Dosimetric Comparision for Rectal Cancer using 3D-CRT, IMRT, Tomotherapy (직장암의 방사선 치료 시 3D-CRT, IMRT, Tomotheray를 이용한 치료계획 및 주변 정상장기 선량 비교)

  • In this paper, we compared the Radiation treatment plan of rectal cancer on 3D-conformal Radiation Therapy, Tomotherapy and Linac Based IMRT using treatment planning system and to find the optimal treatment technique. The results of the comparison of treatments are as follows. In tumor tissue absorption dose more than 95% of the dose prescription dose and normal tissues(bladder, small bowel, fumer bone head) was NOT Normal tissue complication rate(V40, V30, V20, V10) but, The most effective treatment(dose distribution) for the three treatments was tomotherapy based IMRT. The worst was 3D-CRT. If this study is applied to patients under their health status and physical environment, patient's prognosis and quality of life will improve.

  • "Sarcopenia", sarcopenia is an old age syndrome, and used to describe the reduction of skeletal muscle. Initially, it was thought that sarcopenia was only a senile disease characterized by degeneration of muscle tissue. However, its cause is widely regarded as multifactorial, with neurological decline, hormonal changes, inflammatory pathway activation, declines in activity, chronic illness, fatty infiltration, and poor nutrition, all shown to be contributing factors. Skeletal muscle mass can be measured by a variety of methods, currently, the commonly used methods are dual-energy X-ray scanning (DXA), computer tomography (CT), magnetic resonance imaging (MRI), etc. Muscular skeletal disorders can also be assessed by measuring appendicular skeletal muscle (ASM), particularly muscle tissue content. At the same time, sarcopenia refers to skeletal muscle cell denervation, mitochondrial dysfunction, inflammation, hormone synthesis and secretion changes and a series of consequences caused by the above process and is a progressive loss of skeletal muscle syndrome, which can lead to the decrease of muscle strength, physical and functional disorders, and increase the risk of death. Sarcopenia is mainly associated with the aging process, but also related to other causes such as severe malnutrition, neurodegenerative diseases, and disuse and endocrine diseases associated with muscular dystrophy, and it is the comprehensive results of multi-factors, so it is difficult to define that sarcopenia is caused by a specific disease. With the aging problem of the population, the incidence of this disease is increasingly common, and seriously affects the quality of the life of the elderly. This paper reviews the etiology and pathogenesis of myopathy, screening methods and diagnosis, the influence of eating habits, etc, and hopes to provide reference for the diagnosis and treatment of this disease. At present, adequate nutrition and targeted exercise remain the gold standard for the therapy of sarcopenia.

  • This paper performed the basic study for developing the Photodynamic Therapy Equipment for medical treatment. We developed the equipment palpating cell proliferation using a high brightness LED. This equipment was fabricated using a micro-controller and a high brightness LED, and designed to enable us to control light irradiation time, intensity, frequency and so on. Especially, to control the light irradiation frequency, FPGA was used, and to control the change of output value, TLC5941 was used. Control stage is divided into 30 levels by program. Consequently, the current value could be controlled by the change of level in Continue Wave(CW) and Pulse Width Modulation(PWM), and the output of a high brightness LED could be controlled stage by stage. And then, each experiment was performed to irradiation group and non-irradiation group for both Rat bone marrow and Rat tissue cells. MTT assay method was chosen to verify the cell increase of two groups and the effect of irradiation on cell proliferation was examined by measuring 590 nm transmittance of ELISA reader. As a result, the cell increase of Rat bone marrow and tissue cells was verified in irradiation group as compared to non-irradiation group. The fact that specific wavelength irradiation has an effect on cell vitality and proliferation is known through this study.

