대퇴부 거동 해석 및 복합재료 보철물 설계 (Behavior Analysis of the Treated Femur and Design of Composite Hip Prosthesis)

  • 무시멘트 인공 고관절 전치환술 후, 복합재료 스템을 갖는 대퇴골의 장기 거동과 인공 대퇴 보철물의 설계 성능을 분석하기 위하여 비선형 유한요소 프로그램이 개발되었다. 한 발로 서 있을 때의 관절 접촉 하중과 근육하중이 적용되었고, 816개의 brick요소를 갖는 타원형 단면의 복합재료 스템으로 치환된 대퇴골이 3차원 유한요소로 모델링 되었다. 프로그램을 사용하여 대퇴골의 밀도 변화, 응력분포, 상대미소운동이 plate cut과 bend mold와 같은 제작 방법에 대한 스템의 적층 각도 변경에 따라서 평가되었다. 결과는 코발트 크롭 합금, 티타늄 합금, 스테인레스 강과 같은 금속 재료보다 AS4/PEEK, T300/976과 같은 복합재료가 적은 골 흡수를 보였다. 대퇴골 보철물의 장기 안정성 증대는 적당한 복합재료의 적층과 적층 각도의 선택에 의하여 얻어질 수 있었다.

X-ray영상과 CAD를 이용한 인공고관절의 마모 및 식립각 측정법 (Wear and Implantation Tilt Measurements using X-ray and CAD)

  • 고관절 시행에서 식립각도는 추후 환자의 운동범위나 힘의 분배조건에 따라 폴리에틸렌 라이너의 마모에도 영향을 미친다. 인공관절요소로부터 마모입자는 골괴사나 다른 생화학적으로 많은 합병증을 일으킨다. 이런 점에서 인공고관절에서 마모와 정렬각도를 수술 후 추시를 통해 측정하는 것은 중요하다. 특히 관절면의 마모를 측정하는 것은 그 량이 크지 않아 상당한 정밀도를 요구한다. 현재 널리 쓰이는 상용 소프트웨어는 재현성과 측정방법의 표준화가 되어 있지 않아 사용에 많은 어려움이 있다. 본 논문에서는 일반 병원의 방사선촬영 환경에서 CAD만 추가로 구비하면 행할 수 있는 인공관절면 마모와 식립각 측정법을 제시한다. 인공관절만의 X-ray영상을 이용한 본 방법의 정확도와 정밀도 평가를 수행하였다. 또한 실제 인공고관절환자의 X-ray영상을 가지고 2년추시에 따른 마모와 식립각을 측정하였다. 본 연구에서 제시한 CAD를 이용한 방법은 마모측정에서 정확도 0.06 mm, 정밀도 0.05 mm, 식립각은 $0.27^{\circ}$의 정밀도를 갖는다.

  • The purpose of this case report and review of the literature is to provide documentation on periprosthetic hip joint infection with Flavonifractor plautii (formerly known as Eubacterium plautii), a strictly anaerobic bacterium, and to report on a successful pathway for management including staged surgical revisions and extended antibiotic therapy. A systematic review of the literature was conducted, which identified this case as only the fifth documented case of human infection with this organism; as a result, conduct of further research is warranted, based on the paucity of reports in the literature addressing anaerobic periprosthetic joint infection.

  • Numerical modeling using the finite element method (FEM) offers crucial insights into the mechanical behavior of prostheses, including stress and strain distribution, load transfer, and stress intensity factors. Analyzing cracking in PMMA surgical cement (polymethylmethacrylate) for total hip prostheses (THP) is essential for understanding the loosening phenomenon, as the rupture of orthopedic cement is a primary cause. By understanding various failure mechanisms, significant advancements in cemented total prostheses can be achieved. This study performed a numerical analysis using a 3D FEM model to evaluate stress levels in different THP models, aiming to model damage in the orthopedic cement used in total hip arthroplasty. Utilizing ABAQUS software, FEM, and XFEM, the damage in three types of THPs-Charnley (CMK3), Osteal (BM3), and THOMPSON was modeled under stumbling loading conditions. XFEM allowed for the consideration of crack propagation between the cement and bone, while the GEARING criterion employed a user-defined field subroutine to model damage parameters. The study's findings can contribute to improving implant fixation techniques and preventing postoperative complications in orthopedic surgery.

