used to visualize logical stream, transition from one flight mode to another flight mode. The performance was approved by applying flight mode transition conditions of G1000 to proposed transition system that is composed of states, switches, events, data and transition conditions. DURMI-II was used as 6-DOF simulation model.

Bayesian Analysis of Binary Non-homogeneous Markov Chain with Two Different Time Dependent Structures

  • We use the hierarchical Bayesian approach to describe the transition probabilities of a binary nonhomogeneous Markov chain. The Markov chain is used for describing the transition behavior of emotionally disturbed children in a treatment program. The effects of covariates on transition probabilities are assessed using a logit link function. To describe the time evolution of transition probabilities, we consider two modeling strategies. The first strategy is based on the concept of exchangeabiligy, whereas the second one is based on a first order Markov property. The deviance information criterion (DIC) measure is used to compare models with two different time dependent structures. The inferences are made using the Markov chain Monte Carlo technique. The developed methodology is applied to some real data.

Predictors of Transition in the Stage of Change for Smoking Cessation of Male University Students (남자 대학생의 금연 변화 단계 이행에 대한 예측 요인)

  • Purpose: This study was conducted to identify predictors of the stage of change for smoking cessation of male university students on the basis of the Transtheoretical model (TTM). Methods: The 388 current smokers or exsmokers who agreed to participate were recruited from three areas in Korea from August 2 to September 5, 2006. Data was analyzed using a SPSS program for descriptive statistics, ANOVA, and multiple logistic regression. Results: Most subjects (76.8%) were current smokers. According to stages of change, there were statistically significant differences in self efficacy, smoking temptation, decisional balance (cons, pros), and processes of change. The predictors of transition from contemplation to preparation were behavioral and experiential processes. The predictors of transition from preparation to action were pros of smoking and experiential process. The predictors of transition from action to maintenance were cons of smoking and behavioral process. Conclusion: Specific nursing interventions based on stages of change need to be developed for smoking cessation of male university students.

  • Technology transition from defense technology to weapon system is an important process for defense acquisition program. Many countries such as USA, UK, Australia and Republic of Korea use technology readiness level (TRL) as a tool for technology transition by identifying critical technology elements (CTEs) and assessing the technology maturity. In this paper we review a transition process for the defense acquisition. Then we suggest a method to evaluate system's TRL using each component TRL and integration readiness level (IRL) between each technologies. We apply the method to an ACTD project. A result show that technology maturity is influenced by integration between technologies.

Study on Transient Improvement through Governor Control under Asynchronous Transition of CTTS (CTTS의 비동기 절체 시 조속기 제어를 통한 과도 개선에 관한 연구)

  • This paper derives the problems that occur when asynchronous transfer in case of phase, frequency, voltage between the emergency generator and the grid and proposed the countermeasure to solve this problem when the transfer switch replace ATS(Automatic Transfer Switch) with CTTS(Closed Transition Transfer Switch) for the non-interrupting switching. In order to simulate above cases, modelling was used the transient analysis program PSCAD/EMTDC. By using this, the customer installed emergency generator and the grid was implemented. We compared three cases of asynchronous transition based on the basic case and proposed improvement by controlling the governor of emergency generator.

The Automation of VOD Content Posting by Detection Black Frame of Broadcasting Program (방송프로그램 블랙프레임 검출을 통한 VOD 콘텐츠 자동생성)

  • As the nonlinear viewing patterns are getting generalized, the needs for VOD service of consumers are increasing in TV, webTV. But today's VOD system is all automated, except the part for posting contents. This study proposes the method for automation of the content posting. For this we analyzed the broadcast program structures by genre in order to detect the transition point between the advertisement and the contents. It was found that regular black frames were set in the transition during fade in and out. We propose an efficient approach to automatically detect the black frames using RGB values of each frame that enable VOD content posting and replace an advertisement in VOD service.

NONTHERMAL BROADENING OF UV LINES OBSERVED AT THE LIMB OF THE QUIET SUN

  • We have done a spectroscopic study of the solar transition region using high resolution UV & EUV data obtained by SUMER(Solar Ultraviolet Measurements of Emitted Radiation) on board SOHO(Solar and Heliospheric Observatory). Optically thin and conspicuous emission lines observed at the solar limb are carefully selected to acquire average values of physical parameters for the quiet region as a function of radial distance. Our main results found from the present study can be summarized as follows. 1) Nonthermal velocities estimated from various UV lines do not decrease with height at least within one total line intensity scale height above the limb. 2) Nonthermal velocity distribution with temperature is very similar to that of the disk center, in the sense that its peak is located around $2{\times}10^5 K$, but the value is systematically larger than that of the disk. 3) It is found that nonthermal velocity is inversely proportional. to quadratic root of electron density up to about 10 arc seconds above the limb, i.e. ${\xi}\~N_e^{-1/4}$, implying that the observed nonthermal broadening can be attributed to Alfven waves passing through the medium. 41 Electron density estimated from the O V 629/760 line ratio is found to range from about $1{\times}10^{10}cm^{-3}$ to $2{\times} 10^{12}cm^{-3}$ in the transition region.