  • The work suggested in this paper addresses a method for collecting beam data for small circular fields. Beam data were obtained from philips 6 and 8 MV LINAC at Dept. Radiation Therapy at Gainesville Incorporated and Shands Teaching Hospital. Specific quantities measured include tissue maximum ratio (TMR), off-axis ratio (OAR) and relative output factor (ROF) In small field irradiation, special collimators were used to produce circular fields of 1 cm to 3 cm diameter in 2 mm steps, measured at SAO (soura axis distance) of 100 cm. Diode detector was chosen for primary beam measurement and compared with measurements made with photographic film and TLD dosimeters. The measured TMRs and OARs were formulated from limited measurements to generate basic beam data for reference set-up. The empirical formula were later, extended and generalized for any possible set-up using the trends of fitting parameters. The measured TMRs and OARs were well represented by the fitting formula developed.

Clinical Experience of Pyoderma Gangrenosum with Extensive Soft Tissue Necrosis (광범위 연조직 괴사를 동반한 괴저농피증의 치험례)

  • Purpose: Pyoderma gangrenosum is a rare cutaneous ulcerative disease. First described in 1930, the condition is characterized by progressive ulceration with deeply undermined purple-red edge. The lower extremities are most commonly affected but other parts of the skin and mucous membranes may also be involved. Although medical treatments with topical wound therapy are commonly used, surgical intervention is still controversial. In this paper, we report an atypical case of pyoderma gangrenosum which was characterized by extensive soft tissue breakdown. Methods: A 27-year-old male patient was referred to our institution with a $7{\times}8cm$ sized deeply undermined ulceration with pus-like discharge and fever. Incision and drainage was performed at another clinic 3 days prior to admission to our institution. After a thorough physical examination and the MRI review, a diagnosis of necrotizing faciitis was made. Accordingly, fasciotomy and debridement was performed. However, the wound enlarged progressively and the patient remained highly febrile for 9 days after the treatment. Septic screening did not reveal any occult infection. After a secondary review of the case, the initial diagnosis of necrotizing fasciitis was rejected and changed to pyoderma gangrenosum. With the use of dexamethasone intravenously, the wound improved dramatically and the fever was eliminated. Steroid mediation was tapered with duration of 1 month. The wound was stabilized and subsequently covered with split-thickness skin graft. Results: Split-thickness skin grafting with 1 : 1.5 mesh was successfully taken. Conclusion: Initial clinical features of pyoderma gangrenosum are very similar to that of necrotizing fasciitis. High fever and progressive ulceration with severe pain could invite earlier surgical approach. The advancing wound margins (the well defined violaceous, undermined border and necrotic ulcer base) and lack of isolation of pathogenic organism was used to make the correct diagnosis of pyoderma gangrenosum. We achieved a good result with proper medication and split-thickness skin graft.

Electron Microscopic Studies of Human Keloid and Hypertrophic Scars (Keloid와 Hypertrophic Scar ( 비후성반흔 )의 형태학적 관찰)

  • Introduction. The human cutaneous scars manifest themselves many ways in different types according to the factors such as the age, sex, race of the patient as well as the location,. kind and heal ing process of the wound. Among the scars it is quiet difficult to verify the clinical course of the hypertrophic or keloidal scars from the true keloids. However, clinical observations indicate that stress, either mechanical or in the forms of chronic infections, can induce a functional change in the fibroblasts causing an excessive production of collagenous matrix. In this study, we preliminary attempt to justify any difference of the cellular structure between keloids and hypertrophic scars by using electron microscope. Material and Methods. A total of 23 cases: 2 scars, 2 hypertrophic scars and 19 keloids are examined. Immediately, the biopsy tissue was fixed in 10% neutral formalin and 4% glutaraldehyde solution in phosphate buffer for 4 hours, post fixed in 1 % osmium tetraoxide for two hours, dehydrated with graded alcohol, and embedded in Epon 812. Thick sections were stained with hematoxylin eosin, periodic acid-Schiff(PAS) and Van Gieson stain. Thin sections were cut and uranyle acetate, lead citratestain and examined with the electron microscope. Result. The morphologic features of keloid showed thick, homogenously eosinophilic bands of collagen and numberous large active fibroblasts. The hypertrophic scar and soft scar are more cellular than keloid and composed thinner collagenous fiber. For this paper in the etiology of keloids can not as be defined, but and interesting keloidal tissue fibroblast showed irregular nucleus with irregular shape dense bodies and fibril materials contained in to the cytoplasm.