족관절 인공관절 치환술 후 관리 및 재활 (Total Ankle Arthroplasty Management and Rehabilitation)

  • Although total ankle arthroplasty (TAA) has increased considerably in the past ten years, reflecting improvements in implant design and survivorship, the clinical outcomes have been less satisfactory than total hip or total knee arthroplasties. Several issues under debate include postoperative management and rehabilitation in TAA. Especially, there is no consensus or evidence for the most appropriate postoperative management and rehabilitation for patients undergoing TAA. This study was therefore undertaken to suggest appropriate postoperative management and rehabilitation in TAA, after reviewing published articles and focusing on the following topics: prehabilitation, hospital stay, immobilization type and duration, weight-bearing management, pharmacological treatment, and adopted rehabilitation protocols. In previous studies, the postoperative management and rehabilitation proposed depended on the surgeon's preference, the patient's characteristics, and the associated surgical procedures performed after TAA. Nonetheless, our research indicates the best approach is to include a prehabilitation program, immobilization in the early postoperative stage (2~4 weeks), range of motion exercise with partial weight-bearing ambulation, followed by full weight-bearing ambulation after six weeks. Further studies are required to develop a standardized rehabilitation protocol and improve the overall quality of care after TAA.

스템 길이에 따른 환자맞춤 인공고관절의 역학적 안정성 비교 (Comparison of Biomechanical Stability of Custom-made Hip Implants using Finite Element Analysis)

  • Designing a morphologically well-fitted hip implant to a patient anatomy is desirable to improve surgical outcomes since a commercial ready-made hip implant may not well conform to the patient joint. In this study, biomechanical stability of patient-specific hip implants with two different stem lengths was compared and discussed using a 3D finite element analysis (FEA). The FEA results in this study showed that an increase in stem length brings about more the peaked von-Mises stress (PVMS) in the prosthesis and less in the femur. However the decrease in von-Mises stress in the femur causes stress shielding phenomenon that usually leads to considerable bone resorption. Although, in biomechanical stability point of view, this work recommends the use of smaller stems, the length of stem must be determined by considering both the von-Mises stress and the stress-shielding phenomenon.

  • Total hip replacement is a crucial intervention for patients with fractured hips who face challenges in natural recovery. The design of durable prostheses requires a comprehensive understanding of the natural processes occurring in bone. This article focuses on static loading analysis, specifically during stumbling activity, aiming to enhance the longevity of prosthetic implants. Three distinct implants, Charnley, Osteal, and Thompson, were selected for a detailed study to determine the most appropriate model. The results revealed critical insights into the distribution of Von Mises stresses on the components of femoral arthroplasty, including the cement, implant, and cortical bone. Furthermore, the examination of shear stress within the cement emerged as a pivotal aspect for all three implants, playing a crucial role in evaluating the performance and durability of hip prostheses. The conclusions drawn from this study strongly suggest that the Thompson model stands out as the most suitable choice for hip joint implants.

Publication Trends in the Pelvic Parameter Related Literature between 1992 and 2022 : A Bibliometric Review

  • Objective : This study aimed to conduct a bibliometric analysis on pelvic parameter related research over the last 30 years, analyzing trends, hotspots, and influential works within this field. Methods : A comprehensive Web of Science database search was performed. The search yielded 3249 results, focusing on articles and reviews published from 1992 to 2022 in English. Data was analyzed using CiteSpace and VOSviewer for keyword, authorship, and citation burst analysis, co-citation analysis, and clustering. Results : The number of publications and citations related to pelvic parameters has increased exponentially over the last 30 years. The USA leads in publication count with 1003 articles. Top publishing journals include the European Spine Journal, Spine, and Journal of Neurosurgery: Spine, with significant contributions by Schwab, Lafage V, and Protoptaltis. The most influential articles were identified using centrality and sigma values, indicating their role as key articles within the field. Research hotspots included spinal deformity, total hip arthroplasty, and sagittal alignment. Conclusion : Interest in pelvic parameter related research has grown significantly over the last three decades, indicating its relevance in modern orthopedics. The most influential works within this field have contributed to our understanding of spinal deformity, pelvic incidence, and their relation to total hip arthroplasty. This study provides a comprehensive overview of the trends and influential research in the field of pelvic parameters.