Using Bayesian Approaches to Reduce Truncation Artifact in Magnetic Resonance Imaging

  • In Fourier magnetic resonance imaging (MRI), the number of phase encoded signals is often reduced to minimize the duration of the studies and maintain adequate signal-to-noise ratio. However, this results in the well-known truncation artifact, whose effect manifests itself as blurring and ringing in the image domain. In this paper, we propose a new regularization method in the context of a Bayesian framework to reduce truncation artifact. Since the truncation artifact appears in t도 phase direction only, the use of conventional piecewise-smoothness constraints with symmetric neighbors may result in the loss of small details and soft edge structures in the read direction. Here, we propose more elaborate forms of constraints than the conventional piecewise-smoothness constraints, which can capture actual spatial information about the MR images. Our experimental results indicate that the proposed method not only reduces the truncation artifact, but also improves tissue regularity and boundary definition without oversmoothing soft edge regions.

Prostate MR and Pathology Image Fusion through Image Correction and Multi-stage Registration (영상보정 및 다단계 정합을 통한 전립선 MR 영상과 병리 영상간 융합)

  • In this paper, we propose a method for combining MR image with histopathology image of the prostate using image correction and multi-stage registration. Our method consists of four steps. First, the intensity of prostate bleeding area on T2-weighted MR image is substituted for that on T1-weighted MR image. And two or four tissue sections of the prostate in histopathology image are combined to produce a single prostate image by manual stitching. Second, rigid registration is performed to find the affine transformations that to optimize mutual information between MR and histopathology images. Third, the result of affine registration is deformed by the TPS warping. Finally, aligned images are visualized by the intensity intermixing. Experimental results show that the prostate tumor lesion can be properly located and clearly visualized within MR images for tissue characterization comparison and that the registration error between T2-weighted MR and histopathology image was 0.0815mm.

Attenuation Estimation of Soft Tissue by the Entropy Method and Statistical Averaging of Speckle Patterns (스펙클 패턴의 통계적 평균과 엔트로피 방식에 의한 초음파 감쇄계수 추정방법)

  • The time domain methods of estimating the attenuation coefficient are generally based on the analysis of statisical properties of the reflected echoes form an attenuating medium. Hence, it is often required to have a large number of data samples in order to obtain a statistically stable estimation result. In the attenuation estimation problem, this means that many different speckle patterns are required in the spatial resolution volume of an attenLlation image. In this paper, by using the fact that the speckle pattern Is sensitive to the point spread function of the ultrasound beam, we suggest a method to generate the statiscally uncorrelated or slightly correlated data samples in a given region by rotating a linear transducer and carrying out lateral scans for all rotating angles. This technique is applied to the entropy method for attenuation estimation proposed recently by the authors where the performance is verified by experiments using a tissue equivalent phantom.

Color Image Analysis of Histological tissue Sections (해부병리조직에 대한 칼라 영상분석)

  • In this paper, we suggest a new direct method for mage segmentation using texture and color information combined through a multivariate linear discriminant algorithm. The color texture is computed in nin 3${\times}$3 masks obtained from each 3${\times}$3${\times}$3 spatio-spectral neighborhood in the image using the classical haralick and Pressman texture features. Among these 9${\times}$28 texture features the best set was extracted from a training set. The resulting set of 10 features were used to segment an image into four different regions. The resulting segmentation was Compared to classical color and texture segmentation methods using both box classifiers and maximum likelihood classification. It compared favourably on the test image from a Fastred-Lightgreen stained prostatic histological tissue section based on visual inspection. The classification accuracy of 97.5% for the new method obtained on the training data was also among the best of the tested methods. If these results hold for a larger set of images, this method should be a useful tool for segmenting images where both color and texture are relevant for the segmentation process.