Functional Outcome after Reimplantation in Patients Treated with and without an Antibiotic-Loaded Cement Spacers for Hip Prosthetic Joint Infections

  • Purpose: A staged revision with placement of a temporary antibiotic-loaded cement spacer after removal of the implant is the "gold standard" for treatment of chronic prosthetic joint infection (PJI). It enables local delivery of antibiotics, maintenance of limb-length and mobility, easier reimplantation. However, bacterial colonization of spacers and mechanical complications can also occur. The aim of this study is to evaluate functional results and infection control in two-stage treatment of total hip arthroplasty (THA) PJI with and without a spacer. Materials and Methods: A retrospective review of 64 consecutive patients was conducted: 34 underwent two-stage revision using a cement spacer (group A), 30 underwent two-stage revision without a spacer (group B). At the final follow-up, functional evaluation of patients with a THA in site, without PJI recurrence, was performed using the Harris hip score (HHS). Measurement of limb-length and off-set discrepancies was performed using anteroposterior pelvic X-rays. Results: Most patients in group B were older with more comorbidities preoperatively. Thirty-three patients (97.1%) in group A underwent THA reimplantation versus 22 patients (73.3%) in group B (P<0.001). No significant differences in limb-length and off-set were observed. The results of functional evaluation performed during the final follow-up (mean, 41 months) showed better function in patients in group A (mean HHS, 76.3 vs. 55.9; P<0.001). Conclusion: The use of antibiotic-loaded cement spacer seems superior in terms of functional outcomes and reimplantation rate. Resection arthroplasty might be reserved as a first-stage procedure in patients who are unfit, who might benefit from a definitive procedure.

슬관절 전치환술 초기의 대퇴사두근 운동 방법에 따른 슬관절과 족관절의 관상면 정렬 비교: 하지 등척성 동시수축과 대퇴사두근 단독 등척성 수축 (A Comparison of Knee and Ankle Coronal Plane Alignment According to Quadriceps Exercise Method in Early Phase of Total Knee Arthroplasty: Lower Extremity Isometric Co-Contraction and Quadriceps Isolated Isometric Contraction)

  • Background: Total knee arthroplasty (TKA) recovers the alignment of the knee joint, but fails to automatically restore the alignment and function of the hip and ankle joints. It may affect the alignment and stability of the knee joint, therefore therapeutic intervention in hip and ankle joint is necessary for the rehabilitation process after TKA. Objects: The aim of this study was to comparison of the effects of the two exercise methods on the coronal plane alignment after TKA. This study conducted an experiment by dividing subjects into a lower extremity isometric co-contraction group (LEIC) and a quadriceps isolated isometric contraction (QIIC) group. Methods: A total of 37 subjects were randomly assigned to the LEIC ($n_1$=19) or the QIIC ($n_2$=18). Exercise was applied to five times per week for three weeks, starting on the eighth day after surgery. Range of motion exercises were performed as a common intervention and then each group performed quadriceps isometric contraction exercises with 10 sets of 5 repetitions. Radiological imaging was performed prior to surgery, one month and six months after surgery. In addition, the hip-knee-ankle angle (HKA) and tibiotalar angle (TTA) were measured. Results: The HKA was close to neutral in the LEIC rather than the QIIC (p<.05). The LEIC showed varus and the QIIC exhibited valgus TTA (p<.05). In a comparison of HKA and TTA over time, there was no significant change in either group (p>.05). According to the comparison of the TTA before surgery, the LEIC showed significant changes in the varus direction (p<.05), while there was no significant change in the QIIC (p>.05). Conclusion: The LEIC method triggered changes in the TTA and brought the HKA close to the neutral. Thus, LEIC is more effective than QIIC in creating stability in the coronal plane alignment of the knee and ankle joints after TKA.
